Description
−電離線を用いないため、電離放射線防護や放射性廃棄物の複雑な管理を必要としない;
−装置が安価、コンパクトであり、使用が容易である;
−取得時間が非常に短い;
−注入される標識の濃度から考えて非常に高感度の技術であり、標識の濃度は、MRIに用いられるよりも非常に低く、PETおよびSPECTに用いられるものと同等である;
−小動物または臓器のスケールで非侵襲的にイメージングを行った場合、核イメージング法(PET、SPECT)と同等の解像度を有する技術であり、顕微鏡技術を用いた場合は細胞解像度を有することができる。
以降本明細書においてICGと称するインドシアニングリーンは、Cardiogreen(エイコーン社(Akorn Inc.))、Infracyanine(サーブ社(Serb))、ICG−Pulsion(パルシオンメディカルシステム社(Pulsion Medical System))の商品名で販売されている。この化合物は以下の式を有する:
しかし、ICGは、その蛍光標識としての使用を問題とするいくつかの特性を有する。
さらに、ICGの蛍光寿命(0.5ns)は、生物組織の自己蛍光の寿命(通常は0.3〜0.4ns)に非常に近い。従って、パルス発光励起に基づく蛍光装置を用いてICG蛍光と組織の自己蛍光とを区別することは難しい
最後に、ICGは、抗体、ペプチド、サッカリド、タンパク質、オリゴヌクレオチド、もしくはアプタマーなどの標的化生物分子または分子との結合を可能とするグラフト基を持たない。標的化分子とのグラフト化は有益であり、それは、全身注入の後にフルオロフォアをインビボにて対象とする領域へ指向させることを可能とし、それによってイメージングすべき領域にICGが選択的に蓄積され、その結果として検出感度が高められるからである。
従って、光凝固による病変部の治療に関して、特許出願WO2001/017561では、その溶解度および化学的安定性を高めることができるICGの製剤を提案しており、この製剤は、アルコール、バッファー、界面活性剤、グリセロール、ポリエチレングリコール(PEG)、ポリビニルピロリドン(PVP)、オイル、赤血球、脂肪酸、および抗菌薬を含んでいる。特許出願US2004/0156782には、肝臓クリアランスもしくは心臓クリアランスの測定、または血流の測定のための血管造影法用として、凍結乾燥した形態のICG製剤が記載されている。
従って、第一の側面によると、本発明は、ナノエマルジョンの形態でのインドシアニングリーンの製剤に関し、このナノエマルジョンは、連続水相および少なくとも1つの分散油相を含み、ここで、この油相は、インドシアニングリーン、少なくとも1つの両親媒性脂質、および少なくとも1つの可溶化脂質を含む。
可溶化脂質は、少なくとも1つの脂肪酸グリセリドを含むことが有利であり、例えば、12〜18個の炭素原子を持つ飽和脂肪酸の少なくとも1つのグリセリドである。
水相は、共界面活性剤、特にエチレンオキシドユニットから成る、またはエチレンオキシドおよびプロピレンオキシドユニットから成る少なくとも1つの鎖を持つ共界面活性剤をさらに含むことが好ましい。この共界面活性剤は、特に、ポリエチレングリコール/ホスファチジルエタノールアミン複合体(PEG−PE)、脂肪酸およびポリエチレングリコールのエーテル、脂肪酸およびポリエチレングリコールのエステル、ならびにエチレンオキシドとプロピレンオキシドとのブロックコポリマーから選択することができる。
第二の側面によると、本発明は、少なくとも1つの連続水相および少なくとも1つの分散油相を含むインドシアニングリーンの製剤を作製するための方法に関し、この方法は:
(i)少なくとも1つの可溶化脂質、両親媒性脂質、およびICGを含む油相を作製する工程;
(ii)ナノエマルジョンを形成するのに十分なせん断作用の下、油相を水相中に分散させる工程;および、

油相は、特に、成分のすべてまたは一部を適切な溶媒に溶解し、次に溶媒を留去することによって作製することができる。
