Description
請求項2の発明は、課題の他の解決手段として、肝実質細胞増殖因子(HGF)遺伝子のプラスミドを有効成分として含有する、聴覚機能障害用医薬を提供する。
請求項3の発明は、課題の他の解決手段として、肝実質細胞増殖因子(HGF)遺伝子、又はそのプラスミドを封入したウイルスエンベロープベクターを有効成分として含有する、聴覚機能障害用医薬を提供する。
請求項4の発明は、課題の他の解決手段として、ウイルスが、センダイウイルス、レトロウイルス、アデノウイルス、アデノ随伴ウイルス、ヘルペスウイルス、ワクシニアウイルス、ポックスウイルス、インフルエンザウイルスから選ばれる1種である、請求項3記載の聴覚機能障害用医薬を提供する。
HGF遺伝子は、適当なベクター、好ましくはHGF遺伝子をプラスミドに組み込んだものを用いることができる。
HGF遺伝子は、RNAを取り除いたウイルスの膜(ウイルスエンベロープ)にHGF遺伝子を封入したもの、又はHGF遺伝子をプラスミドに組み込んだものを封入したウイルスエンベロープベクターを用いることができる。
ここで用いるウイルスは、野生型ウイルスであっても、組み替え型ウイルスであっても良い。このウイルスとしては、センダイウイルス(HVJ)、レトロウイルス、アデノウイルス、アデノ随伴ウイルス、ヘルペスウイルス、ワクシニアウイルス、ポックスウイルス、インフルエンザウイルスから選ばれる1種が好ましい。これらの中でも、HVJがより好ましく、特に不活性化HVJが好ましい。この不活性化とは、ゲノムを不活性化したウイルスである。

このような物質としては、リン酸、クエン酸、コハク酸、酢酸、その他の有機酸又はこれらの塩のような緩衝剤;アスコルビン酸のような抗酸化剤;低分子量(約10残基未満の)ポリペプチド(例えば、ポリアルギニン又はトリペプチド);タンパク質(例えば、血清アルブミン、ゼラチン、イムノグロブリン);親水性ポリマー(例えば、ポリビニルピロリドン);アミノ酸(例えば、グリシン、グルタミン酸、アスパラギン酸、アルギニン);単糖、二糖及び他の炭水化物(例えば、グルコース、マンノース、蔗糖、デキストリン、セルロース又はその誘導体);キレート剤(例えば、EDTA);糖アルコール(例えば、マンニトール、ソルビトール);対イオン(例えば、ナトリウム);非イオン界面活性剤(例えば、ポリソルベート、ポロキサマー);ポリエチレングリコール等を挙げることができる。
すなわち、HGF遺伝子をカナマイシン処置直前に投与することにより聴覚障害を予防することができ、またカナマイシンによる聴覚障害惹起後でも聴覚機能が回復した。
これらの結果は、HVJ-Eベクターを用いるHGF遺伝子治療の、極めて高い有用性を示している。
i) 蝸牛切開術による、蝸牛中への直接注射
ii) 膜を通した注射あるいは、ベクターを含むジェル(gel)小片を無傷の膜上に置き浸透させることによる、正円窓膜を通じての投与
iii) 切開術による、後半規管を通しての内耳内投与
iv) 内リンパ嚢中への投与
これまでアデノウイルスベクター、ヘルペスウイルスベクター、アデノ随伴ウイルスベクター等のいくつかのウイルスベクターは、上記4つの手法のいずれかを用い、内耳内に直接投与されてきた。
しかしながらそれぞれの手法は、侵襲性および有効性の観点から、それぞれ利点および欠点を有している。
この方法により、脳脊髄液中における導入遺伝子の発現を酵素活性および免疫染色により確認し、また脳あるいは耳組織内において有意な損傷を認めなかった。
この事実は、HVJ-Eベクターそのものが、クモ膜下脳脊髄液への投与後、内耳内螺旋神経節細胞に到達したことを示唆しており、クモ膜下脳脊髄液から内耳への、いくつかの可能性ある経路が示された。
ベクターを膜内に注入した際、離れた器官において、いかなるルシフェラーゼ活性も観察されなかったので、クモ膜下脳脊髄液から内耳に到達する最もあり得る経路は、蝸牛管経由だと考えられる。
仮にベクターが血流経由で全身に広まったのであれば、静注後ルシフェラーゼ活性はまず脾臓において認められるので、脾臓や肺のような離れた器官においても導入遺伝子の発現が検出されるはずである。
従来NGF, BDNF, GDNF and NT-3等の向神経因子が、聴覚の治療に用いられてきた。
しかしながら、これまでHGFはこの目的には用いられてこなかった。
肝臓への効果に加え、海馬、大脳皮質、知覚ニューロン、運動ニューロン等においてHGFは向神経活性を示すことが明らかになっているが、今回我々は、ヒトHGFが、クモ膜下脳脊髄液および螺旋神経節細胞の相方に認められ、それがラットの内因性HGFを誘導したことを示した。
