Description
本出願は、2002年9月4日に出願された米国仮出願第60/408,026号の利益を主張している。
1.発明の分野
本発明は、概して局所麻酔に関し、さらに詳しくは、例えば、麻酔遮断薬投与(administration of an anesthetic blockade)、神経モニタリング、筋電図検査及び治療介入のような、種々の方法との関連で神経刺激針を位置決めするための装置に関する。
局所麻酔の分野は、術後疼痛の軽減、及び外傷又は慢性疼痛の長期の軽減のために、外科手術中に特定の人体の部位に麻酔薬投与を行うことに関連する。しばしば、局所麻酔は、安全性の増加、術後疼痛管理の利用、及び麻酔費用低減の理由により、全身麻酔よりも好ましいとみなされる。
神経刺激針電極の先端は、神経又は神経叢の登録点の位置を定めるために皮膚電極として機能している間、真皮を通って侵入することを物理的に抑止されている。針は、一端に滑らかで鈍いシールドを有する電気的に遮蔽された非導体キャリアのボア内に位置決めされ、キャリアが皮膚層に対して及びそれに沿って側方に変位して押圧されている間、針の先端がシールドを越えて表皮及び真皮に侵入する程度の距離だけ突出することがないような位置にロックされる。

Claims (14)
- 対象の標的神経又は神経叢の位置を皮膚上で定めるための位置決め装置であって、
キャリアと、
前記キャリアを通って延びるボアと、
前記ボア内に担持される針とを有し、
前記ボアは近位端と遠位端とを有し、前記キャリアは前記ボアの遠位端において滑らかな外形面を備える表面を有し、前記表面は電気的に非導体であり、前記針は先端を有し、前記針の先端は電流供給源に選択的に接続されて電流フローへの神経反応を誘発し、
前記針の先端は、前記ボアの遠位端であって操作可能皮膚電極位置にあり、前記表面が前記対象の皮膚層に押圧される際に前記針の先端が侵襲的侵入なしに皮膚層に接触するようになっており、前記針の先端は、前記操作可能皮膚電極位置にあるとき前記キャリアに対する変位が抑止され、これによって経皮電流が前記針の先端から前記対象を通って流れて前記標的神経又は神経叢近傍における皮膚の位置が定められ、その後前記針はその定められた皮膚の位置で前記ボアの遠位端を越えて皮膚層を通り、前記標的神経又は神経叢の皮下位置に向かって前記対象の中に進められる位置決め装置。 - 前記キャリアの一部は、前記針と係合状態にあり、前記針の先端を操作可能皮膚電極位置に解除可能に維持する請求項1に記載の位置決め装置。
- 前記針は、前記針の先端を操作可能皮膚電極位置に解除可能に維持するために、前記キャリアの一部と係合する溝を有する請求項2に記載の位置決め装置。
- 前記針はハブとシャフトとを有し、前記シャフトは、前記針の先端を操作可能皮膚電極位置に解除可能に維持するために、前記キャリアの一部と係合する請求項2に記載の位置決め装置。
- 前記針はハブとシャフトとを有し、前記ハブは、前記針の先端を操作可能皮膚電極位置に解除可能に維持するために、前記キャリアの一部と係合する請求項2に記載の位置決め装置。
- 前記針と係合状態にある前記キャリアの一部は、位置決めネジを有する請求項2に記載の位置決め装置。
- 前記針と係合状態にある前記キャリアの一部は、ピンを有する請求項2に記載の位置決め装置。
- 前記キャリアは前記ボアから延びる軸方向スロットを有し、前記針は径方向アームを有し、前記スロットはその長手方向縁に沿って形成された少なくとも1つのノッチを有し、前記ノッチは前記径方向アームを受容する寸法になっていて操作可能皮膚電極位置に前記針の先端を解除可能に維持する請求項1に記載の位置決め装置。
- 前記ボアは第1ボアを有し、前記キャリアは第2ボアを有し、前記第1ボアと前記第2ボアとは同軸であり、前記第2ボアは第1ボアの近位端から延び、前記針は近位端と遠位端とを有し、前記先端は前記針の前記遠位端に位置決めされ、ハブは前記針の前記近位端に位置決めされ、前記第2ボアは前記ハブを受容する寸法になっている請求項1に記載の位置決め装置。
- 前記キャリアは前記ボアから延びる軸方向スロットを有し、前記針は前記スロットに受容される径方向アームを有し、それによって前記径方向アームは、前記標的神経又は神経叢の位置が定められた後に前記針を前記対象の中に進めるために用いられる請求項1に記載の位置決め装置。

前記先端は電流供給源に接続され、それによって電流が前記対象の皮膚層を通って流れ、前記標的神経又は神経叢に対する前記表面位置の関数として変化する神経反応を誘発する位置決め装置。 - 前記キャリアは皮下部分とグリップ部分とを有し、前記両部分は、形状が円筒形であり、径方向の段部によって分かれており、前記ボアは前記両部分を通って延びており、前記グリップ部分は前記皮下部分よりも大きい直径を有し、前記皮下部分は前記ボアの遠位端を有する請求項11に記載の、対象の標的神経又は神経叢の位置を定めるための位置決め装置。
- 真皮を含む皮膚層を有する対象の標的神経又は神経叢の位置を皮膚上で定めるための位置決め装置であって、
キャリアと、
先端を備える電極とを有し、
前記キャリアは滑らかな鈍いシールドを有し、前記シールドは電気的に非導体であり、前記先端は前記シールドに対する操作可能位置を有し、前記先端は電流供給源に選択的に接続されて電流フローへの神経反応を誘発し、前記操作可能位置は、前記シールドが前記皮膚層に対して押圧されるとき前記先端が真皮に侵襲的に侵入することなく皮膚層に接触するようになっている位置決め装置。 - 真皮を含む皮膚層を有する対象の標的神経又は神経叢の位置を定めるための位置決め装置であって、
キャリアと、
電極とを有し、
前記キャリアは滑らかな外形面を備えるシールドを有し、前記シールドは電気的に非導体であり、前記電極は先端を有し、前記先端は電流供給源に選択的に接続され、前記先端は、前記シールドが前記皮膚層に対して押圧されるとき前記先端が前記真皮に侵襲的に侵入することなく皮膚層に接触し、かつ、電流が前記先端に供給されるとき電流は前記対象を通って、前記標的神経又は神経叢に対する前記皮膚層の前記先端の側方位置の関数として変化する神経反応を誘発するような、前記シールドに対する操作可能位置を有する位置決め装置。
