Description
本出願は、米国特許法規則1.53(b)(1)に基づき出願した非仮出願であり、米国特許法119条(e)に基づき、2006年4月24日に出願の仮出願第60/794393号の優先権を主張するものであり、その全体の開示内容が出典明記により本明細書に援用される。
本発明は全体的に自己免疫性疾患の分子的決定の分野に関する。より具体的には、本発明は、自己免疫性疾患の様々な態様と関係している固有の分子シグネチャーに基づいた方法および組成物に関する。
現在、自己免疫性疾患の多くは、様々な自己抗原に対する自己抗体の産生に特徴があると考えられている。例えば、全身性ループスエリテマトーデス(SLE)は、自己抗体が宿主細胞及び組織に結合し、血管組織に蓄積して免疫細胞を活性化する免疫複合体を形成することによって臓器障害を引き起こす自己免疫性疾患である。シェーグレン症候群は、身体の腺の炎症に特徴がある自己免疫性疾患である。また、IgAネフロパシ、乾癬、関節リウマチ、多発性硬化症、強直性脊椎炎などを含む他の自己免疫性疾患も一般的に見られる。
インターフェロンα(IFN-α)は、SLEなどの多くの免疫疾患の病因に非常に関係しているI型インターフェロンである。IFN-αシグナル伝達の破壊を伴う治療手法はこのような疾患の有効な治療でありうると考えられている。IFN-αレベルはSLEにおいて上がっていることが知られており、IFN-αの患者の治療によりレシピエントのSLEと類似した症状を可逆的に引き起こすことが観察されている。多くの他の種の証拠がIFN-αとSLEを関連づけている。
自己免疫性疾患の臨床管理における最も困難な試みの一つは、患者の疾患を正確かつ迅速に同定することである。最後に、患者の疾患の存在及び/又は程度を客観的に同定するために用いることができる分子基準の診断方法を有することは非常に有用であろう。本明細書において記述される発明は、これらの方法および他の利点を提供するものである。
特許出願及び出版物を含め、本明細書において引用したすべての文献は、出典明記によってこの全体を援用する。
本発明は、全身性エリテマトーデス(SLE)の存在及び/又は程度と関係する発現を有する遺伝子(一又は複数)の同定に少なくとも一部が基づいている、自己免疫性疾患を同定するための方法及び組成物を提供するものであって、このときのSLEは言い換えると、その疾患関連の遺伝子シグネチャーが他の自己免疫性疾患にも応用することができる原型自己免疫性疾患である。例えば、本明細書において記述されるように、一実施態様では、IFN-αによるシグナル伝達に応答して調節される遺伝子が同定された。次いで、この手法によって生じる情報を試験し、修正して、細胞又は組織試料が自己免疫性疾患に特徴的な応答を表す程度の簡明かつ定量的な基準を開発した。本明細書において示すように、本明細書において開示される一又は複数の特定の遺伝子の検出は患者の自己免疫性疾患の存在および/または程度の有用かつ有益な指標であるといえる。さらに、インターフェロン関連の疾患の提示および/または重症度を表す測定基準又は同等の指数は、バイオマーカー遺伝子発現情報の適切な形質転換によって作り出されてもよい。例示的な形質転換および結果として生じる測定基準は本明細書において開示されており、また、本明細書において開示される遺伝子発現データに基づいて作り出される。
ある態様では、本発明は、自己免疫性疾患治療に対する被検体の応答性を予測する方法であって、被検体が、少なくとも2、3、4、5、6、7、8、9、10、11、12、13、14、15、16、17、18、19、20、21、22、23、24、25、26、27、28個、又は最大すべてまでのいずれかの数の表1、2及び/又は3に挙げる遺伝子を正常な参照試料におけるそれぞれの遺伝子の発現レベルよりも多いレベルで発現する細胞を含んでいるか否かを決定することを含む方法であって、このような細胞が存在する場合に該被検体が自己免疫性疾患治療に応答性があることが示される方法を提供する。

ある態様では、本発明は、被検体において抗dsDNA抗体の存在及び/又は上昇を決定するための方法であって、被検体が、少なくとも2、3、4、5、6、7、8、9、10、11、12、13、14、15、16、17、18、19、20、21、22、23、24、25、26、27、28個、又は最大すべてまでのいずれかの数の表1、2及び/又は3に挙げる遺伝子を正常な参照試料におけるそれぞれの遺伝子の発現レベルよりも多いレベルで発現する細胞を含んでいるか否かを決定することを含む方法であって、このような細胞が検出される場合に該被検体において抗dsDNA抗体の存在及び/又は上昇が示される方法を提供する。
当業者に明らかであるように、本発明のいずれかの方法では、ある遺伝子の増加した発現の検出はある疾患の特徴(例えば、疾患の存在、段階又は程度)を陽性に示すのに対して、ある遺伝子の発現の増加が検出されないことはこの疾患の相互関係の特徴を示す上で有益であろう。
ある態様では、本発明は、本発明の組成物と、少なくとも2、3、4、5、6、7、8、9、10、11、12、13、14、15、16、17、18、19、20、21、22、23、24、25、26、27、28個、又は最大すべてまでのいずれかの数の表1、2及び/又は3に挙げる遺伝子の発現が正常な参照試料におけるそれぞれの遺伝子の発現レベルよりも多いレベルであるか否かを決定することによって自己免疫性疾患を検出するための該組成物の使用についての指示書とを、具備するキットを提供する。一実施態様では、本発明の組成物は、少なくとも2、3、4、5、6、7、8、9、10、11、12、13、14、15、16、17、18、19、20、21、22、23、24、25、26、27、28個、又は最大すべてまでのいずれかの数の表1、2及び/又は3に挙げる遺伝子に特異的にハイブリダイズすることができるアレイ/遺伝子チップ/遺伝子セットを含む。一実施態様では、本発明の組成物は、表1、2及び/又は3に挙げるある遺伝子によってコードされたポリペプチドの少なくとも一部をコードする核酸分子を含む。一実施態様では、本発明の組成物は、表1、2及び/又は3に挙げるある遺伝子の少なくとも一部に結合して重合(例えば増幅)に作用することができる核酸プライマーを含む。一実施態様では、本発明の組成物は、表1、2及び/又は3に挙げるある遺伝子(または、その相補鎖)(または、対応する遺伝子産物)を特異的に検出する結合剤(例えばプライマー、プローブ)を含む。一実施態様では、本発明の組成物は、表1、2及び/又は3に挙げるある遺伝子によってコードされるポリペプチドの少なくとも一部に特異的に結合する結合剤を含む。
(a) プローブセットによって表される遺伝子の発現が疾患の特徴と関係しているパターンと総合的に関連しているプローブセットのグループを評価する工程、
(b) 該プローブセットのグループの傾向に対する個々のプローブセットの一致の程度を反映する尺度に従ってプローブセットに重みを付けるウェイティング因子(weighting factor)を作り出し、算出した平均プロファイルに対するそれぞれのプローブセットの相関係数を算出する工程、
(c) 個々のプローブセットを1に対して尺度をとるために必要な値であるスケーリング因子(scaling factor)を決定する工程、
(d) スケーリング因子にウェイティング因子を乗算して、混成因子(composite factor)を作り出す工程、
(e) 混成因子を有する正常な血液試料のシグネチャーを乗算し、平均値を得るためにプローブセットと試料全体の結果として生じた値の平均を求め、該平均値の逆数をとって(inverting)全体のスケーリング因子(global scaling factor)を得る工程、
(f) 各々のウェイティング因子に該全体のスケーリング因子を乗算してスカラー値(scalar value)のベクトルを得て、該スカラー値に対象の試料の発現シグネチャーを乗算し、結果として得た値の平均を求めて、該試料中のI型インターフェロンと関係している遺伝子発現の程度を表す単一測定基準を得る工程
を含んでなる方法を提供する。
前段落の前記方法の一実施態様では、工程(b)において、前記因子は、1に対して尺度をとる(scaling to 1)手段を含むZ-スコアにプローブセットのグループの発現データを変換して、つまり2を底とする対数変換を行って、1の平均値の標準偏差に対して尺度をとる(acaling)ことによって作り出されたものである。
