Description
10人のうち1人の成人が耳鳴、すなわち、外部の聴覚刺激によらない音を感知している。さらに、少なくとも100人に1人の成人が、耳鳴によって睡眠、リラックス、または集中力に対して深刻な影響を受け、また疲労、苛立ち、不安、絶望、失望、または憂鬱な気分になっている。効果が証明された薬物はこれまでないため、耳鳴治療には、満たされていない大きな医療的需要が残されている。多くの試みが行われ、耳鳴の病態を明らかにし、薬理学的治療法および他の治療法を同定したにもかかわらず、これまで、どの試みも治療効果を約束できるほどのものではなかった(Langguth et al., 2009)。

いくつかの実施形態において、R7は常に存在しており、X−は常に存在していないことが好ましい。より具体的には、いくつかの実施形態において、式IXの化合物はトルセミドではない。
図1は、耳鳴を引き起こす聴覚外傷(120dB、10kHz、2時間)が、ラットの内有毛細胞および螺旋神経節ニューロンにおけるKCC2の発現の減少につながることを示す。
A)蝸牛の中回転(midbasal turn)における螺旋神経節ニューロンのin situハイブリダイゼーション。曝露から14日後、騒音に曝露された動物(AT)のKCC2 mRNAの下方制御がはっきりと認められた(b)。偽曝露されたコントロール動物との比較は(a)に示されている。
B)KCC2のウェスタンブロット。第1レーンは、140kDaの明確なバンド(KCC2である)を有するポジティブコントロールとしての海馬組織を示す。第2レーンは曝露されていないコントロール動物の、曝露から7日後の蝸牛組織に対応し、第3レーンは騒音に曝露された動物の、曝露から7日後の蝸牛組織に対応している。騒音に曝露された動物(右レーン)において、コントロール動物と比べて、KCC2タンパク質の下方制御がはっきりと確認できる。

<実施例1>
(目的)
ループ利尿薬フロセミドは耳鳴の(可逆性の)治療薬として提案されてきた。フロセミドは蝸牛内の電位を低下させることによって聴神経の炎症を和らげるとの仮定が立てられてきた(Risey et al., 1995)。しかしながら、同時に、フロセミドの副作用として耳鳴があることも知られている。本発明者らは、耳鳴に対する新規な薬物療法の開発の出発点を特定するために、耳鳴を誘発しやすい蝸牛への損傷後、蝸牛内の塩素イオン共輸送体の発現が変化するのかどうか、そして、もし変化するのであればどのように変化するのかを解明しようと努めた。
消音ブースにおいて、麻酔を投与した13匹のメスの成体ウィスターラットの耳内に、SPL強度120dB、周波数10kHzの音を2時間継続的に曝露した。この曝露はラットに耳鳴を誘発させやすい(例えば、Tan et al., 2007)。この聴覚刺激は、上記動物の頭の高さで出るように調整した。麻酔を投与した4匹のコントロール動物を、スピーカーを切った状態で消音ブースに同じ時間置いた。14日後、音に対する曝露または偽曝露を行った動物を殺した。蝸牛を採取し、2%パラホルムアルデヒド(PFA)、125mMスクロース(100mMのリン酸緩衝食塩水(PBS),pH7.4に溶解)に2時間浸漬することで固定した。これらの蝸牛はRapid Bone Decalcifier(Eurobio社,Les Ulis Cedex,フランス)中で脱灰したのち、25%スクロースを含むハンクス緩衝食塩水中で一晩インキュベートした。翌日、蝸牛をO.C.T.コンパウンド(Miles Laboratories,Elkhart,Ind.,社,アメリカ)に包埋した。使用前に、in situハイブリダイゼーションおよび免疫組織化学のために、組織サンプルを10μmの厚さにおいて凍結切片にし、SuperFrost(登録商標)/plus 顕微鏡スライドに載せて、−20℃で保管した。
耳鳴を誘発しやすい過度の騒音に曝露された動物は、曝露から14日後、明らかなKCC2の下方制御を示した(図1a)。免疫組織化学は、曝露されていないコントロール動物と比較して、内有毛細胞におけるKCC2タンパク質の発現が著しく減少したことを示した。また、in situハイブリダイゼーションによって、騒音に曝露された蝸牛の螺旋神経節ニューロンにおいて、曝露されていない蝸牛と比べると、明らかなKCC2 mRNAの下方制御が示された。ウェスタンブロット分析によって、図1bに示すように、騒音に曝露された動物の蝸牛組織におけるKCC2タンパク質が、コントロール動物と比較して、明確に下方制御(7.8倍現象)されることが示された。この実験の結果は、聴覚外傷などによる蝸牛への損傷に伴い、KCC2の発現が下方制御させることを初めて示すものであった。この実験の結果に対し、この下方制御が失聴に関連するのか、耳鳴に関連するのか、または両方に関連するのかについての解明が残る。したがって、さらなる解明を行うために、実施例2を設定した。
(目的)
第2の実験のねらいは、最初の実験で認められたKCC2の下方制御が失聴の病態生理に関連するのか、耳鳴の病態生理に関連するのか、または両方の病態生理に関連するかどうかを分析することである。この下方制御は、フロセミドによる耳鳴の誘発が一般的に推測されているような蝸牛内電位の緩和(例えば、Risey et al., 1995)に関連することよりも、むしろフロセミドがKCC2に及ぼす公知の効果に関連することの示唆となり得るものである。この目的のため、Ruttiger et al., 2003に記載されている行動モデルを用いて、耳鳴を有する動物と耳鳴がない動物とを識別した。
36匹のメスの成体ウィスターラットを、一定の音が発生している際は液体給餌器に積極的にアクセスするよう条件付けチャンバ内で訓練を行った。無音の間は報酬を与えなかった。条件付けは、動物が無音の期間よりも音が存在する期間に報酬用の給餌器にアクセスするようになったところで完了した。
A群およびB群の何れにおいても、騒音曝露から6日後および30日後には、耳鳴の行動相関を示す動物において、耳鳴行動を示さない動物と比べて、KCC2の発現が著しく下方制御された(図2aに示されるとおり)。免疫組織化学は、耳鳴症状のない動物と比べて、耳鳴症状のある動物の内有毛細胞におけるKCC2タンパク質の発現が明らかに低下していることを示す。in situハイブリダイゼーションにおいてもまた、耳鳴症状のない蝸牛に比べて、耳鳴症状のある動物の螺旋神経節ニューロンにおけるKCC2 mRNAの下方制御が明らかになった。さらに、耳鳴症状のない動物に比べて、聴力に障害を受けた耳鳴症状を有する動物のIHCリボンは、中回転および基底回転において大幅に減少した(動物数n=4、二元ANOVAに関してp<0.001、試験後のスチューデントのt検定(両側検定)に関してp<0.02、何れもα=0.05)(図2b参照)。
(目的)
第3の実験のねらいは、耳鳴を誘発する騒音外傷を受けた後の蝸牛細胞内の塩素イオンレベルに対する薬理学的調節が耳鳴を抑制できるかどうかを分析することである。ブメタニドは、KCC2に比べてNKCC1に対してかなり高い親和性を有する(Payne et al., 2003)ため、本発明のこれまでの発見を鑑みると、NKCC1に対する阻害効果がKCC2に対するあらゆる(望ましくない)阻害効果よりも優位となることから、ブメタニドの投与によって細胞内のCl−レベルが低下することを考慮にいれるべきである。
20匹のメスの成体ウィスターラットに、これまでの実施例と同様に麻酔を投与した。まず、聴性脳幹反応(ABR)の測定を行った。実施例2と同様に、ラットに、消音ブースにおいて、SPL強度120dB、周波数10kHzの音を1.5時間継続的に曝露し(A群;n=10)、また同様の設定で偽曝露(B群;n=10)を行った。
偽曝露された耳(B群)において、APまたはブメタニドのどちらで処理されたかにかかわらず、処理後15日経過した蝸牛組織サンプルからのKCC2のリアルタイムPCRに違いはみられなかった。しかしながら、騒音に曝露された耳(A群)において、偽曝露のみと比較して統計的に有意なKCC2の下方制御が、APで処理した耳においてみられた(スチューデントのt検定(両側検定)においてp<0.05)が、ブメタニドで処理した耳ではみられなかった。騒音に曝露され、ブメタニドで処理した耳におけるKCC2の発現は、偽曝露された耳におけるレベルと類似していたが、騒音に曝露され、APで処理した耳とは統計学的に有意に異なっていた(p<0.01)(図3a参照)。

