精密質量強度が、本明細書、例えば表5にさらに詳細に記載されるように、ダルトンで表示される水素及び電子で調整された精密質量又は中性の精密質量で、又はそれと実質的に同等な質量で測定される、方法が提供される。

[実施例]
材料&方法:
1.患者試料選択
臨床試料は、大阪医科大学、日本から得た。試料は、医師のチームによって一定の方法で集められ、処理され、保存された。すべての試料は、適切に同意され、詳細な病理報告書が添付されていた。
血清試料は、分析のために解凍するまで−80℃で保存した。1回のみ解凍した。すべての抽出は、氷上で実施した。血清試料は、FTICR−MS分析のために、最初に、1%水酸化アンモニウム及び酢酸エチル(EtOAc)を、それぞれ1:1:5の比で用いて、3回、等量の血清を連続抽出することによって調製した。試料を、抽出の間に、3500rpm、4℃で10分間遠心分離し、有機層を回収し、新しい試験管に移した(抽出物A)。3回目EtOAc抽出後、0.33%のギ酸を加え、続いて、さらに2回EtOAc抽出した。最後の有機抽出後、残りの水性成分を、水で2回さらに抽出し、3:1アセトニトリルを用いる沈殿によってタンパク質を回収した(抽出物B)。次いで、1:5比のEtOAc対ブタノール(BuOH)を窒素下で蒸発させて、元のBuOH出発容量(抽出物C)にした。すべての抽出物は、FTICR−MS分析まで−80℃で保存した。
抽出物を、負のイオン化モードのためにメタノール:0.1%(v/v)水酸化アンモニウム(50:50、v/v)に、又は正のイオン化モードのためにメタノール:0.1%(v/v)ギ酸(50:50、v/v)に3倍又は6倍希釈した。APCIには、試料抽出物を希釈せずに直接注入した。すべての分析は、7.0Tアクティブシールド型超伝導磁石(Bruker Daltonics社、Billerica、MA)を備えたBruker Daltonics APEX IIIフーリエ変換イオンサイクロトロン共鳴質量分析計で実施した。試料は、エレクトロスプレーイオン化法(ESI)及び大気圧化学イオン化法(APCI)を1時間あたり600μLの流速で使用して直接注入した。機器同調及び較正条件の詳細は、これまでに報告されている(Gray, G. R., and D. Heath. 2005. A global reorganization of the metabolome in Arabidopsis during cold acclimation is revealed by metabolic fingerprinting. Physiologia Plantarum 124: 236-248)。種々の試料抽出物を別個に分析したが、各試料の質量スペクトルデータを、スペクトル処理後に組み合わせた。すべての試料ピークを、各内部標準質量ピークが、理論的質量に対して1ppm未満の質量誤差を有するように内部標準物質を使用して較正した。
4.1有機抽出物
5種の膵臓癌試料及び5種の正常試料から得た500μLの血清の酢酸エチル抽出物を、窒素ガス下で別個に蒸発させ、50μLのイソプロパノール:メタノール:ギ酸(10:89.9:0.1、v/v/v)で各々再構成した。LC/MSフルスキャン及びMS/MSの両方のために、20μLの再構成された試料をHypersil ODSカラム(5μm、150×4.6mm)、移動相:溶媒A:94.9% H2O、5% MeOH及び0.1%ギ酸、溶媒B:100% MeOH、1mL/分の流速で勾配 15分で100% Aから79% A及び21% B、次いで、25分で、100% Bへ、次いで、30分まで維持を用いるHPLC(Agilent 1100、Agilent Technologies社)分析に付した。HPLCからの溶出液を、APCI供給源が取り付けられたABI QSTAR(登録商標)XL質量分析計を使用して分析し、負のモードでデータを集めた。フルスキャンモードでのスキャンの種類は、1.0000秒のスキャン時間、50〜1500Daの質量範囲及び30分の期間を有する飛行時間型(TOF−MS)とした。供給源パラメータは以下のとおりとした:イオン供給源ガス1(GS1)80;イオン供給源ガス2(GS2)10;カーテンガス(CUR)30;ネブライザー電流(NC)−3.0;温度400℃;デクラスタリングポテンシャル(DP)−60;フォーカシングポテンシャル(FP)−265;デクラスタリングポテンシャル2(DP2)−15。MS/MSモードでは、スキャンの種類は、プロダクトイオンとし、スキャン時間は、1.0000秒とし、スキャン範囲は、50〜1500Daとし、期間は、30分とした。すべての供給源パラメータは、上記と同一であり、−35Vの衝突エネルギー(CE)及び5の衝突ガス(CAD、窒素)を有していた。
