Description
(i)患者において1つ以上の腫瘍抗原に対する第一免疫応答を誘導する工程、および
(ii)患者において1つ以上の腫瘍抗原に対する第二免疫応答を誘導する工程であって、前記第二免疫応答が、患者の癌細胞中に存在する癌特異的体細胞突然変異に特異的である工程、を含む方法に関する。
(a)癌患者の腫瘍標本中で発現される腫瘍抗原、特に共通腫瘍抗原を同定する工程;および
(b)好ましくは、各々の製造前ワクチン生成物が好ましくは共通腫瘍抗原に対する免疫応答を誘導する、製造前ワクチン生成物を含むセットからワクチン生成物を選択することによって、工程(a)で得られた腫瘍抗原プロフィール、特に共通腫瘍抗原プロフィールを特徴とするワクチンを提供する工程、を含む方法によって提供され得る。
(a)癌患者の腫瘍標本中で癌特異的体細胞突然変異を同定し、患者の癌突然変異シグネチャーを提供する工程;および
(b)工程(a)で得られた癌突然変異シグネチャーを特徴とするワクチンを提供する工程、を含む方法によって提供され得る。
i)癌患者からの腫瘍標本および、好ましくは癌患者に由来する、非腫瘍形成性標本を提供する工程;
ii)腫瘍標本のゲノム、エクソームおよび/またはトランスクリプトームと非腫瘍形成性標本のゲノム、エクソームおよび/またはトランスクリプトームとの間の配列相違を同定する工程;
iii)工程(ii)で決定された配列相違を組み込んだエピトープを含むペプチドまたはポリペプチドを設計する工程;
iv)工程(iii)で設計されたペプチドもしくはポリペプチドまたは前記ペプチドもしくはポリペプチドをコードする核酸、好ましくはRNAを提供する工程;ならびに

−例えば転写産物を分析することによって、発現されたタンパク質改変突然変異を同定する工程;
−潜在的に免疫原性である突然変異を同定する、すなわち得られたデータを、確認されている免疫原性エピトープの利用可能なデータセット、例えばhttp://www.immunoepitope.orgのIMMUNE EPITOPE DATABASE AND ANALYSIS RESOURCEなどの公共免疫エピトープデータベースに含まれるものと比較することによって同定する工程;
の1つ以上、好ましくは全部を含む。
1)例えば、Ronaghi et al.1998:A sequencing method based on real−time pyrophosphate.Science 281(5375),363−365に最初に記載された、Roche関連会社である454 Life Sciences(Branford,Connecticut)のGS−FLX 454 Genome Sequencer(登録商標)において実行されるピロシークエンス法として公知の「合成によるシークエンシング」技術。この技術は、一本鎖DNA結合ビーズが、激しくボルテックスすることにより、エマルジョンPCR増幅のために油に取り囲まれたPCR反応物を含有する水性ミセル中に被包される、エマルジョンPCRを用いる。ピロシークエンシング工程中、ポリメラーゼがDNA鎖を合成するにつれて、ヌクレオチド組込みの間にリン酸分子から放出される光が記録される。
2)可逆的色素−ターミネータに基づく、例えばIllumina/Solexa Genome Analyzer(登録商標)およびIllumina HiSeq 2000 Genome Analyzer(登録商標)において実行される、Solexa(現在はIllumina Inc.,San Diego,Californiaの一部)によって開発された「合成によるシークエンシング」アプローチ。この技術では、4つすべてのヌクレオチドを、DNAポリメラーゼと共にフローセルチャネル中のオリゴプライミングしたクラスターフラグメントに同時に添加する。架橋増幅は、シークエンシングのために4つすべての蛍光標識ヌクレオチドを有するクラスター鎖を伸長させる。
3)例えばApplied Biosystems(現在はLife Technologies Corporation,Carlsbad.California)のSOLid(登録商標)プラットフォームにおいて実行される、「ライゲーションによるシークエンシング」アプローチ。この技術では、固定された長さのすべての可能なオリゴヌクレオチドのプールを配列決定された位置に従って標識する。オリゴヌクレオチドをアニーリングし、連結する;配列をマッチさせるためのDNAリガーゼによる選択的なライゲーションは、その位置のヌクレオチドの情報を与えるシグナルをもたらす。シークエンシングの前に、DNAをエマルジョンPCRによって増幅する。各々同じDNA分子のコピーだけを含有する、生じたビーズをスライドガラス上に載せる。2番目の例として、Dover Systems(Salem,New Hampshire)のPolonator(登録商標)G.007プラットフォームも、ランダムに配置したビーズに基づくエマルジョンPCRを使用して並列シークエンシングのためにDNAフラグメントを増幅することによる、「ライゲーションによるシークエンシング」アプローチを用いる。
4)例えばPacific Biosciences(Menlo Park,California)のPacBio RSシステムまたはHelicos Biosciences(Cambridge,Massachusetts)のHeliScope(登録商標)プラットフォームにおいて実行されるような、単一分子シークエンシング技術。この技術の明らかな特徴は、単一分子リアルタイム(SMRT)DNAシークエンシングと定義される、単一DNAまたはRNA分子を増幅せずに配列決定するその能力である。例えばHeliScopeは、各々のヌクレオチドが合成されると共にそれを直接検出する高感度蛍光検出システムを用いる。蛍光共鳴エネルギー移動(FRET)に基づく同様のアプローチがVisigen Biotechnology(Houston,Texas)から開発されている。他の蛍光に基づく単一分子技術は、U.S.Genomics(GeneEngine(登録商標))およびGenovoxx(AnyGene(登録商標))からのものである。
5)例えば複製中の一本鎖上のポリメラーゼ分子の動きを観測するためにチップ上に配置された様々なナノ構造を用いる、単一分子シークエンシングのためのナノ技術。ナノ技術に基づくアプローチの非限定的な例は、Oxford Nanopore Technologies(Oxford,UK)のGridON(登録商標)プラットフォーム、Nabsys(Providence,Rhode Island)によって開発されたハイブリダイゼーション支援ナノポアシークエンシング(HANS(登録商標))プラットフォーム、およびコンビナトリアルプローブアンカーライゲーション(cPAL(登録商標))と呼ばれるDNAナノボール(DNB)技術を用いた、特許保護されているリガーゼに基づくDNAシークエンシングプラットフォームである。
