Description
本発明は、組織切除処置など、侵入型の医療処置を実行するための装置およびシステムに関する。

図2を参照して、カニューラ201の詳細を説明する。カニューラ201は、近位端204と、遠位端206、及び、近位端204と遠位端206との間に延在するルーメン208を有するシャフト202を具える。図に示す実施例では、シャフト202は、円形断面形状を有し、断面のディメンションが0.25ないし1.5インチである。シャフト202は、その他の断面形状とディメンションを有していても良い。図2Aに示すように、シャフト202の遠位端206は、実質的にプレシェープした直線ジオメトリを有する。代替的に、遠位端206は、シャフト202の縦軸211からはなして切断カテーテル104を案内するの使用することができる、プレシェープした曲線ジオメトリ(図2B)を有していても良い。
図3を参照して、切除カテーテル104の詳細を述べる。切除カテーテル104は、ルーメン301を有する始動シース300と、シース300のルーメン301内に摺動可能に配置したカテーテル部材302と、を具える。切除カテーテル104は更に、隣接する組織に切除エネルギィを伝送する電極構造310と、この組織に関連する電極構造310を安定化させるためのカテーテル部材302の遠位端306に装着した真空始動スタビライザ400を具える。切除カテーテル104は、更に、カテーテル部材302の近位端304に装着されたハンドルアッセンブリ320を具える。ハンドルアッセンブリ320は、外科医が切除カテーテル104を操作する手段を提供するハンドル321と、電極構造310に切除エネルギィを提供する切除ソース108に連結された電気的コネクタ362を具える。ハンドルアッセンブリ320は、更にスタビライザ400用の真空力を発生する真空598に接続された真空ポート408と、電極構造310に加圧膨張媒体を供給するポンプ409に接続された膨張ポート336を具える。
ここで、図20−26を参照して、グラウンドカテーテル106について述べる。図20及び21に示す実施例では、グラウンドカテーテル106(1)は、近位端602と遠位端604を有するカテーテル部材600と、遠位端604上に担持された複数の電極エレメント606と、近位端602に固定されたハンドルアッセンブリ608を具える。カテーテル部材600は、例えば、ポリエチレンあるいはポリウレタン、またはPEBAX(登録商標)材料(ポリウレタンとナイロン)などの、ポリマの導電材料でできている。ハンドルアッセンブリ608は、外科医にカテーテル部材600を操作する手段を提供するハンドル609と、電極エレメント606に切除エネルギィを提供する切除ソース108に連結された電気コネクタ610を具える。ハンドルアッセンブリ608も、遠位端604を操作するためのステアリング機構612を具えている。ステアリング機構612は、切除カテーテル104について上述したステアリング機構500と同様である。更に、グラウンドカテーテル106(1)は、組織の温度をモニタするための温度センサ(図示せず)を担持していても良い。
図27−29を参照して、マッピングカテーテル700の詳細を説明する。マッピングカテーテル700は、心臓の電気信号を検出して、これによって切除すべき心臓のターゲット位置を決定するように構成されている。
Claims (10)
- 身体の器官内に位置する組織治療用の切除システムにおいて:
電源端子と、リターン端子を有する切除ソースと;
前記切除ソースの電源端子に電気的に接続され、切除電極エレメントを具える切除プローブと;
前記切除ソースのリターン端子に電気的に接続され、不関電極エレメントを含むグラウンドプローブと;を具え、
前記グラウンドプローブが、器官の内部にイントラバスキュラリに導入されるように構成されており、前記切除プローブが、器官の外部に前記器官の外部と接触してエキストラバスキュラリに配置されるように構成され、更に前記グラウンドプローブが、前記不関電極エレメントを覆う保護エレメントを含み、それが固体組織と接触することを防ぎ、前記不関電極エレメントが体液と接触することを可能にすることを特徴とする切除システム。 - 請求項1に記載のシステムにおいて、前記切除エレメントが、内部領域を有する拡張可能−折りたたみ可能なボディ、および前記拡張可能−折りたたみ可能なボディの内部領域から前記拡張可能−折りたたみ可能なボディの外部へイオンが移動できるサイズの複数の孔を具えることを特徴とするシステム。
- 請求項2に記載のシステムにおいて、前記切除エレメントが更に、前記拡張可能−折りたたみ可能なボディ内に位置する電極を具えることを特徴とするシステム。
- 請求項1乃至3のいずれかに記載のシステムにおいて、前記切除ソースが高周波発生器であることを特徴とするシステム。
- 請求項1乃至4のいずれかに記載のシステムにおいて、前記保護エレメントがケージアセンブリを具えることを特徴とするシステム。
- 請求項5に記載のシステムにおいて、前記ケージアセンブリが近位端、遠位端、および前記近位端と前記遠位端との間に固着した複数の支柱を含むことを特徴とするシステム。
- 請求項6に記載のシステムにおいて、前記ケージアセンブリの近位端が、前記不関電極エレメントに対して摺動可能であることを特徴とするシステム。
- 請求項1乃至7のいずれかに記載のシステムにおいて、前記保護エレメントが、非導電材料から作られることを特徴とするシステム。
- 請求項1乃至8のいずれかに記載のシステムにおいて、シースを更に具え、前記保護エレメントが摺動可能に配置されることを特徴とするシステム。
- 請求項9に記載のシステムにおいて、前記保護エレメントが、前記シースの外側にあるとき拡張した形状を、そして前記シースの内側にあるとき折りたたまれた形状を有することを特徴とするシステム。
