Description
(i)第一の尿タンパク質分子は、α−2−HS−糖タンパク質前駆体、または少なくとも10個のアミノ酸残基を有するフラグメント、例えば、成熟したα−2−HS−糖タンパク質、VVSLGSPSGEVSHPRKT(配列番号1)、またはMGVVSLGSPSGEVSHPRKT(配列番号2)である;

ステージ1:特徴は、正常なGFRと正常なアルブミン尿(ACR<30mg/g)の糖尿病;
ステージ2:特徴は、糸球体過剰ろ過(120mL/分/1.73m2より大きい)、および正常なGFRと正常なアルブミン尿(ACR<30mg/g)を伴う腎腫大;
ステージ3:特徴は、微量アルブミン尿;
ステージ4:特徴は、顕在的アルブミン尿、およびGFRの進行性低下;および
ステージ5:特徴は、15mL/分/1.73m2より低いGFR。
一般に、ステージ1−3は早期とみなされ、ステージ4と5は末期とみなされる。
(2)各患者群中、バイオマーカーを構成するタンパク質分子と臨床学的因子のレベル/値を決定する。
(3)タンパク質レベルと臨床学的因子の値に適切な分析を行い、疾患スコアを計算するモデル(例えば、公式)を構築する。
(4)各患者群を示す疾患スコア(例えば、平均値)とその他の関連要因、例えば、感度、特異性、陽性予測値(PPV)と陰性予測率(NPV)に基づいて、各疾病のステージのカットオフ値を決定する。
(i)被検者
台湾台北市の三軍総医院(Tri−General Military Hospital)で、83人の糖尿病患者(「DM被検者」と称される)、および82人のDN患者(「DN被検者」と称される)が募集されたが、これは、米国糖尿病協会に基づいて設立された下記基準によるものである:
DM:糖尿病を患っているが、DNを有していない(基準は以下のとおり);
DN:糖尿病を患い、尿タンパク質の分泌濃度が一日当たり1gより高く、生検により、DNまたは尿毒症が確定される。
上述の各被検者から、朝一番に排出された尿サンプルと血清サンプルを採集した。尿サンプルに含まれるペプチドは、尿のマトリクス支援レーザー脱離/イオン化飛行時間型質量分析装置(MALDI−TOF−MS)、およびiTRAQ(isobaric tags for relative and absolute quantification)により測定した。
検査した各タンパク質の尿と血清タンパク質濃度のデータを統計分析し、DN被検者とDM被検者を区別する独自の能力を示すのにauROC範囲0.44−0.87で表した。各被検者に対する、数値の相関関係は、常態テストの結果に基づき、SpearmanやPearson分析により決定した。群の平均または中央値の比較は、Student T−test、またはNonparametric Mann−Whitney Testにより適切に行った。p<0.05のとき、統計的有意性が得られた。統計は、平均値±標準誤差(SEM)、または[25%,75%]を伴う中央値で示した。
(i)患者の特徴
下記の表1と表2に、訓練群とテスト群中の患者の特徴、およびDM群とDN群の患者の特徴を示す。
尿中プロテオーム分析により、以下の表3でリストされるペプチドは、DM被検者とDN被検者からの尿サンプルで異なって存在することが分かった。
二つのタンパク質モデル
DM被検者とDN被検者中のuDN2、uDN5、uGR3、uDNO、およびsDNOの二つの組み合わせレベルに対し、判別関数分析、ロジスティック回帰分析、およびリッジ回帰分析を行った。これらの研究で得られた結果は、五種のタンパク質、またはそれらのフラグメントの任意の二つの組み合わせが、DNのステージを決定する信頼できるマーカーとして利用できることを示している。