6)単一分子シークエンシングのための電子顕微鏡検査に基づく技術、例えばLightSpeed Genomics(Sunnyvale,California)およびHalcyon Molecular(Redwood City,California)によって開発されたもの。
7)DNAの重合の間に放出される水素イオンの検出に基づくイオン半導体シークエンシング。例えばIon Torrent Systems(San Francisco,California)は、この生化学的工程を超並列的に実施するために微細機械加工したウェルの高密度アレイを使用する。各々のウェルは異なるDNA鋳型を保持する。ウェルの下にはイオン感受性層があり、その下には特許保護されているイオンセンサーがある。
1.所与の患者から腫瘍組織/細胞および健常組織を採取する。
2.遺伝物質を癌性および健常細胞から抽出し、次に標準的な次世代シークエンシング(NGS)プロトコルを用いてそのエクソーム(DNA)を配列決定する。NGSのカバレッジは、少なくとも5%の頻度を有するヘテロ接合体対立遺伝子を検出できる範囲である。トランスクリプトーム(RNA)も癌細胞から抽出し、cDNAに変換して、配列決定し、いずれの遺伝子が癌細胞によって発現されるかを決定する。
3.発現された非同義一塩基変異(SNV)を本明細書で述べるように同定する。健常組織中のSNPである部位をフィルタリング除去する。
4.(3)からのN=96の突然変異を種々の頻度にわたって選択する。蛍光検出に基づくSNP遺伝子型決定アッセイをこれらの突然変異について設計し、合成する(そのようなアッセイの例には、Life TechnologiesによるTaqManに基づくSNPアッセイまたはFluidigmによるSNPtypeアッセイが含まれる)。アッセイは、所与のSNVを含むアンプリコンを増幅するための特異的標的増幅(STA)プライマーを含む(これはTaqManおよびSNPtypeアッセイにおいて標準的である)。
5.個々の細胞をレーザーマイクロダイセクション(LMD)または単細胞懸濁液への分離のいずれかによって腫瘍および健常組織から単離し、次いで以前に記述されているように(Dalerba P.et al.(2011)Nature Biotechnology 29:1120−1127)選別する。細胞を事前選択せずに(すなわち偏りなく)選択することができるか、あるいは癌性細胞を濃縮することができる。濃縮方法には、特異的染色、細胞の大きさによる選別、LMD中の組織学的検査等が含まれる。
6.個々の細胞を、マスターミックスとSTAプライマーを含むPCR管中で単離し、SNVを含むアンプリコンを増幅する。あるいは単一細胞のゲノムを、以前に記述されているように(Frumkin D.et al.(2008)Cancer Research 68:5924)全ゲノム増幅(WGA)によって増幅する。95℃の加熱工程または専用の溶解緩衝液のいずれかによって細胞溶解を達成する。
7.STA増幅した試料を希釈し、Fluidigm遺伝子型決定アレイに負荷する。
8.健常組織からの試料を陽性コントロールとして使用し、ホモ接合体対立遺伝子クラスター(突然変異なし)を決定する。NGSデータはホモ接合体突然変異が極めてまれであることを示すので、典型的には2つのクラスター:XXおよびXYだけが予測され、X=健常である。
9.実行し得るアレイの数は制限されず、実際には約1000個までの単一細胞を検定することが可能である(約10個のアレイ)。384プレートで実施した場合、試料調製を数日間に短縮することができる。
10.次に各々の細胞についてのSNVを決定する。
1.上記の工程1から6は、N(検定されるSNVの数)が96よりはるかに大きくてもよいことを除き、同一である。WGAの場合は、その後に数サイクルのSTAを実施する。STAプライマーは、各プライマー上に2つの万能タグ配列を含む。
2.STA後、バーコードプライマーをアンプリコンへとPCR増幅する。バーコードプライマーは、固有のバーコード配列と上記万能タグ配列を含む。したがって各々の細胞は固有のバーコードを含む。
3.すべての細胞からのアンプリコンを混合し、NGSによって配列決定する。多重化できる細胞数への実際的な制限は、調製できるプレートの数である。試料を384プレートで調製することができるので、実際的な制限は約5000個の細胞である。
4.配列データに基づき、個々の細胞のSNV(または他の構造的異常)を検出する。
ハイスループット全ゲノム単一細胞遺伝子型決定法に関連して上述した単一細胞アッセイに基づき、各々のSNVの頻度を正確に推定することができ、存在する最も豊富なSNVを、癌の個別化ワクチン(IVAC)を提供するために選択することができる。
腫瘍からのNGSデータは、ホモ接合体突然変異(両方の対立遺伝子でヒット)がまれな事象であることを示唆する。それゆえ、ハプロタイピングの必要はなく、腫瘍体細胞突然変異の系統樹を単一細胞SNVデータセットから作成することができる。生殖系列配列を使用して系統樹を根付かせる。祖先配列を再現するためのアルゴリズムを使用して、系統樹の根に近い節の配列を再現する。これらの配列は、原発性腫瘍中に存在すると予測される最初期の突然変異(本明細書では一次基底突然変異/抗原と定義される)を含む。2つの突然変異がゲノム上の同じ位置の同じ対立遺伝子で起こる確率は低いため、祖先配列における突然変異は腫瘍中で固定されていると予測される。
すべての腫瘍部位を最大限にカバーする抗原を得るための別のアプローチは、腫瘍からいくつかの生検を採取することである。1つの戦略は、NGS分析によってすべての生検中に存在すると同定された抗原を選択することである。基底突然変異を同定する確率を改善するために、すべての生検からの単一細胞突然変異に基づく系統発生分析を実施することができる。
転移性腫瘍は単一細胞に由来すると考えられている。それゆえ所与の患者の種々の腫瘍から抽出した個々の細胞を遺伝子型決定することと併せて患者の血中循環腫瘍細胞(CTC)を遺伝子型決定することにより、癌の進化の歴史を再構築することができる。原発性腫瘍に由来するCTCのクレードを通してもとの腫瘍から進化する転移性腫瘍を観測することが期待される。
