Description
本発明の方法を記載する前に、本発明は記載する特定の方法に限定されず、当然変化し得るものと理解すべきである。本発明の範囲は添付の請求の範囲によってのみ限定されるので、本明細書で用いる用語は特定の実施形態を記載する目的のみのためであり、そして限定を意図するものではない。
T細胞免疫抑制剤。T細胞免疫抑制剤は、通常その他の細胞、例えばB細胞、単球、骨髄造血前駆細胞等の増殖および活性の一般的な抑制を伴わずに、T細胞、特にTヘルパー細胞の活性を阻止する化合物である。T細胞免疫抑制を検定する方法は当分野で周知であり、インビトロアッセイ、例えば抗原の存在下でのTヘルパー細胞によるIL−2の放出、抗原または刺激剤、例えばCon Aの存在下での3HチミジンのDNAへの取り込み、同種刺激細胞の存在下での51Crの放出等がある。インビボアッセイはT細胞の増殖、サイトカインの放出、活発に抑制されている間の移植片拒絶の不能性等の測定に依存し得る。
哺乳動物宿主において寛容原およびT細胞免疫抑制剤を同時に投与することにより、抗原特異的免疫寛容性を誘導し、これをさらに前記したような抗増殖性物質の投与と組み合わせることができる。これらの作用物質を別個にまたは一緒に、通常別個に処方することができる。共通の処置スケジュールの経過中に同一時点で、または異なる時点で宿主に導入する場合、作用物質は同時に投与されるといえる。後者の場合、望ましい効果を達成するために化合物を十分に接近した時間で投与する。典型的には、1つの作用物質がその他の作用物質のインビボでほぼ半減期以内に投与される場合、2つの作用物質は同時に投与されると考えられる。患者においていずれかの活性物質は、治療上有効になる十分な組み合わせレベルで存在すべきである。
[実施例]
正常およびMPS Iのイヌにおけるヒトα−L−イズロニダーゼに対する寛容性の誘導
ムコ多糖症Iはα−L−イズロニダーゼ遺伝子の変異により引き起こされる遺伝的症状であり、酵素イズロニダーゼの欠損に至る。この欠損は、粗大化顔貌、巨大舌、巨大肝臓および脾臓、呼吸器障害、心臓障害、関節硬直、並びに骨疾患進行性の多重システムリソソーム貯蔵障害に至る。該疾患は通常10歳代または20歳代で患者の死亡に至る。
動物。正常およびMPS IのイヌをHarbor−UCLAのMPS Iイヌ・コロニーから入手した。イヌは、ビーグルとプロットハウントの雑種であり、体重は平均12から20kgである。これらの実験用にイヌは2年齢以下であり、そして少なくとも4月齢であった。

実験計画の基本的な概要を図1に示す。イヌの全コホートを免疫抑制剤で前処置し、続いて予定されている場合はIT注射を行い、そして次に2週後の初めにイズロニダーゼの一連の毎週誘発を行った。イヌはその他の処置(胸腺内注射、モノクローナル抗体)を伴うかもしくは伴わない隔日のCsA(前記の処置計画参照)を用いる非寛容性誘導処置、またはその他の処置を伴うかもしくは伴わないCsA毎日投与を用いる寛容性誘導処置のいずれかを受けた。1匹のイヌ、PAには毎日CsAを投与したがAzaは投与しなかった。
寛容性の誘導は、臨床で有用な治療用タンパク質に対する臨床上有意な免疫応答を防御しなければならない。イズロニダーゼを用いる酵素代償療法を受けているMPS Iイヌは、酵素のクリアランスを遅延させるか、または安定性を変化させ、酵素の取り込みを妨げ、そして酵素療法の有効性を制限する可能性のある高力価抗体に応答する。同一の現象はその他の動物モデルにおいても報告された。
動物。MPS IイヌをHarbor−UCLAのMPS Iイヌ・コロニーから入手した。イヌはビーグルとプロットハウントの雑種であり、体重は平均12から20kgである。これらの実験用にイヌは2年齢以下であり、そして少なくとも4月齢であった。
3匹のMPS Iイヌでは寛容性が誘導されたが、対照MPS Iイヌでは誘導されなかった。毎週の酵素注入を開始する前にCsA+Azaの18日を含む寛容化療法を用いて3匹のMPS Iイヌが寛容化された。1匹のMPS Iイヌは対照として提示され、そして寛容化療法を受けなかった。4匹のイヌは1から12週でイズロニダーゼ0.056mg/kg/週の静脈内注入を受けた。ELISAにより測定される、3匹の寛容MPS Iイヌ(図3;中抜きの記号)および1匹の非寛容MPS Iイヌ(図3;塗りつぶした記号)のイズロニダーゼに対する抗体力価を示す。持続的な抗原誘発を伴う低抗体レベル(<20 OD)は寛容性を示す。寛容化薬物処置計画(CsA+Aza)を酵素誘発の7週で終え、そしてその点を超える低抗体レベルは寛容性が誘導されたことを示している。
