本発明は、デングウイルス、特に血清型1、に対するヒト中和モノクローナル抗体に関する。本発明はさらに、脊椎動物対象におけるデングウイルス感染を治療または予防するための組成物および方法に関する。感染からの再発を抑制する、排除する、または防止するのに有効な量で脊椎動物対象に抗体を投与するための方法が提供される。
デング熱は、ヒトに影響を及ぼす最も重大な蚊媒介性のウイルス疾患である。現在、熱帯/亜熱帯ベルトの100を超えるデング熱流行国に住んでいる25億人近い人々は、デング感染のリスクがあると考えられている。都会に生息する蚊種であるネッタイシマカ(Aedes aegypti)がヒトへのウイルスの主要な伝播体である。デングウイルスによる感染は、無症状の感染から、急性の熱病であるデング熱(DF)を経て、血管漏出に代表される重篤な、生命を脅かす合併症であるデング出血熱(DHF)とデングショック症候群(DSS)に至るまでの、広範な臨床症状をもたらす。現在の治療は症状を緩和するための鎮痛薬の使用に限られており、利用可能なワクチンは存在しない。デング熱病は、頻繁かつ再発性の流行病で、毎年5000万人が罹患している。1990年代には、厳格な蚊の防除にもかかわらず、世界のさまざまな地域でデング熱病の再来が見られ、シンガポールでは2005年に過去最大の発生でピークに達した。報告されたケースの80%以上は、作業能力への、さらには治療のための多大な医療費への関連影響がある若者であった。それゆえに、デング関連疾患を短期的に抑えるのに役立つように、デングワクチンに代わるもの、例えば、受動的抗体療法および/または抗ウイルス薬などが緊急に必要とされている。提案されたこうした治療薬は、重症型の疾患(全体の約10%)を発症するリスクのある個体に適用しただけでも、多数の感染者を助ける可能性がある。米国南部やオーストラリアなどの先進国でのデング罹患率の増加、ならびにワクチンの不足のため、そうした抗体は有効な薬物治療を提供するだろう。本発明は、これらおよびその他のニーズを満たすために、完全にヒトのモノクローナル抗体を提供する。

本発明は、全体として、脊椎動物対象におけるデングウイルス感染を予防または治療するための組成物および方法に関する。特に、本発明者らは、国立大学病院(NUH)の感染症部門に入院したデング感染者からCD22+ B細胞を単離した。こうしたB細胞はインビトロでEBVを用いてポリクローナル細胞株として不死化した。ポリクローナルB細胞活性化剤(CpG配列)を、ヒトB細胞増殖因子のインターロイキン2およびインターロイキン4(それぞれ1000U/ml)と共に、B細胞不死化の効率を高めるために添加した。ヒトB細胞株は96ウェルの丸底ウェル内で作製した。2週間後、これらのクローンからの上清は、デングウイルスに対する結合/中和活性を分析するために、酵素結合免疫吸着アッセイ(ELISA)、プラーク減少中和試験(PRNT)および細胞変性効果アッセイ(CPE)でスクリーニングした。陽性抗体を産生するB細胞株は、抗体重鎖および軽鎖の遺伝子増幅のためのmRNA源として使用した。抗体の重鎖および軽鎖配列はインハウスpCMVベクターにクローニングして、高濃度の組換え抗体を産生するFreestyle(登録商標)293F細胞にトランスフェクトした。この方法を用いて、本発明者らは、極めてデング血清型1に特異的でありかつ種々のデング血清型1遺伝子型に対して広範な特異性を有する組換え抗体をクローニングして発現させた。この抗体はフラビウイルス属の他のウイルスに結合することがなく、そのため、他のフラビウイルスへのマクロファージの感染増強を、デング血清型1に予想されるものを超えて、示すことはほとんどないか、まったくない。インビボでの実験は、デング感染のマウスモデルにおいて顕著な予防および治療効果を示した。したがって、この抗体は、利用可能な最も優れた、現存するデング1感染のための候補治療薬に相当する。