腫瘍は、免疫系および治療法の選択圧に起因する突然変異を抑制するように進化すると考えられている。同じ細胞上に共起し、腫瘍中でも高頻度である複数の抗原を標的とする癌ワクチンは、腫瘍回避機構を無効にするより大きな可能性を有し、それゆえ再発の可能性を低減する。そのような「カクテルワクチン」は、HIV陽性患者のための抗レトロウイルス併用療法に類似する。共起する突然変異は、系統発生分析によってまたはすべての細胞のSNVアラインメントを検査することによって同定できる。
1.腫瘍の生検を入手し、体細胞突然変異の地図を決定する。
2.選択肢1:これまでに確立された優先順位付けスキームに基づくさらなる検討のためにN≧96の突然変異を選択する。
選択肢2:単一細胞アッセイ(上述したハイスループット全ゲノム単一細胞遺伝子型決定法参照)、次いで系統発生分析を実施し、多様性を最大化するためにN≧96の一次基底突然変異および場合により最近の突然変異を選択する。前者の突然変異はCTCを同定するために(以下参照)、および後者は系統発生分析を行うために有用である(「同じ細胞上に共起する抗原の同定(「カクテル」IVAC)」の章参照)。
3.癌患者から全血を得る。
4.赤血球を溶解する。
5.CD45+細胞を枯渇させる(例えば選別、抗CD45抗体に結合した磁気ビーズ等による)ことによって白血球を除去し、CTCを濃縮する。
6.DNAアーゼ消化によって遊離DNAを除去する。遊離DNAの起源は、血液中に存在するDNAまたは死細胞からのDNAであり得る。
7.残りの細胞をPCR管に選別し、STAを実施して(選択した突然変異に基づく)、Fluidigm(上述したハイスループット全ゲノム単一細胞遺伝子型決定法)でスクリーニングする。CTCは、一般に複数のSNVについて陽性であるはずである。
8.次に、スクリーニングしたSNVのパネルに基づき、癌性と同定された細胞(=CTC)を系統発生的にさらに分析することができる(「同じ細胞上に共起する抗原の同定(「カクテル」IVAC)」の章参照)。

突然変異の検出および優先順位付け
我々は、最初に、バイアスのない方法で体細胞突然変異を同定するための腫瘍試料および正常試料の配列プロファイリングを明らかにする。我々はこれをバルク腫瘍試料に関して明らかにするだけでなく、初めて、個々の血中循環腫瘍細胞から突然変異を同定する能力も実証する。次に、突然変異の予測される免疫原性に基づきポリネオエピトープワクチンに含めるための突然変異を優先順位付け、同定された突然変異が実際に免疫原性であることを実証する。
CTCを使用する論理的根拠:癌患者の末梢血からの血中循環腫瘍細胞(CTC)の検出は、腫瘍の臨床経過についての広く認められている独立した予後マーカーである(Pantel et al,Trends Mol Med 2010;16(9):398−406)。長年にわたって、CTCの臨床的重要性は腫瘍学における熱心な学術および臨床研究の主題となってきた。転移性乳癌、前立腺癌および結腸直腸癌を有する患者の血液中のCTCの検出は予後と関連性があり、従来の画像化技術および他の予後腫瘍バイオマーカーに付加的な情報を提供することが示されている。治療薬による処置(全身的または標的化)の前、初期段階中および処置後に患者から採取した連続的な血液試料は、処置の応答/失敗に関する情報を提供する。薬剤耐性CTCの分子分析は、個々の患者における耐性機構(例えば特定のシグナル伝達経路における突然変異または標的発現の喪失)へのさらなる洞察を提供し得る。CTCのプロファイリングおよび遺伝的特性付けからのさらなる可能性は、新しい標的療法の開発のための新規癌標的の同定である。この新しい診断戦略は「液体腫瘍生検」と称される。このプロファイリングは迅速で繰り返し行うことができ、患者の血液しか必要とせず、手術は不要であるので、腫瘍の状態の「リアルタイム」表示を提供する。
試料:プロファイリング実験のために、試料は、C57BL/6マウスからの5〜10mmの尾部試料(「Black6」)およびもともとはBlack6マウスに由来する、高度に攻撃的なB16F10マウスメラノーマ細胞(「B16」)を含んだ。
次に、我々は、ワクチンに含めるための突然変異の優先順位付けパイプラインの可能性を明らかにする。「個別癌突然変異検出パイプライン」(iCAM)と呼ばれるこの方法は、複数の最先端アルゴリズムとバイオインフォマティクスの方法を組み込んだ一連の工程を通して体細胞突然変異を同定し、優先順位付ける。この工程のアウトプットは、可能性の高い免疫原性に基づいて優先順位付けられた、体細胞突然変異のリストである。
DNAエクソームリシークエンシングからの体細胞突然変異を2つの方法、すなわち突然変異領域のリシークエンシングおよびRNA−Seq分析のいずれかによって確認した。

IVAC選択アルゴリズムは免疫原性突然変異の検出を可能にする
B16F10メラノーマ細胞からの確認された突然変異に対して特異的T細胞応答が誘導され得るかどうかを検討するため、ナイーブC57BL/6マウス(n=5/ペプチド)を、突然変異または野生型アミノ酸配列(表2参照)のいずれかを含むペプチド100μg(+アジュバントとしてPolyI:C 50μg)で皮下的に2回(0日目、7日目)免疫した。すべてのペプチドは27アミノ酸の長さで、中心位置に突然変異/野生型アミノ酸を有していた。12日目にマウスを犠死させ、脾細胞を採取した。読み出し法として、5×105脾細胞/ウェルをエフェクターとし、ペプチド(2μg/ml)を負荷した5×104骨髄樹状細胞を標的細胞として使用して、IFNγ ELISpotを実施した。エフェクター脾細胞を突然変異ペプチド、野生型ペプチドおよびコントロールペプチド(水疱性口内炎ウイルスヌクレオタンパク質、VSV−NP)に対して試験した。

同定された突然変異は治療的抗腫瘍免疫を提供することができる
同定された突然変異が、ナイーブマウスへのワクチン接種後に抗腫瘍免疫を付与する潜在能を有するかどうかを検証するため、突然変異選択的T細胞反応性を誘導することが示された突然変異番号30についてのペプチドを用いてこの問題を検討した。B16F10細胞(7.5×104)を0日目に皮下的に接種した。マウスにペプチド30(表1参照;ペプチド100μg+PolyI:C 50μg s.c.)を−4日目、+2日目および+9日目にワクチン接種した。コントロール群にはPoly I:C(50μg s.c.)だけを与えた。腫瘍成長を1日おきに観測した。+16日目に、ペプチドワクチン群の5匹のマウスのうち1匹だけが腫瘍を発症し、コントロール群では5匹のマウスのうち4匹が腫瘍成長を示した。