このように製剤した場合、ICGの光学特性が大きく改善され(約10倍の性能の向上)、これによって、イメージングにおける性能の改善、または注入用量の削減が可能となる。例えば、0.27mg/mlのナノエマルジョンとして製剤されたICGの2〜10mlは、2.5mg/mlのICG溶液2〜8mlと少なくとも同様の強度の蛍光シグナルを発する(循環血液量および心拍出量の測定のために体重70kgのヒトへ静脈内注入される用量)。
本製剤の別の利点は、154mM塩化ナトリウムなどの等張性媒体中で作製することができる点であり、そのような媒体中では凝集してしまうために低張性媒体(5%グルコース水)中で注入される懸濁させたICGとは異なる。
本発明に従うICGの製剤は、さらに、作製が非常に容易であり、安価であることから、容易に入手可能である。
最後に、本発明に従う製剤は官能化が可能であり、従って、標的化生物分子または分子とグラフト化することにより、イメージングすべき対象となる領域へICGを輸送可能とすることができ、このことは、蛍光イメージングに新しい臨床応用をもたらすものである。

このような両親媒性脂質は、親水性部分および親油性部分を含む。これらは、一般的には、親油性部分が8〜30個の炭素原子を持つ直鎖状もしくは分岐鎖状の飽和または不飽和鎖を含む化合物から選択される。これらは、天然もしくは合成由来のリン脂質類、コレステロール類、リゾ脂質類(lysolipids)、スフィンゴミエリン類、トコフェロール類、グルコ脂質類(glucolipids)、ステアリルアミン類、カルジオリピン類;ソルビタンエステルなどのエーテルもしくはエステル官能基によって親水性基と結合した脂肪酸から構成される分子、例えば、シグマ社からSpan(登録商標)の商品名で市販されているソルビタンモノオレエートおよびモノラウレート;高分子化脂質;ICIアメリカ社(ICI Americas Inc.)からTween(登録商標)の商品名で、ユニオンカーバイド社(Union Carbide Corp.)からTriton(登録商標)の商品名で市販されている非イオン性界面活性剤などの、短鎖ポリエチレンオキシド(PEG)と結合した脂質;サッカロースモノ−およびジ−ラウレート、モノ−およびジ−パルミテート、モノ−およびジ−ステアレートなどの糖エステル類、から選択することができ;前記界面活性剤は、それ単独で用いることも、または混合物として用いることも可能である。
本発明に従うエマルジョンは、可溶化脂質をさらに含む。
可溶化脂質は、両親媒性脂質に対してその可溶化を可能にするのに十分な親和性を持つ化合物から選択される。それはオイルまたはワックスであってよい。
有利には、それは、異なるグリセリドの混合物である。
ICGは、エタノール、DMSO、またはメタノールなどの適切な有機溶媒中の濃縮溶液として用いてよい。しかし、インビボでの用途に対しては、耐容性が良好である溶媒が好ましい。
それは生体適合性オイルであることが好ましい。生体適合性オイルは、エマルジョンの形成前に化学的もしくは物理的修飾を行わずに用いることが好ましい。
このエマルジョンは、当然、連続水相をさらに含む。
有利には、本発明に従うエマルジョンは、共界面活性剤をさらに含む。
[作製方法]
上述の製剤は、超音波処理によるものを例とする、公知の乳化方法の1つによって得ることができる。
この工程によってフルオロファオがより効果的にカプセル化され、従って、得られた製剤の蛍光量子収率および安定性が高まる。
(i)少なくとも1つの可溶化脂質、両親媒性脂質、およびICGを含む油相を作製する工程;
(ii)ナノエマルジョンを形成するのに十分なせん断作用の下、油相を水相中に分散させる工程;および、
(iii)このようにして形成したナノエマルジョンを回収する工程、
を含むことが好ましい。
さらに、油相は、成分のすべてまたは一部を適切な溶媒に溶解し、次に溶媒を留去することによって作製することが有利である。
続いて、連続相の成分を混合することで作製した水相を添加することができる。
油相への水相の添加は、油相が液体であるように加熱下で行うことが好ましい。
本発明の好ましい態様によると、製剤は、ナノエマルジョン表面における生物リガンドまたは対象とする分子のグラフト化によって官能化することが可能である。
