本開示の治療の方法、レジメン又は使用を実施するに際して、治療上有効量のIL−17アンタゴニスト、例えば、IL−17結合性分子(例えば、セクキヌマブなどのIL−17抗体)又は及びIL−17受容体結合性分子を対象、例えば、哺乳動物(例えば、ヒト)に投与する。IL−17アンタゴニスト、例えば、IL−17結合性分子(例えば、セクキヌマブなどのIL−17抗体)又は及びIL−17受容体結合性分子は、本開示の方法により単独で、又は例えば、乾癬用の追加の薬剤及び療法と併用するなど、他の療法と併用して、投与することができる。1つ又は複数の追加の薬剤と併用投与する場合、IL−17アンタゴニスト、例えば、IL−17結合性分子(例えば、セクキヌマブなどのIL−17抗体)又は及びIL−17受容体結合性分子は、他の薬剤と同時又は連続して投与することができる。連続して投与する場合、主治医がIL−17アンタゴニスト、例えば、IL−17結合性分子(例えば、セクキヌマブなどのIL−17抗体)又は及びIL−17受容体結合性分子を他の薬剤と併用投与する適切な順序を決定する。

[実施例]
完了済みの概念実証(PoC)試験(CAIN457A2102)において、活動性慢性局面型乾癬を有する36例の患者における単回静脈内注入として3mg/kgで投与したセクキヌマブの効果をプラセボの効果と比較した。試験において、乾癬の面積及び重症度指標(PASI)並びに治験責任医師による包括的評価(Investigator Global Assessment)(IGA)エンドポイントに基づいて4週エンドポイントにおける有効性及び12週における持続的有効性が実証された。
(実施例2.1)プロトコール
主要目的
プラセボと比較して、12週間の治療後のPASI75の達成に関して、中等度から重度の慢性局面型乾癬を有する患者に皮下投与したセクキヌマブの3種の導入レジメンの有効性を評価すること。
この多施設試験では並行群無作為化二重盲検デザインを用いる。試験の図示的説明を図2に示す。試験は、スクリーニング期間、導入期間、維持期間及びフォローアップ期間の4つの期間からなる。4週間のスクリーニング期間は、患者の適格性を評価し、有効性を否定された(disallowed)薬物を患者から漸減させるために用いる。ベースライン来院時に、適格患者を導入治療アームの1つに無作為化する。無作為化は、体重(≧90kg又は<90kg)に従って層別化する。患者を以下のように無作為化する。
患者を以下の4つの導入治療アームの1つにそれぞれ1:2:2:1の比率で割り付ける。
・単回注射による導入−「単回」:1週目にセクキヌマブ150mgをs.c.投与する
・月1回の注射による導入−「月1回」:1、5、9週目にセクキヌマブ150mgをs.c.投与する
・早期負荷導入−「早期」:1、2、3、5週目にセクキヌマブ150mgをs.c.投与する
・プラセボ−「プラセボ」:1、2、3、5、9週目にプラセボを投与する。
実薬治療アームのそれぞれでは、試験中、盲検を維持するためにプラセボ注射を投与するものとする。
13週目に、患者を反応者(少なくともPASI75を達成)、部分反応者(PASI50を達成するが、PASI75は達成しない)又は非反応者(PASI50を達成しない)と分類する。13週目における反応者を以下の維持治療アームの1つに1:1の比率でさらに無作為化する。
・固定時間間隔レジメン−「FI」:13週目及び25週目にセクキヌマブ150mg(s.c.)を、再発の発現を観察する定期計画来院時にプラセボを投与する
・再発発現時治療レジメン−「SR」:13週目及び場合により再発の発現が認められない場合に25週目にプラセボを投与し、再発の発現を観察する定期計画来院時にセクキヌマブ150mg(s.c.)を投与する。
プラセボレジメンでの反応者は、プラセボアームに留まり、13週目及び25週目にプラセボを、再発の発現を観察する定期計画来院時にプラセボを投与する。
13週目における非反応者及び部分反応者並びに13週目以後の計画来院時に2連続再発を経験する患者は、オープンラベル相−「OL」:4週間ごとのセクキヌマブ150mg(s.c.)投与に入ることが適格であるものとする。試験に留まっているすべての患者に対する最終被験薬投与は、33週目であり得る。すべての無作為化患者は、安全性をモニターするために最終被験薬投与の4週間後に無治療フォローアップ期間に入り、12週間にわたりモニターするものとする(来院13、14及び15)。主目的を問うための評価は、導入期間の終了時(13週目)に実施する。