本明細書中で用いる用語「B細胞」、「Bメモリー細胞」、「Bリンパ球」、「Bメモリーリンパ球」、「メモリー細胞」、「メモリーB細胞」、およびこれらの異形は交換可能に使用され、液性免疫応答のB細胞を指す。当技術分野で理解されるように、B細胞は、(T細胞によって支配される細胞性免疫応答とは対照的に)液性免疫応答の役割を果たすリンパ球である。B細胞の少なくとも1つの機能は、抗原に対する抗体を産生し、抗原提示細胞(APC)の役割を果たし、最終的に、抗原相互作用による活性化後にメモリーB細胞へと発達することである。B細胞は適応免疫系の一構成要素である。
特定のアイソタイプを有する抗体を発現する細胞の、選択されて刺激された集団は、ウイルス不死化剤を用いて不死化することが可能である。さまざまな不死化剤が、不死化された抗体分泌細胞を得るために、抗体分泌細胞に対して使用され得る。
いくつかの態様において、形質転換されたおよび/または活性化されたB細胞は、所望の抗原特異性を有するものについてスクリーニングすることができ、その後、個々のB細胞クローンを陽性細胞から生産することができる。スクリーニング段階は、ELISA、組織もしくは細胞(トランスフェクト細胞を含む)の染色、中和アッセイ、および/または所望の抗原特異性を同定するための当技術分野で知られた多くの他の方法のいずれかによって、実施することができる。アッセイは単純な抗原認識に基づいて選択するか、または所望の機能(例えば、単に抗原結合抗体ではなく中和抗体)にさらに基づいて選択することができる。
本開示の方法はまた、選択されたB細胞クローンからの抗体のための核酸の取得および/または配列決定、ならびに関心対象の抗体を発現することができる宿主細胞を作製するための該核酸の利用を提供する。
本明細書に開示される主題は、本開示に従って産生される抗体を含む薬学的組成物を提供する。いくつかの態様では、形質転換されたおよび/または活性化されたB細胞を含む薬学的組成物が提供される。いくつかの態様では、薬学的組成物は本明細書に開示される方法を用いて産生された1つまたは複数のモノクローナル抗体を含むことができる。いくつかの態様では、本明細書に開示される主題のモノクローナル抗体と形質転換および/または活性化B細胞の両方を薬学的組成物中に含めることができる。いくつかの態様では、本開示に従って産生されたモノクローナル抗体のパネルを薬学的組成物中に含めることができる。いくつかの態様では、本開示に従って産生されたモノクローナル抗体および/またはB細胞は、1つまたは複数の追加の薬剤、例えば抗ウイルス薬または鎮痛薬、と一緒に含めることができる。
本発明の薬学的組成物は、投与方法に応じて、さまざまな単位剤形で投与することができる。典型的な抗体の薬学的組成物の投与量は当業者によく知られている。そのような投与量は一般的に勧告されるものであり、そして特定の治療状況または患者の耐容性に応じて調整される。それを達成するのに十分な抗体の量は「治療有効用量」として規定される。投与計画およびその使用に有効な量、すなわち「投薬レジメン」は、疾患または症状の病期、疾患または症状の重症度、患者の一般的な健康状態、患者の身体状況、年齢、活性薬剤の製剤処方および濃度などを含めて、さまざまな要因に左右されるだろう。患者に対する投薬レジメンを計算する際には、投与方法もまた考慮される。投薬レジメンはまた、薬物動態、すなわち、薬学的組成物の吸収率、生物学的利用能、代謝、クリアランスなどを考慮に入れる必要がある。例えば、最新のRemington's; Egleton, Peptides 18: 1431-1439, 1997; Langer, Science 249: 1527-1533, 1990を参照されたい。
