Description
生検用針器具は、悪性度を判断するために、患者に与える傷を最小限にしつつ、顕微鏡検査のために組織片を得るために使用されることが多い。通常、こういった器具は、カニューレと呼ばれる中空状の針内にスタイレットと呼ばれる細長いプローブを密嵌した構成となっている。スタイレット及びカニューレは、手動で制御してもよく、最初にスタイレットを組織塊内に突出させてカニューレに追わせる射出機構を用いてもよい。スタイレットは、スタイレットが組織塊に進入したときに、組織が突出して入り込むノッチを有する。カニューレがスタイレット上方を摺動すると、組織小片が組織塊から切断され、スタイレットのノッチ内に捕捉される。その後器具を抜去し、組織片がスタイレットのノッチから取り出される。切断針とカニューレがバネ作用によって時間差で作動するように進行させられる、二段階作動機構を含む器具の例としては、ベイツ(Bates) による特許文献1に開示されている。なお、特許文献1は、その全内容が本明細書において開示されたものとする。
一実施形態においては、本発明は、体内において位置を確認可能な試料採取部を有する生検針システムを特徴とする。試料採取部は、試料採取領域を有するスタイレットと、カニューレとを備える。スタイレット及びカニューレは、カニューレを試料採取領域上方に配置するために、スタイレットの軸線に沿って相対移動可能である。サンプルマーカは、試料採取部において位置確認が可能であり、試料採取部から放出可能である。
本発明の一実施形態は、生検針システムであって、体内において位置確認可能であり、かつ、試料採取領域を有するスタイレット、カニューレ、及びスタイレットの基端からマーカ出口まで延びるマーカルーメンを備える試料採取部であって、前記スタイレット及びカニューレは、カニューレを試料採取領域上に配置するために、スタイレットの軸線に沿って相対移動可能である、試料採取部と、前記試料採取部において位置確認可能であるとともに、該試料採取部から放出可能な試料採取マーカとを備え、前記マーカルーメンがスタイレットとカニューレの間に位置する、生検針システムを提供する。
複数の実施形態において、以下の特徴のうち一つ以上を有することができる。試料マーカは、スタイレット及びカニューレの間で位置を確認することができる。生検システムは、スタイレット及びカニューレ間にマーカルーメンを備えることができる。マーカルーメンは、複数のマーカの供給部を備えることができる。供給部内の複数のマーカは、スタイレットとほぼ平行に順次配置されてもよい。マーカの供給部は、スタイレットの周囲において、径方向に設けられてもよい。マーカの供給部は、スタイレットの軸線を中心に回動可能であってもよい。
(詳細な説明)
(構造)
図1乃至2Bに基づき説明する。生検器具5は、制御ハンドル6及び試料採取部7を備える。試料採取部7は、試料採取ノッチ30を有するスタイレット18と、同心円状に配置されるカニューレ20とを備える。スタイレット18及びカニューレ20は、制御ハンドル6から突出する。試料を採取するために、スタイレット18及びカニューレ20は、制御ハンドル6の制御ボタン12,14を用いて射出準備状態となるように後退させられる。後退させられたスタイレット及びカニューレは、制御ハンドル6のそれぞれ対応するバネ44及び40を圧縮する。試料採取部7は、試料を採取する組織近傍に配置される。器具は、スタイレットバネ44を解放する射出ボタン22を作動させることにより射出され、これによってスタイレット18が組織塊を貫くように拡張される。カニューレバネ40が続いて解放されて、組織塊から試料を切断するカニューレ20を拡張させる。試料は、スタイレット18の試料採取ノッチ30内に捕捉される。
図3A以降の図には、組織を採取する部位にマーカを配置することを含む生検方法が示されている。各図は、制御ハンドル6及び試料採取領域7の拡大断面図を示している。
図3Aにおいては、器具は、(例えば使用前の)ニュートラル位置にある。スタイレット18及びカニューレ20はともに、スタイレット18の試料採取ノッチ30を覆うカニューレ20とともに延びる。試料採取部は、ルーメン27にマーカ19,19’,19’’等の供給部を有する。マーカ19,19’,19’’は、開口29を介してルーメン27内に装填される。マーカは、プッシャ75の端部が最も基端側のマーカ19’’の基端近傍に位置するように、(例えばワイヤを用いて)ルーメン27内へ押し込まれる。