前段落の前記方法の一実施態様では、工程(e)において、前記の全体のスケーリング因子は、対象の試料から得た平均のプローブセットの結果を測定基準に変換するために有用であり、該測定基準が該試料が正常な健康な被検体である場合に1である。
前段落の前記方法の一実施態様では、前記のプローブセットのグループには、少なくとも2、3、4、5、6、7、8、9、10、11、12、13、14、15、16、17、18、19、20、21、22、23、24、25、26、27、28、29、30、31、32、33、34ないし最大すべてまでのいずれかの数の表1、2および/または3に挙げる遺伝子が含まれる。一実施態様では、前記のプローブセットのグループには、表1、2および/または3に挙げるすべてが含まれる。
一態様では、本発明は、自己免疫性疾患治療に対する被検体の応答性を予測する方法であって、被検体から得た試料について本明細書中に記載した方法によって得た第一測定基準を参照(例えば、正常、健康、罹患していない)試料から得た参照測定基準と比較することを含む方法であって、このとき第一測定基準が参照測定基準より高い場合に、該被検体が自己免疫性疾患治療に応答性があることが示される方法を提供する。
一態様では、本発明は、自己免疫性疾患の状態を検出するための方法であって、自己免疫性疾患の状態を有することが予測される被検体から得た試料について本明細書中に記載した方法によって得た第一測定基準を参照(例えば、正常、健康、罹患していない)試料から得た参照測定基準と比較することを含む方法であって、このとき第一測定基準が参照測定基準より高い場合に、該被検体において自己免疫性疾患の状態の存在が示される方法を提供する。
一態様では、本発明は、被検体において自己免疫性疾患を診断するための方法であって、被検体から得た試料について本明細書中に記載した方法によって得た第一測定基準を参照(例えば、正常、健康、罹患していない)試料から得た参照測定基準と比較することを含む方法であって、このとき第一測定基準が参照測定基準より高い場合に、該被検体が該自己免疫性疾患を有することが示される方法を提供する。
一態様では、本発明は、被検体において抗dsDNA抗体の存在及び/又は上昇を決定するための方法であって、被検体から得た試料について本明細書中に記載した方法によって得た第一測定基準を参照(例えば、正常、健康、罹患していない)試料から得た参照測定基準と比較することを含む方法であって、このとき第一測定基準が参照測定基準より高い場合に、該被検体において抗dsDNA抗体の存在及び/又は上昇が示される方法を提供する。
一又は複数のバイオマーカーの発現を試験するための方法における工程は、mRNA発現を検出するアッセイ(限定するものではないが、mRNAをcDNAに変換して、場合によってその後に核酸増幅することを含む)、酵素活性の存在を検出する酵素アッセイ、及び免疫組織化学アッセイを含む、様々なアッセイ様式で行われてもよい。場合によって、組織又は細胞の試料は疾患の組織又は細胞を含む。
いくつかの実施態様では、バイオマーカーは、表1、2および/または3に挙げるものから選択される。
実施態様
1. 被検体が、表1、2及び/又は3に挙げる遺伝子の少なくとも2つを正常な参照試料におけるそれぞれの遺伝子の発現レベルよりも多いレベルで発現する細胞を含んでいるか否かを決定することを含む方法であって、このような細胞が存在する場合に該被検体が自己免疫性疾患を有することが示される方法。
2. 自己免疫性疾患治療に対する被検体の応答性を予測する方法であって、被検体が、表1、2及び/又は3に挙げる遺伝子の少なくとも2つを正常な参照試料におけるそれぞれの遺伝子の発現レベルよりも多いレベルで発現する細胞を含んでいるか否かを決定することを含む方法であって、このような細胞が存在する場合に該被検体が自己免疫性疾患治療に応答性があることが示される方法。
3. 自己免疫性疾患について治療された被検体において微小残存病変をモニタリングするための方法であって、被検体が、表1、2及び/又は3に挙げる遺伝子の少なくとも2つを正常な参照試料におけるそれぞれの遺伝子の発現レベルよりも多いレベルで発現する細胞を含んでいるか否かを決定することを含む方法であって、このような細胞が検出される場合に微小残存病変が存在することが示される方法。
4. 被検体の自己免疫性疾患の状態を検出するための方法であって、被検体が、表1、2及び/又は3に挙げる遺伝子の少なくとも2つを正常な参照試料におけるそれぞれの遺伝子の発現レベルよりも多いレベルで発現する細胞を含んでいるか否かを決定することを含む方法であって、このような細胞が検出される場合に該被検体において自己免疫性疾患の状態の存在が示される方法。
5. 自己免疫性疾患を発症する被検体の素因を評価するための方法であって、被検体が、表1、2及び/又は3に挙げる遺伝子の少なくとも2つを正常な参照試料におけるそれぞれの遺伝子の発現レベルよりも多いレベルで発現する細胞を含んでいるか否かを決定することを含む方法であって、このような細胞が検出される場合に該被検体が自己免疫性疾患を発症する素因が示される方法。
6. 被検体において自己免疫性疾患を診断するための方法であって、被検体が、表1、2及び/又は3に挙げる遺伝子の少なくとも2つを正常な参照試料におけるそれぞれの遺伝子の発現レベルよりも多いレベルで発現する細胞を含んでいるか否かを決定することを含む方法であって、このような細胞が検出される場合に該被検体が該自己免疫性疾患を有することが示される方法。
7. 前記少なくとも2つの遺伝子が、(i) OAS3およびHERC5、または、(ii) ESPTI1およびHERC5、または、(iii) ESPTI1およびTYKI、または、HERC5およびTYKIを含む、実施態様1から6のいずれか一に記載の方法。

20. 被検体又は試料における自己免疫性疾患の存在および/または程度と相関する測定基準値を同定する方法であって、
(a) プローブセットによって表される遺伝子の発現が疾患の特徴と関係しているパターンと総合的に関連しているプローブセットのグループを評価する工程、
(b) 該プローブセットのグループの傾向に対する個々のプローブセットの一致の程度を反映する尺度に従ってプローブセットに重みを付けるウェイティング因子を作り出し、算出した平均プロファイルに対するそれぞれのプローブセットの相関係数を算出する工程、

21. 工程(a)において、前記プローブセットのグループには、ある疾患の特徴と関連するサブクラスターにおいてコアの最も緊密に相関するプローブセットの対を含むか、又はそのプローブセットの対の周りに集まるプローブセットが含まれる、実施態様20に記載の方法。
22. 工程(b)において、前記因子が、1に対して尺度をとる手段を含むZ-スコアにプローブセットのグループの発現データを変換して、つまり2を底とする対数変換を行って、1の平均値の標準偏差に対して尺度をとることによって作り出されたものである、実施態様20又は21に記載の方法。

本発明で用いるプライマー、オリゴヌクレオチド及びポリヌクレオチドは、当分野で公知の標準的な技術を用いて生成することができる。
特に定義しない限り、本明細書で使用する技術用語および科学用語はすべて、本発明が属する分野の当業者によって一般的に理解されているのと同じ意味を有する。Singleton 等, Dictionary of Microbiology and Molecular Biology 2nd ed., J. Wiley & Sons (New York, N.Y. 1994), and March, Advanced Organic Chemistry Reactions, Mechanisms and Structure 4th ed., John Wiley & Sons (New York, N.Y. 1992)により当分野の技術者は本出願に使用した多くの用語の一般的な理解が得られる。
本明細書で用いる「アレイ」又は「マイクロアレイ」なる用語は、基質上でのハイブリダイズ可能なアレイ成分、好ましくはポリヌクレオチドプローブ(例えばオリゴヌクレオチド)の規則正しい整列を指す。基質は、ガラススライドなどの固体基質、又はニトロセルロースメンブレンなどの半固体基質であってもよい。ヌクレオチド配列は、DNA、RNA又は何れかのその並べ換えでありうる。