本発明は、例えば、上述した治療用途のために使用することができる、本開示に従って産生された抗体を含むキットを提供する。その製品はラベルの付いた容器を含む。適切な容器としては、例えば、ボトル、バイアル、および試験管が挙げられる。容器はガラスまたはプラスチックなどの各種材料から形成することができる。容器は、上述したような治療用途に有効である活性薬剤を含む組成物を保持する。組成物中の活性薬剤は抗体を含むことができる。容器上のラベルは、その組成物が特定の治療または非治療用途に使用されることを示し、また上述したような、インビボまたはインビトロでの使用の指示を示すことができる。
倫理上の声明
インフォームド・コンセントを得て、すべての手順を国立大学治験審査委員会から承認されたプロトコルの下で行った(NUS-IRB番号は06-196である)。
C6/36細胞およびBHK-21細胞は、以前に記載されたように培養した(28)。EHIおよびPVP 159株を除くすべてのデング株は、シンガポールのノバルティス熱帯病研究所(NITD)から得た。EHI株はシンガポールの環境衛生研究所(EHI)から入手し、PVP 159 (DENV1/SG/07K3640DK1/2008)はEDEN患者コホートから入手した(29)。
B細胞の単離および不死化は、以前に記載されたように実施した(10)。15日間の培養後、上清をELISAおよびPRNTでDENV特異的抗体についてスクリーニングした。
96ウェル平底プレート(Maxisorpプレート、Nunc社)にマウス4G2抗体を5μg/mlで一晩コーティングした。プレートをPBS/Tween-20 0.01%で3回洗浄した。異なるDENV株をウェルあたり50μl中1×105pfuで添加し、さらに2時間インキュベートした。プレートをPBS/Tween-20 0.01%で3回洗浄した。HM14C10をプレートに加えて、さらに1時間インキュベートした。プレートをPBS/Tween-20 0.01%で3回洗浄した。抗ヒトIgG結合HRP(Pierce社、シンガポール)を添加して1時間インキュベートした。TMB基質(GE healthcare社、シンガポール)を添加し、0.1M硫酸を用いて反応を停止させた。
RNA抽出キット(Qiagen社)を用いて、B細胞からRNAを抽出した。組換え抗体のクローニングおよび発現は、以前に記載されたとおりに行った(30)。
デングウイルス(5×102pfu/ml)を培地、個々のモノクローナル抗体(HM4G2、HM14c10またはHM14c10 N297Q)またはHM14c10モノクローナル抗体のサブクラス(IgG1、IgG2、IgG3またはIgG4)と一緒にプレインキュベートし、次に105個のK562細胞に添加した。1時間後、細胞をPBSで十分に洗浄して、未結合のウイルスとモノクローナル抗体を除去した。さらに48時間後、上清を回収して、ウイルス力価をBHK-21細胞でのプラークアッセイにより測定した。
AG129マウスはIFN-α/βおよび-γ受容体が欠損している(31)。マウスは動物実験委員会(Institutional Animal Care and Use Committee)の勧告に従って取り扱った(IACUCプロトコル番号:018/11)。HM14c10対PBS処理対照の予防的および治療的適用を詳述した略図を図21に提供する。マウスを犠牲にして、確立されたプラークアッセイによりウイルス血症を定量した(32)。
すべてのタイムラプス(time lapse)生細胞顕微鏡法は、Plan-Apochromat 100X 1.4開口数(N.A.)レンズを用いて倒立A1Rsi共焦点顕微鏡(ニコン、日本)で行った。生細胞イメージングは、チャンバーホルダー(ニコン、日本)に取り付けた25mmガラス製カバースリップ(Marienfeld GmbH、ドイツ)上で成育させた未固定の生存BHK細胞を用いて行った。実験の1日前に細胞を4×104個/ウェルの密度で播種し、10%FCSを添加したRPMI 1640中で培養した。