Description
(1)緑内障(glaucoma)
緑内障は40才以上で発症率が3.5%である網膜疾患で、わが国では192万人、世界中で1億500万人の患者が推定されている。症状としては、眼圧上昇を基調とし、網膜萎縮、視神経乳頭の陥没を起こす。予後が悪く、最悪の場合には失明までに至る場合があり、日本では失明原因の第4位、米国では年間12万人もの患者が報告されている。
緑内障は、眼圧上昇が基調となるため、眼圧降下を目的とした薬剤が多く開発されてきた。作用点の違いによって、副交感神経作動薬、交感神経刺激剤、交感神経β遮断剤、炭酸脱水素酵素阻害剤、高浸透圧剤の5つに分類されるが、全て眼圧降下によって薬効を示す。しかし眼圧を下げても予後が悪く、視神経への障害をきたす症例が認められる。また、眼圧上昇を伴わない緑内障、すなわち低眼圧緑内障が数多く報告され、米国では眼圧上昇を付随した緑内障患者200−300万人に対して、低眼圧緑内障は100万人もの患者が推定されている。従って眼圧降下を目的としたものでなく、視神経細胞に対して直接に保護効果をもたらす薬剤が望まれる。
(2)網膜中心動脈閉塞症(central retinal artery occlusion)
網膜中心動脈閉塞症は、網膜中心動脈の篩状板通過点で血栓が詰まることによって発症する。症状としては、片眼性の急激な視力低下を起こし、視神経萎縮を引き起こす。慢性の虚血とは異なり、後に血管の新生は認められない。網膜中心動脈は終動脈であるため、急性の虚血状態が30〜40分間続くと不可逆な変化を起こし、網膜は壊死に至る。従って完全閉塞の場合は視力予後は不良である。
治療法は、発見後直ちに眼球マッサージを行い、血流再開に努める。薬物処置としては、血栓溶解を目的として、ウロキナーゼ+デキストランを、血管拡張・血栓形成阻止を目的として、プロスタグランディンE1(アルプロスタジル)を使用する。
(3)網膜中心動脈分枝閉塞症(branch retinal artery occlusion)
網膜中心動脈分枝閉塞症は、血栓が眼内の分枝部で詰まり、その支配領域のみに障害が発症するものである。治療法は、網膜中心動脈閉塞症と同様に行われている。
(4)網膜中心静脈閉塞症(central retinal vein occlusion)/網膜中心静脈分枝閉塞症(branch retinal vein occlusion)
網膜中心静脈閉塞症〔出血の有無により、出血性網膜症(hemorrhagic retinopathy)と静脈うっ帯網膜症(venous stasis retinopathy)に分類される。〕および網膜中心静脈分枝閉塞症は、篩状板に発生した血栓によるものである。
出血性網膜症は、老人に起きることが多く、半数以上が動脈硬化を原因としている。症状としては、視神経乳頭を中心として、神経線維の走行に沿って、網膜表層に火炎状に広がる出血を起こし、視力の著しい低下を引き起こす。
治療法としては、網膜中心動脈閉塞症と同様にウロキナーゼ+デキストランを使用し、血管強化を目的としてカルバゾクロムスルホン酸ナトリウムまたはメシル酸アドレノクロムグアニルヒドラゾンを適用する。また、黄班浮腫あるいは新生血管の防止のため、レーザー光凝固を行う。
静脈うっ帯網膜症は、炎症を起因とするものと動脈硬化を起因とするものが知られており、前者は若者、後者は老人に多く見られる。症状としては、静脈の強い拡張、蛇行、さらに視神経乳頭の発赤が起きるが、視力低下は出血型に比べて軽微である。
治療法は出血性と同じ薬物処置を行うが、レーザー光凝固は行わない。
(5)虚血性視神経症(ishchemic optic neuropathy)
虚血性視神経症は、栄養血管の閉塞により、視神経に虚血壊死が生じ、視機能障害が現れる病気であり、前部虚血性視神経症(anterior ishchemic optic neuropathy)とも呼ばれる。
原因としては、側頭動脈炎と、動脈硬化症、高血圧症、糖尿病などの全身疾患により発症する特発性虚血性視神経症に分けられる。
治療法は薬物治療のみがとられる。浮腫を軽減する目的で、ステロイド(プレドニゾロン)が、篩状板付近の血流改善を目的として、眼圧降下剤(炭酸脱水酵素阻害剤,アセタゾラミド)が、神経賦活を目的として、ビタミンB1,B12が適用されている。
(6)糖尿病性網膜症
