PTA−イムノアッセイ
抗PTA抗体の生成
免疫化のためのペプチド/コンジュゲート:
免疫化のためのペプチドを、ウシ血清アルブミン(BSA)へのペプチドのコンジュゲートのために、追加のN末端システイン残基を用いて合成した(JPT Technologies,Berlin,Germany)。そのペプチドを、Sulfo−SMCC(Perbio−science,Bonn,Germany)を使用することによってBSAに共有結合で連結した。カップリング手順を、Perbioのマニュアルに従って実施した。
BALB/cマウスを、0及び14日目に100μgのペプチド−BSAコンジュゲート(100μlの完全フロイントアジュバント中に乳化)、21及び28日目に50μg(100μlの不完全フロイントアジュバンド中)で免疫した。融合実験を実施する3日前に、動物に、1回の腹腔内注射及び1回の静脈内注射として与えられる、100μlの生理食塩水中に溶解した50μgのコンジュゲートを与えた。
抗体を標準的な抗体製造法によって産生させ(Marx et al,Monoclonal Antibody Production,ATLA 25,121,1997)、Protein A−クロマトグラフィーによって精製した。抗体純度は、SDSゲル電気泳動分析に基づき>95%であった。
以下の手法に従って、アクリジニウムエステルを用いて全ての抗体を標識した:
標識された化合物(トレーサー、抗PTA3−22):100μg(100μl)抗体(PBS中1mg/ml、pH7.4)を10μlのアクリジニウムNHS−エステル(アセトニトリル中1mg/ml、In Vent GmbH,Germany)(EP0353971)と混合し、室温で20分間インキュベートした。標識された抗体をBio−Sil SEC400−5(Bio−Rad Laboratories,Inc.,USA)上のゲル濾過HPLCによって精製した。精製された標識抗体を(300mmol/lのリン酸カリウム、100mmol/lのNaCl、10mmol/lのNa−EDTA、5g/1ウシ血清アルブミン、pH7.0)に希釈した。最終濃度は、200μlあたり、標識化合物の約800,000相対光単位(RLU)(約20ngの標識抗体)であった。アクリジニウムエステルの化学発光は、AutoLumat LB953(Berthold Technologies GmbH & Co.KG)を用いて測定された。
固相:ポリスチレンチューブ(Greiner Bio−One International AG,Austria)を抗PTA22−36抗体(1.5μg抗体/0.3ml 100mmol/L NaCl、50mmol/l Tris/HCl、pH7.8)でコーティングした(18時間室温)。5%ウシ血清アルブミンでブロッキングした後、チューブをPBS、pH7.4で洗浄し、真空乾燥させた。
50μlのサンプル(又は較正因子)をコーティングしたチューブにピペットで入れ、標識抗体(200μl)を添加後、チューブを2時間18〜25℃でインキュベートした。非結合のトレーサーを洗浄溶液(20mmol/l PBS、pH7.4、0.1%Triton X−100)で5回(各1ml)洗浄することによって除去した。チューブに結合した標識抗体は、Luminumeter LB953、Berthold、Germansyを用いて測定された。
20mM K2PO4、6mM EDTA、0.5%BSA、50μMアムスタチン、100μMロイペプチン、pH8.0中で希釈した合成P37の希釈物を用いてアッセイを較正した。PTA対照血漿は、ICI−diagnostics、Berlin、Germanyで入手可能である。
集団研究/PTA
方法
本発明者らは、1991〜1994年のMalmo Diet and Cancer Study基準試験ベースの集団(年齢58±6歳)の2559人の女性の参加者からの空腹時血漿中のPTAを測定した。本発明者らは、12年を超える中央値経過観察期間中の最初の事象の試験終点のそれぞれに対する、基準PTA(対数変換PTAの標準偏差増大毎あたりのハザード比)を関連付けるために、多変数補正された(すべての従来の心疾患リスク因子、糖尿病リスク因子、癌の分析において癌の遺伝も同様である)コックス比例ハザードモデルを使用した。終点は、Swedish National Hospital Discharge Registry,the Swedish Myocardial Infarction Registry,the Stroke in Malmo Registry and the Swedish Cancer Registryを通じて検索した。これらの登録簿を通じた終点の検索は、正当性が立証され、且つ、正確であることがわかっていた(Belting et al Cancer Epidemiol Biomarkers Prev;1−10.2012 ACRも参照されたい)。インスリンは、標準実験室法によって測定された。
女性人口におけるPTAの平均値は、54.3pmol/L、標準偏差±+1.4pmol/Lであった。すべての結果は、アッセイの測定範囲内であった。最も低いPTA5濃度は9.1pmol/Lであった。これらの結果は、使用したアッセイの適合性を指示する(アッセイ感度4.4pmol/L)。