本明細書で用いる「標的配列」、「標的核酸」又は「標的タンパク質」は、本発明の突然変異が存在すると思われる又は存在することがわかっており、その検出が望まれる、対象とするポリヌクレオチド配列である。通常、本明細書中で用いる「鋳型」は標的ヌクレオチド配列を含有するポリヌクレオチドである。場合によっては、「標的配列」、「鋳型DNA」、「鋳型ポリヌクレオチド」、「標的核酸」、「標的ポリヌクレオチド」及びその変異体が交換可能に用いられる。
第一試料における遺伝子又はバイオマーカーの発現レベル/量が、第二試料における遺伝子又はバイオマーカーの発現レベル/量の少なくともおよそ1.5倍、1.75倍、2倍、3倍、4倍、5倍、6倍、7倍、8倍、9倍又は10倍である場合に、第一試料における遺伝子又はバイオマーカーの発現/量は、第二試料におけるレベル「より多い」レベルである。発現レベル/量は、限定するものではないが、mRNA、cDNA、タンパク質、タンパク質断片および/または遺伝子コピーを含め、当分野で公知の任意の好適な基準に基づいて決定されてもよい。発現レベル/量は、質的におよび/または量的に決定されてもよい。
「プライマー」は、一般に、標的配列とハイブリダイズすることによって、対象の試料中に存在しうる標的に結合して、その後、標的に相補的なポリヌクレオチドの重合を促進する、一般に遊離した3'-OH基を有する短い単鎖のポリヌクレオチドである。
「遺伝子増幅」なる語句は、遺伝子又は遺伝子断片の複数のコピーが特定の細胞又は細胞系統において形成されるプロセスを意味する。複製された領域(増幅されたDNAのストレッチ)は、しばしば「アンプリコン」と称される。通常は、生成されるメッセンジャーRNA(mRNA)の量、即ち遺伝子発現レベルも、発現された特定遺伝子の作成されたコピー数に比例して増加する。
「阻害する」ことは、参照物質と比較して、活性、機能及び/又は量を減らす又は低くすることである。
一般的に「3'」なる用語は、同じポリヌクレオチド又はオリゴヌクレオチドの他の領域又は位置からポリヌクレオチド又はオリゴヌクレオチド3'(下流)の領域又は位置を指す。一般的に「5'」なる用語は、同じポリヌクレオチド又はオリゴヌクレオチドの他の領域又は位置からポリヌクレオチド又はオリゴヌクレオチド5'(上流)の領域又は位置を指す。
本明細書中で用いる「診断」なる用語は、分子の又は病理学的状態の同定、例えば自己免疫性疾患の同定を指す。本明細書中で用いる「予後」なる用語は、例えば、自己免疫性疾患の再発、再燃、及び薬剤耐性を含む自己免疫性疾患が原因となる疾患症状の可能性の予測を意味する。本明細書中で用いる「予測」なる用語は、患者が薬剤又は一連の薬剤に対して有利又は不利に応答する可能性を意味する。一実施態様では、予測はその応答の程度に関する。一実施態様では、予測は、例えば特定の治療的薬剤による治療、及び疾患再発のない一定期間の後に患者が生存しているか改善しているかどうか、及び/又はその可能性に関する。本発明の予測方法を用いて任意の特定の患者のために最も好適な治療様式を選択することによって、治療決定を臨床的に行うことができる。本発明の予測方法は、患者が、治療投薬計画、例えば特定の治療剤や組み合わせの投与、外科的介入、ステロイド治療などを含む特定の治療投薬計画に有利に応答するかどうか、又は治療投薬計画の後に患者が長期に生存しているかどうかを予測する際の有用なツールである。
本発明に従って用いられる場合、特定の治療薬又は治療選択に対する「耐性の増加」なる用語は、薬剤の標準的な用量又は標準的な治療手順に対する応答の減少を意味する。
本発明に従って用いられる場合、特定の治療薬又は治療選択に対する「感受性の減少」なる用語は、薬剤の標準的な用量又は標準的な治療手順に対する応答の減少を意味し、この応答の減少は薬剤の用量や治療の強度を増やすことによって、(少なくとも部分的に)補われうるものである。
「患者応答」は、限定するものではないが以下のものを含む患者に利益を示す任意のエンドポイントを使用して評価できる。(1) 緩徐化及び完全な停止を含む、ある程度の疾患進行の阻害、(2) 疾患発症及び/又は症状の数の減少、(3) 損傷サイズの減少、(4) 近接する末梢器官及び/又は組織への疾患細胞浸潤の阻害(すなわち減少、緩徐化又は完全な停止)、(5) 疾患の拡がりの阻害(すなわち減少、緩徐化又は完全な停止)、(6) 必ずではないが病変の退行又は消失が生じうる自己免疫応答の低減、(7) 疾患と関係する一又は複数の症状の、ある程度の軽減、(8) 治療後の疾患がない期間の増加、及び/又は(9) 治療後の特定の時点での死亡率の減少。
「抗体」及び「イムノグロブリン」なる用語は互換性をもって広義な意味で使われ、モノクローナル抗体(例えば完全長又は完全なモノクローナル抗体)、ポリクローナル抗体、単価抗体、多価抗体、多特異性抗体(例えば所望の生物学的活性を示す限りの二重特異性抗体)及び(本明細書で詳細に記載される)特定の抗体断片が含まれる。抗体はキメラ、ヒト、ヒト化及び/又は親和性成熟したものであり得る。
「抗体断片」は完全な抗体の一部のみを含んでなるものであり、その一部は、完全な抗体に存在する場合のその一部に通常関連する機能の少なくとも一、好ましくはその機能のほとんどないしはすべてを保持することが好ましい。一実施態様では、抗体断片は完全な抗体の抗原結合部位を含んでなるために、抗原結合能を有する。他の実施態様では、抗体断片は、例えばFc領域を含んでなるものは、完全な抗体に存在する場合のFc領域に通常関連する生物学的な機能、例えばFcRn結合、抗体半減期の調節、ADCC機能及び補体結合の少なくとも一を保持する。一実施態様では、抗体断片は、完全な抗体と実質的に類似したインビボ半減期を有する一価性抗体である。例えば、このような抗体断片は、インビボ安定性を断片に与えることができるFc配列に結合した抗原結合アームを含んでもよい。
本明細書中のモノクローナル抗体には、具体的に、重鎖及び/又は軽鎖の一部が特定の種由来の抗体あるいは特定の抗体クラス又はサブクラスに属する抗体の対応する配列と同一であるか相同であり、鎖の残りの部分が他の種由来の抗体あるいは他の抗体クラスあるいはサブクラスに属する抗体の対応する配列と同一であるか相同である「キメラ」抗体、並びにそれが所望の生物的活性を有する限りそれら抗体の断片が含まれる(米国特許第4,816,567号;及び、Morrison 等, Proc. Natl. Acad. Sci. USA 81:6851-6855 (1984))。
「親和性成熟」抗体は、その一又は複数のCDR/HVRに一又は複数の変更を有する抗体であって、そのような変更を有しない親抗体と比較して、抗原に対する抗体の親和性を向上させたものである。好ましい親和性成熟抗体は、標的抗原に対して、ナノモル単位の、さらにはピコモル単位の親和性を有する。親和成熟抗体は、当技術分野において既知の方法により生産できる。Marks等は、Bio/Technology, 10:779-783(1992年)において、VHドメインとVLドメインのシャフリングによる親和成熟を開示している。CDR/HVRおよび/またはフレームワーク残基のランダムな突然変異誘発が、Barbas他、Proc Nat. Acad. Sci, USA 91:3809-3813(1994);Schier他、Gene, 169:147-155 (1995);Yelton他、J. Immunol., 155:1994-2004 (1995);Jackson他, J. Immunol., 154(7):3310-9 (1995);およびHawkins他, J. Mol. Biol., 226:889-896 (1992)に開示されている。
ここで用いられる「細胞障害剤」という用語は、細胞の機能を阻害し又は妨害し、及び/又は細胞の破壊を引き起こす物質を称する。この用語は放射性アイソトープ(例えば、At211、I131、I125、Y90、Re186、Re188、Sm153、Bi212、P32及びLuの放射性同位元素)、化学療法剤、及び細菌性、真菌性、植物性又は動物性起源の小分子毒素又は酵素的に活性な毒素等の毒素又はその断片を含むことが意図されている。