Alexa Fluor-488標識抗体とAlexa Fluor-647標識DEN1ウイルスの同時検出のために、488nmラインのアルゴンイオンレーザーと633nmのヘリウムネオンレーザーの光をHFT UV/488/633ビームスプリッターに向けて、Alexa Fluor-488検出用の505-530バンドパスフィルターとAlexa Fluor-647検出用の650ロングパスフィルターを組み合わせたNFT 545ビームスプリッターを用いて蛍光を検出した。画像は30秒間隔で1フレーム毎秒(fps)にて30〜60分間撮影した。すべての生細胞イメージング実験は、5%CO2顕微鏡用ケージインキュベーターシステム(OkoLab社、イタリア)内37℃でインキュベートした細胞を用いて行った。画像は、ニコン画像統合ソフトウェア(NIS)エレメンツCソフトウェア(64ビット、バージョン3、SP7/build 547)[ニコン、日本]で解析し、処理した。
DENV1のエンドサイトーシスに対する抗体の効果は、生存細胞内の蛍光の相対レベルを測定することによって評価した。個別の抗体で処理した後、少なくとも100個の細胞の画像を、3つの独立した実験からA1Rsi共焦点顕微鏡を用いてランダムに取得した。その後、細胞の細胞内領域を、NIS Elementsソフトウェア(ニコン、日本)の「関心領域」[region of interest: ROI]機能を用いて個別に手動で画定し、そして各細胞内のAlexa Fluor-488の相対蛍光レベルを該ソフトウェアのROI統計機能を用いて測定した。平均値、標準偏差およびスチューデントのt検定は、Microsoft Excelを用いて各細胞集団について計算した。未処理のDEN1感染細胞集団からの蛍光を100%に正規化して、抗体で処理した感染細胞との比較に使用した。
デングウイルス(PVP 159株)を以前に記載されたとおりに調製した(3)。ウイルスをFab HM14c10と1:1のモル比で混合し、37℃で30分、次に4℃で2時間インキュベートした。その後、複合体を、連続したカーボンの薄層でコーティングされたレーシーカーボングリッド上で液体エタンにて急速凍結した。ウイルス粒子は300kV FEI Titan Kriosを用いて以下の条件で撮影した:電子線量16e-/Å2、倍率47,000、デフォーカス範囲1μm〜3μm。画像は1ピクセルあたり1.9Åのピクセルサイズをもたらす4K×4K Gatan CCDカメラにより記録した。合計5,566個の粒子がボックス化され、EMAN(33)プログラムスイートで、それぞれプログラムboxerおよびctfitを用いることにより、コントラスト伝達関数パラメータを測定した。粒子の配向は、マルチパス・シミュレーテッドアニーリング(multi-path simulated annealing: MPSA)プロトコル(34)を用いて測定した。西ナイルウイルスを初期モデルとして使用した(26)。EMANでプログラムmake3dを用いることによって三次元マップを作成した。最終的なマップの分解能は、0.5のフーリエシェル(fourier shell)係数カットオフで測定して、7Åの分解能であることが分かった。DENV1融合後Eタンパク質の結晶構造(18)は、剛体としてcryoEM密度マップにうまくフィットせず、したがってEタンパク質のドメイン類を分割してから個別にフィットさせた。cryoEMマップ(4σ等高線レベルで設定)への分子のフィットはその後、Chimera(35)の「フィットインマップ」(fit-in-map)機能を用いて最適化した。HM14c10可変領域のホモロジーモデルを作成するために、最高の配列一致を有する構造が選択され(PDBコード2GHW)、そしてホモロジーモデルがModeller(19)を用いて作成された。ホモロジーモデルの重鎖および軽鎖は、Fabの2つの可能な配向でcryoEMマップ(3σ等高線レベルで設定)に別々にフィットさせた(図19)。
