脳組織(皮質および脳幹)を採取し、従前に記載されている通りに、アッセイタンパク質抽出物ごとに新しく調製する(Spillmann et al. 1998, Identification and characterization of a bovine neurite growth inhibitor (bNI-220), J Biol Chem. 1998 Jul 24;273(30):19283-93)。簡単に言えば、凍結組織片(例えば0.25g)を、プロテアーゼ遮断剤(10μg/mlアプロチニン−5μg/mlロイペプチン−1μg/mlペプスタチン−1mM PMSF)を含む3〜4倍容量の60mM Chaps−20mM Tris pH8.0−1mM EDTA中、4℃でホモジナイズする。ホモジネートを回転装置に4℃で30分間入れ、4℃にて100’000gで45分間、TLA100.3ローター(Beckman TL-100超遠心機)で遠心分離する。上清から、吸光分光光度計を用いてタンパク質濃度を測定する。
成体Lewisラットを、脊髄の背側半分を両側的に第8胸椎のレベルで横に石灰することにより、顕微手術的に傷つける。椎弓切除術、麻酔および手術は、Schnell and Schwab 1993 (Eur. J. Neurosci. 5: 1156-1171)に記載されている。
本発明の結合分子をIgGとして精製し、PBS中で3mg/mlに濃縮する。マウス血清由来IgG(Chemicon Int., Temecula/CA, USA)またはコムギオーキシンに対するmAB(AMS Biotechnology, Oxon/UK)を対照処理として使用する。この研究では、2匹の雄成体マカクザル(カニクイザル)を髄腔内注入に使用する。
ケタミン(Ketalar(登録商標);Parke-Davis、5mg/kg、i.m.)の筋肉内注射により麻酔を誘導する。アトロピンをi.m.注射(0.05mg/kg)し、気管支分泌を低減させる。プロポフォール1%(Fresenius(登録商標))とグルコース4%溶液の混合物(プロポフォール1容量とグルコース溶液2容量)で継続的に潅流するために、静脈内カテーテルを大腿静脈に留置し、より深い麻酔を誘導する。次いで、その動物を定位固定の枠組みに置く。無菌条件下、C2からTh1まで垂直正中皮膚切開を行う。筋膜を切断し、C2からTh1の脊髄突起を露出させる。脊椎傍筋を牽引し、C6、C7およびTh1の薄膜を剥離する。次いで、C6完全椎弓切除術および上部C7片側椎弓切除術を行う。硬膜を露出させ、第6頸髄薄膜により覆われた脊髄部分の吻側域に相当する、第7および第8頸髄節の上で長軸方向に切開する。hNogo−A抗体を送達する浸透圧ポンプ(Alzet(登録商標)、2ML1;流速:50μg/時)に接続したポリエチレンチューブ(長さ10cm)を硬膜下に挿入し、数ミリメートル吻側に押し、硬膜に縫合で取り付ける。浸透圧ポンプを、椎弓切除の数センチメートル下の左側に、背筋の塊で作られた空洞に置いて固定する。チューブをその軌道に沿って縫合で筋肉組織に固定する。それらの筋肉と皮膚を縫合し、通常プロポフォールの静脈潅流の中断の15〜30分後に、動物は麻酔から回復する。動物を手術後に抗生物質(アンピシリン10%、30mg/kg、s.c.)で処置する。さらに、カルプロフェン(Carprofen)を1週間毎日投与する。
サルの脳および脊髄を注意深く切り出し、30%スクロース中で凍結保護し、クライオスタットにて40μmの切片に切る。注入したmABの検出のために、抗ヒト二次抗体を使用する(Jackson Laboratories)。二重標識のために、以下の抗体を使用することができる:内因性Nogo−A用にウサギAS472(親和性精製したもの)(Chen, 2000)、星状細胞用にGFAPに対するウサギ抗体、およびリソソーム局在用にカテプシンD(DAKO)に対するウサギ抗体。抗血清は全て、対応する二次抗体に結合したTRITCまたはFITCにより、またはABC−DABシステム(Vector)を用いて可視化する。切片をZeiss Axiophotにて落射蛍光により、または共焦顕微鏡(ZEISS LSM 410)により分析する。