聴覚系へのHGF遺伝子治療は、ニューロトロフィンを利用する従来の遺伝子治療法に勝るいくつかの利点を有すると思われる。
また人工内耳とHGF遺伝子治療の併用、すなわち人工内耳手術中にHGF遺伝子を投与することも効果的であろう。
聴覚障害は有毛細胞および螺旋神経節細胞の喪失を伴い、これらの喪失予防はアポトーシスによる細胞死に対するHGFの保護作用によりなされる。
HGFの発現は、カナマイシン処置による聴覚障害惹起後の機能回復にも効果的であった。
このように、知覚神経性の聴覚障害治療にあたり、HGF遺伝子治療は可能性の高い方法の一つである。
本発明の聴覚機能障害用医薬は、特に難聴の予防、治療等を目的とした遺伝子治療用の医薬として好適である。
pCMV−lacZ(9.2kb)は、pSV−β−ガラクトシダーゼ(Promega Corp.、マディソン、ワイオミング州、アメリカ)のHindIII−BamHIフラグメントを、pcDNA3(5.4kb)(インビトロゲン、サンディエゴ、カリフォルニア州、アメリカ)内へ、HindIII−BamHI部位で挿入することによって構築した。
pCMV−ルシフェラーゼ−GL3(pcLuc−GL3:7.4kb)は、pGL3ベーシックベクター(プロメガ)からのルシフェラーゼ遺伝子をpcDNA3(インビトロゲン)内にクローニングすることによって構築した。
pVAX1−hHGF(5.2kb)は、ヒトHGF cDNAをpVAX1(3.0kb)(インビトロゲン)内にBamHI及びNotI部位で挿入することによって構築した。
プラスミド類は、キアゲン・プラスミド単離キット(キアゲン、ヒルデン、ドイツ)を用いて精製した。
センダイウイルス(HVJ;Hemagglutinating virus of Japan)エンベロープベクター(HVJ−E)は、WO-A 01/57204 の実施例8に準じて、プラスミドDNAを不活化HVJ粒子内に組み込むことによって調製した。但し、HVJは遠心分離により精製し、紫外線照射により不活性化した。
10,000赤血球凝集単位であるUV不活化HVJ(Z菌株)を、200μgのプラスミドDNA及び0.3%トライトンXと混合し、平衡生理食塩液(BSS:137mMのNaCl、5.4mMのKCl、10mMのTris−HCl、pH7.6)を用いて洗浄し、クモ膜下注入のためにBBSを用いて100μLに調製した。本実施例では、pCMV−lacZ、pcLuc−GL3、pVAX1−hHGF、pcDNA3又はpVAX1プラスミドDNAを含有するHVJ−Eを使用した。
正常なプライエル反射を示すSprague−Dawley系雄性ラット(6週齢;体重200〜210g)を日本Charles River社から入手した。すべての処置は、大阪大学実験動物委員会ガイドラインに従って実施した。
動物は、保護群、救済群、保護/救済群、ベクター対照群及び非処置群の5つの群に分けた。全群の動物は、アミノグリコシド中毒によって両耳の聴覚を喪失させるため、硫酸カナマイシン(明治製菓、東京)を連続14日間に渡り、皮下注射(400mg/kg/日)によって投与した。
保護群及び保護/救済群には、カナマイシン処置の初日にhHGF遺伝子(pVAX1−hHGF)を含有するHVJ−E懸濁液をクモ膜下注入した。
ベクター対照群には、上記と同一方法でクモ膜下注入により、対照ベクター(pVAX1)を含有するHVJ−E懸濁液を投与した。
救済群及び保護/救済群には、カナマイシン処置の最終日(第14日)にpVAX1−hHGFを含有するHVJ−Eを注入した。
更に、組織化学的解析及びルシフェラーゼ定量のため、pCMV−lacZ又はpcLuc−GL3を含有するHVJ−Eを動物に投与した。
このような処置後、トランスフェクション7日後にβ−ガラクトシダーゼの発現を観察し、トランスフェクションの1日後にルシフェラーゼ活性を測定した。
本試験では、CNS及び内耳内へのin vivo遺伝子導入法として、HVJ−Eの注入を使用する小脳延髄槽内への遺伝子導入法を実施した。
クモ膜下腔内へ注入するために、ケタミン(三共、日本)及びキシラジン(バイエル)を用いて動物に麻酔をかけた後、各動物の頭部を腹臥位で固定し、後頭脳正中線切開部を通して環椎後頭膜を露出させた。
ステンレス製カニューレ(27ゲージ;ベクトン・ディッキンソン)を小脳延髄槽(クモ膜下腔)内へ挿入し、カニューレの位置を確認して、脳内圧上昇を回避するために脳脊髄液(100μL)を取り出した後、マーカー遺伝子、hHGF遺伝子又は対照ベクターを含有するHVJ−E(100μL)を50μ/minの速度で注入した。
その後30分間は、動物の頭部を下に向けておいた。投与後に体重減少、活動低下又は行動変化を示したラットはいなかった。