「ブロッキング(以下「ブロック」とも言う)抗体」又は「アンタゴニスト」抗体は、結合する抗原の生物学的活性を阻害するか、減弱するものである。このようなブロッキングはレセプターへのリガンド結合などのタンパク質-タンパク質相互作用を阻害することによるなどの任意の方法によって行うことができる。一実施態様では、ブロッキング抗体又はアンタゴニスト抗体は、抗原の生物学的活性を実質的にないしは完全に阻害する。
「有効量」とは所望の治療又は予防結果を達成するために必要な用量及び時間での効果的な量を意味する。治療剤の「治療的有効量」は、例えば個体の疾病ステージ、年齢、性別、及び体重、並びに個体に所望の応答を誘発する抗体の能力などの因子に従って変わりうる。また、治療的有効量は治療剤の任意の毒性又は有害な効果よりも治療的に恩恵のある効果が上回るものである。「予防的有効量」とは所望の予防結果を達成するために必要な用量及び時間での効果的な量を意味する。典型的には必ずではないが、予防的用量は疾患の初期ステージ又はその前の患者に用いるので、予防的有効量は治療的有効量よりも少ないであろう。
本明細書において使用する「第2のヒトインターフェロンα(hIFN-α)レセプター」、「IFN-αβR」、「hIFNAR2」、「IFNAR2」、及び「Novic鎖」なる用語は、Domanski等, J. Biol. Chem., 37:21606-21611 (1995)によってクローニングされた515のアミノ酸レセプタータンパク質として定義され、これには、Domanski等による文献の21608頁の図1に示されるように、217の残基からなる細胞外ドメイン、21の残基からなる膜貫通ドメイン、及び250の残基からなる細胞間ドメインが含まれる。一実施態様では、前記の用語には、IFNAR2の細胞外ドメイン(ECD)(又はECDの断片)を含むIFNAR2の断片と、免疫グロブリン配列の少なくとも一部に融合したIFNAR2 ECD等のIFNAR2の可溶性形態が包含される。
本明細書中で用いられる「バイオマーカー」なる用語は、一般的に、遺伝子、タンパク質、糖質構造又は糖脂質を含む分子を指し、哺乳動物組織又は細胞中ないしは組織又は細胞上での該分子の発現は標準的な方法(又は本明細書中で開示される方法)によって検出されうるものであり、哺乳動物細胞又は組織の、1型インターフェロンなどのインターフェロンの阻害に基づく治療投薬計画への感受性を予測、診断及び/又は予後の予測をするものである。場合によって、前記バイオマーカーの発現は、コントロール/参照の組織又は細胞の試料について観察される発現より高いことが決定される。場合によって、例えば、前記バイオマーカーの発現は、試験組織又は細胞の試料において、PCR又はFACSアッセイにおいて、コントロール組織又は細胞の試料について観察される発現の少なくともおよそ5倍、少なくともおよそ10倍、少なくともおよそ20倍、少なくともおよそ30倍、少なくともおよそ40倍、少なくともおよそ50倍、又は好ましくは少なくともおよそ100倍であることが決定されるであろう。場合によって、前記バイオマーカーの発現は、IHCアッセイにおいて染色強度について少なくとも2以上のスコアであることが決定されるであろう。場合によって、前記バイオマーカーの発現は遺伝子チップに基づくアッセイを用いて決定されるであろう。
本明細書中で用いる「試料」なる用語は、例えば理学的、生化学的、化学的及び/又は生理学的特徴に基づいて特性を示す又は同定される、細胞実体及び/又は他の分子実体を含有する対象とする被検体から得られる、又は対象とする被検体由来の組成物を指す。例えば、「疾患試料」なる表現及びこの変形は、特徴付けられている細胞実体及び/又は分子実体を含むことが予測される、又はそうであることが知られている対象の被検体から得た任意の試料を指す。
「相関」又は「相関する」は、任意の方法で、第一の分析又はプロトコルの成績及び/又は結果を、第二の分析又はプロトコルの成績及び/又は結果と比較することを意味する。例えば、第二のプロトコルを行う際に第一の分析又はプロトコルの結果を用いてもよいし、及び/又は第一の分析又はプロトコルの結果を用いて、第二の分析又はプロトコルを行うかどうかを決定してもよい。遺伝子発現分析又はプロトコルの実施態様に関し、遺伝子発現分析又はプロトコルの結果を用いて、特定の治療投薬計画を実行するかどうかを決定してもよい。
本明細書中で用いられる「標識」なる用語は、核酸プローブや抗体などの試薬に直接的又は間接的にコンジュゲートないしは融合され、コンジュゲートないしは融合した試薬の検出を容易にする化合物又は組成物を指す。標識自体が検出可能なもの(例えば放射性標識又は蛍光性標識)であってもよく、酵素標識の場合、検出可能な基質化合物ないしは組成物の化学的変化を触媒するものであってもよい。
標的分子を含む試料は当分野で公知の方法によって採取されてもよく、その方法は対象とする疾患の特定のタイプや位置に適切なものである。罹患組織の代表的な一片を採取するために組織生検が用いられることが多い。あるいは、細胞は、対象の罹患細胞を含むことがわかっているか又は含むと考えられる組織/体液の形態で間接的に得られてもよい。例えば、疾患病変の試料は、切除術、気管支鏡検査法、細針吸引、気管支擦過によって、あるいは痰、肋膜体液又は血液から採取されてもよい。遺伝子又は遺伝子産物は、罹患組織から、あるいは尿、痰又は血清などの他の身体試料から検出されうる。疾患試料中の標的遺伝子又は遺伝子産物の検出について上記に述べた同じ技術は他の身体試料に応用されうる。罹患細胞は、疾患病変から脱離し、前記身体試料中に見られる。前記身体組織をスクリーニングすることによって、これら疾患のために簡便で迅速な診断法が達成されうる。加えて、治療の経過は、前記身体試料を標的遺伝子又は遺伝子産物について試験することによって、より容易にモニターされうる。
罹患細胞について組織調製物を濃縮するための手段は当分野で公知である。例えば、組織は、パラフィン又はクリオスタット切片から単離してもよい。また、罹患細胞は、フローサイトメトリー又はレーザー捕獲顕微解剖によって正常細胞から切り離されてもよい。これら、並びに罹患細胞を正常細胞から切り離すための他の技術は当分野で公知である。罹患組織が正常細胞と非常に混合している場合、混合および/または偽陽性/陰性結果を最小化するための技術は公知であり、そのいくつかは本明細書中の以下に記載されるが、シグネチャー遺伝子発現プロファイルの検出はより難しいかもしれない。例えば、また、試料は、対象の罹患細胞と関係しているが対応する正常細胞とは関係していない(又は逆も然り)ことがわかっているバイオマーカー(突然変異を含む)の存在について評価してもよい。
核酸をガラススライドなどの固形基質に接着させるための技術は当分野で多くのものが周知である。ある方法は、アミン基、アミン基の誘導体又は正荷電した他の基などの固形基質に接着することができる分子を含む修飾した塩基又は類似体を、合成された核酸分子に組み込むことである。その後、合成された生成物は、増幅された生成物上にあり、ガラススライドに共有結合する反応基と共有結合を形成しうる他の反応基又はアルデヒドによってコートされるガラススライドなどの固形基質と反応させる。アミノプロピルシリカ界面化学を用いた方法などの他の方法は、http://www.cmt.corning.com and http://cmgm.stanford.edu/pbrown1に開示されるように当分野で公知である。
用いた技術に必要であれば、及び/又は用いた技術に許容されるならば、固形基質への接着の前又は後に、断片への切断により又は検出可能な標識の付着によって、増幅された核酸がさらに修飾されてもよい。
本明細書において開示される方法およびアッセイは、哺乳動物の組織又は細胞の試料における一又は複数のバイオマーカーの発現の試験に関するものであり、このとき一又は複数の該バイオマーカーの発現によって、組織又は細胞の試料がインターフェロンインヒビターの使用に基づく治療に感受性があるか否かが予測される又は示される。前記方法およびアッセイには、表1、2および/または3に挙げるものの一又は複数などのバイオマーカーの発現を調べるものが含まれる。