毎日CsA+Aza、続いて免疫抑制剤を漸減させながら寛容化抗原の注入の処置計画を用いる、正常およびMPS Iイヌにおけるイズロニダーゼ注入に対する寛容性の誘導を実証した。高親和性取り込み特性を有する別の酵素に対して寛容性を誘導することができることを実証するために、組換えヒトアルファ・グルコシダーゼを調製し、そして寛容化療法を用いて研究した。2匹の正常なイヌを、1匹は寛容化療法を用いて、そして1匹は対照として研究した。グルコシダーゼでの毎週の注入を開始し、そして3週までに対照のイヌは免疫力価を上昇させた。5週までに、対照のイヌは力価が100倍高く、そして処置したイヌは力価が有意でなかった。データにより、その他の抗原を用いて寛容化療法が成功し得ることが示される。

対照イヌSTは薬物処置を受けず、そして毎週の静脈内注入によりアルファ・グルコシダーゼの投与を開始した。3週までに、有意な力価が検出され、そして4週までに力価は100 OD/血清μl以上まで上昇した。続いて、力価は100から200単位/血清μlの範囲になった。対照的に、イヌSCはCsA+Aza処置計画を受け、そして12週間の注入の間にいずれかの免疫応答がある場合、最低であった。これは免疫抑制剤を投与しなかった7から12週を含む。
寛容性プロトコールの開発に関する初期の研究では胸腺注射、免疫抑制剤および成熟T細胞を枯渇させるモノクローナル抗体の組み合わせに注目された。寛容化抗原発現(胸腺内注射)、成熟抗原特異的T細胞応答の抑制(CsA+Aza)および抗原に応答できる成熟T細胞の枯渇の完全な組み合わせが活性化免疫応答を遮断し、そして抗原暴露を受け入れる免疫系を調製するために必要であると考えられた。この研究の開発中に、イヌJHはITIおよびCsA+Azaのみを受けたが寛容になった。このイヌのさらなる分析により、このイヌのCsA代謝によりCsAの血清レベルが実質的に高くなったことが実証された。この知見が、免疫抑制剤および寛容原のみで寛容性の導入に至るさらなる実験を可能にする中心的な結果になった。

酵素注入。1mg/ml イヌアルブミン、および10mM NaPO4を含む食塩水の50ml注入液中α−L−イズロニダーゼ 14,000単位/kg/週を2時間かけて静脈内投与した(1時間目で3,000単位/kg/時間、2時間目で11,000単位/kg/時間)。アナフィラキシー反応に関して動物をモニター観察した。
初期の寛容性実験は隔日の間隔でのCsA投与を含めた。これらの初期の実験は、ITI、免疫抑制剤および/またはT細胞を枯渇させるモノクローナル抗体が寛容性の誘導に必要であるかどうかを確立しようとした。図5はITIおよび薬物を用いるいくつかの初期のイヌの実験の結果を示す。
寛容原投与がなくても寛容状態が長期持続性である場合、寛容原による寛容性の誘導は特に有用である。寛容状態の安定性に取り組むために、寛容および非寛容のイヌを種々の時間間隔で保持し、そして次にイズロニダーゼ注入で再誘発した。データは少なくとも6か月間、寛容動物が持続性の寛容原または抗原暴露がなくても、イズロニダーゼ注入に対して寛容を維持することを示している。
記載した各イヌに関して(同一の頭文字を使用した)前記の実施例で記載するように寛容性を誘導した。JOはその他のイヌと異なって、隔日にCsAを投与して寛容化されたイヌであった。このイヌは最初完全な寛容性を有さなかったが、抗体応答は控えめであった。その他の寛容イヌはその他の実施例で記載した研究に基づいて十分に寛容であった。イズロニダーゼは同一の前記したのと同一の高取り込みイズロニダーゼであった。週基盤で3から6用量の静脈内投与した酵素0.58mg/kgでイヌを誘発した。
図6では、寛容および非寛容であったイヌの力価を、寛容原または治療用注入を行わない期間に充てたギャップと共に示す。最低5週から最大6か月の寛容原の中断は寛容イヌの応答を変化させず、イヌは寛容のままで、そして有意な免疫応答を上昇させなかった。非寛容イヌRUは4.5か月の中断の後酵素の2用量の投与後に抗体力価の>20倍の誘導を示した。イヌJHは、再誘導の3週で最も寛容であったイヌの応答を超えるある程度の応答を示したが、感作された非寛容イヌRUの応答には達しなかった。JHはまた元来の誘発を行われた場合にさらに有意な応答性を有し、そして最適処置計画で寛容化されたイヌほど寛容ではなかった。そのイヌの元来の力価パターンもまたプラトーを示し、そして継続的な注入で低下し、それをこの症例で研究する。
