図1に示される実施形態において、近位部12は患者の体外にとどまる。近位部12はハンドル30および任意選択の押し込みスタイレット20を具備する。
図1および2に示される送達システム10は中間部送達装置14を具備する。本発明に従えば、中間部送達装置14は送達装置10の近位部12(図1、2)と遠位部13(図1、2)との中間にある。用語「中間にある」は、中間部送達装置14が2つの極部の間に介在する、挟まっている、存在しているまたは現れている、または遠位部13の遠位先端と近位部12の近位先端との間で―必ずしも等距離であるとは限らないが―空間的に中間の位置、状態、または性質にある本発明の実施形態を説明することが意図される。さらに、中間部送達装置14は遠位部13および/または近位部12の一部に重ねられてもよく、または部分的に挿入されてもよい。別の実施形態において、中間部送達装置14(以下で説明されるシース50など)の一部および遠位端部分外部誘導管部材80(以下で考察される;図4、5、6、7を参照)は細長いチューブ状カテーテルまたは一体構造のフレクサー(Flexor)(登録商標)シースであってもよい。
ここで本発明に従う医療装置送達システムの遠位部13の実施形態を参照すると、図4、5、6、および7は遠位部13が比較的チューブ状の本体であることを示す。脈管、脈管路、内視鏡の作業導管の形態、または操縦されるべき内視鏡と併せて使用される外部付属導管器具にかんがみて、遠位が縮径された、円形化された、面取りされた、または矢尻形であるほぼチューブ状の遠位端が患者にはより許容し易くあり得る。さらに、特定の実施形態において、遠位部13の遠位部は軟性で、円形化され、かつ可撓性であることにより、患者にさらなる保護および配慮を提供し得る。
図8は、内部誘導管部材70と内部圧迫部材41を機能的に連結するための、本発明の一実施形態による内部継手46を示す、システムの遠位部分の破断縦断面図である。それによって、内部継手46は、医師が、内部圧迫部材41、内部誘導管部材70、およびステントを外部誘導管部材80および外側シース50と比較して静止状態に又は比較的静止状態に保ちつつ、外部誘導管部材80を内部誘導管部材70に対して移動させることを可能にする。本発明の更に別の態様は、鋭角θの内部誘導管部材の第2の端部分77を有する内部誘導管部材70を具備する。前記図面、実施形態、および前記説明の同様の要素には同じ標識が付けられる。本発明による実施形態はこれらの特徴の1つ以上を有してもよく、各特徴を以下で説明し、対応する図面に示す。
内部継手46および/又は鋭角の内側部材近位端を有する自己拡張型ステントを送達するための医療用装置の製造方法および提供方法も提供する。
Claims (14)
- 患者の体内に植え込み可能な自己拡張型機器を迅速に挿入送達するための送達器具であって:
システム近位部分とシステム遠位部分の中間に延びる、細長く長さ方向に続く、可撓性の中間部送達装置であって、前記中間部送達装置が外側シースと金属製内側圧迫部材を備え、前記内側圧迫部材が外側係合面を備える遠位結合端部分を有する、中間部送達装置と;
前記システム遠位部分に配置されるプラスチック製内側誘導管部材であって、入口ポートを有する遠位の第1の端部分と出口ポートを有する近位の第2の端部分とを具備し、それらの間に誘導管を画定する内側誘導管部材であり、前記内側誘導管部材の第2の端部分が内面と外面を有する内側誘導管部材と;
軸線方向において前記内側誘導管部材と関連する外側誘導管部材であって、開口部を有する遠位の第1の端部分と出口ポートを有する近位の第2の端部分とを具備し、前記第1の端部の開口部と第2の端部の出口ポートが誘導管を画定する外側誘導管部材であり、外側誘導管部材が外側誘導管部材の第1の端部分と第2の端部分の中間の第1の外径と、外側誘導管部材の第2の端部分に又はその近傍に位置する第2の、より小さい外径とを備える階段状プロファイルを有するように構成された外側誘導管部材と;
前記外側誘導管部材内の前記システム遠位部分に配置された自己拡張型展開装置取り付け部位であって、近位拘束具、内側誘導管部材の外面、および外側誘導管部材の内面を具備する取り付け部位;および
前記自己拡張型展開装置取り付け部位の近位とされ前記システム遠位部分に配置される移行部位であって、前記内側誘導管部材の出口ポートが前記外側誘導管部材出口ポートと連通し、前記内側誘導管部材の出口ポートの近位に位置する後部開口部を備える、移行部位を備え;
