Description
本出願は、引用をもってその内容全体を本願明細書に援用する2006年4月6日に提出された米国特許出願第11/398,859号、および2006年7月19日に提出されたPCT特許出願US06/027859号の優先権の利益を主張する。
吸入による投与に好適な徐放技術では、リポソームなどの脂質を用いた薬物製剤を用いて、徐放による肺および全身における薬物の持続的な治療効果と疾患部位への薬物取り込みの標的および促進能力を得ている。
本願発明の目的は、脂質対薬物比が低い脂質を用いた製剤を提供することである。
本願発明は一部、脂質と活性剤をコアセルベートと混合するステップを含む脂質を用いた活性物質製剤を調製する方法に関連する。さらなる実施態様では、前記コアセルベートは脂質と混合する前に形成する。さらなる実施態様では、前記コアセルベートは脂質と混合している間に形成する。さらなる実施態様では、前記コアセルベートは脂質と混合した後に形成する。さらなる実施態様では、前記コアセルベートは前記活性物質のコアセルベートである。さらなる実施態様では、前記コアセルベートは前記脂質および活性物質以外の第三の成分のコアセルベートである。さらなる実施態様では、前記第三の成分には、前記活性物質と交換可能な対イオンが含まれる。

本願発明は、薬物周辺に脂質の二重層形成を誘発する、薬物コアセルベートの形成によって調製される、脂質薬物製剤を開示する。本願方法によって低い脂質対薬物比が得られ、その結果、脂質製剤と、外部の薬物濃度よりも3乃至5倍高い内部の薬物濃度を得ることができる。本願発明はまた、コアセルベーション技術を用いたこの脂質製剤の調製方法、この脂質製剤を噴霧によって投与するための高送達速度、およびこの脂質製剤をその必要がある患者に投与するステップを含む肺感染症の治療の方法を開示する。

本願発明のある実施態様では、肺感染症の患者を治療する方法であって、低L/D比を有する脂質薬物製剤を、その必要のある患者に、治療上有効な量投与するステップであって、当該薬物が抗感染性であるステップを含む方法を含む。

脂質薬物製剤のプロセスには「溶媒注入」プロセスが関与する。これは、少量、好ましくは最少量のプロセスに適合する溶媒に1種類以上の脂質を溶解し、脂質の懸濁液または溶液(好ましくは溶液)を作成するステップと、その後、薬物を含有する水性溶媒にその溶液を注入するステップを含むプロセスである。典型的には、プロセスに適合する溶媒は、透析またはダイアフィルトレーションなどのように水性プロセスで洗浄することができるものである。溶媒注入の一種である「エタノール注入」は、少量、好ましくは最少量のエタノールで1種類以上の脂質を溶解し、脂質の溶液を作成するステップと、その後、薬物を含有する水性溶媒にその溶液を注入するステップを含むプロセスである。「少量」の溶媒とは、注入プロセスにおいてリポソームまたは脂質複合体に適合する量である。注入プロセスのための条件はコアセルベート形成を生じさせなければならないという点は、重要である。活性物質の水溶液を脂質溶液に注入する基本的な条件は、本願明細書に提示する実施例と、以下に教示する各種パラメータの作用に基づいて決定されなければならない。さらに当業者に有用なのは、 Bunderberg de