糖尿病性網膜症は、数年以上の高血糖持続により、細小血管内皮細胞の変性、壊死、血栓形成、血液凝固能亢進から細小血管閉塞が起こり、虚血状態となり発症する。インシュリン依存性で39%、非依存性で47%の患者が発病し、日本国内で約60万人の患者が推定されている。
治療法として、単純期では全身性に血糖値のコントロールをし、出血のある場合は血管強化剤、ストレプトキナーゼ・ストレプトドルナーゼ(バリダーゼ)を適用し、前増殖期では汎網膜光凝固を行う。さらに増殖期では牽引性網膜剥離を生じた場合硝子体手術を行う。網膜神経の保護を目的とした薬物はなく、補助的にコハク酸トコフェロールカルシュウム(ビタミンE)、酢酸トコフェロール(ユベラ)が適用されているのみである。従って虚血時の障害を軽減する新薬開発が望まれる。
(7)黄班変性症
黄班部は光軸が通過する網膜部であり、視野の中心部を担い、視力を決定する重要な領域である。黄班変性症は、この部分に異常をきたした症状の総称であり、おもに5つの疾患が知られている。すなわち、中心性漿液性脈絡網膜症(central serious chorioret)、中心性滲出性脈絡網膜症(central exudative chorioretinopathy)、老人性円板状黄班変性(senile disciform macular degeneration)、老人性萎縮性黄班変性(senile atrophic macular degeneration)、特発性硝子体網膜界面黄班症(idiopathic vitreouretinal interface maculopathy)に分類される。
中心性漿液性脈絡網膜症および中心性滲出性脈絡網膜症は、脈絡膜より漿液あるいは出血(滲出)が、破綻した色素上皮細胞を通過して網膜下に滞留する疾患である。
老人性円板状黄班変性は黄班部に滲出性の変化と出血、脈絡膜からの血管新生を起こす疾患で、老人性萎縮性黄班変性では滲出性の変化が認められず、網膜色素上皮細胞の萎縮を特徴とする。
特発性硝子体網膜界面黄班症は、黄班部に透明あるいは不透明な網膜前膜が形成し、黄班に走行する血管が蛇行する。
老人性萎縮性黄班変性を除いていずれの疾患も、血管障害に起因することから、虚血性の障害が考えられる。
治療法としては、基本的には新生血管が発生している場合には光凝固による処置を行い、薬物療法としては、網膜下の漿液の吸収を目的として、高滲浸透剤(イソソルビルやグリセリン)の使用、網脈絡膜の循環改善を目的として、カリジノゲナーゼの使用、血管拡張を目的として、ストレプトキナーゼ・ストレプトドルナーゼ(バリダーゼ)の使用、その他、ビタミン剤、ステロイド剤が適用されているが、網膜神経細胞に作用する薬剤はなく新薬開発が期待される。
(8)未熟児網膜症(retinopathy of prematurity)
未熟児網膜症は、早産児への高濃度の酸素投与が起因して、未熟な網膜血管の末梢部に閉塞性変化が生じ、酸素投与停止後、低酸素状態になる。その治療法は、冷凍凝固を行う方法等の対症療法的なものであり、根本的な治療を目的とした薬物は無く、従って虚血時の障害を軽減する新薬開発が望まれる。
また、脳神経疾患または脳血管障害においても、脳塞栓症、一過性脳虚血、鎖骨下動脈盗血症候群、Wallenberg症候群(延髄外側症候群)、脳血栓症、ラクナ梗塞、可逆性虚血性神経障害、脳梗塞、もやもや病(ウィリス動脈輪閉塞症)、低酸素性脳症、静脈洞血栓症、または術後脊髄虚血などで虚血による障害が関与しており、これらの疾患における虚血時の障害を軽減する新薬開発が望まれる。
一方、 セマフォリンは、神経成長円錐を退縮させ軸索の伸長を抑制する因子として同定された内因性のタンパク質であり、これまでに約20種の分子種が知られているが、最も良く研究されているのがクラス3型と呼ばれるサブファミリーの遺伝子群である。これらの遺伝子がコードする蛋白質はインビトロで強い神経突起伸長抑制活性、成長円錐退縮活性を有していることが知られている。中でも最も良く研究されているのがセマフォリン3A(Sema3A)(Cell,75,p217(1993)およびCell,75,p1389(1993)を参照)であり、この蛋白は10pMという低濃度で短時間のうちに培養神経細胞の成長円錐退縮を誘発する。