ポリエピトープ抗原提示を裏付けるデータ
患者のタンパク質コード領域からの実証された突然変異は、RNAワクチンのGMP製造のための前駆物として使用されるポリネオエピトープワクチン鋳型のアセンブリの候補物をその中から選択することができるプールを構成する。ワクチン骨格として適切なベクターカセットは既に記述されている(Holtkamp,S.et al.,Blood,108:4009−4017,2006;Kreiter,S.et al.,Cancer Immunol.Immunother.,56:1577−1587,2007;Kreiter,S.et al.,J.Immunol.,180:309−318,2008)。好ましいベクターカセットは、コード領域および非翻訳領域(UTR)において修飾されており、長期間にわたってコードされるタンパク質の最大化された翻訳を確実にする(Holtkamp,S.et al.,Blood,108:4009−4017,2006;Kuhn,A.N.et al.,Gene Ther.,17:961−971,2010)。さらに、ベクター骨格は、細胞傷害性T細胞ならびにヘルパーT細胞の同時増殖のための抗原経路指定モジュールを含む(Kreiter,S.et al.,Cancer Immunol.Immunother.,56:1577−1587,2007;Kreiter,S.et al.,J.Immunol.,180:309−318,2008;Kreiter,S.etal.,Cancer Research,70(22),9031−9040,2010)(図5)。重要な点として、我々は、そのようなRNAワクチンが複数のMHCクラスIおよびクラスIIエピトープを同時に提示するために使用できることを実証した。
RNAポリネオエピトープ構築物についての科学的概念実証
RNAポリネオエピトープの概念は、リンカー配列によって連結された、突然変異ペプチドをコードする連続的に配置された配列から成る長いインビトロ転写mRNAに基づく(図6参照)。コード配列は非同義突然変異から選択され、常に、もとの配列状況からの30〜75塩基対の領域に隣接された突然変異アミノ酸についてのコドンで構築される。リンカー配列は、細胞の抗原プロセシング機構によって選択的にプロセシングされないアミノ酸をコードする。
ポリネオエピトープワクチンの設計−リンカーの関連性
ポリネオエピトープRNA構築物は、リンカーペプチド配列で連結された複数の体細胞突然変異コードペプチドが配置される骨格構築物を含む。コドン最適化ならびに骨格によるRNA安定性および翻訳効率の増大に加えて、RNAポリネオエピトープワクチンの1つの実施形態は、抗原ペプチドのMHCクラスIおよびII提示を増大させ、有害なエピトープの提示を減少させるように設計されたリンカーを含む。

RNAポリネオエピトープワクチン
RNAポリネオエピトープワクチン構築物は、T7プロモーター、タンデムβグロビン3'UTR配列および120bpのポリ(A)尾部を含むpST1−A120ベクターに基づいており、これらはRNAの安定性および翻訳効率を高め、それにより、コードされる抗原のT細胞刺激能力を増強することが示されている(Holtkamp S.et al.,Blood 2006;PMID:16940422)。加えて、MHCクラスIシグナルペプチドフラグメントならびにエピトープをクローニングするためのポリリンカー配列に隣接する終結コドン(MHCクラスI輸送シグナルまたはMITD)を含む膜貫通ドメインおよびサイトゾルドメインを挿入した(Kreiter S.et al.,J.Immunol.,180:309−318,2008)。後者は、抗原提示を増大させ、それにより抗原特異的CD8+およびCD4+T細胞の増殖を増強して、エフェクター機能を改善することが示されている。
臨床適用は以下の工程を含む:
・適格患者が次世代シークエンシングによるDNA分析に同意しなければならない。
・通常の診断手順から得られる腫瘍標本(パラフィン包埋し、ホルマリン固定した組織)および末梢血細胞を入手し、上述した突然変異分析のために使用する。
・発見された突然変異を確認する。
・優先順位付けに基づき、ワクチンを設計する。RNAワクチンのために、遺伝子合成およびクローニングによってマスタープラスミド鋳型を作製する。
・プラスミドを、臨床グレードのRNAの作製、RNAワクチンの品質管理および放出のために使用する。
・ワクチン薬剤製品を、臨床適用のためにそれぞれの治験施設に送付する。
・RNAワクチンは、製剤緩衝液中で裸のワクチンとしてまたは例えばリンパ節への直接注射、皮下、静脈内、筋肉内注射のためにナノ粒子もしくはリポソーム中に封入して使用することができる。あるいは、RNAワクチンは、例えば養子移入のための樹状細胞の、インビトロトランスフェクションに使用することができる。
腫瘍転移の同定および腫瘍ワクチン接種のためのその利用
我々は、B16F10マウスメラノーマ細胞株における突然変異発見のためにNGSエクソームリシークエンシングを適用し、962の非同義体細胞点突然変異を同定し、563を発現遺伝子中で同定した。潜在的なドライバー突然変異は、古典的腫瘍サプレッサー遺伝子(Pten、Trp53、Tp63、Pml)ならびに細胞増殖(例えばMdm1、Pdgfra)、細胞接着および移動(例えばFdz7、Fat1)またはアポトーシス(Casp9)を制御する癌原遺伝子シグナル伝達経路に関与する遺伝子において起こる。さらに、B16F10は、ヒトメラノーマにおいて頻繁に変化すると以前に記述されたAim1およびTrrapの突然変異も保有する。
C57BL/6マウス(Jackson Laboratories)をUniversity of Mainzにおいて動物研究に関する連邦および州政府の政策に従って飼育した。
B16F10メラノーマ細胞株を2010年にAmerican Type Culture Collectionから購入した(製品:ATCC CRL−6475、ロット番号:58078645)。初期(3代目、4代目)継代の細胞を腫瘍実験に使用した。通常はマイコプラスマ属(Mycoplasma)に関して細胞を試験した。受領時以降に細胞の再確認は実施しなかった。
核酸抽出および試料調製:バルクB16F10細胞からのDNAおよびRNAならびにC57BL/6尾部組織からのDNAを、Qiagen DNeasy Blood and Tissueキット(DNA用)およびQiagen RNeasy Microキット(RNA用)を使用して三つ組で抽出した。
