Description
本出願は、2009年11月24日に出願された米国仮特許出願第61/263,927号の利益を主張し、その内容全体が本明細書に参照により援用される。
(技術分野)
本開示は、概して外科手術処置において用いられるシールに関する。より詳細には、本開示は、組織の切開部または天然の開口の中に挿入するように適合され、実質的に流体密閉シールが組織および一外科手術物体または複数の外科手術物体の両方と共に形成されるように、1つ以上の外科手術物体を密閉して受容するように適合されるシールアンカ部材に関する。
最小侵襲性外科手術処置は、外科医が皮膚の小さな開口部を通って、または天然の開口(例えば、口、肛門、または膣)を通って患者の体に入る処置である。そのような処置は、従来の開放性外科手術よりもいくつかの利点を有する。特に、従来の開放性外科手術と比較すると、最小侵襲性外科手術処置は、結果として患者の外傷および回復時間の減少をもたらす。そのような処置は、概して、患者の腹部に対して行なわれる場合を除いて、「内視鏡処置」と呼ばれ、患者の腹部に対して行われる場合、処置は「腹腔鏡処置」と呼ばれる。本開示の全体を通して、用語「最小侵襲性」は、内視鏡処置および腹腔鏡処置の両方を包含するものと理解されるべきである。

前部分と、
後部分と、
該前部分と該後部分との間に配置される中間部分と、

該前部分は、第1の構成と第2の構成との間で遷移可能である、外科手術装置。
(項目2)
上記第1の構成は要素の第1の配列を有し、上記第2の構成は要素の第2の配列を有する、上記項目のいずれかに記載の外科手術装置。
(項目3)
上記前部分は、上記第1の構成における第1の直径と、上記第2の構成における第2の直径とを有し、該第1の直径は該第2の直径とは異なる、上記項目のいずれかに記載の外科手術装置。
(項目4)
上記第1の構成と上記第2の構成との間の遷移は、上記内腔のうちの少なくとも1つを通る外科手術器具の挿入に応答する、上記項目のいずれかに記載の外科手術装置。
(項目5)
上記前部分は、複数の位置決め部材を備え、該複数の位置決め部材は、横方向における上記前部分の圧縮を容易にするために、隣接する位置決め部材間に少なくとも1つの間隙を規定する、上記項目のいずれかに記載の外科手術装置。
(項目6)
上記前部分は、上記第2の構成において実質的に平らな外形を有する、上記項目のいずれかに記載の外科手術装置。
(項目7)
上記前部分は、1つ以上の位置決め区分を含み、該位置決め区分は、上記中間部分に対して放射状に平行移動可能である、上記項目のいずれかに記載の外科手術装置。
(項目8)
上記1つ以上の位置決め区分は、上記前部分を上記組織路の中に挿入することを容易にするために、上記第1の構成にあるときに内側に向けられる、上記項目のいずれかに記載の外科手術装置。
(項目9)
上記1つ以上の位置決め区分は、上記シールアンカ部材を上記組織路内に固定するために、上記第2の構成にあるときに外側に向けられる、上記項目のいずれかに記載の外科手術装置。
(項目10)
上記1つ以上の位置決め区分は、内側に付勢される、上記項目のいずれかに記載の外科手術装置。
(項目11)
上記1つ以上の位置決め区分は、上記1つ以上の内腔を通る器具の挿入に応答して、外側に向けられるように放射状に平行移動する、上記項目のいずれかに記載の外科手術装置。
(項目12)
下にある体腔にアクセスする組織路内に位置決めするための外科手術装置であって、
前部分と後部分とを有するシールアンカ部材と、
該前部分と該後部分との間に配置される中間部分と、
該後部分に関連付けられる第1の位置決め部材と、
該前部分に関連付けられる第2の位置決め部材であって、該第2の位置決め部材は、非円形形状を有する、第2の位置決め部材と
を備えている、外科手術装置。
(項目13)
上記非円形形状は、長方形である、上記項目のいずれかに記載の外科手術装置。
(項目14)
上記前部分の上記長方形形状は、上記組織路の中にそして該組織路を通って該前部分を挿入することを容易にする、上記項目のいずれかに記載の外科手術装置。
(項目15)
上記後部分と上記前部分との間で概ね長手方向に延びる少なくとも1つのポートをさらに備えている、上記項目のいずれかに記載の外科手術装置。
下にある体腔にアクセスする組織路内に位置決めする外科手術装置は、前部分と、後部分と、前部分と後部分との間に配置される中間部分とを含むシールアンカ部材を含む。シールアンカ部材の前部分は、シールアンカ部材を組織路の中に容易に挿入するように構成されかつ適合される。シールアンカ部材の挿入後、シールアンカ部材の前部分はまた、組織路内にシールアンカ部材を確保しかつ/または固定することを容易にするように構成されかつ適合される。
図面および以下の説明において、同様な参照数字は類似または同一の要素を識別し、従来のようにそして当該分野において公知であるように、用語「近位」は、使用中における医療従事者に最も近い、装置の端部をいい、一方、「遠位」は、医療従事者に最も遠い端部をいう。

Claims (12)
- 下にある体腔にアクセスする組織路内に位置決めするための外科手術装置であって、
該外科手術装置は、シールアンカ部材を備え、
該シールアンカ部材は、
第1の位置決め区分と第2の位置決め区分とを含む前部分であって、該第1の位置決め区分および該第2の位置決め区分のそれぞれは、それぞれの第1の構成と第2の構成との間で移動可能である、前部分と、
後部分と、
該前部分と該後部分との間に配置された中間部分と、
該後部分と該中間部分とを通って延びる第1の内腔であって、該第1の内腔は、該第1の位置決め区分がそれの第1の構成にあるときに該第1の位置決め区分と整列している、第1の内腔と、
該後部分と該中間部分とを通って延びる第2の内腔であって、該第2の内腔は、該第2の位置決め区分がそれの第1の構成にあるときに該第2の位置決め区分と整列している、第2の内腔と
を備え、
該第1の位置決め区分および該第2の位置決め区分のそれぞれは、該第1の内腔および該第2の内腔をそれぞれ通って挿入される物体によって、該第1の位置決め区分および該第2の位置決め区分のそれぞれの第1の構成から該第1の位置決め区分および該第2の位置決め区分のそれぞれの第2の構成まで独立して遷移可能である、外科手術装置。 - 前記第1の構成は、要素の第1の配列を有し、前記第2の構成は、要素の第2の配列を有する、請求項1に記載の外科手術装置。
- 前記前部分は、前記第1の構成における第1の直径と、前記第2の構成における第2の直径とを有し、該第1の直径は、該第2の直径とは異なる、請求項1に記載の外科手術装置。
- 前記第1の構成と前記第2の構成との間の遷移は、前記内腔のうちの少なくとも1つを通る外科手術器具の挿入に応答する、請求項1に記載の外科手術装置。

- 前記第1の位置決め区分および前記第2の位置決め区分は、内側に付勢される、請求項7に記載の外科手術装置。
- 前記第1の位置決め区分および前記第2の位置決め区分は、前記第1の内腔および前記第2の内腔のそれぞれを通る器具の挿入に応答して、外側に向けられるように放射状に平行移動する、請求項7に記載の外科手術装置。
- 前記第1の位置決め区分および前記第2の位置決め区分は、それぞれの第1の内腔および第2の内腔を通って挿入された前記器具の軸方向の移動が、該第1の位置決め区分および該第2の位置決め区分の放射状平行移動およびその後の外側への移動をもたらすように構成されている、請求項11に記載の外科手術装置。