このような分子の共界面活性剤への結合は、乳化前または乳化後のいずれで行ってもよい。乳化後では、エマルジョンが不安定化しないように、化学グラフト反応が、水溶液中にて、極端な酸性でも塩基性でもないpH(pH5〜11)で行われることが好ましい。従って、化学グラフト反応の実施が困難である場合、第一の変形(乳化前のグラフト化)が原理的には好ましい。
a)生物リガンド
i)生物標的化リガンド:ある細胞(例としては、例えばS. Achilefu, Technology in Cancer Research & Treatment, 2004, 3, 393-408に記載の腫瘍細胞)もしくはある臓器の特異的な認識を可能とする生物的要素(抗体、ペプチド、サッカリド、アプタマー、オリゴヌクレオチドなど)または化学的要素(例えば葉酸)、
ii)酵素活性を例とするある生物活性に対するマーカーである生物リガンド。例えば、そのような生物リガンドは、あるプロテアーゼによって開裂可能なペプチドであり、その末端にICG蛍光阻害剤がグラフト化することになる。この種のリガンドは、C.H. Tung, Biopolymers, 2004, 76, 391-403の論文に記載のように、プロテアーゼの酵素活性の特異的なイメージングを可能とする。別の例は、その蛍光の阻害剤から標識を分離するジスルフィド架橋を有する生物リガンドによって構成される。このような生物リガンドは、プローブの細胞内への内部移行の特異的なイメージングを可能とし、例えば、フランス特許出願FR2888938に記載されている;
b)隠匿剤(furtivity agent):これは、生物内でのICGの循環時間を延長し、その排出を遅延する効果を有する要素である;
c)「集合ベクター(assembly vector)」:これは、1もしくは複数の蛍光標識、および/または1もしくは複数の生物標的化リガンド、および/または1もしくは複数の隠匿剤、および/または1もしくは2つ以上の機能(例えば、医薬の送達、その他のイメージング様式、治療機能)の集合を可能とする要素である。
−MRI(磁気共鳴画像法)、PET(陽電子放出断層撮影法)、SPECT(単一光子放射型コンピュータ断層撮影法)、超音波診断法、放射線撮影法、またはX線断層撮影法などのその他のイメージング様式のためのイメージング剤;または、
−治療効果を持つ分子(DNA、オリゴヌクレオチド、化学分子など)。
いかなる理論にも束縛されるものではないが、現在のところ、ICGは、恐らくナノエマルジョンの内部にカプセル化されると共に、その膜(またはシェル)にインターカレートされていると推測される。ICGがさらに乳化の前に存在する場合、ICGが表面に吸着されることは非常に可能性が低いと考えられる。
さらに、これは、あるいくつかの用途において対象とする標的化リガンドによる官能化が可能である。
[エマルジョンの使用方法]
提案されるICG製剤は、特に診断薬として用いることができる。
重要な用途は、センチネル節の検出である。
本発明を、実施例および添付の図面によってより詳細に説明する。
図3:実施例1で述べるようにして作製されたエマルジョンの分散相の平均径のヒストグラムである:透析の10日後(黒棒グラフ)および40日後(白棒グラフ)。測定は、ZeitaSizer Nano(マルベルンインスツルメント社(Malvern Instrument))での動的光散乱法により、種々の量のICG(充填率を0〜1500μMまで変化させる)を含有する非常に少量のナノエマルジョン(0.5〜2μl)を添加した0.1×のPBS溶液1mlで行う。
図11:図11a)およびb)は、作製後(a)、および周囲温度で4ヶ月保存後(b)のナノエマルジョンに、Universal V3.8B TAデバイスを用いた示差走査熱量測定(DSC)を行うことで得られたサーモグラムである(℃による温度の関数としての熱流(W/g))(実施例6)。
エマルジョンの形態のICGの製剤