上記のように、様々な自己免疫性疾患と関係している1型インターフェロンなどのインターフェロンの異常な発現と関係している罹患したヒト細胞種の集団がいくつかある。したがって、開示した方法及びアッセイにより、患者を治療するために適切又は効果的な治療法を評価する際に有用なデータ及び情報を得るための、簡便、効果的及び費用効果が良いと思われる手段が提供されると考える。例えば、免疫関連の症状と診断されている患者は、組織又は細胞の試料を得るために行われた生検を持ちうる。この試料を、様々なインビトロアッセイによって試験し、患者の細胞がインターフェロンインヒビター(例えば、抗インターフェロンα抗体又はインターフェロンαレセプターに対する抗体)などの治療剤に感受性があるか否かを決定することができる。
以下に示すように、試料における様々なバイオマーカーの発現は、多くの方法論によって分析されてもよく、これらの多くは当分野で公知かつ当業者に理解されるものであり、これらには限定するものではないが、免疫組織化学および/またはウェスタン分析、定量的な血液に基づくアッセイ(例えば血清ELISAなど)(例えば、タンパク質発現のレベルを調べるために)、生化学酵素活性アッセイ、インサイツハイブリダイゼーション、mRNAのPCR分析及び/又はノーザン分析、並びに遺伝子及び/又は組織アレイ分析によって実施することができる非常に様々なアッセイの一つが含まれる。遺伝子及び遺伝子生成物の状態を評価するための代表的なプロトコールは、例としてAusubel et al. eds., 1995, Current Protocols In Molecular Biology、ユニット2(ノーザンブロッティング)、4(ウェスタンブロッティング)、15(イムノブロッティング)および18(PCR分析)にみられる。
試料において特定のバイオマーカー、例えば表1、2および/または3に挙げるものを検出することに関する以下のプロトコールは例示として挙げる。
本発明の特定の実施態様では、試料におけるIRGの発現は、免疫組織化学及び染色プロトコールを用いて試験される。組織切片の免疫組織化学染色は、試料中のタンパク質の存在を評価ないしは検出するための確実な方法であることが示されている。免疫組織学法(「IHC」)技術は、抗体を用いて、一般的には色素生産性方法または蛍光性方法によって、インサイツで細胞性抗原を探索して視覚化する。
前記組織試料は従来の方法によって固定(すなわち保存)されてもよい(例として、“Manual of Histological Staining Method of the Armed Forces Institute of Pathology,” 3rd edition (1960) Lee G. Luna, HT (ASCP) Editor, The Blakston Division McGraw-Hill Book Company, New York; The Armed Forces Institute of Pathology Advanced Laboratory Methods in Histology and Pathology (1994) Ulreka V. Mikel, Editor, Armed Forces Institute of Pathology, American Registry of Pathology, Washington, D.C.を参照)。当分野の技術者は、組織学的染色ないしは他の分析に供する試料の目的に応じて固定液を選択することは理解するところであろう。また、当分野の技術者は、組織試料の大きさおよび用いる固定液に応じて固定の長さを決定することも理解するであろう。実施例では、中性緩衝ホルマリン、ブアン固定液またはパラホルムアルデヒドを用いて試料を固定してもよい。
包埋材料としてパラフィンを用いた場合、組織切片は通常、脱パラフィン化して、水に再水和させる。組織切片は、いくつかの従来の標準的な方法によって、脱パラフィン化してもよい。例えば、キシレンおよび段階的に減少するアルコールを用いてもよい(例として、上掲の"Manual of Histological Staining Method of the Armed Forces Institute of Pathology"を参照)。別法として、Hemo-De7(CMS, Houston, Texas)などの市販の脱パラフィン化非有機薬剤が用いられてもよい。
一般的に、免疫組織化学に使用する一次および/または二次抗体は、検出可能な成分にて標識されるであろう。通常、以下の種類に分類できる多くの標識が利用可能である:
(b) コロイド金粒子
(c) 希有土類キレート(ユウロピウムキレート)、テキサスレッド、ローダミン、フルオレセイン、ダンシル、リサミン、ウンベリフェロン、フィコクリセリン(phycocrytherin)、フィコシアニン又はSPECTRUM ORANGE7およびSPECTRUM GREEN7などの市販の蛍光体および/または上記の何れか一ないしは複数の誘導体を含むが、これらに限定されるものではない蛍光標識。蛍光標識は、例えば、上記のImmunologyのCurrent Protocolsに開示される技術を用いて抗体にコンジュゲートすることができる。蛍光は、蛍光計を用いて定量化することができる。
(d) 様々な酵素基質標識が利用可能であり、米国特許第4,275,149号にはこの概説がある。一般に、酵素は、様々な技術を用いて測定することができる色素生産性基質の化学変化を触媒する。例えば、酵素は、分光測光法で測定することができる基質の変色を触媒するかもしれない。あるいは、酵素は、基質の蛍光又は化学発光を変えうる。蛍光の変化を定量化する技術は上記の通りである。化学発光基質は、化学反応によって電子的に励起され、測定することができる(例えば化学発光計測器を用いて)か、またはエネルギーを蛍光アクセプターに与える光を発しうる。酵素標識の例には、ルシフェラーゼ(例えば、ホタルルシフェラーゼおよび細菌ルシフェラーゼ;米国特許第4737456号)、ルシフェリン、2,3-ジヒドロフタルアジネジオン(dihydrophthalazinediones)、リンゴ酸デヒドロゲナーゼ、ウレアーゼ、西洋わさびペルオキシダーゼ(HRPO)などのペルオキシダーゼ、アルカリホスファターゼ、β-ガラクトシダーゼ、グルコアミラーゼ、リソチーム、サッカライドオキシダーゼ(例えばグルコースオキシダーゼ、ガラクトースオキシダーゼおよびグルコース-6-リン酸デヒドロゲナーゼ)、複素環のオキシダーゼ(例えばウリカーゼおよびキサンチンオキシダーゼ)、ラクトペルオキシダーゼ、ミクロペルオキシダーゼなどが含まれる。抗体に酵素をコンジュゲートする技術は、O'Sullivanら., Methods for the Preparation of Enzyme-Antibody Conjugates for use in Enzyme Immunoassay, in Methods in Enzym. (ed J. Langone & H. Van Vunakis), Academic press, New York, 73:147-166 (1981)に記載されている。
(i) 基質として水素ペルオキシダーゼを有する西洋わさびペルオキシダーゼ(HRPO)、ここで水素ペルオキシダーゼが染料前駆体(例えば、オルソフェニレン(orthophenylene)ジアミン(OPD)又は3,3',5,5'テトラメチルのベンジジン塩酸塩(TMB))を酸化する;
(ii) 色素生産性基質としてリン酸パラグラフ-ニトロフェニルを有するアルカリホスファターゼ(AP);及び

多数の他の酵素基質の組合せは当業者にとって利用可能である。これらの一般的な概要については、米国特許第4275149号および同第4318980号を参照。標識は、抗体と間接的にコンジュゲートされることがある。これを行うための様々な技術は当分野の技術者に公知である。例えば、抗体は、ビオチンとコンジュゲートさせることができ、前述した大きな4つの分類のうちの何れかはアビジンとコンジュゲートさせることができ、その逆もまた可能である。ビオチンは選択的にアビジンと結合し、したがって、標識はこの間接的な方法で抗体にコンジュゲートさせることができる。あるいは、抗体と標識を間接的にコンジュゲートさせるために、抗体は小ハプテンとコンジュゲートさせ、前述した標識の異なるタイプのうちの1つは抗ハプテン抗体とコンジュゲートさせる。したがって、抗体と標識は間接的にコンジュゲートすることができる。