また、ラット視神経軸索切断モデルにおいて、網膜神経節細胞の細胞死(アポトーシス)を、抗セマフォリン3A抗体が抑制することが知られている(The Journal of Biological Chemistry,277,p49799(2002)を参照)。前記モデルは軸索(神経線維)のみが切断され、神経脈管構造や視神経への血液供給にはダメージを与えていない創傷障害モデルである。さらに、ウサギを用いた先天性緑内障モデルの網膜において、セマフォリンの発現が上昇していることも知られている(Graefe’s Archive for Clinical and Experimental Ophthalmology,241,p673(2003)を参照)。一方、ラットを用いた中大脳動脈閉塞モデルでは、セマフォリン3AのmRNAが発現していることも知られている(Brain Research,914,p1(2001)を参照)。
一連のキサントン化合物が、セマフォリン阻害活性を有し、神経再生促進作用を有することが知られている(国際公開第02/09756号パンフレット、国際公開第03/062243号パンフレット及び国際公開第03/062440号パンフレットを参照)。しかし、これらのセマフォリン阻害活性を有する化合物が、虚血時の細胞死(アポトーシス)、例えば網膜神経細胞死や脳における神経細胞死を抑制し、虚血障害に有効な治療剤又は予防剤として優れた効果を示すことは知られていなかった。
本発明者らは、セマフォリン阻害活性を有する化合物が、虚血性障害が関与する神経細胞死を抑制することを見出し、虚血性神経障害の治療剤又は予防剤として有用であることを見出した。
すなわち、ラット前房室内を、血圧測定装置を用いて約150mmHgの圧力で負荷することにより、眼圧上昇を施行し、虚血性障害を惹起するモデル動物を作成した。該モデル動物に対して、ペニシリウム・エスピー(Penicillium sp.)SPF−3059株を培養することにより得られるセマフォリン阻害活性を有する化合物を投与したところ、優れた治療効果を示した。従って、セマフォリン阻害活性を有する化合物は、虚血性障害を治療もしくは要望する薬剤として、好ましくは網膜における虚血性障害を治療もしくは予防する薬剤として有効であることがわかった。
すなわち、本発明は、

図5は、外顆粒細胞層(ONL:outer nuclear layer)の厚みを測定した結果を示す図である〔Student’s t−test,compared to(IOP上昇負荷+PBSを投与)Group,*:〈0.05〕。横軸の番号の意味は以下のとおり。すなわち、1 正常眼圧;2 眼圧(IOP)上昇負荷+PBSを投与;3 IOP上昇負荷+SPF−3059−5を前投与処置;4 IOP+SPF−3059−5を後投与処置。図5より、眼圧上昇負荷およびSPF−3059−5の投与は共に外顆粒細胞層へは影響を及ぼさないことがわかる。

本明細書において、アシルオキシ基とは、炭素数2〜6の直鎖もしくは分枝のアシルオキシ基を表し、具体的には、アセトキシ基、プロピオニルオキシ基、ブチリルオキシ基、ピバロイルオキシ基等を例示することができる。
ペニシリウム・エスピー(Penicillium SP.)SPF−3059株を培養することにより得られるセマフォリン阻害活性を有する化合物であって、式[1]で表される化合物としては、国際公開第02/09756号パンフレット又は国際公開第03/062243号パンフレットに開示された一連のセマフォリン阻害活性を有する化合物を挙げることができる。また、該化合物の誘導体としては、国際公開第03/062440号パンフレットに開示された一連のキサントン誘導体を例示することができる。
式[1]において、R7及びR8における有機基としては特に限定は無く、R7及びR8は結合して飽和もしくは不飽和の環構造を形成していてもよい。
式[1]、[2]、[3]、[4]および[6]で表される化合物において、R1は、水素原子、カルボキシ基又はアルコキシカルボニル基を表す。また、前記アルコキシカルボニル基としてはメトキシカルボニル基、エトキシカルボニル基、プロポキシカルボニル基等の炭素数2〜4のアルコキシカルボニル基を挙げることができ、中でもメトキシカルボニル基が好ましい。特に、式[2]、[3]及び[6]におけるR1は、好ましくは水素原子又はカルボキシ基を表し、式[4]におけるR1は、好ましくは水素原子、カルボキシ基又はメトキシカルボニル基、更に好ましくは水素原子又はカルボキシ基を表す。