適切な容器中に、大豆油(シグマ‐アルドリッチ社)0.05g、Suppocire(登録商標)NC(ガッテフォッセ社)の商品名で市販されている半合成グリセリド0.150g、ならびに、ジメチルスルホキシド(DMSO)溶液としてのICG(シグマアルドリッチ社のカーディオグリーン、またはセルブラボラトリーズ社のインフラシアニン)0.310mgおよび9.30mg、さらにはLipoid(登録商標)S45の商品名でリポイド社(Lipoid)から市販されている大豆レシチン0.100g(45%ホスファチジルコリンに濃縮)から構成される予備混合物を作製した。
グリセロール0.05g、ICIアメリカ社からMyrj(登録商標)53の商品名で市販されているエチレンオキシドユニットを50個有するポリオキシエチレンステアレート0.331g、および154mMの塩化ナトリウム溶液を混合して混合物を1.7gとし、連続相を作製した。次に、乳化前に、この溶液を加熱して維持した(50〜60℃)。
得られたエマルジョンを0.22μmフィルターでろ過し、殺菌および凝集物の除去を行う。こうして、このエマルジョンは、希釈後、インビボでの機能イメージング用蛍光プローブとして直接用いることができる。透析前の得られた製剤の組成を以下の表1にまとめる。
従って、これらすべての特性に照らして、本発明に従う製剤を、すぐに使用できる形態で市販することが可能であろう。
血管新生のイメージング
雄のスプラーグドーリーラット(ハーラン(Harlan)、フランス)にイソフルラン吸入(4%で誘導、2%で維持)による麻酔を施し、次にイメージング装置の下に配置する。この装置は:1)ICGの励起帯で発光し、その励起パワーが数mW/cm2のオーダーである光源、および2)観察されるサンプルに適合させたレンズと組み合わせたCCDカメラから構成される。この集合体は、励起光とフレアを回避し、蛍光のみの検出を可能とするフィルターを備えている。次に頚部領域を切除して頚動脈および頚静脈を取り出す。次に、実施例1で述べたように作製した、透析後の充填率350μMのICG製剤を静脈内注入する。続いて、この領域の血管系の蛍光イメージングを行い、図7に示すように、頚動脈の視覚化をまず行い、次に頚静脈の視覚化を行う。この蛍光イメージングシステムを用いれば、手術の過程において外科医の動きのガイドおよび/または管理が可能となる。
センチネルリンパ節のイメージング
雌のヌードマウス(ジャンビエ(Janvier))にイソフルラン吸入(4%で誘導、2%で維持)による麻酔を施し、次に上述の蛍光イメージング装置の下に配置する。
腫瘍イメージング
マウス由来のTs/Apc腫瘍細胞を、雌のヌードマウス(ジャンビエ)の背中へ皮下注入し(106細胞)、注入部位の領域における腫瘍成長を、イメージングセッションに先立つ期間全体にわたってモニタリングする。
実施例5
グラフト化cRGDによるエマルジョン
攪拌手段を備えた適切な容器中にて、メタノール1mlの中、25mgのホスファチジルエタノールアミン(PE、シグマ社)および100mgのSCM−PEG5000−マレイミド(クリエイティブバイオケム(Creative Biochem))を5μlのトリエチルアミンと混合することでPE−PEG(5000)−マレイミドを作製する。攪拌は周囲温度で行なう。
ICGをカプセル化するエマルジョンを実施例1のようにして作製するが、上記の125mgの大豆油、375mgのSuppocire、350mgのレシチン、および1.5mgのフルオロフォアに加えて、上記で作製したPE−PEG(5000)−マレイミドの25mgを分散相に添加する。
ナノエマルジョンの安定性の実証
可溶化脂質によってナノエマルジョンに付与された安定性を実証するために、以下の実験を行った。
255mgのSuppocire(登録商標)NC(ガッテフォッセ社)(可溶化脂質)、85mgの大豆油(シグマアルドリッチ社)(オイル)、345mgのMyrj52(登録商標)(ICIアメリカ社)(共界面活性剤)、65mgのLipoid(登録商標)s75(レシチン、両親媒性脂質)、およびリン酸バッファー(PBS)を含むナノエマルジョンを、実施例1のプロトコルに従って作製した。
