Description
本願は、参照により本明細書に組み込まれる2012年5月23日出願の米国特許仮出願第61/650,615号の利益を主張するものである。
本発明は、組織焼灼システムに関する。より詳細には、本発明は、気管支カテーテルにおける改良に関する。
ここで、本発明の代替的実施形態による、気管支内プローブの遠位部分のアセンブリ82の部分図である図7を参照する。アセンブリ82は中空の管状セグメント84、86を含み、該セグメントは、上述したような生検ツール及び焼灼針などの器具を所有するプローブのための進入を提供し、また他の構成要素のための支持を提供する。例えば、セグメント86は、バルーン88を支持する。
ここで、本発明の代替的実施形態による、気管支内プローブを介して、針を通して配置されたワイヤ上に配置され得るシースのシャフト158の一部分の側面図である図13を参照する。繋留アセンブリは、径方向に突出するワイヤ160のアレイを含み、ワイヤ160のそれぞれは、2つの孔162、164を通してシャフト158から退出している。ワイヤ160は、医療手技中、必要に応じて、ハンドル20(図1)上の制御部を用いて後退され、又は延伸されて気管支の壁と係合することができる。
ここで、本発明の代替的実施形態による、気管支内プローブの遠位部分のアセンブリ166の部分分解図である図14を参照する。アセンブリ166は、アセンブリ82(図7)と類似している。しかしながら、この実施形態では、バルーン168はディフレクタ90上に直接装着される。膨張された際、バルーン168はディフレクタ90の一部分又は殆どを包囲し得るが、出口孔92を遮断しない。遠位セグメント170は、対応するセグメント86(図7)と比較して短縮されてもよく、又は完全に省略されてもよい。この配置は、一旦アセンブリ166が操作者によって配置され、バルーン168が膨張されると、気管支の内壁に高い程度の出口孔92の接近を提供する。
上記の記載は、針が生検を行い、生検及び/又は焼灼部位を封止するように使用されることを想定していた。代替的実施形態では、針は他の機能を行うように構成されてもよく、該機能のいくつかを以下に列挙する。
(1) 内視鏡装置であって、
ルーメンと、長軸と、遠位セグメントとを有する細長いアセンブリであって、前記ルーメンが前記長軸に概ね沿って延伸する、アセンブリと、
前記遠位セグメント内に配置されたディフレクタであって、前記長軸に対してある角度で前記ディフレクタを貫通して延伸するボアを有し、前記ボアが、出口孔と、前記ルーメンとの横向きの連絡部とを有し、前記ルーメン、前記ボア、及び前記横向きの連絡部が、前記ルーメン、前記横向きの連絡部、前記ボア、及び前記出口孔を通したツールの通過を受け入れる寸法を有する、ディフレクタと、
前記遠位セグメント内に配置された位置センサであって、前記位置センサからの信号に応答して前記遠位セグメントの位置を算出するよう作動する位置プロセッサと接続可能である、位置センサと、
前記遠位セグメント内に配置された超音波イメージャであって、前記超音波イメージャにより提供されるデータを処理する電子回路に接続可能である、超音波イメージャと、
複数の案内部と、前記複数の案内部内に通されたそれぞれのワイヤとを含むプッシュプル繋留システムであって、前記ワイヤが、前記ワイヤが前記案内部内に後退されている第1の位置と、前記アセンブリが気管支内部に挿入された際、前記ワイヤが前記案内部を越えて延伸し、前記長軸から十分に分岐して気管支と係合する第2の位置との間で移動するよう作動する、プッシュプル繋留システムと、
前記遠位セグメント上に配置された膨張可能なバルーンと、を備える、内視鏡装置。
(2) 前記アセンブリが別のルーメンを有し、前記ディフレクタが、前記別のルーメンと連絡する入口孔を有し、前記ディフレクタの前記ボアが、前記入口孔に繋がり、前記別のルーメンから前記入口孔を通し、前記出口孔を通した前記ツールの通過を受け入れる、実施態様1に記載の装置。
(3) 近位方向に位置する制御ハンドルを更に備え、前記繋留システムが前記ディフレクタと前記制御ハンドルとの間に配置され、前記ワイヤが前記制御ハンドルから制御される、実施態様1に記載の装置。
(4) 前記ワイヤが、前記第2の位置において、前記長軸から外方向に、略近位方向に延伸する、実施態様1に記載の装置。
(5) 前記位置センサが3軸磁場センサである、実施態様1に記載の装置。
(7) 後退可能な生検針と少なくとも1つの後退可能な焼灼針とを更に備え、前記生検針及び前記焼灼針が、前記ディフレクタを介して留置可能である、実施態様1に記載の装置。
(8) 内視鏡装置であって、
近位セグメントと、遠位セグメントと、肺内に貫通するように構成されている遠位端と、を有するプローブと、
前記肺に関連した前記遠位端の位置を示す信号を生成するように構成されている位置センサと、