材料および方法
患者。ムコ多糖症Iの患者を処置のために選択する。ベースラインおよび6、12、26、および52週に詳細な臨床試験、腹部および脳の核磁気共鳴映像法、心エコー検査法、可動域測定、睡眠ポリグラフ計、臨床検査評価、白血球α−L−イズロニダーゼ活性の測定、並びに尿グリコサミノグリカン排泄により患者を評価する。
非寛容イヌで観察されるIgG応答に比較して、イズロニダーゼに対する全IgG応答の低下により明らかであるように、イヌの寛容化療法はIgGの誘導を防御する。本実施例は、IgGの誘導の防御に加えて、その他の免疫グロブリンサブタイプの誘導を寛容化療法が防御することを示すさらなる証拠を提供する。寛容(イヌPEおよびSA)または、寛容化療法を受けなかったイヌ対照(イヌRUおよびUM)のいずれかであるイヌの免疫血清の分析を、イズロニダーゼに対するIgG、IgAおよびIgE抗体の存在に関して研究した。
本発明の別の態様では、発明者は既存の免疫応答を低減または排除することが可能であることを実証した。本実施例はイヌNitroから作製されたデータを論考する。Nitroは元来寛容化されているが、ほぼ6か月の抗原暴露からの中断期間の後に、再暴露が著明な既往反応を誘導し、力価は400+に達した。抗原に対する暴露を行わない数週間の後、イヌを寛容化療法に置き、そして低用量(0.056mg)のイズロニダーゼの毎日注入を与えた。力価は最初CsA+Aza処置計画の投与中に典型的な既往反応で100以上まで上昇し、そして次に20以下まで速やかに低下した(図18参照)。処置計画により応答は低減され、そして酵素に対する免疫応答は、以前に0.5mg/kg/週の注入での>400レベルと比較して相対的に控えめであった。これらのデータにより、動物が寛容原組成物中の抗原に対する免疫応答を既に有している場合でさえ、寛容原に対する寛容性を低減させることができることが実証される。
寛容原における高親和性部分の影響を前記の実施例1で論考したが、これはマンノース6−ホスフェートのような部分が抗原の寛容化能力を増大させるのに有用であろうということを示した。寛容原組成物のこの態様をさらに研究し、そしてその結果を本実施例および図9にて報告する。イズロニダーゼは通常マンノース6−ホスフェートを含み、そしてこの高取り込み部分はマンノース6−ホスフェートレセプターを介して標的細胞によるイズロニダーゼの取り込みを促進する。
血友病Aの処置は有効量の第VIII因子を血友病患者に分配する能力に依存する。しかしながら、第VIII因子治療に対する抗体またはインヒビターが血友病A疾患の患者において重大な問題になり得る。第VIII因子に対する抗体インヒビターを発達させた血友病患者は第VIII因子治療を無効にする制御不能な出血を経験することがある。実施例1から9は、宿主を寛容原に対して寛容化するのに十分な期間の免疫抑制の処置計画と組み合わせた、寛容原としてα−L−イズロニダーゼまたはアルファ・グルコシダーゼの抗原特異的免疫寛容性投与を誘導するための方法および組成物を記載する。本実施例は第VIII因子に対する免疫寛容性を誘導することを志向する。本実施例は第VIII因子に対するインヒビターの形成を防御し、そして逆転させるために用いるプロトコールを提供する。
β細胞抗原、例えばグルタミン酸デカルボキシラーゼ(GAD)に対する抗体および細胞性免疫応答は膵臓のβ細胞の破壊および糖尿病の発症の原因となる。これらの抗原に対する寛容性の誘導はI型糖尿病の症状の危険にあるか、または発症している患者における糖尿病の進行および発症を防御するための重要な方法を提供する。GADは1つの重要な抗原であるが、その他の抗原、例えばIA2およびインスリンをこの方法で研究することもできる。

前記哺乳動物宿主がまず、前記シクロスポリンAおよび前記治療用ポリペプチドの組み合わせで寛容化療法の間に維持され、次に前記シクロスポリンAの投与をやめた後も前記可溶性治療用ポリペプチドで依然維持され、
前記哺乳動物宿主が、継続的な免疫抑制の不在下で、前記寛容化療法を経験していない哺乳動物宿主において前記治療用ポリペプチドによって誘導される抗体力価に比べて低減された治療用ポリペプチドへの抗体力価を示すことを特徴とするキット。 - さらに抗増殖性物質として6−メルカプトプリン薬を含む請求項1に記載のキット。