選択:突然変異は、選択されるには以下の基準を満たさねばならなかった:(i)すべてのB16F10三つ組中に存在し、すべてのC57BL/6三つ組中に存在しない、(ii)FDR≦0.05、(iii)C57BL/6中で均一、(iv)RefSeq転写産物中で生じる、および(v)真の突然変異とスコアされる非同義変化を引き起こす。実証および免疫原性試験のための選択は、突然変異が発現された遺伝子であることを必要とした(レプリケート全体にわたって平均RPKM>10)。
オボアルブミンクラスI(OVA258−265)、クラスII(OVAクラスII330−338)、インフルエンザヌクレオタンパク質(Inf−NP366−374)、水疱性口内炎ウイルスヌクレオタンパク質(VSV−NP52−59)およびチロシナーゼ関連タンパク質2(Trp2180−188)を含むすべてのペプチドをJerini Peptide Technologies(Berlin,Germany)から購入した。合成ペプチドは27アミノ酸長で、14位に突然変異(MUT)または野生型(WT)アミノ酸を有していた。ポリイノシン酸:ポリシチジル酸(poly(I:C),InvivoGen)を皮下注射用アジュバントとして使用した。Inf−NP366−374ペプチドに特異的なMHC五量体をProImmune Ltd.から購入した。
年齢をマッチさせた雌性C57BL/6マウスに、PBS中に製剤化したペプチド100μgおよびpoly(I:C)50μg(総容量200μl)を側腹部に皮下注射した(各群につき5匹のマウス)。すべての群を0日目と7日目に2つの異なる突然変異コードペプチドで免疫し、一方の側腹部につき1つのペプチドとした。初回注射の12日後にマウスを犠死させ、免疫学的試験のために脾細胞を単離した。
酵素結合免疫スポット(ELISPOT)アッセイ(Kreiter S et al.,Cancer Res 2010;70:9031−40)および刺激因子としての同系骨髄由来樹状細胞(BMDC)の生成は以前に記述されている(Lutz MB et al.,J Immunol Methods 1999;223:77−92)。BMDCを、ペプチドでパルスするか(2μg/ml)、または指示される突然変異もしくはコントロールRNA(eGFP−RNA)をコードするインビトロ転写(IVT)RNAでトランスフェクトした。各々が25位に突然変異を有する50アミノ酸を含み、9アミノ酸のグリシン/セリンリンカーによって分離された2つの突然変異を示す配列をpST1−2BgUTR−A120骨格にクローニングした(Holtkamp S et al.,Blood 2006;108:4009−17)。この鋳型からのインビトロ転写および精製は以前に記述されている(Kreiter S et al.,Cancer Immunol Immunother 2007;56:1577−87)。アッセイのために、5×104ペプチドまたはRNA操作したBMDCを、抗IFN−γ抗体(10μg/mL、クローンAN18;Mabtech)で被覆したマイクロタイタープレート中で5×105の新鮮単離した脾細胞と共に同時インキュベートした。37℃で18時間後、サイトカイン分泌を抗IFN−γ抗体(クローンR4−6A2;Mabtech)で検出した。スポット数をカウントし、ImmunoSpot(登録商標)S5 Versa ELISPOT Analyzer、ImmunoCapture(登録商標)Image AcquisitionソフトウェアおよびImmunoSpot(登録商標)Analysisソフトウェアバージョン5で解析した。統計解析はスチューデントt検定およびマン・ホイットニー検定(ノンパラメトリック検定)によって行った。検定でp値<0.05が与えられるか、平均スポット数が>30スポット/5×105エフェクター細胞である場合、応答を有意とみなした。反応性は平均スポット数によって評価した(−:<30;+:>30;++:>50;+++>200スポット/ウェル)。
ELISPOTアッセイ用に調製した脾細胞のアリコートを、細胞内フローサイトメトリによるサイトカイン産生の分析に供した。このために各試料につき2 ×106脾細胞を、96ウェルプレート中でゴルジ阻害剤ブレフェルジンA(10μg/mL)を添加した培地(RPMI+10%FCS)に塗布した。各動物からの細胞を、2× 105のペプチドパルスしたBMDCで37℃にて5時間再刺激した。インキュベーション後、細胞をPBSで洗浄し、PBS 50μl中に再懸濁して、以下の抗マウス抗体:抗CD4 FITC、抗CD8 APC−Cy7(BD Pharmingen)で4℃にて20分間細胞外染色した。インキュベーション後、細胞をPBSで洗浄し、その後外膜の透過処理のためにCytofix/Cytoperm(BD Bioscience)溶液100μL中に4℃にて20分間再懸濁した。透過処理後、細胞をPerm/Wash−Buffer(BD Bioscience)で洗浄し、Perm/Wash−Buffer中に50μL/試料で再懸濁し、以下の抗マウス抗体:抗IFN−γ PE、抗TNF−α PE−Cy7、抗IL2 APC(BD Pharmingen)で4℃にて30分間細胞内染色した。Perm/Wash−Bufferで洗浄した後、フローサイトメトリ分析のために1%パラホルムアルデヒドを含有するPBS中に細胞を再懸濁した。BD FACSCanto(登録商標)II血球計算器およびFlowJo(バージョン7.6.3)を用いて試料を分析した。
腫瘍ワクチン接種実験のために、7.5×104 B16F10メラノーマ細胞をC57BL/6マウスの側腹部に皮下的に接種した。予防的設定で、突然変異特異的ペプチドでの免疫を腫瘍接種の4日前および腫瘍接種後2日目と9日目に実施した。治療的実験のために、ペプチドワクチンを腫瘍注射後3日目と10日目に投与した。腫瘍サイズを3日ごとに測定し、腫瘍径が15mmに達したときにマウスを犠死させた。
我々の目的は、B16F10マウスメラノーマにおける潜在的に免疫原性の体細胞点突然変異をNGSによって同定し、これらをマウスのペプチドワクチン接種によってインビボ免疫原性に関して試験し、誘発されるT細胞応答をELISPOTアッセイによって測定することであった(図9A)。我々は、C57BL/6野生型バックグラウンドゲノムおよびB16F10細胞のエクソームを、それぞれ三つ組で抽出し、捕獲して、配列決定した。各試料について、1億個を超えるシングルエンド50ヌクレオチドリードが生成された。これらのうち80%がマウスmm9ゲノムに特異的に整列し、49%が標的上に整列して、標的の濃縮が成功したことを明らかにし、三つ組試料の各々において標的ヌクレオチドの70%について20倍以上のカバレッジをもたらした。同じく三つ組でプロファイリングしたB16F10細胞のRNA−Seqは、平均3000万個のシングルエンドの50ヌクレオチドリードを生成し、このうち80%がマウストランスクリプトームに整列する。
