前記粘着剤の塗布部分に剥離紙が貼付されて被覆されているとともに、前記情報記入用紙の情報記入欄形成面に前記粘着剤に対して離型性を示す離型剤が塗布され、
前記剥離紙を剥離して粘着剤塗布部分を露出させた状態で、粘着剤塗布用紙の粘着剤塗布部面と情報記入用紙の情報記入欄形成面とを重なり合うように折り畳んだときに、前記情報記入用紙と粘着剤塗布用紙とが再剥離可能に接着されて、前記隠蔽情報が隠蔽状態とされる隠蔽用紙であって、
前記離型剤が、線数５０〜４００本／インチの規則的な網目模様、あるいはランダムなパターンで構成された絵柄の網目模様に塗布され、かつ、離型剤の塗布面積が、塗布部の用紙面積の５〜８０％であり、
前記粘着剤が水性エマルジョン系粘着剤であり、
情報記入用紙と粘着剤塗布用紙とを接着させたのち２４時間経過後の再剥離時における、情報記入用紙と粘着剤塗布用紙との粘着強度がＪＩＳ Ｚ ０２３７の１０にある１８０度引き剥し粘着力の測定で３．００Ｎ／２５ｍｍ〜２５Ｎ／２５ｍｍである、