図3Dは、カニューレ20が射出された状態を示している。カニューレ20は、スタイレット18のノッチ30内へ突出した試料37を組織から切断する。(カニューレの動きが迅速であるため、ルーメン27内のマーカは、スタイレット18及びカニューレ20が射出されたときとほぼ同じ位置に留まる。)
図4A乃至4Cには別の実施形態が示されており、マーカの供給部は、制御ハンドルに設けられている。制御ハンドルは、マーカ19,19’,19’’を保持するルーメン77を有するマーカカートリッジ72を備える。マーカカートリッジ72は、摺動ボタン80を用いて、マーカを収容しているスロットがスタイレット18のルーメン27と軸方向において整合するように、レール74に沿って移動させることができる。プッシャ75は、プッシャの先端がカートリッジのスロットに入り込んでマーカをカートリッジからルーメン27へ押し込み、マーカが開口29を介して試料採取部位に射出されるように作動させられる。マーカ配置後、プッシャは、その先端がカートリッジの基端側に位置するように後退させられ、マーカカートリッジがレール74に沿って摺動させられて、マーカを収容した別のスロットがルーメン27と整合するように配置される。続いて試料が採取され、当該部位をマーキングする。
Claims (24)
- 生検針システムであって、
体内において位置確認可能であり、かつ、試料採取領域を有するスタイレット、カニューレ、及びスタイレットの基端からマーカ出口まで延びるマーカルーメンを備える試料採取部であって、前記スタイレット及びカニューレは、カニューレを試料採取領域上に配置するために、スタイレットの軸線に沿って相対移動可能である、試料採取部と、
前記試料採取部において位置確認可能であるとともに、該試料採取部から放出可能な試料採取マーカとを備え、
前記マーカルーメンがスタイレットとカニューレの間に位置する、生検針システム。 - 前記試料採取マーカがスタイレットとカニューレの間において位置確認可能である請求項1に記載のシステム。
- 前記ルーメン内に複数のマーカの供給部を有する請求項1に記載のシステム。
- 前記供給部に配置された各マーカが、スタイレットとほぼ平行に順次配置される、請求項3に記載のシステム。
- 前記マーカの供給部が、径方向において前記スタイレットの周囲に設けられる、請求項3に記載のシステム。
- 前記マーカの供給部がスタイレットの軸線に対して相対回動可能である請求項5に記載のシステム。
- 前記スタイレットの先端近傍にマーカ出口を備える請求項1に記載のシステム。
- 前記スタイレットが側部ノッチを備え、前記出口が該側部ノッチの先端側に位置する、請求項7に記載のシステム。
- 前記出口は、マーカをスタイレットとほぼ平行に射出するように指向している、請求項8に記載のシステム。
- 前記スタイレットが側部ノッチを備え、マーカ出口が該側部ノッチの基端側に位置する、請求項1に記載のシステム。
- 前記スタイレットが側部ノッチを備え、少なくとも1個のマーカ出口が該側部ノッチの端部と整合する、請求項1に記載のシステム。
- 第1出口が試料採取領域の第1軸線周囲近傍に配置され、第2出口が試料採取領域の第2周囲近傍に配置される、請求項11に記載のシステム。
- 前記第1出口がカニューレに設けられる請求項12に記載のシステム。
- 前記第2出口がカニューレに設けられる請求項12に記載のシステム。
- マーカの供給部を有する制御ハンドル部を備える請求項1に記載のシステム。
- マーカを前記試料採取部に選択的に配置するためにマーカプッシャを備え、前記ハンドル
部より該プッシャを作動させる、請求項15に記載のシステム。 - マーカを前記試料採取部に選択的に配置するためにマーカプッシャを備え、前記ハンドル部より該プッシャを作動させる、請求項1に記載のシステム。
- 前記マーカは、試料が採取される組織領域の軸方向における長さの指標となる、請求項1に記載のシステム。
- 前記マーカが長尺状要素を備える請求項18に記載のシステム。
- 前記長尺状要素が、その長さに沿って、MRI、超音波、又は透視法により識別可能な複数の領域を有する、請求項19に記載のシステム。
- 前記識別可能な複数の領域は、試料が採取される組織領域の長さの指標となるように離間して配置される、請求項20に記載のシステム。
- 前記マーカの少なくとも一部が生分解可能である請求項21に記載のシステム。
- マーカの供給部を備え、任意のマーカが、MRI、超音波、又は透視法により供給部内の別のマーカとは識別可能である、請求項1に記載のシステム。
- 前記マーカは、マーカが組織と接触したときにマーカが基端側へ移動することを妨げる組織係合縁を備える、請求項1に記載のシステム。
