Description
本願は、ダルババンシン組成物ならびに細菌感染の治療方法におけるかかる組成物の使用方法に関する。
本願は、2002年11月18日出願の米国仮出願第60/427654号、2003年7月8日出願の米国仮出願第60/485694号、2003年8月13日出願の米国仮出願第60/495048号、および2003年8000月19日出願の米国仮出願第60/496483号の利益を主張し、これらすべての開示を本明細書に一体化させる。
U.S. Center for Disease Control and Preventionによれば、院内血流感染は米国における死因の最たるものである。毎年市米国において700万人の中央動脈カテーテル挿入患者(CVCs)の約5%が少なくとも1回は血流感染の関連している(1年に約350000人)。カテーテルに関連した血流感染は、細菌が静脈カテーテルを通して血流に侵入したときに起こり、致命的となりうるものである。
本発明は、ダルババンシンを用いる、細菌感染の治療または予防のための組成物、方法およびキットを提供する。驚くべきことに、ダルババンシンの安定化処方は、5〜7日またはそれ以上の期間に約1回の治療規則を可能にするファーマシューティカル・ウィンドウならびに長い半減期を示し、その一方でインビボにおいて抗細菌特性を保持することが示された。
本発明は、ダルババンシンの改良された投与規則および新規組成物、ならびに抗生物質耐性細菌感染の改良された処置方法を提供する。詳細には、本発明は、1種またはそれ以上の抗生物質に耐性のある株、例えばMRSAに対して活性のあるダルババンシン組成物を提供し、それは5〜7日あるいはそれより長い時間に1回の投与規則にて投与されうる。

本発明は、上で説明した方法によるダルババンシンの投与のために処方された医薬組成物を提供する。本発明の医薬組成物は、個体に投与された場合、数日間、しばしば少なくとも約3日間、少なくとも約5日間、あるいは少なくとも約1週間またはそれ以上にわたり、治療上または予防上有効なダルババンシン血漿レベルを提供するに十分な量のダルババンシンおよび医薬上許容される担体を含むダルババンシンの1回分用量の形態であってもよい。一般的には、治療上または予防上有効なダルババンシン血漿レベルは、血漿1リットルあたり少なくとも約4mgである。上記方法のようなよく知られた方法によりダルババンシン血漿レベルを測定してもよい。
高用量レベルでの毎週のダルババンシン投与(すなわち、驚くほど高く長期にわたる血清レベルをもたらす)は、本明細書の実施例により示されるように、毎日1回または毎日2〜4回投与される慣用的な抗生物質の低い用量の標準的な治療において観察されるのと同様あるいはそれよりも良好な安全性プロファイルを示す。ダルババンシンの驚くほど高い用量(すなわち、驚くほど高く長期にわたる血清レベルをもたらす)を、他の抗生物質よりも低頻度で投与してもよく、その場合、不利な副作用はなく、改善された有効性および患者のコンプライアンスを得ることが可能である。
本発明は、細菌感染の治療または予防方法において使用するキットも提供する。該キットは本発明の医薬組成物を含み、例えば、少なくとも1回分用量のダルババンシン、および細菌感染を治療または予防のための用法に関するヘルスケアプロバイダーへの情報を提供するための説明書を含む。説明書は印刷された形態あるいはフロッピーディスク、CD、またはDVDのごとき電子媒体の形態、あるいはかかる情報を得ることのできるウェブサイトアドレスの形態として提供されうる。しばしば、1回分用量のダルババンシンは、個体に投与された場合に、ダルババンシンの治療上または予防上有効な血漿レベルが個体において少なくとも5日間維持されるような用量を含む。いくつかの具体例において、キットは、少なくとも5日間隔で、しばしば約1週間間隔で投与される2つの1回分用量を含み、しばしば、2回目の用量よりも約1.5ないし約3倍の1回目のダルババンシン用量を含む。しばしばダルババンシンは滅菌水性医薬組成物または乾燥粉末組成物中(例えば、凍結乾燥されて)に含まれる。
このランダム化されコントロールされた研究は、ダルババンシンの2つの投与規則の安全性および有効性を評価するものであった。皮膚および皮膚深部構造を含む軟組織の感染(SSTI)を有する、あるいは外科的介入を必要とする成人患者を、3つの群にランダマイズした:研究アーム(study arm)1には1日目に1100mgのダルババンシンを静脈注射により与えた;研究アーム2には1日目の1gのダルババンシンIVそして8日目に500mgのダルババンシンIVを与えた;研究アーム3には「標準のケア(standard of care)」を与えた。臨床的および微生物学的応答ならびに不利なイベントを評価した。
62人の患者が研究に参加した。すべての患者に少なくとも1の試験医薬用量を与えた。安全性および有効性に関して4つの研究集団を評価し、それらの集団は以下のように定義された:治療意図(ITT)集団は、研究医薬の少なくとも1の用量を与えられたすべての患者を包含した(すべてのランダム化されて研究対象)。微生物学的治療意図(MITT)集団は、ベースラインにおいて培養により確認されたグラム陽性病原体を有するすべてのITT患者であった。臨床的に評価可能な集団を、1)すべての研究エントリー基準を満たす者、2)経口ステップダウン療法(ケア群の標準に適用されただけ)を除き、4日目以降にグラム陽性感染に対する抗微生物剤療法を変更しなかった者、3)フォローアップ(FU)評価通院のために戻った者、および4)プロトコル承認されていない同時投与抗微生物剤を与えられなかった者と定義した(治療が失敗しない場合)。微生物学的に評価可能な集団は、ベースラインにおいて培養により確認されたグラム陽性病原体を有する臨床的に評価可能な患者の部分集合であった。
