Description
[0060]ブタを使い動物を背殿位にして3通りの急性処置を遂行した。切開を剣状突起付近に作り、型番4194リードを胸骨下/胸骨後方空間へ6996Tトンネリング用具及びシースを使用して送達した。能動的缶型エミュレータ(ACE:active can emulator)を右胸(第1の急性実験)又は左中腋窩(第2及び第3の急性実験)の何れかの皮下嚢部に設置した。様々なペーシング構成を試し、異なった機種を刺激供給源として使用した。ペーシングパルス送達には複数のパルス幅を使用した。それら実験に亘って、幾通りかの異なる胸骨下/胸骨後方リード電極場所を利用した。
[0063]第1の急性研究では、メドトロニック・アテイン(MEDTRONIC ATTAIN)双極OTW4194リードを胸骨下/胸骨後方に植え込み、2つの能動缶型エミュレータを右背外側領域(ACE1)に1つと右中腋窩(ACE2)に1つ位置決めした。4194リードは、リード先端及びリード本体を胸骨の長さに平行に走らせた状態で、胸骨の下側、縦隔に直接植え込んだ。様々なペーシング構成を試し、電気的データを収集した。
[0068]第2の急性実験を行った。但し、第2の急性では、動物は心膜が胸骨へ癒着した状態を呈した。心膜癒着に因り心陰影の心室面が胸骨から離れて回転―解剖学的差異がこの実験全体を通してより高い閾値を生じさせたのかもしれない。
[0072]胸骨下/胸骨後方ペーシングの実現可能性を実証する第3の最終的な急性実験を行なった。先の2つの急性実験と同じく4194リードを胸骨の下に設置した。能動缶型エミュレータを左中腋窩へ設置した。この実験では胸骨下/胸骨後方4194リードを意図的にリード先端が当初は心室の心陰影のはるか上方、第2肋骨付近に来るように位置決めした。次いでリード先端を(剣状突起に向かって)一度に肋骨空間1つ分ずつ引き戻し、各位置で電気的データを収集していった。先の実験と同じく、低い捕捉閾値は、ペーシング電極を蛍光透視法により観測して心陰影の心室面のほぼ上に位置決めしたときに取得された。リード先端が心陰影の心室面の上になっていないときは多くの場合「無捕捉」となった。

Claims (4)
- 患者に皮下的に植え込まれるように構成されている植え込み式カルディオバータ除細動器(ICD)と、
植え込み式医用電気リードであって、
近位端及び遠位部分を有する細長リード本体、
前記ICDへ連結するように構成されている前記リード本体の前記近位端のコネクタ、及び、
前記細長リード本体の前記遠位部分に沿った1つ又はそれ以上の電極、を含んでいる植え込み式医用電気リードと、を備え、前記植え込み式医用電気リードは、前記1つ又はそれ以上の電極が患者の胸骨下の位置になるように移植できるようになっている、植え込み式カルディオバータ除細動器(ICD)システムにおいて、
前記ICDは、前記植え込み式医用電気リードを介して、前記患者の胸骨下場所にて前記1つ又はそれ以上の電極により感知される電気信号を取得し、及び電気刺激を前記患者の前記胸骨下場所から心臓へ前記1つ又はそれ以上の電極を使用して送達し、
前記胸骨下場所は実質的に前縦隔内である、ICDシステム。 - 前記ICDは35ジュール(J)未満のエネルギーを有するカルディオバージョンショックと除細動ショックのうちの前記一方を送達するように構成されている、請求項1に記載のICDシステム。
- 前記1つ又はそれ以上の電極は少なくとも1つのペーシング電極を含んでおり、前記ICDは徐脈ペーシングと抗頻拍(ATP)ペーシングのうちの一方を前記患者の前記胸骨下場所から前記心臓へ前記少なくとも1つのペーシング電極を介して提供するように構成されている、請求項1及び請求項2の何れか一項に記載のICDシステム。
- 前記ICDは2ミリ秒より大きいパルス幅及び大凡1ボルトから20ボルトの間のパルス振幅を有するペーシングパルスを送達するように構成されている、請求項1から請求項3の何れか一項に記載のICDシステム。