また、式[1]、[2]、[3]、[4]及び[6]において、R2は、水素原子、水酸基又はアシルオキシ基、好ましくは水素原子又は水酸基を表す。また、前記アシルオキシ基としては、アセトキシ基、プロピオニルオキシ基、ピバロイルオキシ基等を挙げることができる。
また、式[4]において、R3は、水素原子、メトキシメチル基又は前記式[15]に示される基を表す。

上記の式[1]〜[14]で表される化合物は、ペニシリウム・エスピー(Penicillium sp.)SPF−3059株の培養物からセマフォリン阻害活性を指標として得ることができる。また、このようにして得られたセマフォリン阻害活性を有する化合物等から公知の転換方法や公知の合成方法により製造した化合物からセマフォリン阻害活性を指標として同定することができる。
また、式[1]〜[14]で表される化合物においては、それらの塩やそれらの誘導体、好ましくは医薬的又は獣医薬的に許容される塩や誘導体も本発明の範疇に含まれる。ここで、塩としては、ナトリウム塩、カリウム塩、カルシウム塩、マグネシウム塩、アルミニウム塩、アンモニウム塩等の無機塩基塩や、トリエチルアンモニウム塩、トリエタノールアンモニウム塩、ピリジニウム塩、ジイソプロピルアンモニウム塩等の有機塩基塩や、アルギニン、リジン等の塩基性アミノ酸塩などを挙げることができる。また誘導体としては、化合物のカルボキシ基や水酸基がエステル基に変換された誘導体が挙げられ、例えば水酸基がアセチル基、プロピオニル基などの炭素数2〜5のアシル基でアシル化された誘導体や、カルボキシ基がメチルエステル、エチルエステルなどの炭素数2から5のエステルへ変換された誘導体等を挙げることができる。前記誘導体は、例えば国際公開第03/062440号パンフレットに記載された方法を参照すればよい。
前記式[1]〜[14]で表される化合物は、いずれも大阪府内土壌より分離したペニシリウム属に属するカビSPF−3059株[本菌株は、特許手続上の微生物の寄託の国際的承認に関するブタペスト条約に基づき、2001年7月13日に経済産業省独立行政法人産業技術総合研究所特許生物寄託センター(〒305−8566 茨城県つくば市東1−1−1 中央第6)に受託番号FERM BP−7663として寄託されている。]を培養することにより効果的に得ることができ、国際公開第02/09756号パンフレット又は国際公開第03/062243号パンフレットに記載された方法に従って得ることができる。
セマフォリン阻害活性を有する本発明の化合物は、虚血性障害による神経細胞死の抑制作用を示したことから、虚血性神経疾患治療剤又は予防剤として使用しうる。ここでいう虚血性神経疾患としては、虚血による網膜神経障害または虚血性脳血管障害が挙げられる。ここでいう網膜神経障害としては、例えば緑内障、網膜中心動脈閉塞症、網膜中心動脈分枝閉塞症、網膜中心静脈閉塞症、網膜中心静脈分枝閉塞症、虚血性視神経症、糖尿病性網膜症、黄班変性症、未熟児網膜症等が挙げられ、中でも糖尿病性網膜症が好ましい。また虚血性脳血管障害としては、例えば、脳塞栓症、一過性脳虚血、鎖骨下動脈盗血症候群、Wallenberg症候群(延髄外側症候群)、脳血栓症、ラクナ梗塞、可逆性虚血性神経障害、脳梗塞、もやもや病(ウィリス動脈輪閉塞症)、低酸素性脳症、静脈洞血栓症、または術後脊髄虚血などが挙げられる。本発明の化合物は網膜神経保護作用を有しており、特に、虚血による網膜神経障害の治療又は予防に有効である。
本発明の虚血性神経障害治療剤又は予防剤は、薬学的に許容される通常の担体、結合剤、安定化剤、賦形剤、希釈剤、pH緩衝剤、崩壊剤、可溶化剤、溶解補助剤、等張剤などの各種調剤用配合成分を添加することができる。またこれら治療剤又は予防剤は、経口的又は非経口的に投与することができる。すなわち経口的には、通常用いられる投与形態、例えば錠剤、丸剤、粉末、顆粒、カプセル剤、シロップ剤、乳剤、懸濁液等の剤型で経口的に投与することができる。
非経口的には、例えば、静脈内注射(点滴剤)、筋注射剤、皮下注射剤、点眼剤、眼軟膏剤等の形態の製剤とすることができる。