Claims (21)
- 被検者が糖尿病性ネフロパシーを患っている可能性を推定する方法であって、
糖尿病性ネフロパシーを患っていると疑われる被検者の尿サンプル中のバイオマーカーのレベルを決定するステップであって、前記バイオマーカーは、
VVSLGSPSGEVSHPRKT(配列番号1)およびMGVVSLGSPSGEVSHPRKT(配列番号2)よりなる群から選択される、α−2−HS−糖タンパク質のフラグメント、および
KGKWERPFEVKDTEEEDF(配列番号3)、MIEQNTKSPLFMGKVVNPTQK(配列番号4)、EDPQGDAAQKTDTSHHDQDHPTFNKITPNLAE(配列番号5)、およびEDPQGDAAQKTDTSHHDQDHPTFNKITPNLAEFA(配列番号6)よりなる群から選択される、α−1抗トリプシンのフラグメントであるステップと;
前記バイオマーカーのレベルに基づいて、前記被検者が糖尿病性ネフロパシーを患っているか評価するステップと;
からなり、
糖尿病性ネフロパシーを患わない被検者と比較して、前記バイオマーカーのレベルが増加するとき、前記被検者は糖尿病性ネフロパシーを患っている可能性が増加していることを示すことを特徴とする方法。 - 前記バイオマーカーは、前記被検者の尿サンプル中のα−1酸性糖タンパク質のフラグメントを更に含み、前記α−1酸性糖タンパク質のフラグメントは、GQEHFAHLLILRDTKTYMLAFDVNDEKNWGLS(配列番号7)である請求項1に記載の方法。
- 前記バイオマーカーは、前記被検者の血清サンプル中のオステオポンチンのフラグメントを更に含み、前記オステオポンチンのフラグメントは、YPDAVATWLNPDPSQKQNLLAPQNAVSSEETNDFKQETLPSK(配列番号8)およびKYPDAVATWLNPDPSQKQNLLAPQTLPSK(配列番号9)よりなる群から選択される請求項1または2に記載の方法。
- 前記評価ステップの後、前記バイオマーカーのレベルと糖尿病性ネフロパシーの状態を関連付けるステップを更に含み、糖尿病性ネフロパシーを患わない被検者と比較して、前記バイオマーカーのレベルが増加するとき、前記被検者が末期の糖尿病性ネフロパシーであることを示す請求項1〜3のいずれか1項に記載の方法。
- 被検者の糖尿病性ネフロパシー治療の有効性を評価する方法であって、
前記被検者の尿サンプル中の治療前のバイオマーカーのレベルを決定するステップであって、前記バイオマーカーは、
VVSLGSPSGEVSHPRKT(配列番号1)およびMGVVSLGSPSGEVSHPRKT(配列番号2)よりなる群から選択される、α−2−HS−糖タンパク質のフラグメント、および
KGKWERPFEVKDTEEEDF(配列番号3)、MIEQNTKSPLFMGKVVNPTQK(配列番号4)、EDPQGDAAQKTDTSHHDQDHPTFNKITPNLAE(配列番号5)、およびEDPQGDAAQKTDTSHHDQDHPTFNKITPNLAEFA(配列番号6)よりなる群から選択される、α−1抗トリプシンのフラグメントであるステップと;
前記患者の尿サンプル中の治療後のバイオマーカーのレベルを決定するステップと;
前記治療後のバイオマーカーレベルの変化に基づいて、治療の効果を評価するステップと;からなり、
前記治療後のバイオマーカーのレベルが前記治療前のバイオマーカーのレベルと同じか低いとき、前記治療が有効であることを示すことを特徴とする方法。 - 前記バイオマーカーは、前記被検者の尿サンプル中のα−1酸性糖タンパク質のフラグメントを更に含み、前記α−1酸性糖タンパク質のフラグメントは、GQEHFAHLLILRDTKTYMLAFDVNDEKNWGLS(配列番号7)である請求項5に記載の方法。
- 前記バイオマーカーは、前記被検者の血清サンプル中のオステオポンチンのフラグメントを更に含み、前記オステオポンチンのフラグメントは、YPDAVATWLNPDPSQKQNLLAPQNAVSSEETNDFKQETLPSK(配列番号8)およびKYPDAVATWLNPDPSQKQNLLAPQTLPSK(配列番号9)よりなる群から選択される請求項5または6に記載の方法。
- 前記被検者はヒトの患者である請求項5〜7のいずれか1項に記載の方法。
- 前記被検者は実験動物である請求項5〜7のいずれか1項に記載の方法。
- 被検者の糖尿病性ネフロパシーのステージを決定する方法であって、
糖尿病性ネフロパシーを患っていると疑われる被検者の尿サンプル中のバイオマーカーのレベルを決定し、選択的に一つ、またはそれ以上の臨床学的因子を決定するステップであって、前記バイオマーカーは、
VVSLGSPSGEVSHPRKT(配列番号1)およびMGVVSLGSPSGEVSHPRKT(配列番号2)よりなる群から選択される、α−2−HS−糖タンパク質のフラグメント、および
KGKWERPFEVKDTEEEDF(配列番号3)、MIEQNTKSPLFMGKVVNPTQK(配列番号4)、EDPQGDAAQKTDTSHHDQDHPTFNKITPNLAE(配列番号5)、およびEDPQGDAAQKTDTSHHDQDHPTFNKITPNLAEFA(配列番号6)よりなる群から選択される、α−1抗トリプシンのフラグメントであり、
前記臨床学的因子が、年齢、性別、HbA1c、アルブミン/クレアチニン比、および糸球体ろ過量よりなる群から選択されるステップと;
前記バイオマーカーのレベルに基づいて、疾患スコアを計算するステップと;
前記疾患スコアに基づいて、異なる糖尿病性ネフロパシーのステージを示す所定のカットオフ値と比較して、被検者の糖尿病性ネフロパシーステージを評価するステップと;
からなることを特徴とする方法。 - 前記バイオマーカーは、前記被検者の尿サンプル中のα−1酸性糖タンパク質のフラグメントを更に含み、前記α−1酸性糖タンパク質のフラグメントは、GQEHFAHLLILRDTKTYMLAFDVNDEKNWGLS(配列番号7)である請求項10に記載の方法。
- 前記バイオマーカーは、前記被検者の血清サンプル中のオステオポンチンのフラグメントを更に含み、前記オステオポンチンのフラグメントは、YPDAVATWLNPDPSQKQNLLAPQNAVSSEETNDFKQETLPSK(配列番号8)およびKYPDAVATWLNPDPSQKQNLLAPQTLPSK(配列番号9)よりなる群から選択される請求項10または11に記載の方法。
- 前記疾患スコアは、リッジ回帰分析、因子分析、判別関数分析、およびロジスティック回帰分析よりなる群から選択される分析により計算される請求項10〜12のいずれか1項に記載の方法。
- 被検者中の糖尿病性ネフロパシーの進行をモニタリングする方法であって、
糖尿病性ネフロパシーを患っていると疑われる被検者の第一と第二の尿サンプル中のバイオマーカーのレベルを決定し、選択的に一つ、またはそれ以上の臨床学的因子を決定するステップであって、前記第二のサンプルは、前記第一のサンプルの2週〜12ヶ月後の前記被検者からのものであり、前記バイオマーカーは、
VVSLGSPSGEVSHPRKT(配列番号1)およびMGVVSLGSPSGEVSHPRKT(配列番号2)よりなる群から選択される、α−2−HS−糖タンパク質のフラグメント、および
KGKWERPFEVKDTEEEDF(配列番号3)、MIEQNTKSPLFMGKVVNPTQK(配列番号4)、EDPQGDAAQKTDTSHHDQDHPTFNKITPNLAE(配列番号5)、およびEDPQGDAAQKTDTSHHDQDHPTFNKITPNLAEFA(配列番号6)よりなる群から選択される、α−1抗トリプシンのフラグメントであり、
前記臨床学的因子が、年齢、性別、HbA1c、アルブミン/クレアチニン比、および糸球体ろ過量よりなる群から選択されるステップと;
前記第一および第二のサンプルのバイオマーカーのレベルのそれぞれに基づいて、第一の疾患スコアおよび第二の疾患スコアを計算するステップと;
前記被検者の疾病進行を評価するステップと;