患者の臨床的応答ならびに明らかになったあるいは推定された微生物学的応答を評価することにより3つの治療規則の有効性を決定した。第1の有効性のエンドポイントは、臨床的に評価可能な集団に関するフォローアップ来院時における臨床的応答であった。EOTおよびFUの両方に関する臨床的応答を、成功(治癒または改善)または失敗(決定されない結果を包含)に分類した。成功に分類された患者には、感染に対するさらなる抗細菌剤での全身的処置を受けた者であってはならない。失敗は、1またはそれ以上の局所あるいは全身的なSSTIの徴候の継続と定義され、その結果、SSTIに対する新たなあるいはさらなる全身的な抗微生物剤治療が必要であった。
臨床的成功率を表4に示す。臨床的に評価可能な集団において、ダルババンシン1回投与の患者の61.5%、ダルババンシン2回投与の患者の94.1%、および標準のケア群の76.2%の患者がFU評価の時点で成功に分類された。ベースラインにおいて深部または合併症のSSTIに分類された患者に関する実地のサブ分析においても、ダルババンシン2回投与療法は、ダルババンシン1回投与療法(成功率58.3%)および標準のケアの療法(73.7%)と比較して、高い臨床的成功率(93.8%)を示した。
異なる病原体に関する異なる治療規則の成功率を表5に示す。微生物学的に評価可能な集団に関し、FU評価における根絶/推定上根絶の割合は、ダルババンシン1回投与の場合58.3%(7/12)、ダルババンシン2回投与の場合92.3%(7/12)、および標準のケア群の患者の場合70.6%(12/17)であった。継続して存在した単離体に関し、ダルババンシンMICにおいては変化がなかった。FUにおいて、エス・アウレウス根絶率は、ダルババンシン1回投与(50%)および標準のケア(60%)と比較すると、ダルババンシン2回投与の場合に高かった。MITT集団に関して同様の知見が観察された。ダルババンシン2回投与によりMRSA単離体の80%が根絶された(表5)。
ダルババンシン治療群にランダム化された患者に関し、ダルババンシン血漿濃度を調べるために8日目に5mlの血液を採取した。8日目に500mgのダルババンシンを投与されるべきランダム化された患者から、2回目の投与直前に血液を得た。ダルババンシン1回投与群にランダム化された患者から10日目および24日目にさらに5mlの血液試料を得、ダルババンシン2回投与群にランダム化された患者から20日目および34日目にさらに5mlの血液試料を得た。
研究薬剤を少なくとも1回投与された各患者(ITT群)を、異常な臨床試験結果およびバイタルサインを含む不利なイベント(AE)のモニタリングにより薬剤安全性に関して評価した。重篤度(軽度、中度、重度、生命を脅かす程度)ならびに研究薬剤との関連性(関連性なし、関連性がありそうもない、関連している可能性あり、あるいはおそらく関連している)に関して研究者によりAEが評点付けされた。
このオープン−ラベルランダム化されたフェーズIIの試験は、ダルババンシンがSSTIを有する成人の治療に有効であることを示す。参加した患者の大部分は深部または合併症のある感染を有し(>90%)、外科的介入を要する感染を有していた(〜70%)が、約45%は糖尿病も有していた。
この研究の第1の目的は、ダルババンシンの薬物動力学を特徴付けて、治療用量の薬剤を投与される健康対象における腎臓での排泄の程度を計算することであった。これはオープンラベルで比較ではない(open label, non-comparative)研究であった。
年齢18歳から65歳までの健康な男性または女性の対象に、30分かけて輸液することにより1000mgIV用量のダルババンシンを1回投与した。
研究1、2、3、4、5、6、7、14、21、28および42日目に血液および尿(24時間収集)を集めた。血液試料をヘパリン処理チューブ中に得て、遠心分離した。血漿を分離し、アッセイ時まで−20℃で保存した。有効性が確認されているLC/MS/MS法を用いて血漿および尿試料をダルババンシンに関してアッセイした。尿および結晶に関するアッセイの定量の下限は500ng/mLであった。
不利なイベントを記録し、重篤度および研究薬剤との関連性について評価した。研究室のデータ(化学パネル、差分CBC、尿分析)を集め、ベースラインからの変化および範囲外の値を評価した。ECG、身体検査およびバイタルサインを得て、ベースラインからの変化を評価した。
ダルババンシン1000mgIVの1回投与は十分に耐えられるものであった。1000mgの1回静脈輸液後、45mg/lよりも高いダルババンシン血漿濃度が少なくとも7日間維持された。これは殺細菌的であることが知られている濃度(4〜32mg/l)よりも高い。このことは、ダルババンシンの1週間に1回の投与規則での使用を支持する。尿での除去プロファイルは、腎臓での排泄が重要な除去経路であり、約40%が尿中に排泄されることを示す。この知見は動物での観察結果と矛盾しない。腎臓経路のみが除去経路ではないので、ダルババンシンの用量調節は腎臓に障害のある患者において必要でないかもしれない。
蛋白へのダルババンシンの結合を、Microcal VP-ITC装置を用いて、20mMホスフェート、150mM NaCl,pH7.4中、25℃および37℃にて等温滴定微小熱量測定(ITC)により測定した。典型的な実験において、25x10μlの蛋白(約150μM)を、ダルババンシン溶液(約5μM)の入った熱量計セルに注入した。280nmにおける吸光度を測定することにより、実際の蛋白およびダルババンシン濃度を決定した。対照実験は蛋白をバッファー(ダルババンシン不存在)に注入して、同一条件下での蛋白の希釈熱を調べることを含んでいた。比較のために、いくぶんかの必要な修飾を行って、テイコプラニンを用いて同様の実験を行った。