錠剤のような固体製剤は有効成分を乳糖、ショ糖、トウモロコシ澱粉などの通常の薬理的に許容しうる担体または賦形剤、ヒドロキシプロピルセルロース、ポリビニルピロリドン、ヒドロキシプルピルメチルセルロースなどの結合剤、カルボキシメチルセルロースナトリウムや澱粉グリコール酸ナトリウムなどの崩壊剤、ステアリン酸やステアリン酸マグネシウムなどの滑沢剤、あるいは保存剤等と混合して調製される。
非経口投与には、有効成分は水、生理食塩水、油、ブドウ糖水溶液などの生理的に許容しうる担体に溶解または懸濁し、これは補助剤として乳化剤、安定化剤、浸透圧調整用塩または緩衝剤を必要に応じて含有してもよい。点眼剤の添加物として、グリセリンや塩化ナトリウムなどの等張化剤、リン酸やクエン酸などの緩衝剤、塩酸や水酸化ナトリウムなどのpH調節剤、ヒドロキシプルピルメチルセルロースやポリビニルアルコールなどの増粘剤、塩化ベンゼトニウムなどの保存剤、あるいは可溶化剤を必要に応じて含有してもよい。また、眼軟膏剤の添加剤としては、ワセリン、ポリエチレングリコール、精製ラノリン、流動パラフィン等が例示される。
投与量及び投与回数は、投与法と患者の年齢、体重、病状等によって異なるが、病床部位に局所的に投与する方法が好ましい。また、1日あたり1回又は2回以上投与することが好ましい。2回以上投与するときは連日あるいは適当な間隔をおいて繰り返し投与することが望ましい。
投与量は成人患者一人一回当たり有効成分の量として数百μg〜2g、好ましくは5〜百mg、更に好ましくは数十mg以下を用いることができ、一日一回または数回にわけて投与することができる。非経口投与では、成人患者一人あたり0.1〜100mg/日、さらに好ましくは0.3〜50mg/日の投与量が挙げられ、一日一回または数回に分けて投与することができる。投与回数を減らすために徐放性製剤を用いることもできる。点眼剤として用いる場合には、有効成分の量として、成人患者一人あたり0.01〜10w/v%、好ましくは0.05〜5w/v%を用いることができ、症状に応じて1回量1〜数滴を1日1〜6回投与することが望ましい。また、眼軟膏剤として用いる場合には、有効成分の量として、0.01〜10w/w%、好ましくは0.1〜5w/w%を用いることができ、症状に応じて1日1〜6回投与することが望ましい。
また、本発明の虚血性神経障害治療剤又は予防剤は、動物薬としての利用も可能である。
[実施例1]
眼圧上昇モデルに対する薬理効果
網膜神経は、網膜外顆粒層、網膜内顆粒細胞および神経節細胞層から構成されており、網膜内顆粒細胞および神経節細胞層は網膜中心動脈から、血液の供給を受けている。前房に圧力をかけると強膜に被われていない篩状板に負荷がかかり、この領域を通過して伸展する網膜中心動脈は閉塞され、その結果、網膜内顆粒細胞および神経節細胞層が虚血状態となり、細胞死を起こす。
網膜内顆粒細胞の障害は、細胞数の変化を「内顆粒細胞層(INL:inner nuclear layer)の厚さ」として容易に評価できる。また、眼圧上昇によって各神経細胞間のシナプスである内網状層も障害を受け薄層化することから、内網状層(IPL:inner plexus layer)の厚さも障害の程度を知る指標となる。この2つのパラメーターを指標として抑制効果を有する網膜神経障害治療剤又は予防剤を探索した。
[試験方法]
(1)実験動物
SD系ラット6週齢を20匹使用し、以下の群分けを行った。
Shamオペレーション(以下Sham opeと略する場合がある。) 5匹(うち解析4例)
IOP(Intra Ocular pressure)処置+PBS(5μl硝子体内投与) 5匹(うち解析4例)
IOP(Intra Ocular pressure)処置+被験物質(5μl硝子体内前投与) 5匹(うち解析4例)
IOP(Intra Ocular pressure)処置+被験物質(5μl硝子体内後投与) 5匹(うち解析4例)
(2)眼圧上昇
以下の工程で、眼圧上昇モデル動物を作成し、薬剤(評価化合物)の投与を行った。
(i)ラットを50mg/kg,i.p.投与麻酔下で、脳固定装置に固定した。
(ii)ラットの頭部を固定し、眼球に血圧測定装置をつないだチューブの先に26Gの針を装着し、ラット前房室内に刺入し、前房室内圧140〜160mmHgの圧で負荷をかけた。
(iii)薬剤の硝子体内投与のタイミングに関しては、前投与処置の場合は眼圧上昇負荷の15分前に投薬を行い、後投与処置の場合は眼圧上昇負荷15分後に投薬を行った。(iv)1時間眼圧上昇を維持し、眼圧負荷後ラットを脳固定装置から解除し、1週間通常飼育を行った。
