Description
また、心室収縮の速度は、心室圧力が増加する点において心室収縮が生じる速度を比較することによって同様に肺動脈圧力センサ信号から解明されうる。心室内のこの収縮期間は、出力波形126上の点130と点132の間に見られる。心房収縮速度の場合と同様に、心室収縮速度(VCR)は、点130から点132までの圧力の変化ΔPVCを130と点132の間の時間ΔTVCの変化で割った商から決定されうる。これは次式から分かる。
一部の実施形態において、不整脈発作中に右心房収縮速度と右心室収縮速度を比較する図3におけるステップ(ブロック84)は、心房収縮速度(ACR)および心室収縮速度(VCR)を上式から計算し、その後にこれらの機械的収縮速度を比較して、電気的出力波形114から最初に検出された不整脈事象の種類を判断するステップを含む。たとえば、心室収縮速度(VCR)が心房収縮速度(ACR)よりも大きい場合、制御ユニット48のアルゴリズム50は事象が心室性頻拍であると結論を下してもよい(ブロック86)。心室が心房よりも高速で収縮しない場合、アルゴリズム50は検出された事象が上室性頻拍であると結論を下してもよい(ブロック88)。

前記電気的活動信号に少なくとも部分的に基づいて前記心臓の事象を検出し、
前記心臓内または前記心臓近傍に位置するセンサであって、前記心臓内の心房収縮および心室収縮の両方を感知するように構成される前記センサから機械的活動信号を受信し、
前記センサから受信した機械的活動信号を解析し、
前記機械的活動信号に少なくとも部分的に基づいて事象を確認すること
を備える方法。

確認された事象の種類に少なくとも部分的に基づいて前記患者に治療を提供することをさらに備える付記1に記載の方法。
(付記3)
前記センサは圧力センサである付記1に記載の方法。
(付記4)
前記電気的活動信号から事象を検出すると、前記体内の前記センサを活性化させることをさらに備える付記1に記載の方法。

前記患者の肺動脈内に埋め込まれた圧力センサを用いて前記心臓の前記心房収縮および心室収縮に関連する1つ以上の圧力測定値を感知し、
前記センサから受信した、感知された圧力信号に少なくとも部分的に基づいて前記事象を確認し、
前記事象がはっきり確認される場合に治療を前記患者に提供すること
を備える方法。

前記センサから受信した前記圧力信号を解析することは、右心房収縮速度を右心室収縮速度と比較することを含む付記12に記載の方法。

Claims (6)
- 患者の体内の調律識別を行うシステムであって、
心臓の電気的活動を感知するように構成された電極と、
前記心臓内または前記心臓近傍に埋め込まれ、前記心臓内で心房の機械的活動および心室の機械的活動の両方を感知するように構成され、感知した心房および心室の機械的活動に関連する一又は複数の血圧測定値を含む機械的活動信号を出力する血圧センサと、
前記血圧センサと通信する埋め込み型医療装置であって、前記血圧センサから受信した前記機械的活動信号および前記電極から受信した電気的活動信号に少なくとも部分的に基づいて、不整脈事象を検出するとともに前記患者に提供する治療を調節するように構成される制御ユニットを含む前記埋め込み型医療装置とを備え、
前記埋め込み型医療装置は、前記電極から受信した電気的活動信号に基づいて不整脈事象が検出されると、その検出された不整脈事象の存在を前記血圧センサから受信した前記機械的活動信号を用いて確認し、その不整脈事象として頻拍が疑われると、前記機械的活動信号を用いて頻拍事象の種類を識別するパルス発生器であるシステム。 - 前記血圧センサは感知した機械的活動信号を前記埋め込み型医療装置に無線送信するように構成される請求項1に記載のシステム。
- 前記制御ユニットは右心房収縮速度を右心室収縮速度と比較することにより不整脈事象を判断するように構成されている請求項1に記載のシステム。
- 前記制御ユニットは、前記心臓の感知した心房収縮の持続期間にわたって生じる血圧の変化を決定することにより、前記右心房収縮速度を決定するように構成されている請求項3に記載のシステム。
- 前記制御ユニットは、前記心臓の感知した心室収縮の持続期間にわたって生じる血圧の変化を決定することにより、前記右心室収縮速度を決定するように構成されている請求項3に記載のシステム。
- 前記制御ユニットは、機械的な房室(A/V)遅延を解析することによって、不整脈事象を判断するように構成されている請求項1に記載のシステム。