本発明者らは、PTAと乳癌の間の関係を評価した(表3)。以前に癌を患っていた全ての女性(N=459)を評価から除外した。PTAと女性における乳癌の間に強い関係があった。完全に調整されたモデルにおいて、PTAの疾患の各SD(本発明者らは逆四分位、revPTAを使用した。表3/4参照)は、将来の乳癌の危険性を28.2%増加と関連付け(表3)、PTAの最高の四分位対最低の四分位は、乳癌の危険性において2.1倍を超える差があると特定した(表5と図2参照)。方程式中のPTAなしのインスリンは、将来の乳癌発症と有意に関連しなかったが、驚くべきことに、PTAが方程式の一部である場合、インスリンは有意になった(p=0.035)。インスリンの増加は、将来の乳癌のSDあたり34.6%の危険性の減少と関連した。PTAの予測力はインスリンによって影響されなかった。
プロ−ニューロテンシンアッセイ
抗体を上記のように作製した。標識用抗体(LA)は、P−NT1−19(H−CSDSEEEMKALEADFLTNMH(配列番号33))に対して作製され、固相抗体(SPA)は、ペプチドP−NT44−62(CNLNSPAEETGEVHEEELVA(配列番号34))に対して作製された。抗体の開発及び製造を上記のように行った。
使用された技術あ、アクリジニウムエステル標識に基づいて、サンドイッチ被覆されたチューブ発光イムノアッセイであった。
ヒト血清を含有するプロ−ニューロテンシンの希釈物を用いて、アッセイを較正した。高いプロ−ニューロテンシン免疫反応性を有するヒト血清のプール(InVent Diagostika,Hennigsdorf,Germany)は、ウマ血清(Biochrom AG,Deutschland)(アッセイ標準)を用いて希釈された。標準は、ヒトプロ−ニューロテンシン−較正因子(ICI−Diagnostics,Berlin,Germany)の使用によって較正された。あるいは、アッセイは、アッセイは、合成若しくは組換えP−NT1−117又はその断片によって較正されてもよい(Emstら、2006も参照)。
50μlのサンプル(又は較正因子)をSPAコーティングしたチューブにピペットで入れ、標識LA(200μl)を添加後、チューブを16〜22時間18〜25℃でインキュベートした。非結合のトレーサーを洗浄溶液(20mmol/l PBS、pH7.4、0.1%Triton X−100)で5回(各1ml)洗浄することによって除去した。チューブに結合したLAは、Luminumeter LB953を用いて測定された。結果は、較正曲線から計算された。典型的な較正曲線を図3に示す。
プロ−エンケファリンイムノアッセイ
抗体の開発
免疫化のためのペプチド/コンジュゲート:
免疫化のためのペプチドを、ウシ血清アルブミン(BSA)へのペプチドのコンジュゲートのために、追加のN末端システイン残基を用いて合成した(JPT Technologies,Berlin,Germany)。そのペプチドを、Sulfo−SMCC(Perbio−science,Bonn,Germany)を使用することによってBSAに共有結合で連結した。カップリング手順を、Perbioのマニュアルに従って実施した。

標識された化合物(トレーサー、CT−MRPENK抗体):100μg(100μl)抗体(PBS中1mg/ml、pH7.4)を10μlのアクリジニウムNHS−エステル(アセトニトリル中1mg/ml、In Vent GmbH,Germany)(EP0353971)と混合し、室温で20分間インキュベートした。標識された抗体をBio−Sil SEC400−5(Bio−Rad Laboratories,Inc.,USA)上のゲル濾過HPLCによって精製した。精製された標識抗体を(300mmol/lのリン酸カリウム、100mmol/lのNaCl、10mmol/lのNa−EDTA、5g/1ウシ血清アルブミン、pH7.0)に希釈した。最終濃度は、200μlあたり、標識化合物の約800,000相対光単位(RLU)(約20ngの標識抗体)であった。アクリジニウムエステルの化学発光は、AutoLumat LB953(Berthold Technologies GmbH & Co.KG)を用いて測定された。
固相:ポリスチレンチューブ(Greiner Bio−One International AG,Austria)を抗体(1.5μg抗体/0.3ml 100mmol/L NaCl、50mmol/l Tris/HCl、pH7.8)でコーティングした(18時間室温)。5%ウシ血清アルブミンでブロッキングした後、チューブをPBS、pH7.4で洗浄し、真空乾燥させた。
50μlのサンプル(又は較正因子)をコーティングしたチューブにピペットで入れ、標識抗体(200μl)を添加後、チューブを2時間18〜25℃でインキュベートした。非結合のトレーサーを洗浄溶液(20mmol/l PBS、pH7.4、0.1%Triton X−100)で5回(各1ml)洗浄することによって除去した。チューブに結合した標識抗体は、Luminumeter LB953、Berthold、Germansyを用いて測定された。