(v)炭酸ガスによる窒息死後、眼球を摘出した。
(vi)パラフィン網膜切片を作成し、0.5%クレシルバイオレットで染色した。
(vii)視神経乳頭より880μmの位置(網膜上方)の組織像を撮影し、外顆粒細胞層(ONL:outer nuclear layer)、内顆粒細胞層(INL:inner nuclear layer)、内網状層(IPL:inner plexus layer)の厚さを測定した。
(3)薬剤の調整および投与方法
被験物質としてSPF−3059−1をPBSに1mg/mlの濃度で溶解し、使用直前にPBSで更に0.1mg/mlに希釈した。希釈した溶液、5μlを30G二重針にて、ラット左眼硝子体内に投与した。
(4)有意差検定
上記(2)の方法で眼圧上昇を施行し、PBSを硝子体内投与したラットに対して、SPF−3059−1投与による網膜保護効果をStudent’s t−testおよびWelch&F−testにて有意差検定を行った。
[薬理評価]
各処置群のラット網膜を用いて、外顆粒細胞層、内顆粒細胞層、内網状層それぞれの層の厚さを測定し、細胞保護効果を評価した。以下、表1及び図1〔外顆粒細胞層(ONL:outer nuclear layer)〕、表2及び図2〔内顆粒細胞層(INL)〕、表3及び図3〔内網状層(IPL)〕に、測定値を示した。また、図4に0.5%クレシルバイオレットで染色した網膜切片を示した。
外顆粒細胞は脈絡層からの血液供給を受けており、眼圧上昇による眼動脈虚血障害を受けないことがわかっているが、表1及び図1の結果より、本実施例でも外顆粒細胞層の厚さは、眼圧上昇による影響を受けないことが示された。以上のことから、本実験で作成した切片は適切に作成されていることが確認できる。
内顆粒細胞は眼動脈からの血液供給を受けており、眼圧上昇によって眼動脈虚血障害を受け、遅発性の細胞死を引き起こすことがわかっている。表2及び図2の結果から、本実施例により、眼圧上昇によって内顆粒細胞が細胞死を引き起こし、同細胞の存在する細胞層の薄層化が観察された。さらにSPF−3059−1の前投与によって内顆粒細胞死が抑制され、同細胞層の薄層化が有意に抑制されることが示された。またSPF−3059−1の後投与によって、統計学的な有意差は認められなかったものの、薄層化の抑制傾向が観察され、内顆粒細胞死に対する細胞死抑制効果が考察された。
更に、内網状層は、内顆粒細胞と神経節細胞間のシナプス形成がなされている層であり、細胞体から進展している軸索のみが存在する層である。内顆粒細胞が眼圧上昇による眼動脈虚血障害を受けることから、その軸索である内網状が退縮し、内網状層の薄層化が起こることが知られている。本実施例によってもこの現象が再現され、眼圧上昇による内網状層の薄層化が観察された。さらにSPF−3059−1の前投与によって内顆粒細胞死が抑制され、内網状層の薄層化が有意に抑制されることが示された。またSPF−3059−1の後投与によって、統計学的な有意差は認められなかったものの、薄層化の抑制傾向が観察され、内顆粒細胞死に対する細胞死抑制効果が考察された。
以上の結果、SPF−3059−1は眼圧上昇による内顆粒細胞の細胞死を抑制することが示された。また内網状層は、内顆粒細胞および神経節細胞両者のシナプス形成の場であり、神経節細胞の軸索も含まれていることから、神経節細胞への保護効果も考えられた。
また、SPF−3059−1の前投与処置では、内顆粒細胞層および内網状層の薄層化に対して統計学的に有意に抑制作用を有していることが示された。更に、内顆粒細胞層および内網状層の薄層化に対して、SPF−3059−1の後投与処置を行うことによっても、抑制傾向を示すことがわかった。
上記の結果から、眼疾患において、内顆粒細胞の虚血性障害が疑われる糖尿病性網膜症、神経節細胞の障害が報告される緑内障等に対し、SPF−3059−1が有効であることがわかった。
[実施例2]
眼圧上昇モデルに対する薬理効果
[試験方法]
(1)実験動物
SD系ラット7週齢を20匹使用し、以下の群分けを行った。

実施例1と同様の工程で、眼圧上昇モデル動物を作成し、薬剤(評価化合物)の投与を行った。
(3)薬剤の調整および投与方法