150mgのSuppocire(登録商標)NC(ガッテフォッセ社)(可溶化脂質)、50mgの大豆油(シグマアルドリッチ社)(オイル)、228mgのMyrj53(登録商標)(ICIアメリカ社)(共界面活性剤)、100mgのLipoid(登録商標)s75(レシチン、両親媒性脂質)、およびリン酸バッファー(PBS)を含むナノエマルジョンを、実施例1のプロトコルに従って作製した。
228mgのMyrj53(登録商標)(ICIアメリカ社)(共界面活性剤)、100mgのLipoid(登録商標)s75(レシチン、両親媒性脂質)、1600μlのリン酸バッファー(PBS)、表3に示す量のSuppocire(登録商標)NC(ガッテフォッセ社)(可溶化脂質)、および大豆油(シグマアルドリッチ社)(オイル)を含む3つのナノエマルジョンを、実施例1のプロトコルに従って作製した。
Claims (14)
- 連続水相および少なくとも1つの分散油相を含み、ここで、前記油相は、インドシアニングリーン、少なくとも1つの両親媒性脂質、および、25℃で固体であり、少なくとも10重量%のC12脂肪酸、少なくとも5重量%のC14脂肪酸、少なくとも5重量%のC16脂肪酸、及び少なくとも5重量%のC18脂肪酸を含む飽和脂肪酸のグリセリドの混合物からなる少なくとも1つの可溶化脂質、を含む、ナノエマルジョンの形態でのインドシアニングリーンの製剤。
- 少なくとも1つの可溶化脂質が、0重量%〜20重量%のC8脂肪酸、0重量%〜20重量%のC10脂肪酸、10重量%〜70重量%のC12脂肪酸、5重量%〜30重量%のC14脂肪酸、5重量%〜30重量%のC16脂肪酸、および5重量%〜30重量%のC18脂肪酸を含む飽和脂肪酸のグリセリドの混合物からなる、請求項1に記載のインドシアニングリーンの製剤。
- 前記両親媒性脂質がリン脂質である、請求項1または2に記載のインドシアニングリーンの製剤。
- 前記油相が少なくとも1つのオイルをさらに含む、請求項1から3のいずれか1項に記載のインドシアニングリーンの製剤。
- 前記オイルの親水性油性バランス(HLB)が3〜6である、請求項4に記載のインドシアニングリーンの製剤。
- 前記油相が、大豆油および亜麻仁油から選択される少なくとも1つのオイルを含む、請求項1から5のいずれか1項に記載のインドシアニングリーンの製剤。
- 前記水相が、共界面活性剤をさらに含む、請求項1から6のいずれか1項に記載のインドシアニングリーンの製剤。
- 前記共界面活性剤が、エチレンオキシドユニットから成るか、またはエチレンオキシドユニットおよびプロピレンオキシドユニットから成る少なくとも1つの鎖を含む、請求項7に記載のインドシアニングリーンの製剤。
- 前記共界面活性剤が、ポリエチレングリコール/ホスファチジルエタノールアミン複合体(PEG−PE)、脂肪酸およびポリエチレングリコールのエーテル、脂肪酸およびポリエチレングリコールのエステル、ならびに、エチレンオキシドとプロピレンオキシドとのブロックコポリマーから選択される、請求項8に記載のインドシアニングリーンの製剤。
- 前記エマルジョンの前記連続相が、生理学的に許容されるバッファーを含む、請求項1から9のいずれか1項に記載のインドシアニングリーンの製剤。
- 請求項1に記載のインドシアニングリーンの製剤を作製するための方法であって、
(i)インドシアニングリーン、両親媒性脂質、および、25℃で固体であり、少なくとも10重量%のC12脂肪酸、少なくとも5重量%のC14脂肪酸、少なくとも5重量%のC16脂肪酸、及び少なくとも5重量%のC18脂肪酸を含む飽和脂肪酸のグリセリドの混合物からなる少なくとも1つの可溶化脂質、を含む油相を作製する工程;
(ii)ナノエマルジョンを形成するのに十分なせん断作用の下、前記油相を水相中に分散させる工程;および、
(iii)このようにして形成した前記ナノエマルジョンを回収する工程、
を含む、方法。 - 前記せん断作用を、超音波処理で発生させる、請求項11に記載の作製方法。
- 前記油相が、前記成分のすべてもしくは一部を適切な溶媒に溶解し、次に前記溶媒を留去することで作製される、請求項11または12に記載の作製方法。
- 請求項1から10のいずれか1項に記載のインドシアニングリーンの製剤の、診断薬としての使用。