PASIスコアは、計画来院時に得られる。PASI採点法において、頭部、体幹、上肢及び下肢を紅斑、肥厚(局面隆起、硬化)及び鱗屑形成(落屑)について別個に評価する(表1参照)。
中間解析結果(12週):
PASI反応
12週目に実施した中間解析から得られるデータは、プラセボアームにおける5%と比較して「単回」アームにおける12%、「月1回」アームにおける43%及び「早期」アームにおける55%のPASI75を示している(表5及び図3)。「月1回」及び「早期」アームの両方がプラセボと比較して統計的有意性(p<0.001)を示した。「早期」アーム(最初の5週間以内の4x150mgセクキヌマブs.c.)は、最高有効性を示し、これは、3mg/kgIV(PoC)より良好であったが、10mg/kgIV(CAIN457A2212)より低かった。
PASI反応に加えて、治験責任医師による包括的評価(IGA)達成に関して主要エンドポイント解析を実施した。0(なし)から5(非常に重度)までのスケールを用いた。0(なし)又は1(ほぼなし)のIGAに達した場合、患者はIGA反応者とみなされた。選択基準は、3以上のベースラインIGAであった。12週間の治療後に、IGA反応は、「単回」アームの患者の4.5%、「月1回」アームの患者の22.6%及び「早期」アームの37.9%により達成され、プラセボアームの患者の3.0%のみがIGA0/1反応を達成した。
中間解析(IA)は、103例の患者が12週間の治療後にPASI75反応に達した16週間後に実施した。このIAにおいて、2種の維持レジメン(「固定間隔」及び「再発の発現時の治療」)を比較する重要な副次的目的を内部データモニタリング委員会(DMC)が評価した。
無作為化時の人口統計データと比較して、この中間解析における集団は、より低い平均体重(無作為化時の93.1kgと比較して85.1kg)を有する患者からなっていた。他の点では、人口統計データは同等であった。反応者のみが二重盲検維持期間に入ったので、平均体重の差は予想されたものであり、より低い体重は、セクキヌマブによる治療後に反応を示すより高い可能性に関連している。
「固定間隔」治療アーム(51例の患者、無作為化の12及び24週間後の150mgのセクキヌマブs.c.による維持治療)において、患者の94.1%が維持に入って4週目に(5.9%の反応の喪失)、8週間後には80.4%(19.6%の喪失)、66.7%が維持期間に入って12週目に(33.3%の喪失)PASI75反応を示した(図4A及び4B)。25週目の来院時に、患者をセクキヌマブにより再治療したところ、4週間後に、PASI75反応を有する患者の百分率は、68.6%にわずかに増加した。
維持期間の開始時(すなわち、被験薬を開始してから12週間後)に、「固定間隔」アームの患者の64.7%がIGA0(「なし」)又は1(「ほぼなし」)反応を示した。維持に入って4週目に、この反応は、患者の70.6%により、8週目に患者の60.8%により示された。患者が維持において最初に再治療を受ける前の維持期間に入って12週目に、患者の52.9%がIGA0/1反応を有し、このレベルは、4週間後に維持されていた(51.0%)。
内部主要エンドポイント解析(PEA)は、すべての患者が12週間の治療の期間に達した後に実施した。結果から、セクキヌマブが「月1回」及び「早期」アームにおいてプラセボと比較して、試験した適応症に有効であり、12週間の治療後のPASI75達成に関するセクキヌマブの効果を示すという主要エンドポイントを満たしていることが確認される。
(実施例3.1)プロトコール
主要目的
プラセボと比較して、治療を開始してから12週間後のPASI75の達成に関して、中等度から重度の慢性局面型乾癬を有する患者における月1回s.c.投与したセクキヌマブの3種の用量(25mg、75mg及び150mg)の又は単回投与(25mg)としての有効性を評価すること。
これは、中等度から重度の慢性局面型乾癬を有する120例の患者における多施設無作為化二重盲検プラセボ対照並行群試験である。患者が約25試験施設で登録されることが予期されるが、施設は、5例以上の患者を登録すべきである。
本試験は、12週間の治療期間(9週目の最終被験薬投与を含む)の用量範囲情報を提供する。本試験の目的は、乾癬の症状の重症度を低減する(プラセボと比較して)セクキヌマブの用量(単数又は複数)を決定することである。
患者は、以下の5種の治療アームの1つに1アーム当たり24例の患者として1:1:1:1:1の比率で割り付けるものとする。
・「3x150mg」アーム: 1、5及び9週目にセクキヌマブ150mgをs.c.投与する