注目すべき点として、突然変異遺伝子の多く(非同義体細胞点突然変異を含む962の遺伝子)が、これまでに癌表現型に関連付けられている。突然変異は、Pten、Trp53(p53とも呼ばれる)およびTp63を含む、確立された腫瘍サプレッサー遺伝子中で認められた。最も広く確立された腫瘍サプレッサーであるTrp53中で(Zilfou JT et al.,Cold Spring Harb Perspect Biol 2009;1:a001883)、タンパク質位置127おけるアスパラギンのアスパラギン酸への突然変異(p.N127D)は、DNA結合ドメイン内に局在し、SIFTによって機能を変化させると予測されている。Ptenは2つの突然変異(p.A39V、p.T131P)を含み、どちらもタンパク質機能に有害な影響を及ぼすと予測される。p.T131P突然変異は、ホスファターゼ活性を低下させることが示された突然変異(p.R130M)に隣接する(Dey N et al.,Cancer Res 2008;68:1862−71)。さらに、突然変異は、DNA修復経路に関連する遺伝子、例えばBrca2(乳癌2、若年性)、Atm(毛細管拡張性運動失調症の変異)、Ddb1(損傷特異的DNA結合タンパク質1)およびRad9b(RAD9ホモログB)中で認められた。さらに、突然変異は他の腫瘍関連遺伝子中でも起こり、これにはAim1(腫瘍サプレッサー「アブセントインメラノーマ1」)、Flt1(癌遺伝子Vegr1、fms関連チロシンキナーゼ1)、Pml(腫瘍サプレッサー「前骨髄球性白血病」)、Fat1(「FAT腫瘍サプレッサーホモログ1」)、Mdm1(TP53結合核タンパク質)、Mta3(転移関連1ファミリー、成員3)、およびAlk(未分化リンパ腫受容体チロシンキナーゼ)が含まれる。我々は、以前に腫瘍中で同定された、MAPK/ERK経路の細胞膜結合受容体チロシンキナーゼ、Pdgfra(血小板由来増殖因子受容体αポリペプチド)(Verhaak RG et al.,Cancer Cell 2010;17:98−110)中のp.S144Fにおける突然変異を認めた。突然変異は、Casp9(カスパーゼ9、アポトーシス関連システインペプチダーゼ)中のp.L222Vで起こる。CASP9は、ポリ(ADP−リボース)ポリメラーゼ(PARP)をタンパク質分解によって切断し、アポトーシスを調節し、いくつかの癌に関連付けられている(Hajra KM et al.,Apoptosis 2004;9:691−704)。我々が認めた突然変異は潜在的にPARPおよびアポトーシスシグナル伝達に影響を及ぼす。最も興味深い点として、Braf、c−Kit、KrasまたはNrasでは突然変異が認められなかった。しかし、Rassf7(RAS関連タンパク質)(p.S90R)、Ksr1(ras 1のキナーゼサプレッサー)(p.L301V)、およびAtm(PI3K経路)(p.K91T)中で突然変異が同定され、そのすべてがタンパク質機能に有意の影響を及ぼすと予測される。Trrap(形質転換/転写ドメイン関連タンパク質)は、今年になって新規潜在的メラノーマ標的としてヒトメラノーマ標本中で同定された(Wei X et al.,Nat Genet 2011;43:442−6)。B16F10では、Trrap突然変異はp.K2783Rで起こり、オーバーラップするホスファチジルイノシトールキナーゼ(PIK)関連キナーゼFATドメインを障害すると予測される。

これらの突然変異の免疫原性試験用の抗原を提供するため、我々は、免疫のために他のペプチドに比べて多くの利点を有する長いペプチドを使用した(Melief CJ and van der Burg SH,Nat Rev Cancer 2008;8:351−60)。長いペプチドは抗原特異的CD8+およびCD4+T細胞を誘導することができる(Zwaveling S et al.,Cancer Res 2002;62:6187−93;Bijker MS et al.,J Immunol 2007;179:5033−40)。さらに、長いペプチドは、MHC分子に提示されるためにはプロセシングを必要とする。そのような取込みは、強力なT細胞応答をプライミングするのに最適な、樹状細胞によって最も効率的に行われる。適合するペプチドは、これに対し、トリミングを必要とせず、非活性化BおよびT細胞を含む、MHC分子を発現するすべての細胞上に外因的に負荷され、免疫学的寛容とフラトリサイドの誘導をもたらす(Toes RE et al.,J Immunol 1996;156:3911−8;Su MW et al.,J Immunol 1993;151:658−67)。50の実証された突然変異の各々について、我々は、中心部に位置する突然変異または野生型アミノ酸を有する27アミノ酸長のペプチドを設計した。したがって、突然変異を担持する8〜14アミノ酸長の任意の潜在的MHCクラスIおよびクラスIIエピトープをこの前駆体ペプチドからプロセシングすることができた。ペプチドワクチン接種のためのアジュバントとして、交差提示を促進し、ワクチン効果を高めることが公知であるpoly(I:C)を使用した(Datta SK et al.,J Immunol 2003;170:4102−10;Schulz O et al.,Nature 2005;433:887−92)。50の突然変異をT細胞の誘導に関してマウスにおいてインビボで試験した。印象的な点として、50の突然変異をコードするペプチドの中から16が、免疫マウスにおいて免疫応答を誘発することが認められた。誘導されたT細胞は種々の反応性パターンを示した(表7)。
表7:突然変異をコードするペプチドでのワクチン接種に続いて測定されたT細胞反応性の要約。統計解析はスチューデントt検定およびマン・ホイットニー検定(ノンパラメトリック検定)によって行った。検定でp値<0.05が与えられるか、平均スポット数が>30スポット/5×105エフェクター細胞である場合、応答を有意とみなした。反応性は平均スポット数によって評価した。−:<30;+:>30;++:>50;+++>200スポット/ウェル)。
