Description
Claims (13)
- 心臓リズム管理システムであって、
センサユニットであって、該センサユニットは、圧力センサ、および該圧力センサを肺動脈内に係留するための係留システムを備え、該圧力センサは、該肺動脈内に位置決めし、肺動脈内の圧力に基づいて収縮期圧、拡張期圧、脈拍圧、心拍数又は前駆出期のうちの少なくとも1つを表すデータを収集するようになっている、センサユニットと、
該センサから遠隔配置されたパルス発生器と、
リードであって、該リードは、該パルス発生器に接続されるようになっている近位端、および心臓組織に電気的に結合されるようになっている遠位端を有し、該パルス発生器が、該リードを通してペーシング信号を送るようになっている、リードと、
該パルス発生器に動作可能に結合された制御モジュールと、
該圧力センサから該制御モジュールに圧力データを通信する無線通信手段と
を有し、制御モジュールは、収縮期圧、拡張期圧、脈拍圧、心拍数又は前駆出期のうちの少なくとも1つを予め設定された値と関連づけ、該予め設定された値に対する該収縮期圧、拡張期圧、脈拍圧、心拍数又は前駆出期のうちの少なくとも1つの現在の値の関係に基づいて該パルス発生器からのペーシング信号の性質を自動的に調整し、このプロセスを、該関係がさらに調整しなくてもよくなるまで自動的に繰り返すようになっているシステム。 - 前記ペーシング信号は、電気刺激のタイミング、振幅又は部位のうちの1つによって調整される請求項1に記載のシステム。
- 前記無線通信手段は、前記圧力センサ及び前記パルス発生器と動作可能に結合された無線周波数通信機器を含む請求項1に記載のシステム。
- 前記無線通信手段は、前記圧力センサ及び前記パルス発生器に動作可能に結合された超音波通信機器を含む請求項1に記載のシステム。
- 前記無線通信手段は、前記圧力センサ及び前記パルス発生器と動作可能に結合された音響通信機器を含む請求項1に記載のシステム。
- 前記センサユニット及び前記パルス発生器と通信する外部機器をさらに含む請求項1に記載のシステム。
- 心臓内に位置決めされ、生理学的パラメータを測定するようにされた第2のセンサと、
該生理学的パラメータを表すデータを前記制御モジュールに通信する通信手段とをさらに有し、該制御モジュールは、該生理学的パラメータデータとともに、前記予め設定された値に対する前記収縮期圧、拡張期圧、脈拍圧、心拍数又は前駆出期のうちの少なくとも1つとの関係に基づいてリズム管理信号の性質を調整するようにされた請求項1に記載のシステム。 - 前記制御モジュールがさらに、心拍数データを受信し、心拍数を決定し、そして該心拍数を予め設定されたレートと比較するようにされており、肺動脈圧データを受信し、肺動脈圧を決定し、そして該肺動脈圧を予め設定された圧力と比較するようにされており、該心拍数が突然上昇しており、かつ、該肺動脈圧データが該予め設定された圧力より低いかどうかを決定し、そしてそうである場合に、該制御モジュールは、対頻脈ペーシング治療を開始するようにされている、請求項1に記載のシステム。
- 前記制御モジュールがさらに、心拍数データを受信し、心拍数を決定し、そして該心拍数を予め設定されたレートと比較するようにされており、肺動脈圧データを受信し、肺動脈圧を決定し、該肺動脈圧を予め設定された圧力と比較するようにされており、該心拍数が突然上昇しており、かつ、該肺動脈圧データが該予め設定された圧力より高いかどうかを決定し、そしてそうである場合に、該制御モジュールは、細動除去ショックを施すようにされている、請求項1に記載のシステム。
- 前記制御モジュールがさらに、肺動脈圧データを受信し、肺動脈圧を決定し、そして該肺動脈圧を予め設定された圧力と比較するようにされており、該肺動脈圧データが該予め設定された圧力より高いかどうかを決定し、そしてそうである場合に、該制御モジュールは、AV遅延を変化させるようにされている、請求項1に記載のシステム。
- 前記制御モジュールが、AV遅延を減少させ、連続的に肺動脈圧をサンプリングし、該肺動脈圧が低下したかどうかを決定するようにされており、そして該肺動脈圧が低下しない場合に、該制御モジュールは、該AV遅延を増大させるようにされている、請求項10に記載のシステム。
- 前記生理学的パラメータが、大動脈内の圧力であり、そして前記制御モジュールが、前駆出期を計算し、該前駆出期を予め設定された値と比較し、前駆出期が該予め設定された値より低いかどうかを決定し、そしてそうである場合、前記心臓に通信されるパルスレートを増大させるようにされている、請求項7に記載のシステム。
- 前記生理学的パラメータが、大動脈内の圧力であり、そして前記制御モジュールが、肺動脈圧が増大し始める時間(t)を決定し、大動脈弁が開くときを決定し、(t)と大動脈弁が開くときとの間の時間間隔を計算し、該時間間隔を予め設定された値と比較し、そして該時間間隔と該予め設定された値との間の差に基づいてAV遅延を変化させるようにされている、請求項7に記載のシステム。