et al, 1998)。提示されている界面活性たんぱく質(SP)は、構造を維持し、呼吸中に生じる肺界面活性物質の拡張および収縮の両方を促進する働きをする。これらの中で、SP-BおよびSP-Cには特に溶解作用があり、リポソームを溶解することができる(Hagwood et al., 1998;
Johansson, 1998)。この溶解作用は、リポソームの緩やかな破壊を促進することができる。リポソームはマクロファージの貪食作用によって直接取り込まれることができる(Couveur et al., 1991;
Gonzales-Roth et al., 1991; Swenson et al, 1991)。肺胞マクロファージによるリポソームの取り込みは、薬物が患部に送達されうるもう一つの手段である。
個々の流速は変化したが、全流速は800 mL/分に維持された。そのために、2つの別々のポンプを異なるポンプ速度に設定して使用した。混合溶液を10秒間NaCl溶液を入れたビーカーに注入し、最終NaCl濃度を1.5%にして、最終エタノール濃度が30%を超えないようにした。混合後、1mLのアリコートをセファデックスG-75ゲルろ過カラムにかけて、封入物から遊離アミカシンを分離した。最高密度の分画1mL(目視の濁度で決定)をさらなる分析用に収集した。表1に結果を示す。脂質/アミカシン流速比が上昇しても、L/Dは300/500mL/分までほぼ一定だった。脂質の速度をさらに上昇させたところ、L/Dは上昇し始め、粒子サイズも大きくなり始めた。同時に、脂質の流速が速く質量が増加するほど、アミカシンの回収(封入効率)がよくなった。
設定はバッチ3と同様に維持したが、加えたNaCl溶液の量は少なく、最終濃度は1.0%になった。注入時間が短いと注入中に添加することが難しいため、再び溶液を加えてから注入を開始した。さらに、注入中、流れの圧力で、混合管の末端にインラインミキサを移動させた。ミキサの位置は、バッチ3の場合には管の末端から0cmだったが、この場合は末端から5cmだった。これは、バッチ20の温度条件40/40℃において得られたL/D比が0.55で、バッチ3のほぼ半分であったことから、重要かもしれない。異なる注入温度におけるアミカシン封入を比較すると、驚くことに、低温のほうが良いL/Dを得られることがわかる。テストした温度のうち、脂質/アミカシン温度が30/30℃と50/室温の場合、L/D比は0.32と0.37で差がなかった。繰り返すが、バッチ1-5では、ゲルろ過によって洗浄したこれらのサンプルから得られる数値は低く、おそらく、バッチがダイアフィルトレーションで洗浄された場合よりも低いだろう。
次に、NaCl溶液の添加のステップと洗浄プロセスに注目した。プロセスのパラメータはさまざまな方向において変化した。流速300/500での注入ステップの直後、混合物のエタノール濃度は34%に達する。アミカシンはこの濃度において一定限度の溶解度を有する(図9)。
以前、50mg/mLアミカシン原液を使うと封入の状態が最良になることが明らかになった。従来のプロセスを用いた場合、アミカシンの原液の濃度を40mg/mLに減らすと、L/Dが上昇した。2ストリームインライン注入プロセスを用いると、エタノール濃度は高レベルに達するので、現在の50mg/mLアミカシンは最適な濃度ではないかもしれない。
リポソームへの薬物(たとえば、アミノグリコシド、アミカシン、トブラマイシン、ゲンタマイシンなどのアミノグリコシド)の注入量を増加するにはいくつか方法がある。一つの方法は、封入される脂質の体積/量が大きい非常に大きなリポソーム(>1μm)の作製である。このアプローチは、リポソームの吸入用(噴霧用)には実用的ではない。なぜなら、1) 大きいリポソーム(> 0.5μm)ほど大きい放出を受けやすいので、リポソームの大きさに応じて噴霧中の剪断ストレスによって破壊されやすい、2) 肺への良好な沈着に必要な小滴のサイズは小さいほどよく、約3μm未満である、という理由からである。そのため、吸入用には、リポソームの大きさは、大きすぎる放出を避けるために、なるべく小さくすることが望ましい。現在、本願明細書に開示されるリポソームの平均直径は約0.4μm未満である(表4)。
薬学的吸入療法のために、基本的には、最短の投与時間で望ましい投与量を達成するための高速の送達速度の確立が望ましい。リポソーム製剤のためには、噴霧中に封入された薬物の保持を最大化することも望ましい。噴霧流速は、単位時間(たとえば、g/分、ml/分)内にエアロゾル化して噴霧器から飛び出す水溶液の容量(または重量)である。薬物送達速度は、単位時間内(たとえばmg/分)に噴霧器によって送達される薬物の量と定義する。
10.1. 肺感染症のバイオフィルムバリア
CF患者の慢性疾患の主因である緑膿菌などの感染症の治療の障害は、上皮細胞上の痰/バイオフィルムバリア内での薬物透過である。図1で、ドーナツ型はリポソーム抗感染製剤を表し、+印は遊離感染剤、−印はムチン、アルギン酸塩、およびDNAであって、実線は緑膿菌を表す。このバリアは、細菌由来のアルギン酸塩またはエキソ多糖類に埋包されたクローン化および植物性緑膿菌(P. aeruginosa)、および傷害された白血球に由来するDNA、および肺上皮細胞に由来するムシンから構成されており、すべて正味のマイナス電荷を有している(Costerton, et al., 1999)。このマイナス電荷は、アミノグリコシドなどプラス帯電している薬物に結合して透過を防ぎ、生物学的に無効にする(Mendelman et al., 1985)。リポソームまたは脂質製剤の中への抗感染物質の封入によって、抗感染物質を痰/バイオフィルムへの非特異的な結合から遮断、または部分的に遮断し、リポソームまたは脂質製剤を(封入したアミノグリコシドとともに)透過させることができる(図1)。
アミカシンの薬物動態は、ラットにおいて、遊離トブラマイシンかまたはアミカシンのリポソーム製剤のいずれかを気管内(IT)投与後に測定した。これらのデータを、遊離トブラマイシンの尾部静脈注射後に肺で得られた分布と比較した。すべての場合において、1回の投与につき、4mg/ラットを投与した。図2に見られるように、注射と比較して、はるかに大きなアミノグリコシドの沈着を、ITで送達することができる。リポソーム抗感染物質技術のデポー効果も、ITまたはIVで投与されたトブラマイシンとの比較すると、アミカシンのリポソーム製剤の場合、投与から24時間、肺の中の残留量が100倍を越えることから実証される。したがって、薬物の治療レベルは、アミカシンのリポソーム製剤の場合、遊離トブラマイシンと比較して長時間持続させることができる。