前記第二の疾患スコアが前記第一の疾患スコアより高いとき、糖尿病性ネフロパシーの悪化を示すことを特徴とする方法。 - 前記バイオマーカーは、前記被検者の尿サンプル中のα−1酸性糖タンパク質のフラグメントを更に含み、前記α−1酸性糖タンパク質のフラグメントは、GQEHFAHLLILRDTKTYMLAFDVNDEKNWGLS(配列番号7)である請求項14に記載の方法。
- 前記バイオマーカーは、前記被検者の血清サンプル中のオステオポンチンのフラグメントを更に含み、前記オステオポンチンのフラグメントは、YPDAVATWLNPDPSQKQNLLAPQNAVSSEETNDFKQETLPSK(配列番号8)およびKYPDAVATWLNPDPSQKQNLLAPQTLPSK(配列番号9)よりなる群から選択される請求項14または15に記載の方法。

前記被検者の第一と第二のサンプルのバイオマーカーのレベルを決定し、選択的に一つ、またはそれ以上の臨床学的因子を決定するステップであって、前記第一のサンプルは、前記治療前の前記被検者からのものであり、前記第二のサンプルは、前記治療後の前記被検者からのものであり、前記バイオマーカーは、
VVSLGSPSGEVSHPRKT(配列番号1)およびMGVVSLGSPSGEVSHPRKT(配列番号2)よりなる群から選択される、α−2−HS−糖タンパク質のフラグメント、および
KGKWERPFEVKDTEEEDF(配列番号3)、MIEQNTKSPLFMGKVVNPTQK(配列番号4)、EDPQGDAAQKTDTSHHDQDHPTFNKITPNLAE(配列番号5)、およびEDPQGDAAQKTDTSHHDQDHPTFNKITPNLAEFA(配列番号6)よりなる群から選択される、α−1抗トリプシンのフラグメントであり、

- 前記バイオマーカーは、前記被検者の血清サンプル中のオステオポンチンのフラグメントを更に含み、前記オステオポンチンのフラグメントは、YPDAVATWLNPDPSQKQNLLAPQNAVSSEETNDFKQETLPSK(配列番号8)およびKYPDAVATWLNPDPSQKQNLLAPQTLPSK(配列番号9)よりなる群から選択される請求項18または19に記載の方法。