前記金属製内側圧迫部材の遠位結合端部分が、前記プラスチック製内側誘導管部材の前記第2の端部分に植え込まれ、前記プラスチック製内側誘導管部材の前記第2の端部分が溶解された状態から冷却により固化した状態で前記金属製内側圧迫部材の遠位結合端部分の前記外側係合面が該第2の端部分に溶融接合されている、
ことを特徴とする送達装置。 - 近位端部分と遠位結合端部分を有する細長い金属製内側圧迫部材であって、前記遠位結合端部分が外側係合面を有する内側圧迫部材と;
第1の端部分、第2の端部分を有し、それらの間にチャネルを画定するプラスチック製内側誘導管部材と;を有し
前記金属製内側圧迫部材の遠位結合端部分が、前記プラスチック製内側誘導管部材の前記第2の端部分に植え込まれ、前記プラスチック製内側誘導管部材の前記第2の端部分が溶解された状態から冷却により固化した状態で前記金属製内側圧迫部材の遠位結合端部分の前記外側係合面が該第2の端部分に溶融接合されてなる、医療用機器に使用される内部継手。 - 前記内側誘導管部材が、前記第1の端部分又はその近傍に入口ポートを、前記第2の端部分又はその近傍に出口ポートを更に備え、前記ポートがそれらの間の前記チャネルと流体連通している、請求項2に記載の内部継手。
- 前記内側誘導管部材の第2の端部分が、ナイロン、ポリエーテルブロックアミド、ポリエーテルエーテルケトン、熱可塑性プラスチック、熱硬化性プラスチック、樹脂、ポリプロピレン、ポリエチレン、ポリエステル、ポリアミド、アイオノマー、ポリカーボネート、ポリフェニレンオキサイド、ポリフェニレンスルフィド、アクリル、液晶ポリマー、ポリオレフィン、ポリエチレンアクリレート、ポリフッ化ビニリデン、ポリビニル、ポリ塩化ビニル、およびポリテトラフルオロエチレンから成る群から選択される溶融接合材料を含む、請求項2又は3のいずれかに記載の内部継手。
- 前記溶融接合が、前記内側圧迫部材遠位結合端部分と前記内側誘導管部材の第2の端部分の間に少なくとも5ニュートンの引き離し強さを与える、請求項2−4のいずれかに記載の内部継手。
- 前記内側圧迫部材の近位端に連結されるシステム近位部分を更に備え、前記システム近位部分がハンドルを更に備え、前記細長い内側圧迫部材が前記ハンドルから前記内側誘導管部材に向かって少なくとも50.0cm延びる、請求項2−5のいずれかに記載の内部継手。
- 前記内側誘導管部材が、患者の体内に植え込み可能な自己拡張型機器を取り付ける部位を更に備える、請求項2−6のいずれかに記載の内部継手。
- 体内に植え込み可能な自己拡張型装置を迅速に挿入送達するように構成された送達システムに使用される内部継手の製造方法であって:
外側係合面を有する遠位結合端部分を有する細長い金属製内部圧迫部材を用意する工程と;
第1の端部分と第2の端部分を有する管状のプラスチック製内側誘導管部材を用意する工程であって、前記内側誘導管部材は、前記第1の端部分が入口ポートを具備し、前記第2の端部分が出口ポートを具備し、前記ポートがそれらの間に誘導管を画定し、前記内側誘導管部材の第2の端部分が内面と外面を有する、工程と;
前記内側誘導管部材の第2の端部分を溶融する工程と;
前記内側圧迫部材遠位結合端部分を植え込む工程であって、前記内側誘導管部材の第2の端部分の内面と外面の間にあり、前記植え込まれた内側圧迫部材の遠位結合端部分が、内側誘導管部材の第2の端部分に内側および外側接触界面を形成し、前記接触界面が前記内側圧迫部材遠位結合端部分の外側係合面に結合するように、前記内側圧迫部材遠位結合端部分を、前記溶融した内側誘導管部材の第2の端部分に植え込む工程;および、
前記植え込まれた内側圧迫部材遠位結合端部分と前記内側誘導管部材の第2の端部分を固体状態に冷却する工程、
を含む方法。 - 前記内側誘導管部材の少なくとも一部の中にマンドレルを位置決めする工程を更に含む、請求項8に記載の方法。
- 前記内側誘導管部材の第2の端部の少なくとも一部の周囲に工具を配置する工程を更に含む、請求項9に記載の方法。
- 前記マンドレルを取り除く工程を更に含む、請求項10に記載の方法。
- 前記器具を取り除く工程を更に含む、請求項10に記載の方法。
- 前記内側誘導管部材の第2の端部分をスカイビング加工する工程を更に含む、請求項8に記載の方法。
- 前記内側誘導管部材にテーパを形成する工程を更に含む、請求項8に記載の方法。