ルシフェラーゼ遺伝子がトランスフェクトされたラットを、トランスフェクション24時間後に麻酔下で屠殺した。器官(脳、肺、脾臓、肝臓及び蝸牛)を採取し、個別に50mLファルコンチューブ内に入れた。
ルシフェラーゼ活性は、ルシフェラーゼ・アッセイキット(プロメガ)を用いて測定した(44 of HVJ−E)。組織抽出物のタンパク質濃度を測定することによってルシフェラーゼレベルを標準化した(44 of HVJ−E)。ルシフェラーゼ単位は、組織タンパク質1g当たりの相対発光量(RLU)として表した。
本試験には、hHGF遺伝子を含有するHVJ−Eの注射の5日及び14日後にラットから採取した脳脊髄液(CSF)(100μL)を使用した。
CSF中のヒトHGF及びラットHGFの濃度は、抗ヒト及び抗ラットHGF抗体を使用するエンザイムイムノアッセイによってアッセイキットの製造業者(免疫研究所、東京)の取扱説明書に従って測定した。
ヒトHGFに対する抗体は、ヒトHGFとのみ反応し、ラットHGFとは反応しなかった。ラットHGFに対する抗体は、ヒト及びラットHGFの両方と反応した。
エーテルを用いてラットに深い麻酔をかけ、断頭し、側頭骨を低温リボヌクレアーゼ無含有生理食塩液中に入れた。耳嚢を除去し、解剖顕微鏡下で全蝸牛を単離した。ラットからの組織を溶解緩衝液中にプールし、ホモジナイザーを使用してホモジナイズし、RNeasy Mini Kit(キアゲン)を使用して全RNAを単離した。RT−PCR用のSuperScript第1鎖合成システム(インビトロゲン)を用いてRNAを逆転写させた。
ヒトHGF及びGAPDHに対する特異的プライマー:HGF、上流プライマー5’−TTCACAAGCAATCCAGAGGTACGC−3’、下流プライマー5’−GAGGGTCAAGAGTATAGCACCATG−3’;GAPDH、上流プライマー5’−TGAAGGTCGGAGTCAACGGA−3’、下流プライマー5’−GATGGCATGGACTGTGGTCA−3’ (以上それぞれ配列表の配列番号3、4,5および6)を使用して、第1鎖cDNAを増幅させた。
PCR条件は、各セットのプライマーに対して最適化した。PCR反応混合液は、5μLのcDNA、5μLの10×PCR緩衝液、4μLの2.5mMのdNTP、2.5μLの20pM上流及び下流プライマー、及び1.5UのTaqポリメラーゼを含有しており、45μLとなるまで蒸留水を添加した。
加熱サイクル条件:HGF、94℃で45秒間、70℃で2分間及び72℃で2分間;GAPDH、94℃で45秒間、58℃で1分間及び72℃で2分間。
聴覚機能の生理的状態を評価するために、聴性脳幹反応(ABR)聴力検査を実施した。
ABRsは、ベースライン値を測定するためにカナマイシン投与初日の前日に測定し、更にカナマイシン処置開始から7、14、21、28及び56日後に再度記録した。
聴覚機能の各試験前に、動物にはケタミン(50mg/kg)−キシランジン(10mg/kg)溶液の筋肉内注射により麻酔をかけた。針電極を同側右耳翼(参照電極)、対側耳翼(接地電極)及び頭頂(能動電極)の皮下に配置した。全記録は防音室にて、日本光電製Neuropack IV(MEM−4104)システムを用いて実施した。
単一波長100μsecのクリック音(10/sec)によって電位を誘発し、これらのモノラル刺激をラウドスピーカーによって右耳へ送達した。反応をデジタルフィルタリングし(バンドパス:50〜3,000Hz)、増幅させ、500回の反応を平均化した。
図1は、0日から56日間の閾値(dB)であり、図2はI波潜時(mS)を示す。なお、図中のHGFは保護群、保護/救済群、救済群を示し、図中のVecはベクター対照群を示し、図中のKMは非処置群を示す。
図1、図2から明らかなとおり、保護群、保護/救済群、救済群は、56日経過後においても聴覚の変化(閾値及びI波潜時の変化)は僅かであったが、ベクター対照群と非処置群では、聴覚の変化(閾値及びI波潜時の変化)が顕著であった。
図3a〜cから明らかなとおり、カナマイシン+HGF処置群の螺旋神経節細胞は、カナマイシン処置群の比べると、無傷のものに近かった。
図5に、カナマイシン+HGF処置群(保護群、保護/救済群、救済群)とカナマイシン処置群(非処置群)における、螺旋神経節細胞密度(10,000mm2当たりの細胞数)を示す。
図4、図5から明らかなとおり、カナマイシン+HGF処置群は、他の処置群に比べて、螺旋神経節細胞密度が高かった。
以上の結果は、HVJ−E懸濁液の投与により、聴覚ニューロンが保存又は再生された結果であると推定される。