・「3x75mg」アーム: 1、5及び9週目にセクキヌマブ75mgをs.c.投与する
・「3x25mg」アーム: 1、5及び9週目にセクキヌマブ25mgをs.c.投与する
・「1x25mg」アーム: 1週目にセクキヌマブ25mgをs.c.投与し、5及び9週目にプラセボをs.c.投与する、及び
・「プラセボ」アーム: 1、5及び9週目にプラセボをs.c.投与する
PASIスコアは、計画来院時に得られる。PASI採点法において、頭部、体幹、上肢及び下肢を紅斑、肥厚(局面隆起、硬化)及び鱗屑形成(落屑)について別個に評価する(表1参照)。
PASI反応
PASI75(主要変数)、PASI50及びPASI90反応の結果を図7に示し、表8に要約する。最高反応は、3x150mgコホートにおいて認められ、81.5%(p<0.001対比プラセボ)のPASI75及び51.9%(p<0.001対比プラセボ)のPASI90を有していた。3x75mgコホートは、12週間の治療後に57.1%(p=0.002対比プラセボ)のPASI75を有していた。したがって、コホート間における明らかな用量反応関係を、プラセボと統計的に異なっていなかった1x25mg群を除くすべてのコホートについて確認することができる(すなわち、3.4%(p=0.308)の1x25mg及び19.2%(p=0.362)の3x25mgコホートは、プラセボと比較したとき、統計的に有意な差を示さなかった(表9))。特に、PASI90は、3x75mgコホートの患者の19%及び3x25mgコホートの7.7%によってのみ達成された。反応率は、1x25mgコホートにおいて非常に低く、いずれの患者もPASI90を達成しなかった。プラセボ反応率は、PASI75について9.1%(患者22例中2例)であり、PASI90について4.5%(患者22例中1例)であった。サブグループ解析(表9)において、最高PASI75(93.8%)反応が3x150mgにより治療し、体重が90kg未満の患者に認められたのに対して、体重が90kg以上の患者の63.6%のみが12週間の治療後にこのコホートにおいてPASI75を達成した。
この12週間用量設定試験の結果は、慢性局面型乾癬におけるセクキヌマブの有効性をさらに確認するものである。3回にわたり月1回s.c.投与したセクキヌマブ150mgは、それぞれ81.5%及び51.9%のPASI75及びPASI90反応率をもたらした(表8)。これらの反応率は、試験A2211における最高反応コホート、すなわち、患者が4xセクキヌマブ150mg(s.c.)(ベースライン、1週目、2週目及び4週目)の投与を受けた「早期」アームにおいて観測された反応率より高かった。
(実施例4.3)モデリング研究
セクキヌマブ用量/レジメン、セクキヌマブ血漿濃度及びPASI反応関係の間の関係をポピュレーションPK/PDアプローチを用いてモデル化した。モデルは、試験CAIN457A2102、CAIN457A2103、CAIN457A2211、CAIN457A2212及びCAIN457A2220からのデータに基づいて付加的に構築し、更新した。
300mgセクキヌマブs.c.は乾癬における第II相試験において試験しなかったが(関節リウマチにおいて試験したが)、患者を乾癬試験CAIN457A2212においてより高用量(3x10mg/kgi.v.まで)に曝露した。図8に示すように、150mg及び300mg(s.c.)を用いる提案された負荷レジメンは、試験CAIN457A2212と比較して低い曝露をもたらす。より具体的には、300mg(s.c.)の提案された投与レジメンは、高曝露ピークを避けると同時に10mg/kgi.v.と類似の曝露プロファイルをもたらし、10mg/kgi.v.で認められたのと同様のPASI75反応率をもたらすと推定される。したがって、CAIN457A2304では、150mg及び300mg用量を用いるs.c.負荷レジメンを続行する。
試験CAIN457A2304は、中等度から重度の慢性局面型乾癬を有する患者における最長1年間にわたる乾癬の面積及び重症度指標(PASI)並びに治験責任医師による包括的評価(IGA)スコアに基づく有効性を実証し、安全性及び忍容性を評価するための、固定間隔又は再発発現時治療維持レジメンでの皮下セクキヌマブの無作為化二重盲検多施設試験であるように計画されている。
上述の実施例で述べたものを含む様々な試験から得られたデータに基づいて、セクキヌマブに関する以下のPK情報を提供する(表10)。

Claims (13)
- 乾癬を治療するための、IL−17抗体を含む医薬組成物であり、