DENV1に対する血清型特異的な結合および中和活性を有する抗体を分泌するBリンパ球細胞株の一群を同定し、サブクローニングし、そして増殖させた。これらの細胞株のうちの1つ、BCL-14c10は、他のものより著しく強い結合および中和活性を有するIgGを産生した(図14A)。この細胞株は、組換えヒトIgG1のPCR増幅および発現のための免疫グロブリン遺伝子テンプレートの供給源として使用した(図14B(a))。1つの組換えヒト抗体(HM)14c10は、親BCL-14c10と同等のDENV1への結合活性をもっていた(図14B(b))。HM14c10は、DENV1に結合して中和したが、DENV2、3または4には結合せず(図14C)、インビトロにおいて0.328μg/mlのPRNT50で強い中和活性を示した(図11A)。
所与の抗体とDENV間の相互作用の正確な性質は、中和を説明するための鍵を握っているに違いない。それを究明するために、Fab HM14c10:DENV1複合体の低温電子顕微鏡(cryoEM)構造を7Åの分解能で解いた(図12A)。完全占有では、120コピーのFab HM14c10がウイルス表面上のEタンパク質の利用可能な180コピーのすべてに結合する。Eタンパク質上のHM14c10のフットプリントを同定するために、DENV1 Eタンパク質の結晶構造(18)をcryoEM密度マップにフィットさせた(図16および表1)。7Å分解能のcryoEMマップは、HM14c10 FabとEタンパク質の間の明確な密度接続を示して、相互作用界面でのEタンパク質残基の同定を可能にした(図12Bおよび図17)。HM14c10によって認識されるエピトープはウイルスの四次構造に依存する。HM14c10の2つのFabはウイルス非対称単位内の3つのEタンパク質に結合する(図12CおよびD)。各抗体は、2つの隣接するEタンパク質にまたがって、E-DIII上のエピトープの半分と、隣接Eタンパク質のE-DIおよびE-DI-E-DIIヒンジ上のあとの半分に結合する。

抗体は、エンドソーム膜へのウイルスの付着もしくは融合の阻害、またはウイルスによって誘起される表面糖タンパク質のコンフォメーション変化のブロックを介してなど、多様なメカニズムによってウイルス感染を中和することができる(20, 21)。DENV1のHM14c10中和のメカニズムを理解するために、タイムラプス共焦点顕微鏡法を用いて、蛍光タグ付けした生存DENVによる細胞の感染を追跡した(22)(図13および21A)。BHK細胞をDENV1およびアイソタイプ対照Mab(非DENV結合性)と一緒にインキュベートしたとき、ウイルスは、複数の、主に核周囲の、細胞内コンパートメント内で合体した(図14A(a))。中和濃度のHM4G2は細胞外空間でのウイルス凝集体の形成を誘導したが、これらもまた、うまく内在化され、HM4G2がウイルスの付着/内在化を阻害しないことが確認された(図13A(b))。対照的に、HM14c10はより小さい凝集体の形成を誘導したが、効率的にウイルスの付着をブロックし、小さいウイルス粒子のほとんどは1時間後に細胞外空間に残存していた(図13A(c))。HM4G2は、アイソタイプ対照に比べて、細胞内ウイルスの蓄積を遅らせた(図13B、上段および中段のパネル)。HM14c10:DENV1複合体は細胞に入ることができなかったが、その表面から外れた状態で見ることができた(図13B、下段のパネル)。3つすべての条件下で内在化された蛍光DENV1の程度が定量化された(図13C)。これらのデータから、HM14c10によるDENV1の阻害の主な様式は、宿主細胞へのウイルス付着の妨害によるものであることが示唆される。