5種の膵臓癌試料及び5種の正常試料から得た10μLのC−ACN画分(水性抽出物)の血清を、0.18mL/分の流速でフルスキャン及びプロダクトイオンスキャン(MS/MS)のためにMeta Sil AQカラム(3μm、100×2.0mm、Varian社)を備えたHPLC(Agilent 1100)に直接注入した。溶媒A:H2O−MeOH−ギ酸(94.9:5:0.1、v/v/v)及び溶媒B:MeOH−ギ酸(99.9:0.1、v/v)を移動相として使用し;100%のAから出発し、11分で80%のB及び20%のA、20分まで維持、次いで、30分で100%のB、次いで、45分まで維持の勾配溶媒プログラムを適用した。HPLCからの溶出液を、ESI供給源を取り付けたApplied Biosystem (AB) QSTAR(登録商標)XL質量分析計を使用して負及び正のモードで分析した。フルスキャンモードでのスキャンの種類は、1.0000秒のスキャン時間、50から1500Da間の質量範囲及び60分の期間を有する飛行時間型(TOF−MS)とした。供給源パラメータは以下のとおりである:イオン供給源ガス1(GS1)、65;イオン供給源ガス2(GS2)、75;カーテンガス(CUR)、30;温度425℃;負のモードのために:イオンスプレー(IS)、−4200V;デクラスタリングポテンシャル(DP)、−60;フォーカシングポテンシャル(FP)、−265;デクラスタリングポテンシャル2(DP2)、−15;また、正のモードのために:イオンスプレー(IS)、5500V;デクラスタリングポテンシャル(DP)、60;フォーカシングポテンシャル(FP)、265;デクラスタリングポテンシャル2(DP2)、15。MS/MSモードでは、スキャンの種類は、プロダクトイオンとして、スキャン時間は、1.0000秒として設定し、スキャン範囲は、50〜1500Daとし、期間は、60分とした。すべての供給源パラメータは、上記と同じであり、それぞれ、−30V及び+30Vの衝突エネルギー(CE)及び5の衝突ガス(CAD、窒素)を有していた。
すべてのLC−MS/MS分析は、(Goodenowe, D. B., L. L. Cook, J. Liu, Y. Lu, D. A. Jayasinghe, P. W. Ahiahonu, D. Heath, Y. Yamazaki, J. Flax, K. F. Krenitsky, D. L. Sparks, A. Lerner, R. P. Friedland, T. Kudo, K. Kamino, T. Morihara, M. Takeda, and P. L. Wood. 2007. Peripheral ethanolamine plasmalogen deficiency: a logical causative factor in Alzheimer's disease and dementia. Journal of lipid research 48: 2485-2498)に従い、以下の改変を施して実施した。具体的には、分析は、Agilent 1100 LCシステムと連結させたトリプル四重極質量分析計(4000Q TRAP、Applied Biosystems社)を使用して実施した。
試料は、各試料の120μLの酢酸エチル画分に15μLの内部標準物質(メタノール中、0.1μg/mLの(24−13C)−コール酸(Cambridge Isotope Laboratories社、Andover、MA))を加えることによって調製した。100μLの試料をフローインジェクション分析(FIA)によって注入し、負の大気圧化学イオン化(APCI)モード下でモニタリングした。方法は、各代謝産物及び(24−13C)−コール酸の1種の親/フラグメント遷移の多重反応モニタリング(MRM)をベースとした(表3)。
12μLのC−ACN画分を、108μLの移動相及び内部標準物質としての15μLのレセルピンと混合した。移動相は、75%アセトニトリル及び25%のddH2O中、1%ギ酸からなる。100μLの試料を、フローインジェクション分析(FIA)によって注入し、正又は負のイオンエレクトロスプレー(ESI)モード下でモニタリングした。方法は、各代謝産物及びレセルピンの1種の親/フラグメント遷移の多重反応モニタリング(MRM)をベースとした(表4)。ホスファチジルコリンの負のESIモード遷移は、以下のとおりに選択された:ギ酸塩付加物及び修飾子(qualifier)(両方とも、同一質量ホスファチジルコリンに共通している)及びsn−2脂肪酸(個々のホスファチジルコリンに対して特異的である)。