IL−17抗体が、
a)導入レジメン中にそれを必要とする患者に投与されるものであり、ここで導入レジメンは、負荷レジメンを含み、負荷レジメンは、ゼロ週目に始めて、150mg〜300mgの用量のIL−17抗体を5回皮下投与するステップを含み、5回の用量のそれぞれは週1回送達され、及び
b)その後、維持レジメン中に投与されるものであり、維持レジメンが、4週間ごと、150mg〜300mgの用量のIL−17抗体を皮下投与するステップを含むこと
を特徴とし、
ここで、前記IL−17抗体は2つの成熟IL−17タンパク質鎖を有するIL−17ホモ二量体のエピトープに結合し、ここで前記エピトープは、1つの鎖上のLeu74、Tyr85、His86、Met87、Asn88、Val124、Thr125、Pro126、Ile127、Val128、His129及び他の鎖上のTyr43、Tyr44、Arg46、Ala79、Asp80を含み、IL−17抗体が100〜200pMのKDを有し、IL−17抗体が23〜30日のインビボ半減期を有する、
前記医薬組成物。 - ステップa)の全ての用量およびステップb)の全ての用量が、150mg又は300mgである、請求項1に記載の医薬組成物。
- 患者の体重が90kg未満である場合、150mgの用量のIL−17抗体を患者に投与し、患者の体重が90kg以上である場合、300mgの用量のIL−17抗体を患者に投与する、請求項2に記載の医薬組成物。
- 導入レジメンが、8週目に150mg〜300mgの用量のIL−17抗体を患者に皮下投与するステップをさらに含む、請求項3に記載の医薬組成物。
- ステップa)の前に、患者が乾癬用全身薬による治療を以前に受けていなかった、請求項1に記載の医薬組成物。
- ステップa)の前に、患者が乾癬用全身薬による治療を以前に受けていた、請求項1に記載の医薬組成物。
- 前記全身薬が、メトトレキセート、シクロスポリン、フマル酸エステル、アシトレチン、アレファセプト、アダリムマブ、エファリズマブ、エタネルセプト、インフリキシマブ、ゴリムマブ及びウステキヌマブからなる群から選択される、請求項6に記載の医薬組成物。
- 乾癬を治療するための、IL−17抗体を含む医薬組成物であり、
IL−17抗体が、
a)150mg〜300mgの5回の用量としてそれを必要とする患者に皮下投与されるものであり、5回の用量のそれぞれは週1回送達され、及び
b)その後、4週間ごと、150mg〜300mgとして患者に皮下投与されるものであること
を特徴とし、
ここで、前記IL−17抗体が、
i)配列番号8として示されるアミノ酸配列を含む免疫グロブリンVHドメイン及び配列番号10として示されるアミノ酸配列を含む免疫グロブリンVLドメイン、
ii)配列番号1、配列番号2及び配列番号3として示される超可変領域を含む免疫グロブリンVHドメイン並びに配列番号4、配列番号5及び配列番号6として示される超可変領域を含む免疫グロブリンVLドメイン、
iii)配列番号11、配列番号12及び配列番号13として示される超可変領域を含む免疫グロブリンVHドメイン並びに配列番号4、配列番号5及び配列番号6として示される超可変領域を含む免疫グロブリンVLドメイン、または
iv)セクキヌマブ
を含む、前記医薬組成物。 - IL−17抗体が、0、1、2、3、4および8週目に150mg〜300mgの用量として皮下投与され、その後4週間ごと皮下投与される、請求項8に記載の医薬組成物。
- ステップa)の全ての用量およびステップb)の全ての用量が、150mg又は300mgである、請求項8または9に記載の医薬組成物。
- 乾癬が尋常性乾癬である、請求項1〜10のいずれかの項に記載の医薬組成物。
- IL−17抗体が
a)配列番号8として示されるアミノ酸配列を含む免疫グロブリンV H ドメイン及び配列番号10として示されるアミノ酸配列を含む免疫グロブリンV L ドメイン、
b)配列番号1、配列番号2及び配列番号3として示される超可変領域を含む免疫グロブリンV H ドメイン並びに配列番号4、配列番号5及び配列番号6として示される超可変領域を含む免疫グロブリンV L ドメイン、
c)配列番号11、配列番号12及び配列番号13として示される超可変領域を含む免疫グロブリンV H ドメイン並びに配列番号4、配列番号5及び配列番号6として示される超可変領域を含む免疫グロブリンV L ドメイン、または
d)セクキヌマブを含む、請求項1〜11のいずれかの項に記載の医薬組成物。 - IL−17抗体がセクキヌマブである、請求項1〜12のいずれかの項に記載の医薬組成物。