場合によって行うブロック処置の後に、一次抗体が組織試料中の標的タンパク質抗原と結合するような好適な条件下と十分な時間、組織切片を一次抗体に曝露させる。これを達成するための好適な条件は慣例的な実験によって決定できる。試料に対する抗体の結合の範囲は、上記の検出可能な標識の何れか一つを用いて決定される。標識は、3,3'-ジアミノベンジジンクロモゲンなどの色素生産性基質の化学変化を触媒する酵素標識(例えばHRPO)であることが望ましい。好ましくは、酵素標識は、一次抗体(例えば、一次抗体はウサギポリクローナル抗体であり、二次抗体はヤギ抗ウサギ抗体である)に特異的に結合する抗体にコンジュゲートさせる。
場合によって、IRGの発現を検出するためのIHC分析に用いられる抗体は、対象のIRGに主に結合するように生成された抗体である。場合によって、抗IRG抗体はモノクローナル抗体である。抗IRG抗体は様々な販売元からなど、当分野で容易に入手可能であり、また、当分野で公知の慣例的な技術を用いて生成されうる。

さらに、本発明の方法には、組織又は細胞の試料中の、mRNA、例えばIRG mRNAの存在および/または発現を調べるプロトコールが含まれる。細胞中のmRNAの評価のための方法は周知であり、例えば、相補的DNAプローブを用いたハイブリダイゼーションアッセイ(例えば、標識したIRGリボプローブを用いたインサイツハイブリダイゼーション、ノーザンブロットおよび関連した技術)および様々な核酸増幅アッセイ(例えば、IRGに特異的な相補的プライマーを用いたRT-PCRおよび、他の増幅型の検出法、例えば枝分れDNA、SISBA、TMAなど)が含まれる。
哺乳動物の組織又は細胞の試料は、例えばノーザン、ドットブロットまたはPCR分析を用いて、IRGのmRNAについて都合よくアッセイすることができる。例えば、定量的PCRアッセイなどのRT-PCRアッセイは公知技術である。本発明の例示的実施態様では、生体試料中のIRGのmRNAの検出方法は、少なくとも一のプライマーを用いて逆転写によって、試料からcDNAを生成し、該IRGのcDNAを増幅するために、IRGのポリヌクレオチドをセンスプライマーおよびアンチセンスプライマーとして用いて産生された該cDNAを増幅し、そして、増幅されたIRGのcDNAの存在を検出することを含む。加えて、このような方法は、生体試料中のIRGのmRNAのレベルを決定し得る一ないし複数の工程(例えば、アクチンファミリメンバーなどの「ハウスキーピング」遺伝子のコントロールmRNA配列と該レベルを同時に検討すること)を含んでもよい。場合によって、増幅されたIRGのcDNAの配列を決定してもよい。
また、組織又は細胞の試料における選択されたバイオマーカーの発現は、機能的又は活性に基づくアッセイによって調べられてもよい。例えば、バイオマーカーが酵素である場合、組織又は細胞の試料において特定の酵素活性の存在を決定するかまたは検出するために当分野で公知のアッセイを実行してもよい。
組織又は細胞の試料が、インターフェロンインヒビターによる治療に感受性があることを表すバイオマーカーの一又は複数を発現するという決定に続いて、有効量のインターフェロンインヒビターが、哺乳動物を悩ましている自己免疫性疾患などの疾患を治療するために該哺乳動物に投与されてもよいことが考慮される。本明細書中に記載の様々な病的状態の哺乳動物の診断は、熟練した専門家によってなされうる。診断技術は、例えば、哺乳動物の自己免疫関連の疾患の診断又は検出を可能とする当分野で利用可能である。
インターフェロンインヒビターの投与にとって有効な用量とスケジュールは、経験的に決定することができ、そのような決定を行うことは当業者の技量の範囲にある。一回又は複数回服用を用いることができる。例えば、単独で使用されるインターフェロンインヒビターの効果的な用量又は量は、1日当り体重の約1μg/kgから約100mg/kg又はそれより多い範囲であると現在考えられている。用量の種間スケーリングは、例えば、Mordenti等, Pharmaceut. Res., 8:1351(1991)に開示されているような、当該分野において既知の方法を用いて実施することができる。
インターフェロンインヒビターのインビボ投与が用いられる場合、正常な投与量は、投与経路に応じて、哺乳動物の体重当たり1日に約10ng/kgから100mg/kgの範囲又は1日当たりより多く、好ましくは約1μg/kg/日から10mg/kg/日とすることができる。特定の用量及び送達方法の指針は文献に与えられている;例えば、米国特許第4657760号、第5206344号、又は第5225212号を参照のこと。異なる製剤が異なる治療用化合物及び異なる疾患に有効であること、例えば一つの器官又は組織を標的とする投与には、他の器官又は組織とは異なる方式で送達することが必要であることが予想される。
さらに付加的な治療が本方法において使用され得ることを考慮する。一又は複数の他の治療には、限定されるものではないが、ステロイドの投与及び問題とする特定の自己免疫性疾患のための他の標準的な治療投薬計画が含まれる。このような他の治療法を、インターフェロンインヒビターとは異なる試薬として用いられうることが考えられる。
典型的に、本発明のキットは、上記の容器と、商業的及び使用者の観点からみて望ましい物質、例えばバッファ、希釈液、フィルター、針、注射器及び使用のための指示書を有するパッケージ挿入物を収容する一ないし複数のその他の容器とを具備する。特定の治療又は非治療的用途に該組成物が使用されることを示すために容器上にラベルがあってもよく、またそのラベルは上記のようなインビボの使用又はインビトロの使用の何れかについての指導を示すものであってもよい。
他の実施態様は、容器と、該容器上のラベルと、該容器内に収容される組成物を具備するキットであり、この場合の組成物はストリンジェントな条件下でIRGのポリヌクレオチドの相補鎖とハイブリダイズするポリヌクレオチドを含有するものであり、該容器上のラベルは、該組成物を用いて少なくとも一種類の哺乳動物細胞中のIRGの存在を評価することができることと、少なくとも一種類の哺乳動物細胞中のIRGのRNA又はDNAの存在を評価するためのIRGのポリヌクレオチドの使用についての指示書を示すものである。
キットの他の任意の成分には、一ないし複数のバッファ(例えばブロックバッファ、洗浄バッファ、基質バッファなど)、酵素標識によって化学的に変化する基質などの他の試薬(例えば色素原)、エピトープ探索溶液、コントロール試料(ポジティブコントロール及び/又はネガティブコントロール)、コントロールスライド(一ないし複数)などがある。
IFN-α応答遺伝子(IRG)の発現は、SLE患者(活動期疾患及び非活動期疾患の患者)及び、University Of Minnesota (Minneapolis, MN)からの正常ドナーから採取した血液、つまり末梢血単核球(PBMC)から得たデータにおいて分析した。
データは以下の通りに作出した。92の血液試料は、18人の活動期SLE患者からの異なるデータを集めたものであり、19の血液試料は5人の非活動期SLE患者からの異なるデータを集めたものであり、4の血液試料は4人の健康なドナーから集められたものである。PBMCは、標準的なフィコール勾配遠心分離法によって全血液から単離した。RNAは、Qiagen (Valencia, CA)からRNA単離キットを用いてPBMC試料から調製し、Agilent (Palo Alto, CA)のWHGオリゴヌクレオチドマイクロアレイチップにハイブリダイズさせた。生データは、標準的なアラインメントFeature Extractionによって処理しアラインメントログ比データを求めた。IFN-αに応答した遺伝子の正常な発現は、健康なドナーからPBMCを単離し、100U/mの組み換えIFN-αとともに4時間培養物中で培養し、そして、IFN-α添加の4、12、28及び52時間後の細胞培養物の試料を採取することによって試験した。