インビボで誘発される免疫応答が腫瘍担持マウスにおける抗腫瘍作用につながるかどうかを評価するため、我々はMUT30(Kif18b中の突然変異)およびMUT44を例として選択した。これらの突然変異は、突然変異ペプチドに対して選択的に強力な免疫反応を誘導し、内因的にプロセシングされることが示されていた(図10A、B)。突然変異ペプチドでワクチン接種することの治療的潜在能を、7.5×105 B16F10の移植の3日後および10日後にMUT30またはMUT44のいずれかとアジュバントでマウスを免疫することによって検討した。腫瘍の成長は、コントロール群と比較して両方のペプチドワクチン接種によって阻害された(図11A)。B16F10は非常に攻撃的に成長する腫瘍であるので、我々は防御的免疫応答も試験した。マウスをMUT30ペプチドで免疫し、4日後に7.5×105 B16F10細胞を皮下的に接種して、腫瘍攻撃誘発の2日後および9日後にMUT30で追加免疫した。MUT30で処置したマウスの完全な腫瘍防御および40%の生存が認められ、一方コントロール処置群のすべてのマウスは44日以内に死亡した(図11B、左)。MUT30での免疫にもかかわらず腫瘍を発症したマウスでは、腫瘍の成長がより緩やかであり、コントロール群と比較して6日間の平均生存期間の延長をもたらした(図11B、右)。これらのデータは、単一突然変異に対するワクチン接種が既に抗腫瘍作用を付与することができることを示唆する。
B16F10メラノーマ細胞株からの50の実証された突然変異を用いて種々のRNAワクチンを構築した。1つ(モノエピトープ)、2つ(バイエピトープ)または16(ポリエピトープ)の異なる突然変異を示すDNA配列を、25位に突然変異を有する50アミノ酸(バイエピトープ)または14位に突然変異を有する27アミノ酸(モノおよびポリエピトープ)を使用して構築し、9アミノ酸のグリシン/セリンリンカーによって分離した。これらの構築物をmRNAのインビトロ転写のためにpST1−2BgUTR−A120骨格にクローニングした(Holtkamp et al.,Blood 2006;108:4009−17)。

ポリエピトープRNAワクチン設計をさらに特性付けるため、1つのMHCクラスIIエピトープ(オボアルブミンクラスI(SIINFEKL)、クラスII(OVAクラスII)、インフルエンザヌクレオタンパク質(Inf−NP)、水疱性口内炎ウイルスヌクレオタンパク質(VSV−NP)およびチロシナーゼ関連タンパク質2(Trp2))を含む5つの異なる公知のモデルエピトープを含有するDNA配列を構築した。突然変異ポリエピトープのために使用した9アミノ酸の同じグリシン/セリンリンカーでエピトープを分離した。この構築物をmRNAのインビトロ転写のためにpST1−2BgUTR−A120骨格にクローニングした。
免疫原性に関して図13で分析した同じポリエピトープを使用して、B16F10腫瘍細胞に対する突然変異コードRNAの抗腫瘍活性を検討した。詳細には、C57BL/6マウスの群(n=10)に、1×105 B16F10メラノーマ細胞を側腹部に皮下的に接種した。3、6、10、17および21日目に、リポソームトランスフェクション試薬を用いてポリエピトープRNAでマウスを免疫した。コントロール群にはリポソームだけを注射した。
実証された突然変異の抗腫瘍活性を、MUT30をペプチドワクチンとして使用して治療的インビボ腫瘍実験によって評価した。詳細には、C57BL/6マウスの群(n=8)に、1×105 B16F10メラノーマ細胞を側腹部に皮下的に接種した。3、10および17日目に、polyI:Cをアジュバントとして使用してMUT30、チロシナーゼ関連タンパク質2(Trp2180−188)または両方のペプチドの組合せでマウスを免疫した。Trp2は、B16F10メラノーマ細胞によって発現される公知のCD8+エピトープである。
信頼度に基づく体細胞突然変異検出のためのフレームワークおよびB16F10メラノーマ細胞への適用
NGSは、ゲノム全体またはタンパク質コードエクソンなどの標的領域内で変異の高スループット発見を可能にするという点でバイアスがない。
ライブラリの捕獲およびシークエンシング
次世代シークエンシング、DNAシークエンシング:この場合はすべての公知のマウスエクソンを捕獲するように設計された、Agilent Sure−Select solutionに基づく捕獲アッセイ[Gnirke,A.,et al.,Nat.Biotechnol.27,182−189(2009)]を用いてDNAリシークエンシングのためのエクソーム捕獲を実施した。
配列リードを、bwa(バージョン0.5.8c)[Li,H.Durbin,R.,Bioinformatics 25,1754−1760(2009)]を用いて、デフォルトオプションを使用して参照マウスゲノムアセンブリmm9[Mouse Genome Sequencing Consortium,Nature 420,520−562(2002)]に整列させた。曖昧なリード−bwa出力によって提供されるようなゲノムの複数箇所にマッピングされるリードを除去した。残りのアラインメントを選別し、インデックス化して、バイナリ圧縮フォーマット(BAM)に変換し、シェルスクリプトを用いてリードクオリティスコアをIllumina標準phred+64から標準サンガークオリティスコアに変換した。
バーコード化されたmRNA−seqcDNAライブラリを、Illumina mRNA−seqプロトコルの修正版を用いて全RNA 5μgから調製した。SeramagOligo(dT)磁気ビーズ(Thermo Scientific)を使用してmRNAを単離した。単離したmRNAを、二価カチオンと熱を用いて断片化し、160〜200bpの範囲のフラグメントを生じさせた。断片化したmRNAを、ランダムプライマーとSuperScriptII(Invitrogen)を用いてcDNAに変換し、次いでDNAポリメラーゼIおよびRNaseHを使用して第二鎖を合成した。cDNAを、T4 DNAポリメラーゼ、クレノウDNAポリメラーゼを使用して末端修復し、T4ポリヌクレオチドキナーゼを使用して5'リン酸化した。平滑末端cDNAフラグメントを、クレノウフラグメントを使用して3'アデニル化した(3'−5'エキソマイナス)。単一の3'TオーバーハングIlluminaマルチプレックス特異的アダプタを、T4 DNAリガーゼを使用してcDNAフラグメントに連結した。cDNAライブラリを精製し、E−Gel 2% SizeSelectゲル(Invitrogen)を用いて300bpでサイズ選択した。濃縮、Illumina6塩基インデックス配列およびフローセル特異的配列の付加を、Phusion DNAポリメラーゼ(Finnzymes)を使用してPCRによって行った。すべての清浄化は1.8倍容量のAgencourt AMPure XP磁気ビーズを使用して実施した。