DENVは、免疫応答性げっ歯類の天然の病原体ではないが、タイプI/II IFNの受容体が欠損しているAG129マウスにおいて用量依存性のウイルス血症を誘発することが可能である。本発明者らは、これらのマウスに未変性DENV1を皮下(モデルI、図21B(a))または腹腔内(モデルII、図21B(b))に注射してから、それぞれ3〜4日後にウイルス血症を定量化した(20)。異なる遺伝子型(EHI-D1遺伝子型I対Westpac遺伝子型IV)を表す2つのDENV1臨床分離株を利用して、HM14c10のインビボ有効性を判定した。両方のモデルにおいて、HM14c10は、DENV1感染の24時間前にマウスに与えたとき、または感染の48時間後に与えたとき、疾患を防いだ(図13D)。ウイルス血症の顕著な減少が観察されたHM14c10の最低濃度はマウスあたり0.6μg(または160pM)であり、これは報告された他の抗DENV治療製剤のいずれによっても達成されていないインビボ効力に相当する。
DENV感染で生じる体液性応答に関する最近のレポート(23, 24)では、主にDENV血清型交差反応性であって弱い中和活性を有する抗体が支配的であることを示唆している。ヒト血清レパートリーでは稀であるが、E-DIII抗体はDENV感染を防ぐことが提案されており(23, 24)、これはDENVへのマウス抗体応答に関する研究と一致している(7)。特性評価されたヒト抗体は、基本的にはウイルスEタンパク質のDIおよびDIIに特異的であった。特性評価された抗体の少数は、ウイルス全体に結合するものの、組換えEタンパク質には結合しないことが観察され、これは四次構造依存性エピトープに対する特異性を示唆している(23)。この研究において、本発明者らは、DENV血清型1に対する強力な中和抗体を単離して、徹底的に特性評価した。この抗体はインビトロ系とインビボ系の両方において高度に中和する。それは、DENV1のみに結合するので、他のDENV血清型による骨髄単核細胞の感染増強を引き起こすことがない。
Claims (22)
- デングウイルス血清型1のエンベロープ(E)タンパク質またはその断片に結合する、単離された抗体またはその抗原結合断片であって、
該抗体が、ウイルスの2つのEタンパク質にまたがって結合する中和活性をもつヒト抗体であり、該2つのEタンパク質にまたがって結合することが、一方のEタンパク質のDIおよびDIとDIIの間のヒンジに、そして隣接Eタンパク質のDIIIに、結合することを含み、
該抗体またはその抗原結合断片が、DIにおける配列番号33のアミノ酸T51、G159、T160、T165、P166、Q167、E172、I173、およびT275、DIIにおける配列番号33のアミノ酸N52およびG274、ならびにDIIIにおける配列番号33のK310に結合し、
該抗体またはその抗原結合断片が、(a)全免疫グロブリン分子、(b) scFv、(c) Fab断片、(d) F(ab')2、および(e)ジスルフィド結合Fvからなる群より選択され、
該抗体またはその抗原結合断片が、CDR1配列SYGMH、CDR2配列VIWYDGSKTYYGDSVKG、およびCDR3配列GIAGGWAFWを含む重鎖可変領域、およびCDR1配列RASQNVYSYLG、CDR2配列GVTSRAT、およびCDR3配列QQYAGを含む軽鎖可変領域を含む、

- 1×10-8M未満の親和定数(KD)で抗原に結合する、請求項1記載の抗体またはその抗原結合断片。
- 1×10-9M未満の親和定数(KD)で抗原に結合する、請求項1記載の抗体またはその抗原結合断片。
- 前記抗体が、デングウイルス感染から回復した患者のB細胞に由来する、請求項1記載の抗体またはその抗原結合断片。
- 対象におけるデングウイルス感染を軽減または予防するのに有効な、請求項1〜7のいずれか一項記載の抗体またはその抗原結合断片および薬学的に許容される担体を含む薬学的組成物。
- 抗ウイルス薬または鎮痛薬をさらに含む、請求項8記載の薬学的組成物。
- 受動免疫のための薬剤の製造における請求項1〜7のいずれか一項記載の抗体またはその抗原結合断片の使用。
- デングウイルス感染の治療のための薬剤の製造における、該疾患を軽減または予防する請求項1〜7のいずれか一項記載の抗体またはその抗原結合断片の使用。