NogoAのヒトNiG−フラグメントに対して高い親和性を有するヒトIgG1モノクローナル抗体を選択した。Medarex Inc., Annandale, NJによって組換えにより再構成された、ヒト免疫グロブリン遺伝子を有するマウス「Medarexマウス」を用い、標準的なハイブリドーマ技術によって誘導されたマウスハイブリドーマ細胞クローンからオリジナルモノクローナルを選択した。ヒトNiGで免疫されたMedarexマウスの作出およびそのハイブリドーマの生産は当技術分野で周知であり、WO2005/028508に記載されているものと同様の条件に従った。ほとんどのハイブリドーマの抗体産生レベルは極めて低かったので、組換えDNA技術を用いて、細胞株において、完全抗体またはFab−フラグメントの高レベル産生に特化した発現ベクターを構築した。精製AbおよびAbのFabフラグメントの作製は周知であり、例えば、WO2005/028508に詳細に記載されている。精製6A3−mAbおよび6A3−Fabの作製の場合も同様のステップに従った。
6A3 mAbはIgG1イソ型である。ヒトIgG1イソ型抗体は細胞Fc受容体に対して高い親和性を有し、抗体依存性細胞毒性(AADC)および補体依存性細胞傷害性(CDC)を誘導し得る(Jerries et al., 2002, Hezareh et al., 2001)。さらに、IgG mAbは、Fc受容体により媒介される逆輸送により、脳から血液脳関門を通って血液へ急速に流出することが報告されている(Zhang et al., 2001)。6A3 IgG1 mAbの潜在的なFc受容体媒介相互作用を除去するため、また、SP2/0細胞および大腸菌における高能力発現のための一価Fabフラグメントの組換え生産のためにも、そのイソ型を組換えによりIgG4に切り換えた。
6A3抗体の可変重鎖および軽鎖の相補性決定領域は、URL: www.bioinf.org.uk/abs/にてKabatデータベースを用いて決定した。このKabat定義は配列の変動に基づき、抗体可変領域のCDRを決定するために最もよく使用されている方法である(Wu TT, Kabat EA, 1970)。
マウス6A3−IgG1 mAb、6A3−IgG4 mAbおよび6A3 Fabの親和性を、BIAcore 2000光学バイオセンサー(Biacore, Uppsala, Sweden)を製造者の説明書に従って用い、表面プラズモン共鳴(SPR)によって測定した。組換えヒトNiGを、アミンカップリング化学を用い、CM5センサーチップのフローセルに共有結合的に固定化した。簡単に言えば、カルボキシメチリル化デキストランマトリックスを、0.025M NHSおよび0.1M EDCを含有する溶液35μlを注入することにより活性化した。センサーチップへの固定化に関しては、組換えヒトNiGを0.01Mクエン酸バッファー(pH4)に希釈し、親和性測定を可能とするカップリングレベルに達するように流速5μl/分で注入した。残っているNHS−エステル基の不活性化合物は、1Mの塩酸エタノールアミン(pH 8.5)35μlを注入することにより行った。このセンサーチップの表面を、5μlの0.1M HClを注入することにより再生した。親和性の測定に関しては、抗体を0.50nM〜100nMの範囲の種々の濃度で、流速200μl/分にて注入した。各注入後に、センサーチップの表面を10μlの0.1M HClを注入することで再生し、表面の結合活性の損失は無かった。速度定数kaおよびkdと親和性定数KAおよびKDを、製造者により供給されているBIAevaluations 3.0ソフトウエアを用いて評価した。
この実施例では、抗体の内因性ヒトNogo−Aとの結合を示す。この目的で、Nogo−Aの乏突起特異的遺伝子発現を示すことが従前に同定されている2種のヒト細胞株を試験し、その後、抗体の特異的結合に関して試験した。ヒト乏突起神経膠細胞株MO3.13およびHOGを用いて、本発明者らの2個の抗Nogo−A抗体NVP−6A3−Ab−NX−1(6A3−Ab)とNVP−IIC7Ab−NX−1(IIC7−Ab)を内因性Nogo−Aに対するそれらの結合に関して特性決定した。さらにこれらの細胞を用い、臨床試験のための種々の抗体バッチの特性決定のためのバイオアッセイを開発した。2つの独立した試験設定において、6A3−Abと、これらの細胞中の内因性ヒトNogo−Aとの結合を分析および検出した。