al., 1968)。マクロファージは両方の感染経路の中央で、全身性(吸入)炭疽病の宿主自己破壊の主な寄与因子である。リポソーム抗感染製剤技術は、徐放および標的化の性質に加えて、細胞取り込みを促進し、薬物標的および送達において、肺胞マクロファージおよび肺上皮細胞を使用することができる。このような性質があるために、このような細胞内感染症を促進し、感染症が肺に生じてマクロファージによって運ばれてしまうと考えられている。さらに重要なのは、このような性質によって抗感染製剤がさらに効果を発揮し、リポソーム抗感染物質は疾患を含有するまさにその細胞によって貪食されるはずという点である。抗感染製物質は、標的化されながら細胞内に放出され、広がる前に感染症を攻撃する。カプセル封入される薬物は、シプロフロキサシン、テトラサイクリン、エリスロマイシン、またはアミカシンなど、すでに認可された薬剤であってよい。リポソームシプロフロキサシン製剤はすでに作製されている。
感染症の近隣における活性な状態の薬物の放出は、本願発明のリポソーム薬物製剤の活性の重要な局面である。そのような標的化された放出の可能性を、CF患者から採取した痰でインキュベーションした場合の薬物放出、緑膿菌をあらかじめ摂取したラットの肺における放出、および緑膿菌の培養物に対する活性をモニターすることによってテストした。
ある実施態様では、本願発明のリポソームアミカシン製剤は、嚢胞性線維症患者の慢性緑膿菌感染症のために調合されたアミカシンのナノサイズ(200-300 nm)リポソームカプセル封入剤形である。それは、吸入してから肺内でアミカシンを徐放するようにデザインされている。肺胞マクロファージはこの大きさの範囲内の粒子を貪食することが知られているため、本リポソーム製剤がこの細胞に与える作用は特に興味深い。洗浄によって得られるラット肺胞マクロファージの基本および刺激された機能を、リポソームアミカシン製剤を投与して、および投与しないで、各種対照と比較して研究した。

本願発明のいずれの組成物の投与量も、症状、患者の年齢および体重、治療または予防する疾患の性質および重篤度、投与経路、および本願組成物の形状によって変化するだろう。いずれの本願製剤も、単回または分割して投与してもよい。本願発明の組成物の投与量は、当業に知られる技術によって、または本願明細書に教示されるとおり、容易に決定することができる。
本願発明の脂質抗感染製剤は、リポソームの水性懸濁液を含んでもよい。本製剤は、リポソームを形成するための脂質賦形剤、および適切な浸透圧およびpHを得るための塩/バッファを含有してもよい。本製剤は、薬学的賦形剤を含んでもよい。薬学的賦形剤は、ある器官または体の部分から別の器官または体の部分へいずれの本組成物またはその成分をも輸送することに関与する、液体、希釈剤、溶媒、またはカプセル封入物質であってよい。各賦形剤は、対象の組成物とその成分と適合性があり患者に有害ではないという意味において「許容」でなければならない。好適な賦形剤には、トレハロース、ラフィノース、マンニトール、スクロース、ロイシン、トリロイシン、および塩化カルシウムが含まれる。その他の好適な賦形剤の例には、(1)ラクトース、およびグルコースなどの糖類、(2)トウモロコシデンプンおよびバレイショデンプンなどのデンプン、(3)カルボキシメチルセルロースナトリウム、エチルセルロース、および酢酸セルロースなどのセルロースおよびその誘導体、(4)粉末状トラガカント、(5)モルト、(6)ゼラチン、(7)タルク、(8)カカオバター、および座剤用ロウなどの賦形剤、(9)ピーナツ油、綿実油、ベニバナ油、ゴマ油、オリーブ油、トウモロコシ油、および大豆油などの油、(10)プロピレングリコールなどのグリコール、(11)グリセリン、ソルビトール、およびポリエチレングリコールなどのポリオール、(12)オレイン酸エチルおよびラウリル酸エチルなどのエステル、(13)寒天、(14)水酸化マグネシウム、および水酸化アルミニウムなどの緩衝剤、(15)アルギン酸、(16)発熱物質を含まない水、(17)等張生理食塩水、(18)リンガー溶液、(19)エチルアルコール、(20)リン酸緩衝溶液、および(21)薬学的製剤に用いられるその他の無毒性適合性物質、が含まれる。