我々は、サンガーリシークエンシングおよびRNAによる検証のためにSNVを選択した。3つすべてのプログラムによって予測され、非同義であり、最低限10RPKMを有する転写産物中で見出されるSNVを同定した。これらのうちで、プログラムによって提供される最も高いSNPクオリティスコアを有する50を選択した。陰性コントロールとして、50%以上のFDRを有し、1つの細胞株試料中だけに存在し、1つの突然変異コーリングプログラムによってのみ予測される44のSNVを選択した。DNAを使用して、選択した変異を、DNA 50ngを使用した領域のPCR増幅、次いでサンガーシークエンシング(Eurofins MWG Operon,Ebersberg,Germany)によって検証した。反応は、陽性および陰性コントロールの50遺伝子座と32遺伝子座についてそれぞれ成功であった。腫瘍RNA−Seqリードの検査によっても検証を行った。
ランダムフォレストクオリティスコア計算:一般的に使用される突然変異コーリングアルゴリズム(DePristo,M.A.et al.,Nature Genetics 43,491−498(2011),Li,H.et al.,Bioinformatics 25,2078−2079(2009),Ding,L.et al.,Hum.Mol.Genet(2010)first published online September 15,2010)は複数のスコアを出力し、それらすべてが潜在的に突然変異コールのクオリティに影響を及ぼす。これらには、装置によって割り当てられる関心対象のベースクオリティ、この位置についてのクオリティアラインメント、この位置をカバーするリードの数またはこの位置で比較した2つのゲノム間の相違についてのスコアが含まれるが、これらに限定されない。偽発見率の計算のためには、突然変異の順位付けを必要とするが、様々なクオリティスコアから矛盾する情報が得られ得るので、これをすべての突然変異について直接実行できるわけではない。
受信者操作特性(ROC)曲線および対応する曲線下面積(AUC)は、分類器を構成し、それらのパフォーマンスを視覚化するために有用である[Fawcett,T.,Pattern Recogn.Lett.27,861−874(2006)]。我々は、実験および計算手順のパフォーマンスを評価するためにこの概念を拡大適用する。しかし、ROCグラフをプロットするには、通常は与えられず、高スループットデータ(NGSデータなど)に関しては確立することが困難な情報である、データセット中のすべての真陽性および偽陽性(TPおよびFP)例の知識を必要とする。したがって、我々は、それぞれのTPおよびFP率を推定するために計算したFDRを使用し、ROCグラフをプロットして、AUCを計算する。中心となる概念は、データセット中の単一突然変異のFDRが、この突然変異がTP/FP突然変異の合計にそれぞれどの程度寄与するかの割合を与えることである。また、TPおよびFPへのランダムな割り当てのリストに関して、生じるROC AUCは、我々の方法では0.5に等しく、完全にランダムな予測を示す。
癌治療のための標的としての腫瘍抗原の組合せの選択
この実施例では、大きな割合の腫瘍患者によって少なくとも部分的に共有され、その中で広いスペクトルの癌患者に適用できるワクチン生成物のセットを提供することができる腫瘍抗原のセットを確立することが可能であるかどうかを評価した。
Claims (8)
- 患者においてメラノーマを予防するまたは処置するための癌ワクチンの組み合せ物であって、
当該癌ワクチンの組み合せ物は、
(i)前記患者において1つ以上の共通腫瘍抗原に対する第一免疫応答を誘導するためのワクチンであって、前記第一免疫応答が、前記患者の癌細胞中に存在する癌特異的体細胞突然変異に特異的ではなく、CD8+T細胞応答を含み、前記第一免疫応答を誘導するためのワクチンは、共通腫瘍抗原の組み合わせもしくは共通腫瘍抗原の組み合わせの1つ以上のT細胞エピトープを含むペプチドもしくはポリペプチド、または前記ペプチドもしくは前記ポリペプチドをコードする核酸を含み、前記組み合わせは、DCT(アイソフォーム1)と、TYRと、TPTEとを含む、第一免疫応答を誘導するためのワクチン、および
(ii)前記患者において1つ以上の腫瘍抗原に対する第二免疫応答を誘導するためのワクチンであって、前記第二免疫応答が、前記患者の癌細胞のエクソーム中に存在する非同義癌特異的体細胞突然変異に特異的であり、CD4+細胞応答を含み、前記第二免疫応答を誘導するためのワクチンは、5つ以上の突然変異に基づくネオエピトープを含むポリペプチドまたは前記ポリペプチドをコードする核酸を含む、第二免疫応答を誘導するためのワクチン、
を含む、癌ワクチンの組み合せ物。 - 前記患者が前記1つ以上の共通腫瘍抗原について陽性である、請求項1に記載の使用のための癌ワクチンの組み合せ物。
- 前記突然変異に基づくネオエピトープを含むポリペプチドが、30までの突然変異に基づくネオエピトープを含む、請求項1または2に記載の使用のための癌ワクチンの組み合せ物。
- 前記ポリペプチドが、癌細胞によって発現される癌特異的体細胞突然変異を含まないエピトープをさらに含む、請求項3に記載の使用のための癌ワクチンの組み合せ物。
- 前記エピトープが、ワクチン配列を形成するように、天然に存在するタンパク質でも前記エピトープに隣接するアミノ酸配列に隣接し、前記ワクチン配列が30アミノ酸長である、請求項3または4に記載の使用のための癌ワクチンの組み合せ物。
- 前記ネオエピトープ、エピトープおよび/またはワクチン配列が、頭−尾方向に並んでいるおよび/またはリンカーによって分離されている、請求項3から5のいずれか一項に記載の使用のための癌ワクチンの組み合せ物。
- 前記第一免疫応答を誘導するためのワクチンおよび/または前記第二免疫応答を誘導するためのワクチンが、RNAワクチンである、請求項1から6のいずれか一項に記載の使用のための癌ワクチンの組み合せ物。
- 前記腫瘍抗原が腫瘍関連抗原である、請求項1から7のいずれか一項に記載の使用のための癌ワクチンの組み合せ物。