細胞株:MO3.13細胞はトロント大学のN. Cashman博士から入手した。それらはヒト横紋筋肉腫(RD)の6−チオグアニン耐性変異株と、外科術検体から培養した成人ヒト乏突起神経膠細胞との融合物に由来するものであった。HOG細胞はシカゴ大学のG. Dawson博士から入手した。この細胞株は外科術で摘出した乏突起神経膠腫から確立したものであった。細胞は全て、Glutamax、10%ウシ胎児血清およびペニシリン/ストレプトマイシンを添加した高グルコース含有ダルベッコの改変イーグル培地(Gibco)で培養した。
RT−PCR:鋳型としてMO3.13 RNAを用いたRT−PCRでは、200bp前後の明瞭に異なるDNAフラグメントが生じた(図6)。陰性対照(逆転写を行わないRNAおよびH2O)では産物は検出されなかった。PCRフラグメントは予測されたサイズの194bpに見られ、陰性対照サンプル(DNアーゼで処理したRNAおよびH2O)では産物は増幅されなかった。
PCRにNogo−A特異的プライマーを用いたMO3.13細胞のRT−PCR分析では、予測されたサイズ(194bp)のDNAフラグメントが得られたが、非逆転写RNAサンプルまたは水対照ではPCR産物は検出されなかった。この結果から、本発明者らは、これらの細胞が内因性Nogo−Aを発現するものと結論付ける。
さらなる研究において、従前に記載されたようにマカクザルに損傷を起こし、損傷時から6A3または対照IgGの4週間髄腔内注入で処置した。損傷を受けた左手の手先の器用さを、本明細書で上述したような条件下で改変型ブリンクマンボード検査を用いて測定した。6A3処置は、対照IgG処置に比べて機能回復の速度および程度を改善した。試験の終了時に損傷のサイズを測定したところ、対照IgGで処置したサルの機能回復は損傷サイズとほぼ逆の相関が見られ、50%損傷の90%から90%損傷の53%まで変動があった。逆に、抗Nogo−A mAbで処置した動物における回復量は損傷サイズによって有意に影響されず、6A3処置マウスでは、損傷サイズが85%と高かった場合でさえ、ほとんどそれらの損傷前の性能に達した。
ヒト対象における6A3抗体のCSF保持および半減期をCSF注入後14日間測定し(一日用量15mg/日)、血清およびCSFで個々の濃度を測定した(図9および10)。
3匹のサルに、正確な細かい指の動きの創出を不能にすることが知られている損傷である、C7/C8境界における脊髄の片側断裂を施し、対照動物においてはマウスIgG抗体または処置動物においては6A3抗体のいずれかを1mg/日の用量で4週間障害部位に髄腔内送達する浸透圧Alzet(登録商標)ポンプを埋め込んだ(図11およびFreund et al., Nat Med 12:7 90-2, 2006)。手先の器用さは改変型ブリンクマンボード検査にて、垂直スロットと水平スロットからのフードペレットの取り出しによって評価する。他の挙動課題としては、引き出しからのペレットの取り出し、腕の弾道運動(ballistic arm movements)、食物を掴む足の運動能および痛みや不快に対する挙動観察が挙げられる。これらの課題は損傷60日前から損傷120日後に一定間隔で行う。
好適な臨床試験は次のように記載される:
この試験は、スクリーニング相(ベースラインを含む)、非盲検処置相および少なくとも22週間の追跡調査相の三相を有する。この試験は独立データ安全性モニタリング委員会(Data Safety Monitoring Board (DSMB))の管理下で実施される。
・コホート1:3人の対麻痺患者が5mg[2.5ml中]を24時間受容
・コホート2:3人の対麻痺患者が30mg[2.5ml中]を24時間受容
・コホート3:6人の対麻痺患者が30mg/日まで[2.5ml/日]を14日間受容
・コホート4:10人の対麻痺および四肢麻痺患者が30mg/日まで[2.5ml/日中]を28日間受容

・順序通りに超可変領域CDR−L1−6A3(配列番号11)、CDR−L2−6A3(配列番号12)およびCDR−L3−6A3(配列番号13)
を含む、単離された分子。 - ・(i)順序通りに超可変領域CDR−H1−6A3(配列番号8)、CDR−H2−6A3(配列番号9)およびCDR−H3−6A3(配列番号10)を含む可変ドメインならびに(ii)ヒト重鎖の定常部分またはそのフラグメントを含む少なくとも1個の免疫グロブリン重鎖またはそのフラグメント;ならびに