DISCOVAmetrics(商標)(Phenomenome Discoveries社、Saskatoon)を使用して、FTICR−MS精密質量アレイアラインメントを実施した。Microsoft Office Excel 2007を使用して、FTICR−MSデータの初期統計分析及びグラフを実施した。膵臓癌及び対照間の有意差の決定のために、両側独立スチューデントt検定を使用した。0.05未満のP値を、有意と考えた。ロジスティック回帰分析からSAS Enterprise Guide 4.2を使用してROC曲線を作成した。

これら20種の最良のFTICRバイオマーカーで受信者動作特性(ROC)分析を実施した。表7には、得られた曲線下面積(AUC)が要約されている。
妥当な分子式のコンピュータによる割り当てを20種の最良のバイオマーカーに実施した。割り当ては、精密な予測のために高い質量正確度を頼りにする、これまでに記載された(Aharoni, A., C. H. Ric de Vos, H. A. Verhoeven, C. A. Maliepaard, G. Kruppa, R. Bino, and D. B. Goodenowe. 2002. Nontargeted metabolome analysis by use of Fourier Transform Ion Cyclotron Mass Spectrometry. Omics 6: 217-234)、一連の数学的及びケモメトリックスルールに基づいていた。アルゴリズムは、その厳密な質量に基づいて、規定の制約内で検出された精密質量に割り当てられ得る炭素、水素、酸素及びその他の元素の数をコンピュータで計算する。表8では、論理的に推定される分子式がコンピュータによって計算された。
タンデム質量分析フラグメンテーションフィンガープリントを、上記のマーカーについて作成した。
FTICR−MS研究に使用した対照コホートから得た血清の選択された酢酸エチル抽出物を、3種のC36バイオマーカー、「576」、「594」及び「596」について、APCI負のイオンモード(1202モード)において四重極飛行時間型(Q−TOF)質量分析計に連結されたHPLCを使用して再分析した。25〜27分あたりの保持時間については、MS/MS及びMS3フラグメンテーションデータは、H2Oの喪失(それぞれ、m/z557、575及び577)及び2分子のH2Oの喪失(それぞれ、m/z539、557及び559)に起因するピーク並びにCO2の喪失(それぞれ、m/z531、549及び551)並びにCO2及びH2Oの喪失(m/z513、531及び533)に対応するより小さいピークに支配されていた(表9;図6〜12)。
ステップ勾配溶出;アセトニトリル−水25:75〜100%アセトニトリルを用いる逆相フラッシュカラムクロマトグラフィーに付された、市販の血清の酢酸エチル抽出物は、LC/MS及びMS/MSによって分析された場合に、2種の膵臓癌C36マーカー(m/z594及び596)が極めて豊富であると見い出された画分をもたらした。この画分のプロトン核磁気共鳴(1H NMR)スペクトル(図13)は、プレグナン環であると考えられる縮合環系を有する化合物の特徴的な共鳴を示した。これらの2種のマーカーは、ステロイド系骨格を有すると考えられ、恐らくは、胆汁酸として知られる化合物のクラスに属する可能性がある。
表6では、16種の化合物が、3種のコリン関連ファミリーに属する推定式を示した。すなわち、519.3、523.3及び541.3に対するリゾホスファチジルコリン(LysoPC)、757.6、779.5、783.6、785.6、803.5、805.6、807.6、809.6、829.6及び833.6に対するホスファチジルコリン(PtdCho)並びに702.6、724.5及び812.7に対するスフィンゴミエリン。
1203分析モードにおける600.5117化合物を、タンデム質量分析マスフラグメンテーションによってさらに分析した。545.5、527.5及び263.3のピークによって支配されているフラグメンテーションパターンによって、表6に示される分子式を有する化合物が存在し、両側鎖で18:2を有する1−アルケニル−2−アシルグリセロールとして分類され得ることが確認される(図40)。
膵臓癌患者の血液におけるコリン関連化合物及びC36バイオマーカーのレベルの低下を、同一集団においてタンデム質量分析アプローチ(方法を参照のこと)を使用してさらに確認した。アプローチは、分析物を定量化するための、親−娘フラグメントイオン組合せの測定(多重反応モニタリング;MRMと呼ばれる)に基づいている。