被験物質としてSPF−3059−5を使用直前にPBSで0.1mg/mlに希釈した。希釈した溶液、5μlを30G二重針にて、ラット左眼硝子体内に投与した。
(4)有意差検定
上記(2)の方法で眼圧上昇を施行し、PBSを硝子体内投与したラットに対して、SPF−3059−5投与による網膜保護効果をStudent’s t−testおよびWelch&F−testにて有意差検定を行った。
[薬理評価]
各処置群のラット網膜を用いて、外顆粒細胞層、内顆粒細胞層、内網状層それぞれの層の厚さを測定し、細胞保護効果を評価した。以下、表4及び図5〔外顆粒細胞層(ONL:outer nuclear layer)〕、表5及び図6〔内網状層(IPL)内顆粒細胞層(INL)〕、表6及び図7〔内顆粒細胞層(INL)内網状層(IPL)〕に、測定値を示した。また、図8に0.5%クレシルバイオレットで染色した網膜切片を示した。

内顆粒細胞は眼動脈からの血液供給を受けており、眼圧上昇によって眼動脈虚血障害を受け、遅発性の細胞死を引き起こすことがわかっている。表5及び図6の結果から、本実施例により、眼圧上昇によって内顆粒細胞が細胞死を引き起こし、同細胞の存在する細胞層の薄層化が観察された。さらにSPF−3059−5の前投与によって、統計学的な有意差は認められなかったものの、薄層化の抑制傾向が観察され、内顆粒細胞死に対する細胞死抑制効果が考察された。またSPF−3059−5の後投与によって、統計学的な有意差をもって、内顆粒細胞死に対する細胞死抑制効果が示された。
更に、内網状層は、内顆粒細胞と神経節細胞間のシナプス形成がなされている層であり、細胞体から進展している軸索のみが存在する層である。内顆粒細胞が眼圧上昇による眼動脈虚血障害を受けることから、その軸索である内網状が退縮し、内網状層の薄層化が起こることが知られている。表6及び図7の結果から、本実施例によってもこの現象が再現され、眼圧上昇による内網状層の薄層化が観察された。さらにSPF−3059−5の前投与並びに後投与によって、内網状層の薄層化が有意に抑制されることが示され、網膜内の神経機能を保護していることが考察された。
以上の結果、SPF−3059−5は眼圧上昇による内顆粒細胞の細胞死を抑制することが示された。また内網状層は、内顆粒細胞および神経節細胞両者のシナプス形成の場であり、神経節細胞の軸索も含まれていることから、神経節細胞への保護効果も考えられた。
また、SPF−3059−5の前投与処置では、内顆粒細胞層に対して抑制傾向を示し、内網状層の薄層化に対しては統計学的に有意に抑制作用を示すことがわかった。
更に、SPF−3059−5の後投与処置を行うことによっても、内顆粒細胞層および内網状層の薄層化に対して、統計学的に有意に抑制作用を有していることが示された。
上記の結果から、眼疾患において、内顆粒細胞の虚血性障害が疑われる糖尿病性網膜症、神経節細胞の障害が報告される緑内障等に対し、SPF−3059−5が有効であることがわかった。
[実施例3]
化合物の製造
本発明の化合物は、いずれも公知化合物であり、国際公開第02/09756号パンフレット又は国際公開第03/062243号パンフレットに開示されており、いずれもペニシリニウム属カビであるSPF−3059株から製造することができる。製造方法及び物理化学的性質についても前記国際公開パンフレットに記載されている。具体的には以下の化合物をそれぞれ調製した。

[実施例8]
ラット中大脳動脈閉塞モデルにおける薬理試験例
脳神経細胞の障害は、永久中大脳動脈閉塞または中大脳動脈閉塞−再開通によって生じる脳梗塞巣の大きさとして評価できる、また脳梗塞巣に存在する神経線維の数、再生神経線維の数、あるいは健常部より梗塞部へと進入する線維の数についても定量化でき、障害の程度を知る指標となる。これらのパラメーターを指標として抑制効果を有する虚血性脳血管障害治療剤または予防剤の薬理作用を確認できる。
[試験方法]
(1)薬剤投与
被験物質としてSPF−3059−1またはSPF−3059−5を、ミニ浸透圧ポンプ(Alza)にて、脳梗塞部へ直接投与する。被験物質は0.1mg/mlにPBSにて希釈し、ポンプのリザーバーへ充填する。対照はPBSを充填する。ポンプは手術2日前より37℃の生理食塩水内にインキュベートし、動作を安定させる。
(2)手術