- ブメタニドまたはその類似体を含有する、耳鳴の治療または予防に使用するための医薬組成物であって、

- 同時のまたは時間をずらした療法プロトコルにおける共治療プロトコルにおいて、1つ以上のGABA作動性作用物質および/または1つ以上のグリシン作用物質と組み合わせて用いられる、請求項1または2に記載の医薬組成物。
- 1つ以上のカルボニックアンヒドラーゼ阻害剤および/またはGABA作動性作用物質および/またはグリシン作用物質をさらに含有する、請求項1〜3の何れか一項に記載の医薬組成物。

- 上記生物分解性ポリマーは、ヒアルロン酸/ヒアルロン酸塩、レシチンゲル、(ポリ)アラニン誘導体、酸化エチレンおよび酸化プロピレンに基づくブロックコポリマー、ポリ(エチレングリコール)、ポロキサマ、キトサン、キシログルカン、コラーゲン、フィブリン、ポリエステル、ポリ(ラクチド)、ポリ(グリコリド)またはそれらの共重合体PLGA、スクロース酢酸イソ酪酸エステル、およびグリセロールモノオレートからなる群より選ばれる、請求項7に記載の医薬組成物。
- 上記耳鳴の治療または予防において、全身的または局所的に投与される、請求項1〜8の何れか一項に記載の医薬組成物。
- 上記耳鳴の治療または予防において、局所的に投与される、請求項9に記載の医薬組成物。
- 上記耳鳴の治療または予防において、中/内耳の境界面に投与される、請求項10に記載の医薬組成物。
- 上記耳鳴の治療または予防において、正円窓膜および/または卵円窓膜に投与される、請求項11に記載の医薬組成物。
- 上記耳鳴の治療または予防において、耳の上または中へ局所的に投与される、請求項10に記載の医薬組成物。
- カルボニックアンヒドラーゼ阻害剤および/またはGABA作動性作用物質および/またはグリシン作用物質と組み合わせて用いられる、請求項1〜13の何れか一項に記載の医薬組成物。