マイクロアレイデータは、一番上の70パーセンタイルの平均シグナルと一番上の70パーセンタイルの変動の係数とを有するプローブについて、xクラスターソフトウェアプログラム(log2シグナルに対するピアソン)を用いて二次元(試料とプローブ)に階層的にクラスター化させた。クラスターデータは、ジャバTreeviewソフトウェアプログラムによって観察した。R(http://www [insert period] r-project [insert period]org/)、JMP(SAS Institute, Cary, NC)及びエクセル(Microsoft, Redmond, WA)を用いて数値的な分析を行った。
すべての試料のマイクロアレイクラスタリングにより、試料と遺伝子の意味のあるグループ化が示された。試料クラスタリングにより、大部分の活動期SLE患者のグループ化が示された。遺伝子クラスタリングにより、明確な生物学的パターンを有する様々な緊密にグループ化された遺伝子サブクラスターが明らかになった。例えば、あるクラスターはB細胞に、また他のクラスターは好中球に特異的であることがわかっている遺伝子について、他のクラスターは抗体について、そして他のクラスターはIGRについて非常に濃縮(enrich)されていた。IRGサブクラスターは、試料に関して興味深いパターンを示した。すべての正常な試料はIRGの発現が低いのに対して、SLE試料は正常様から非常に高い範囲にまで変化する広範囲な発現を示した。
密接なサブクラスター内のプローブの発現プロファイルは非常に類似していて識別できない。非常に類似したプロファイル間の差異はかなり、生物学的な原因か又は技術的な原因によるノイズによるものであるかもしれない。例えば、いくつかの遺伝子は複数のプローブによってマイクロアレイに表され、同じ遺伝子の発現を表すIRGサブクラスター領域に数対のプローブがある。この場合、互いに近くにクラスター化されたプローブは非常に隣接していることが多い。したがって、明らかなパターンが存在し、多くのプローブに反映されること、そしてデータのノイズの干渉を低減するためにいくつかのプローブからのデータを利用することにより最もはっきりとパターンを識別することができることが明らかとなった。しかしながら、識別された遺伝子は、疾患の存在と相関する遺伝的識別子として個別に用いられてもよい。
発現がインターフェロンαの存在によって非常に誘導される遺伝子を同定するために、健康なドナーのPBMC試料を組み換えインターフェロンαにより処置し、細胞培養物の試料を上記のAgilent WHG発現分析に供した。これらのハイブリダイゼーションのログ比率データを双方向分散分析(時間と処置)によって分析し、処置p値<5×10−7のフィルタリングによって142プローブを同定した。このセットの遺伝子は発現がインターフェロンαによって誘導される遺伝子のサブセットであり、コクラスタリングについて共通の原則がインターフェロンαによる誘導である他の実験において遺伝子のクラスターを同定するための有効な手段となる。
IRGにおける転写活性化のパターンは、特定のサブグループのプローブのAgilent比率のレベルに対する単一の測定基準の比率を算出することによって測定した。例として、本発明者等はIRGプローブを用いた以下の手法を記載する。まず、SLE試料とコントロール試料のクラスターヒートマップを有するPBMC試料に、インターフェロンαによって誘導されるプローブの密度プロットを整列させることによってパターン(IRGの集合体プロファイル)を定義した。2つの最も高く相関したプローブから始めて、このプローブセットがその中心に最も明らかな発現シグネチャーを含むように(しかしこの時点では異なるシグネチャーの意味ある貢献を含んでいなくともよい)、次に最も高く相関したプローブか又は分枝したプローブを加えてセットを拡げることによって、プローブをIRGとして定義した。このセットは表1に挙げる35のプローブからなる。
そして、このグループの発現データをz-スコア(平均を1に対して尺度をとり(scale)、2を底とする対数に変換し、1の標準偏差に対して尺度をとったもの)に変換し、平均プロファイルに対する各々のプローブのプロファイルの相関係数を算出した。これらの相関係数をウェイティング因子として用いて、グループの傾向に強く一致するプローブを相対的に重くウェイティングし、他の情報やノイズに明らかに影響を受けるものを相対的に軽くウェイティングした。
プローブを1に対して尺度をとるために必要な因子に、ウェイティング因子を乗じて、単一ハイブリダイゼーションについて正規化してウェイティングした測定基準を得ることができる混成因子を作出した。正常な血液試料のシグネチャーに、プローブと試料について平均化した因子を乗じた。この数を逆数にして、試料から得たプローブの平均の結果を、健康なドナーから得た試料を1にした場合の測定基準に変換する全体のスケーリング因子を求めた。。各々の正規化した因子/ウェイティング因子にこの因子を乗じた。結果は、試料発現シグネチャーを乗じて、平均を取り、試料におけるIFN-α転写応答のレベルを測定する単一の測定基準であるI型インターフェロン応答遺伝子測定基準(IRGM)を求めた、スカラー値のベクトルであった。
SLEDAIなどのSLE疾患の活性と重症度の臨床測定は患者の疾患症状を数量化するものであって、疾患の病因となる遺伝子の発現と相関しうる。この仮定を調べるために、個々の患者のIRGMデータを患者の臨床スコアとlab試験結果と比較した。IRGMとSLEDAIとの間で有意な相関は観察されなかったが、活動期SLE患者の多くにおいて血清中の抗dsDNA抗体の力価がIRGMと非常に相関した(図3)。この相関性は、他のアッセイの代用になるいずれかのアッセイの基準といえる。また、SLEの治療の合理的な設定の基準として役立ちうる生物学的関係を示す。
IRGM試験と、このような試験を構成する遺伝子の発現(表1に示す)は、相対的に高いIRGMスコアを有するために遮断されうるIFN-αシグナル伝達を有する患者を同定することによる、自己免疫性疾患(例えばSLE)のためのIFN-αをベースとする治療により利益を得る患者を選別するために有用である。同等に、患者によっては高いIRGMスコアを表さないために、破壊されうる活性なIFN-αシグナル伝達を現在のところ持っていないので、IFN-αベースの治療から利益を得ないことを予測するために用いられてもよい。
IRGM試験と、このような試験を構成する遺伝子の発現(表1に示す)は、上記のような様々な薬剤開発、診断、予後予測及び治療設定における有用な指標である。例えば、この情報を用いて、抗IFN-α治療に十分に応答した患者が処置前にIFN-αのシグナル伝達標的の発現のレベルが高いか否か、その後にその処置によりその発現が無くなったか否かを調べることができる。特定の処置がIFN-αシグナル伝達経路にどの程度作用するかについての有用な基準であろう。時間とともに治療の効果のプロファイルを測定することは、有用な生物関連マーカー又は薬力学的マーカーであるかもしれない。
上記の測定基準を基にした手法は、疾患経路、作用機構および薬物動態を特徴付ける際の様々な方法に利用されうる例えば、異なるインターフェロン分子はおそらく、異なるマイクロアレイデータ及び/又は分析に基づいて同じ方法で作られたIRGM及び/又は試験により測定及び解明が促されるような異なる特性を有する。例えば:
1) I型インターフェロンはすべて同じヘテロダイマーレセプターによりシグナル伝達されるが、それらの半減期、レセプター親和性、又は標的細胞においてシグナル伝達を開始する力は異なってもよい。これらの大きさの相違はIRGMによって容易かつ正確に測定されうる。この種の測定値は、細胞培養実験又は臨床場面において実行されうる。同様に、臨床場面において使用される候補薬物又は薬剤の作用はこの手法を用いて測定されてもよい。
2) 異なるインターフェロンにより処置した培養血液試料のマイクロアレイアッセイによって、異なるIRGM様試験を行うことができる。試験が互いに異なる限りにおいて、臨床試料に応用して、異なるインターフェロンおよび/または薬剤の相対活量を決定することができた。