被験物質の薬効は、脳卒中易発症ラット(SHRSPラット)を使った永久中大脳動脈閉塞モデル、およびWistarラットを使った中大脳動脈閉塞−再開通モデルにて評価する。いずれも被験物質投与のための大脳皮質実質へのカニューレ埋め込み手術後に行う。
カニューレ埋め込み手術
(i)ラットをハロセン麻酔し、脳定位固定器に固定する。
(ii)頭部皮膚を切開し、頭蓋骨を露出する。
(iii)ドリルにて左頭蓋骨に穴を開け、左大脳皮質実質内に薬剤を充填したミニ浸透圧ポンプに接続したカニューレを刺入する。
(iv)カニューレを歯科用セメントにて頭蓋に固定、ポンプを背側皮下に挿入する。創部を縫合する。
永久中大脳動脈閉塞モデル
SHRSPラットに対し、以下の方法で永久中大脳動脈閉塞を作成する。
(i)左側頭部の筋肉を除去、ドリルにて側頭骨に穴をあけ、中大脳動脈を露出する。
(ii)中大脳動脈をbipolar coagulator(両極性焼灼器)にて焼灼し、血液を途絶させる。
(iii)ラットの創部を縫合し、体温を維持しつつ回復させる。
中大脳動脈閉塞−再開通モデル
Wistarラットに対し、以下の方法で永久中大脳動脈閉−再開通を作成する。
(i)頸部皮膚を切開し、頚動脈を露出する。
(ii)外頚動脈を絹糸で結紮する。
(iii)外頚動脈と内頚動脈を結ぶ細い血管を焼灼切断する。総頚動脈を結紮する。
(iv)内頚動脈の血流を一時的に途絶させ、外頚動脈との分岐部に穴を開け、栓子(ナイロン糸にシリコンを塗ったもの)を挿入する。
(v)翼状口蓋動脈に栓子が行かないように注意しつつ、さらに奥へ挿入する。
(vi)栓子を血管と結紮し固定する。
(vii)創部を縫合し、体温を維持しつつ回復させる。
(3)組織学的評価
薬効評価としてTTC(2,3,5−triphenyltetrazolium chloreideの略)染色による梗塞巣の大きさの測定および、免疫染色による脳梗塞巣に存在する神経線維の数、再生神経線維の数を測定する。
TTC染色による評価
(i)手術1週間後にラットは麻酔下で経心的に生理食塩水にて灌流を行う。
(ii)脳を摘出し、ブレインスライサーにて2mm厚の切片を作成、カバーグラスに瞬間接着剤にて貼り付ける。
(iii)カバーグラスに貼り付けた切片を0.8% TCC/PBS溶液に37℃で10分間浸し染色を行う。
(iv)4℃の10%ホルマリン緩衝液中に移し、染色を停止する。
(v)実体顕微鏡写真を撮影、コンピューターにてデジタルファイル化し、画像解析ソフトにて脳梗塞巣の大きさを測定する。
免疫染色による評価
(i)手術1週間後にラットは麻酔下で経心的にPBSにて灌流を行い、つづいて4%パラフォルムアルデヒド/PBS溶液にて灌流固定を行う。
(ii)脳を摘出し、同固定液にて1晩の後固定を行い、30%シュクロース/PBS溶液中で4℃にて浸漬保存する。
(iii)クリオスタットにて40μmの凍結切片を作成し、TBS中に浮遊切片として採取する。
(iV)ABC法にて免疫染色を行い、プレパラートを作成する。抗体は、神経線維の指標として抗ニューロフィラメント抗体、再生神経線維の指標として抗GAP−43抗体を用いる。
(v)顕微鏡で梗塞部の写真を撮影、コンピューターにてデジタルファイル化し、画像解析ソフトにて梗塞巣に含まれる神経線維および再生神経線維の数、あるいは、健常部より梗塞部へと進入する線維の数を定量化する。
Claims (6)
- 式[22]:
で表される化合物、又はその薬学上許容される塩を有効成分として含有する虚血性神経障害における虚血による神経細胞死の抑制剤。 - 虚血性神経障害が、網膜神経障害である、請求項1に記載の抑制剤。
- 網膜神経障害が、緑内障、糖尿病性網膜症、黄班変性症又は未熟児網膜症である、請求項2に記載の抑制剤。
- 網膜神経障害が、網膜中心動脈閉塞症、網膜中心動脈分枝閉塞症、網膜中心静脈閉塞症、網膜中心静脈分枝閉塞症又は虚血性視神経症である、請求項2に記載の抑制剤。
- 網膜神経障害が、糖尿病性網膜症である、請求項2に記載の抑制剤。
- 虚血性神経障害が、脳塞栓症、一過性脳虚血、鎖骨下動脈盗血症候群、Wallenberg症候群(延髄外側症候群)、脳血栓症、ラクナ梗塞、可逆性虚血性神経障害、脳梗塞、もやもや病(ウィリス動脈輪閉塞症)、低酸素性脳症、静脈洞血栓症、または術後脊髄虚血である、請求項1に記載の抑制剤。