正及び負両方のエレクトロスプレーイオン化法モードにおいて、多重反応モニタリングに基づくタンデムMSアプローチを使用して、FTICR−MS分析のためと同一の水性抽出物を使用して、患者と対照間でのLysoPCレベルの相違を確認した(式及び遷移については方法参照のこと)。図41には、表6に列挙される3種のリゾホスファチジルコリン及び20種のさらなるLysoPCのレベルは、対照に対して膵臓癌患者において大幅に低下することの確認が報告されている。予想され得るように、20種の最良のFTICRバイオマーカー中に存在するLysoPCについて、MRMによって試験されたすべてのLysoPCの中で最低のp値が得られ、正のESI分析モードにおける最小の値、2.69E−15は、LysoPC18:2、FTICRによる第2の最良の推定LysoPCについて得られる。全体として、23種のLysoPCにおいて観察された大幅な低下は、膵臓癌血清においては、全ファミリーがダウンレギュレートされることを示唆する。
FTICR−MS分析のためと同一の水性抽出物を、正の分析モードにおいて、表6に列挙された10種のうち7種のPtdCho及び6種のさらなるPtdChoについて、並びに負の分析モードにおいて、表6に列挙された10種のうち9種のPtdCho及び多数のさらなるPtdChoについて、的を絞った方法によって分析した。図42a及び42bには、正及び負両方のESI分析モードにおいて試験したすべてのPtdChoの血清レベルが、対照に対して膵臓癌患者において大幅に低下していることの確認が報告されている。FTICRの最良のバイオマーカーの中で最良の推定PtdCho、「785.6」はまた、正のESI分析モードにおけるMRMによって試験されたすべての中でも最良のPtdChoであり、5.77E−18のp値を有する。試験されたすべてのPtdChoが、その側鎖とは無関係に膵臓癌血清において低下しており、正のESI分析モードにおいて5.31E−10の最大p値を有し、このことが、全ホスファチジルコリンファミリーは、膵臓癌血清において集合的にダウンレギュレートされることを実証することは興味深い。
FTICR−MS分析のためと同一の水性抽出物を使用して、患者及び対照間のスフィンゴミエリンレベルの相違を確認するために、多重反応モニタリングをベースとするタンデムMSアプローチを開発した。図43には、試験された5種のスフィンゴミエリン(FTICR分析によって同定された2種、SM(d18:1/16:0)及びSM(d18:1/24:1(15Z)を含む)の血清レベルが、対照に対して膵臓癌患者において極めて大幅に低下していることが報告されている。FTICRによって検出されなかったSM(d18:1/24:0)が、最強の低下を示し、7.81E−15のp値を有する。
FTICR−MS分析のためと同一の酢酸エチル抽出物を使用して、患者及び対照間のC36バイオマーカーレベルの相違を確認するために、多重反応モニタリングをベースとするタンデムMSアプローチを開発した。2Aにおいて説明されるように、表5に列挙されるすべての質量の中で、いくつかは、H2O分子又はいくつかの不飽和だけ異なり、C36において同一のファミリーに属すると思われ、タンデムMS法を、全「C36ファミリー」(式及び遷移については方法参照のこと)に広げた。
疾患進行状態に関する情報を含めた。したがって、疾患進行とバイオマーカー低下の間に相関があるかどうかを決定した。所望の3種のLysoPC、7種のPtdCho及び3種のC36マーカーのMRMデータを、癌ステージに従って再分析した(図45)。ステージあたり少量の患者でのこの予備研究は、いかなる傾向も示さないと思われる。
化学放射線療法状態に関する情報を含めた。したがって、この種類の治療とバイオマーカー低下の間に相関があるかどうかを決定した。所望の3種のLysoPC、7種のPtdCho及び3種のC36マーカーのMRMデータを、治療状態に従って再分析した(図46)。少量の患者でのこの予備研究は、バイオマーカーに対して化学放射線療法の効果がないことを示すと思われる。
本発明者らは、膵臓癌血清試料の包括的な的を絞らないメタボロミックプロファイリングを実施し、0.90を上回るほとんどのAUCによって示されるような、この癌の極めて強力な特徴を同定した。識別力があるとFTICRによって同定されたマーカーのファミリーを、的を絞った分析によって検証した。その低下が膵臓癌と関連している4種のファミリーが同定された:ホスファチジルコリン、リゾホスファチジルコリン、スフィンゴミエリン及びステロイド様化合物であり得るC36マーカー。

- 1種又は2種以上の参照血液試料が、1人又は2人以上の膵臓癌陰性のヒトに由来する、請求項1〜6のいずれかに記載の方法。
- 患者に由来する血液試料を分析して、1種又は2種以上の内部標準分子の定量化データを得るステップと、
前記1種又は2種以上の内部標準分子について得られたレベルに対する、1種又は2種以上の代謝産物マーカーのレベルの各々の比を得るステップと
をさらに含み、
比較ステップ(b)が、各比を、1種又は2種以上の参照血液試料について得られた1又は2以上の対応する比と比較するステップを含む、請求項1〜7のいずれかに記載の方法。
