Description
(i) 前記抗生物質治療開始前、又は治療開始後6時間以内に少なくとも1回、そして
(ii) 患者に対する抗生物質治療の開始から12時間後から1週間後に少なくとも1回、
前記患者から得られた試料中の、プロカルシトニン又は少なくとも12アミノ酸長のそのフラグメントのレベルを測定し、そして、
プロカルシトニン又はそのフラグメントの前記レベルを、細菌感染の存在と相関させることを含み、24時間当たり少なくとも20%の前記レベル(ng/mL)の減少が、患者における細菌感染の存在を示す、
患者における細菌感染の存在を診断するためのin vitro方法に関する。
臨床検査機関の患者の4997個の連続血清試料を、B.R.A.H.M.S PCT感受性LIA(ルミネッセンス・イムノアッセイ)キット(B.R.A.H.M.S AG, ドイツ国Hennigsdorf, 製品番号109.050)を使用してプロカルシトニン(PCT)のレベルを測定するように分析した。患者の血清は、種々の医療分野、例えば腎臓学、泌尿器学、腫瘍学、小児科、内科、一般内科、及びその他の分野から、種々異なる診察専門医によって検査機関に分析のために送られてきたものである。機能アッセイ感度(FAS)を高め、そして低濃度範囲(0.05〜0.25ng/mL)のPCTレベル濃度を信頼性高く測定するために、試料体積を50μLから100μLに増大させることを除けば、キットに同梱されているマニュアルに従って、アッセイを行った。
プロカルシトニン(PCT)を記載のように測定することができる(Morgenthaler NG他:Clin Chem, 2002 May, 48(5), 788-790)。PCTのカルシトニン部分に対してヒツジ抗体を形成し、PCTのカタカルシン部分に対して、マウス・モノクローナル抗体を形成した。管に抗カタカルシン抗体を被覆した。抗カルシトニン抗体に、MACNアクリジニウムエステル(InVent GmbH、ドイツ国Hennigsdorf)を標識付けし、これはトレーサーとして役立った。正常なウマ血清中の組み換えPCTの希釈体が標準として役立った。100μlの試料又は標準を、30分間にわたって、被覆された管内でインキュベートし、200μlのトレーサーを添加した。2時間にわたってインキュベートした後、管を1mlのLIA洗浄溶液(B.R.A.H.M.S AG)で4回洗浄し、そして固定した。LB952T照度計(ドイツ国、Berthold)を使用して、化学発光を測定した。
Claims (7)
- 感染症ではない原発性疾患を有する患者において、まだ発現していない、そして/あるいはまだ症候性でない更なる疾患又は病状に罹る患者のリスクを見極めるためのin vitroプロカルシトニン・アッセイ方法であって、前記患者から得られた、血液試料、血清試料、血漿試料、又は前記試料のうちのいずれかの抽出物、を含む群から選択される試料中の、プロカルシトニン又は少なくとも12アミノ酸長のそのフラグメントのレベルを測定し;そして、プロカルシトニン又はそのフラグメントの前記レベルを、まだ発現していない、そして/あるいはまだ症候性でない更なる疾患又は病状に罹る患者のリスクと相関させることを含み、前記in vitro方法の相関ステップが、プロカルシトニン又はそのフラグメントの前記レベルを、閾値レベルと比較することを含み、プロカルシトニン又はそのフラグメントの前記レベルが0.02〜0.25ng/mLの閾値レベルを超えているときには、前記患者は前記リスクに対する素因がある、in vitroプロカルシトニン・アッセイ方法。
- プロカルシトニン、又は少なくとも12アミノ酸長のそのフラグメント、又はプロカルシトニン及び/又はそのフラグメントの混合物のレベルが前記患者由来の試料中で測定される、請求項1に記載のin vitroプロカルシトニン・アッセイ方法。
- 原発性疾患を有する患者において、更なる疾患又は病状に罹るリスクを階層化するための、請求項1又は2に記載のin vitroプロカルシトニン・アッセイ方法。
- 前記更なる疾患又は病状の治療又は予防をコントロールするための、請求項3に記載のin vitroプロカルシトニン・アッセイ方法。
- 前記治療又は予防は患者に抗生物質を投与することを含む、請求項4に記載のin vitroプロカルシトニン・アッセイ方法。
- 前記アッセイが異なるプロカルシトニン部分に対する2つの抗体を含むサンドイッチ・アッセイである、請求項1〜5のいずれか1項に記載のin vitroプロカルシトニン・アッセイ方法。
- 一方の抗体がプロカルシトニンのカルシトニン部分に対する抗体であり、そして他方の抗体がプロカルシトニンのカタカルシン部分に対するモノクローナル抗体である、請求項6に記載のin vitroプロカルシトニン・アッセイ方法。