前記肺を画像化するように構成されている超音波イメージャと、
後退可能な生検針であって、前記生検針の留置後、前記肺の試料を吸引するように構成されている、後退可能な生検針と、
少なくとも1つの後退可能な焼灼針であって、前記少なくとも1つの焼灼針の留置後、前記肺の一区画を焼灼するように構成されている、少なくとも1つの後退可能な焼灼針と、を備える、内視鏡装置。
(9) バルーンを更に備え、前記バルーンが、前記遠位セグメントの一部分を包囲するように、かつ、前記バルーンの膨張後に前記肺の前記一区画を封止するように、構成される、実施態様8に記載の装置。
(10) 前記生検針が、前記肺の試料採取部分にシーラントを適用するように構成されている、実施態様8に記載の装置。
(12) 内視鏡検査の方法であって、
ルーメンと、長軸と、遠位セグメントと、前記遠位セグメント内に配置されたディフレクタとを有する細長いアセンブリを肺の気管支内に挿入する工程であって、前記ルーメンが前記長軸に概ね沿って延伸し、前記ディフレクタが、前記長軸に対してある角度で前記ディフレクタを貫通して延伸するボアを有し、前記ボアが、出口孔と、前記ルーメンとの横向きの連絡部とを有する、工程と、
前記ディフレクタ内にツールを導入する工程と、
前記遠位セグメント内に配置された位置センサからの信号を、前記位置センサからの前記信号に応答して前記遠位セグメントの位置を算出するよう作動する位置プロセッサに伝送する工程と、
前記遠位セグメント内に配置された超音波イメージャを使用して前記肺内の標的を画像化し、前記超音波イメージャにより提供されたデータを電子回路に伝送して前記データを処理する工程と、
前記遠位セグメント上に配置された膨張可能なバルーンを膨張させることにより、前記ディフレクタの前記出口孔を、前記気管支の壁に対して押し付ける工程と、
その後、前記ツールを用いて、前記出口孔を介して前記気管支の前記壁を貫通して、前記ツールにより前記肺内の前記標的に到達する工程と、
複数の案内部と、前記複数の案内部内に通されたそれぞれのワイヤと、を含むプッシュプル繋留システムを使用して、前記ワイヤを、前記ワイヤが前記案内部内に後退されている第1の位置と、前記ワイヤが前記案内部を越えて延伸し、前記長軸から十分に分岐して前記気管支の前記壁と係合する第2の位置との間で移動することにより、前記アセンブリを繋留して、前記気管支の前記壁を貫通する間、前記アセンブリ上に逆−牽引を提供する工程と、
前記ツールを使用して前記標的上で手術を行う工程と、を含む、方法。
(13) 前記ツールを導入する工程が、前記ルーメン、前記横向きの連絡部、及び前記出口孔を通して前記ツールを通過させる工程を含む、実施態様12に記載の方法。
(14) 前記アセンブリが別のルーメンを有し、前記ディフレクタが、前記別のルーメンと連絡する入口孔を有し、前記ディフレクタの前記ボアが、前記入口孔に繋がり、前記ツールを導入する工程が、前記別のルーメンから前記入口孔を通し、前記出口孔を通して前記ツールを通過させる工程を含む、実施態様12に記載の方法。
(15) 前記アセンブリが、近位セグメントと、前記近位セグメント上に配置されたワイヤ制御部を有するハンドルとを更に含み、前記アセンブリを繋留する工程が、前記ワイヤ制御部を作動させることにより前記ワイヤを移動する工程を含む、実施態様12に記載の方法。
(17) 前記位置センサが、複数の身体表面電極と協働して、インピーダンス測定信号を前記位置プロセッサに報告する電極である、実施態様12に記載の方法。
Claims (3)
- 内視鏡装置であって、
近位セグメントと、肺の気管支内に貫通するように構成されている遠位端を含む遠位セグメントと、を有するプローブと、
前記肺に関連した前記遠位端の位置を示す信号を生成するように構成されている位置センサと、
前記肺の一区画を画像化するように構成され、前記遠位セグメントに配置されている、超音波イメージャと、
後退可能な生検針であって、前記生検針の留置後、前記肺の試料を吸引するように構成されている、後退可能な生検針と、
少なくとも1つの後退可能な焼灼針であって、前記少なくとも1つの焼灼針の留置後、前記肺の前記一区画内の標的を焼灼するように構成されている、少なくとも1つの後退可能な焼灼針と、
膨張可能なバルーンであって、前記バルーンが、前記超音波イメージャの近位側の前記遠位セグメントの一部分を包囲するように構成され、かつ、前記バルーンを膨張させて前記気管支の遠位部分を前記気管支の残りの部分から隔離することによって前記バルーンの膨張後に前記肺の前記一区画を封止するように構成される、バルーンと、を備える、内視鏡装置。 - 前記生検針が、前記肺の試料採取部分にシーラントを適用するように構成されている、請求項1に記載の装置。
- 前記焼灼針が、該焼灼針を用いた焼灼後に、前記肺の焼灼部分にシーラントを適用するように構成されている、請求項1に記載の装置。