20mM K2PO4、6mM EDTA、0.5%BSA、50μMアムスタチン、100μMロイペプチン、pH8.0中で希釈した合成P37の希釈物を用いてアッセイを較正した。プロ−エンケファリン対照血漿は、ICI−diagnostics、Berlin、Germanyで入手可能である。
乳癌予測のためのPTA、プロ−ニューロテンシンとHRT、及びPTA、プロ−ニューロテンシンとプロ−エンケファリンの組み合わせアッセイ
最近、プロ−ニューロテンシンとプロ−エンケファリンが乳癌について高度に予測的であることが示されたため、本発明者らは、乳癌予測についてこれらのバイオマーカーを組み合わせた。本発明者らは、PTAの増分値を示すために、乳癌について既知の危険因子として、HRT(ホルモン補充療法)を加えた。
PTAとプロ−ニューロテンシンの間に有意な相関はなかった(p=0.71)。PTAとPNTを用いたインスリン及びホルモン補充療法(HRT)を含む組み合わせモデルにおいて(表8)、本発明者らは、乳癌予測においてそれらはともに独立であることを見出した。両マーカーは、非常に有意であった(PTAについてp=0.05、PNTについてp<0.001)。
低いPTA値が乳癌発症の危険性の増加を示すため、本発明者らは、PTA四分位を逆転させた(revPTA):第1四分位PTA=第4四分位PTA revPTA;第2四分位PTA=第3四分位 revPTA;第3四分位PTA=第2四分位 revPTA;第4四分位PTA=第1四分位 revPTA(表9)。
PTAとプロ−エンケファリンの間に有意な相関があった(p=0.001、r=0.35)。インスリン、PTA、PNTとプロ−エンケファリンを含む組み合わせモデルにおいて、本発明者らは、全てのマーカーが、乳癌予測についての情報を独立して追加することを見出した(表10)。全てのマーカーは、非常に有意であった(PTAについてp=0.028、PNTについてp<0.001、インスリンについてp=0.009、及びプロエンケファリンについてp<0.001)。PTAは独立したままであるが、それは、プロエンケファリンに非常に相関する。
Claims (29)
- 乳癌を被っていない対象において乳癌となる危険性を予測するための方法であって、下記:
−該対象から得られた体液中の配列番号1であるプロ−タキキニン又は配列番号2であるプロ−タキキニン1−37のレベルを決定し;及び
−該プロ−タキキニン又は該プロ−タキキニン1−37のレベルと乳癌となる危険性を相関させる
ことを含み、ここで、レベルの減少は、乳癌となる危険性の増大を示す上記方法。 - 以下の工程:
−前記対象から得られた体液中のプロ−ニューロテンシン又はその少なくとも5つのアミノ酸の断片のレベルを決定し;及び
−追加的に、プロ−ニューロテンシン又はその少なくとも5つのアミノ酸の断片と乳癌となる危険性を相関させる
ことを含み、ここで、プロ−ニューロテンシン又はその断片のレベルの増加は、乳癌となる危険性の増大を示す、請求項1に記載の方法。 - 以下の工程:
−前記対象から得られた体液中のプロ−エンケファリン又はその少なくとも5つのアミノ酸の断片のレベルを決定し;及び

ことをさらに含み、ここで、インスリンのレベルの減少は、乳癌となる危険性の増大を示す、請求項1〜3のいずれか1項に記載の方法。 - 追加的に相関させることが、個々のバイオマーカーによって得られた乳癌発症についての相対的な危険因子を考慮することによって、決定されたバイオマーカーレベルの組み合わせた分析を意味する、請求項2〜4のいずれか1項に記載の方法。
- プロ−タキキニン、そのスプライス改変体又はその断片のレベルの減少は、閾値を下回るレベルであり、閾値は、100pmol/L以下である、請求項1〜5のいずれか1項に記載の方法。
- 閾値が、80pmol/L以下である、請求項6に記載の方法。
- 閾値が、60pmol/L以下である、請求項6に記載の方法。
- 閾値が、50pmol/L以下である、請求項6に記載の方法。
- 閾値が、45.6pmol/L以下である、請求項6に記載の方法。
- 閾値が、40pmol/Lである、請求項6に記載の方法。
- プロ−ニューロテンシン又はその断片のレベルの増加は、閾値を上回るレベルであり、閾値は、78pmol/L以上である、請求項2〜11のいずれか1項に記載の方法。
- 閾値が、100pmol/L以上である、請求項12に記載の方法。
- 閾値が、150pmol/Lである、請求項12に記載の方法。
- プロ−エンケファリン又はその断片のレベルの減少は、閾値を下回るレベルであり、閾値は、100pmol/L以下である、請求項3〜14のいずれか1項に記載の方法。

- 年齢、真性糖尿病の存在、現在の喫煙を含む群から選択される追加的に少なくとも1つの臨床パラメータが決定される、請求項1〜23のいずれか1項に記載の方法。

- 対象において乳癌となる危険性をモニターするために、又は治療経過をモニターするために、1回を超えて該方法が行われる、請求項1〜25のいずれか1項に記載の方法。
- 採用された予防的及び/又は治療的測定に対する該対象の応答を評価するために、該モニターリングが行われる、請求項26に記載の方法。