また、IRGM試験を生成するために使用する方法は、細胞の状態ないしは活性または細胞(一又は複数)の種類の、いずれかの発現シグネチャーに応用されてもよい。例えば、SLE患者の中には、プラズマ細胞による抗体産生の指標である免疫グロブリン遺伝子発現の顕著な上方制御を示す者がいる。これらの遺伝子の発現を表すマイクロアレイプローブは、プラズマ細胞活性および抗体産生の全体のレベルの測定値の算出をまとめて立証しうる。他の例では、活動中の有糸分裂細胞複製と関係している特定の転写変化がある。これらの転写変化は、どの程度活性に作用しているかを測定するために様々な生物学的試料に適用される試験に集約されてもよい。あるいは、他の例では、特定の種類の免疫細胞に特異的な発現を有する遺伝子は、どの種類の細胞がそれらを発現するかによって分類することができ、各々の種類の細胞について試験を行うことができる。その後、試験のこの情報を様々な臨床試料(SLE患者の血液、クローン病患者の腸管生検など)のいずれかに応用して、免疫細胞の種類のバランスを決定することができる。
材料と方法
IFN-α応答遺伝子(IRG)の発現は、Gene Logic Inc. (Gaithersburg, MD)によって得られる健康なドナーとSLE患者の白血球(WBC)からのデータにおいて分析した。
データは以下の通りに作出した。活動期SLE患者から72の血液試料を集め、健康なドナーから46の血液試料を集めた。RNAは、Qiagen (Valencia, CA)のRNA単離キットを用いてWBC試料から調製し、Affymetrix, Inc. (Santa Clara, CA)のHGU133オリゴヌクレオチドマイクロアレイチップにハイブリダイズさせた。生データをAffymetrix MAS5.0 Feature Extractionによって処理しシグナルデータを求めた。

このセットのすべてのプローブとそれらの対応する遺伝子は、I型インターフェロンに対する血液細胞の応答のレベルに関する有用なマーカーである。これらマーカーは、インターフェロンシグネチャー測定基準(ISM)を作出するために既に示したように、個々の応答又はいずれかの数と組み合わせでの応答に有益である。この目的のための発現レベルの測定は、様々な標準的な技術のいずれか、例えば発現マイクロアレイ(例えばAffymetrix HGU133のような市販のアレイ)又はリアルタイムPCR(例えばTaqman)を用いて効率よく達成することができる。
本明細書中で同定された一又は複数の遺伝子を含む遺伝子の組み合わせがインターフェロン応答遺伝子シグネチャーとどうの程度相関するかをさらに評価するために、24の選択した遺伝子の総当たり3遺伝子組み合わせ(表4A)のピアソン相関を評価した。データを表4Bに示す。
Qiagen/PreAnalytix(Valencia, CA)のPAXgeneチューブを用いて、35のSLE試料と10の健康なドナーの全血液を集めた。Qiagen/PreAnalytix(Valencia, CA)の血液RNA単離キットを用いてRNAを調製し、24のインターフェロン-α(IFN)の応答遺伝子の発現を、慣例的な方法を用いて、例えば、ABI (Foster City, CA)のTaqMan試薬とプライマー/プローブを用いてアッセイした。RPL19に発現を正規化して相対的な量を決定した。1つの「健康な」ドナー試料が、おそらく少し前にウイルス感染したことによってIFN応答遺伝子を異常に高く発現していたために、分析から外した。以下の方法でインターフェロンシグネチャー測定基準(ISM)スコアを決定した。
1.正常な試料において各々の遺伝子の平均発現を算出した(「平均正常発現」)。
2.平均正常発現(ステップ#1)に対して相対的な発現の比率を表に表す。
3.1セットの遺伝子を用いて各試料についてのISMスコアを決定する。ISMスコアは、所定の試料における遺伝子セットについての発現比率(ステップ#2)の平均とした。
24のIFNα応答遺伝子から、2024の異なる3-遺伝子サブセットを生成することができた。各々の可能な2024の3-遺伝子組み合せについて、3-遺伝子ISMスコアと24-遺伝子ISMスコアとの間のピアソン相関を算出した。R(http://www [insert period] r-project [insert period] org/)を用いてすべての数的分析を行った。
ほとんどの健康なドナー試料が1に近いISMスコアであったのに対して、有意な割合のSLE患者は顕著に高いISMスコアであった。さらに、すべての3-遺伝子組み合せISMスコアは、24-遺伝子ISMスコアの非常に質の高い代用物として機能した。24-遺伝子ISMスコアとの3-遺伝子ISMスコアの相関性を図5の棒グラフに示す。最も低いピアソン相関性は0.73であり、相関性の70%は0.95より大きかった。
表4Bから明らかなように、すべての組み合わせは有意な相関値を示し、最も低い値はおよそ0.73であった。これは、本明細書中に開示した遺伝子の疾患マーカーとしての有用性と可動性(フレキシビリティ)を示す。すべてではないが24の選択した遺伝子のほとんどは表1、2および/または3に示すものである。表1、2および/または3に挙げていない遺伝子(一又は複数)を含む組み合わせを考慮しても、高い相関性が観察されたことから、本明細書中に開示した遺伝子の疾患マーカーとしての有用性と広い適用性がさらに確認された。

- 被検体においてSLEの存在を決定するための下記遺伝子の組み合わせを発現する細胞の存在を検出する方法であって、被検体から入手した試料の細胞が、IFI44L、CIG5およびIFI44を含む遺伝子の組み合わせを正常な参照試料におけるそれぞれの遺伝子の発現レベルよりも多いレベルで発現するか否かを決定することを含み、該遺伝子の組み合わせが表1に列挙する他の遺伝子のいずれも含んでおらず、このとき多いレベルで遺伝子の組み合わせを発現する細胞の存在が該被検体がSLEを持つことを示す、方法。
- ハウスキーピング遺伝子の使用をさらに含む、請求項1に記載の方法。
- ハウスキーピング遺伝子がRPL19である、請求項2に記載の方法。

- SLE治療に対する被検体の応答性を予測するための下記遺伝子の組み合わせを発現する細胞の存在を検出する方法であって、被検体から入手した試料の細胞が、IFI44L、CIG5およびIFI44を含む遺伝子の組み合わせを正常な参照試料におけるそれぞれの遺伝子の発現レベルよりも多いレベルで発現するか否かを決定することを含み、該遺伝子の組み合わせが表1に列挙する他の遺伝子のいずれも含んでおらず、このとき多いレベルで遺伝子の組み合わせを発現する細胞の検出が該被検体がSLE治療に応答することを示す、方法。
- SLEについて治療された被検体中の微小残存病変をモニタリングするための下記遺伝子の組み合わせを発現する細胞の存在を検出する方法であって、被検体から入手した試料の細胞が、IFI44L、CIG5およびIFI44を含む遺伝子の組み合わせを正常な参照試料におけるそれぞれの遺伝子の発現レベルよりも多いレベルで発現するか否かを決定することを含み、該遺伝子の組み合わせが表1に列挙する他の遺伝子のいずれも含んでおらず、このとき多いレベルで遺伝子の組み合わせを発現する細胞の検出が微小残存病変の存在を示す、方法。
- 被検体中のSLE類似症状を検出するための下記遺伝子の組み合わせを発現する細胞の存在を検出する方法であって、被検体から入手した試料の細胞が、IFI44L、CIG5およびIFI44を含む遺伝子の組み合わせを正常な参照試料におけるそれぞれの遺伝子の発現レベルよりも多いレベルで発現するか否かを決定することを含み、該遺伝子の組み合わせが表1に列挙する他の遺伝子のいずれも含んでおらず、このとき多いレベルで遺伝子の組み合わせを発現する細胞の検出が該被検体中にSLE類似症状があることを示す、方法。
- SLEを発症する被検体の素因を評価するための下記遺伝子の組み合わせを発現する細胞の存在を検出する方法であって、被検体から入手した試料の細胞が、IFI44L、CIG5およびIFI44を含む遺伝子の組み合わせを正常な参照試料におけるそれぞれの遺伝子の発現レベルよりも多いレベルで発現するか否かを決定することを含み、該遺伝子の組み合わせが表1に列挙する他の遺伝子のいずれも含んでおらず、このとき多いレベルで遺伝子の組み合わせを発現する細胞の検出が該被検体にSLEを発症する素因があることを示す、方法。
- ハウスキーピング遺伝子の使用をさらに含む、請求項10から13のいずれか一項に記載の方法。
- ハウスキーピング遺伝子がRPL19である、請求項14に記載の方法。