インライン注入プロセス
約20mg/mlの総脂質(重量比でDPPC:コレステロール = 2:1)のエタノール溶液と、約75 mg/mlの硫酸アミカシン(約50
mg/mlアミカシン)の水溶液を混合して、2ストリーム・インライン注入法によって反応容器に入れた。2つの溶液を、それぞれ約1.0 L/分と約1.5L/分の速度でY字型連結器に注入した。2ストリーム注入中、新たに水を、脂質溶液の流速と類似する流速(約1.0L/分)で反応容器に加えた。反応容器に注入したアミカシン脂質懸濁液を、瞬時に水を連続注入して希釈した。この加水によって、エタノールを希釈することによって膜の密封を促進し、懸濁液の粘性も低下させてダイアフィルトレーション・カートリッジの内圧を低下させる。注入後、この懸濁液を、ダイアフィルトレーションを使って体積を半分に減らすことによって濃縮した。新たに3.0% NaCl溶液を供給している間に、濃縮懸濁液をダイアフィルトレーションによって洗浄した。洗浄した懸濁液は、望ましい総アミカシン濃度になるまで、ダイアフィルトレーションによってさらに濃縮した。結果を表10に示す。
コアセルベーション技術によるウシ血清アルブミン(BSA)のカプセル封入
BSAは等電点pI=4.9を有するたんぱく質である。その点より上のpHでは、正味のマイナス電荷を有するコロイドとしてみなすことができる。媒質のイオン強度、pHおよび温度に影響する、ポリ(アリルアミンヒドロクロリド)などの各種高分子電解質との複合体化コアセルベートを形成する。アルブミンへの非溶媒(エタノール)を加えると、コアセルベーションも誘発することができることが明らかになった。BSAをpH7.0の水に溶解し、加えたエタノール濃度が約45重量%を超えると、BSA分子が凝集してコアセルベート相の小滴を形成し、光散乱が大きく増大する。NaClを加えると(イオン強度が上昇)、コアセルベーションを誘発するのに必要なエタノールの量が少なくなる。pHを低くすると似たような効果がある(図12)。二価イオン(たとえばMg2+)は、BSAコアセルベーションを誘発するのに必要な臨界エタノール濃度を低くするよりさらに強い効果がある。もっとも劇的な効果は、低分子量ポリカチオンPEIをBSA溶液に加えたときに見つかった(図14)。したがって、0.05mg/mLのPEIはモル単位で約60μMの濃度であって、BSA 3分子につきPEI 約1分子でしかない。
60:5:40だった。すべての溶液を30℃に予熱した。試験管に入れた1mL BSAに脂質溶液(0.4mL)をピペットで加え、ただちにボルテックスして完全に混合した。20秒後、5%スクロース溶液0.6mLを加え、再びボルテックスした。BSAのカプセル封入を測定するために、得られたリポソーム懸濁液0.8mLを5-20%のスクロース勾配に置き、30分間、30,000RPMで遠心分離した。負荷されたリポソームは20%スクロースより重いペレットを形成した。そのペレットを収集し、脂質とBSAについて定量した。脂質を逆相HPLCで測定し、BSAを蛍光で測定した(励起280nm、発光320nm)。そのペレットは脂質1.6mgとBSA 1.3mgを含有し、L/D比は1.2で、通常たんぱく質で見られる値よりも低いことがわかった(たとえば、米国特許第6,843,942号では、DPPC-コレステロール-ステアリルアミン形成における組み換えヒト超酸化ジスムターゼ(rh-SOD)のカプセル封入を調製したところ、L/D比約5だった)。

- 肺感染症の治療のための脂質を用いたアミノグリコシド製剤であって、前記脂質を用いたアミノグリコシド製剤が、噴霧によって10乃至25mg/分の速度で送達されることを特徴とし、前記脂質を用いたアミノグリコシド製剤が、脂質二重層およびその中に封入されたアミノグリコシドを含み、前記脂質を用いたアミノグリコシド製剤の前記脂質対前記薬物(L/D)比が0.40乃至0.49である、脂質を用いたアミノグリコシド製剤。
- 噴霧後の脂質に封入されたアミノグリコシドが、噴霧前の封入されたアミノグリコシドの45%超である、請求項1に記載の脂質を用いたアミノグリコシド製剤。
- 前記脂質がリン脂質とステロールの混合物である、請求項1に記載の脂質を用いたアミノグリコシド製剤。
- 前記リン脂質がジパルミトイルホスファチジルコリン(DPPC)であり、かつ前記ステロールがコレステロールである、請求項3に記載の脂質を用いたアミノグリコシド製剤。
