【発明の属する技術分野】
本発明は複合ステント、複合ステント装具(器具)を含むステントアセンブリー(組み立て品)に関し、またステントを製造する方法に関する。
【0002】
【従来の技術および発明が解決しようとする課題】
ステントは人体あるいは動物の体の内腔で使用される。内腔に適正に配置されていると、ステントはその内腔の壁に接触してその壁を支持したりあるいはその壁を外側に押し付けることができる。

ステントは、圧縮力を取り除いた時に外側に戻って、内腔の壁に接触することができるようにステントを横に弾力的に圧縮できるような材料から作ることができる。合金のマルテンサイト相とオーステナイト相との間の変態の結果として形状記憶合金から得られる高弾性により、この用途に特によく適合したステントが製造される。形状記憶合金の超弾性変態特性は、T.W.Duering らによるButterworth-Heinemann(1990年)の「形状記憶合金の工学的見方”Engineering Aspects of Shape Memory Alloys”」の370頁に議論されている。その文献に開示された内容は、その文献に言及することにより本明細書に含める。形状記憶合金の主な変態は、初めは応力と共に略直線的に歪みが増大する。この挙動は元に戻すことができ、従来の弾性変形に対応する。引き続き歪みを増大させても、「ローディングプラトー(荷重平坦域)」の末端までの制限された歪みの範囲では、応力がほとんど増加しないか全く増加しない。荷重平坦域の応力は応力/歪みグラフ上の変曲点で定義される。引き続き歪みを増大させると応力が増大する。荷重を取り外す(アンローディング)と、「アンローディングプラトー(無荷重平坦域)」の開始端まで、歪みが減少するとともに応力が減少し、このアンローディングプラトーの開始端は、歪み減少とともに応力が殆ど変化しない変曲点の存在によって明らかである。アンローディングプラトーの末端で、歪みの低下と共に応力が低下する。アンローディングプラトー応力も応力/歪みグラフ上の変曲点で定義される。ゼロの応力までのアンローディングの後の残留歪みは全てサンプルの永久歪みである。この変形、ローディングプラトー、アンローディングプラトー、弾性率、プラトーの長さおよび永久歪み(特定の全変形に対し定義された)の特徴が、例えば「形状記憶合金の工学的見方”Engineering Aspects of Shape Memory Alloys”」の376頁に確立され、定義されている。

非線形超弾性は、合金の冷間加工プロセス、例えば、プレス、鍛造あるいは引き抜きを含むプロセスによって形状記憶合金に採り入れることができる。冷間加工工程(ステップ)の後にはアニール工程が行なわれ、冷間加工で生じた転位が結合して転位が整合するのに十分高い温度での冷間加工工程によりそのコンポーネントはその形態で拘束される。これにより冷間加工で生じる変形を確実に保持することができる。

超弾性を導入するための技術は上記技術から変化させることができる。例えば、変形した形態に拘束しながら合金を熱処理にかける代わりに、その合金を特定の所望の形態以上に変形させ、次に、以下説明する種類の熱誘導により、特定の所望の形態に向かう形態変化をとるような熱処理を行なうものである。超弾性の導入は、高温(例えば、合金の再結晶温度程度)でアニールを行ない、続いて迅速に冷却し、次に、低温での熱処理を行なっても良い。

形状記憶合金から作られるステントを使用することは、所定の場所にステントを配置した後、連続的にステントを使用すべき内腔に対しステントが外向きの力をかけることができるため、魅力的である。これにより内腔を開けたままにすることができる。それはステントが所定の場所に留まることを意味するものでもある。形状記憶合金の弾性が向上すると、取り付け後、ステントが内腔と共に動き、曲がることがきるようになる。これは、ステントが大腿動脈あるいは頚動脈のような露出内腔に位置する場合に特に重要になることがある。これらの管に外部から力を加えるとそれらの管は一般に丸い断面から実質的に平坦化される。

本発明は、塑性変形できるとともに、形状記憶ステントスリーブが外側に広げられる最大横寸法(最大径)を制限できるような拘束要素を有する複合ステント装具を提供する。

従って、一つの形態では、本発明は(a)ステント装具を配置すべき内腔に対して外への力を作用させることができるように処理された形状記憶合金ステントスリーブと、(b)前記ステントスリーブが外側に広がることができる最大の横寸法(最大径)を制限する拘束手段を含む複合ステント装具を提供する。

本発明のステント装具は、サイズが正確にわからない内腔で使用することができる。ステント装具が内腔で広がる(拡張する)横寸法(径)は、適切な力が内腔に作用する程度の大きさになるまで、拘束要素の変形により調整することができる。ステント装具が内腔に配置されている間、通常、ステント装具の変形が起きる。その変形は拘束された要素の変形を伴い、それにより、回復力が除かれたゆったりした形態にステントスリーブがさらに戻る。これは、ステント装具内に配置した場合に拘束要素の塑性変形でステント装具を広げることができる拡張装具によって達成することができる。従って、他の形態では、本発明は、(a)上記説明したタイプのステント装具と、(b)ステント装具内に配置した場合に前記拘束要素の塑性変形で前記ステント装具が広がることができる最大の横寸法(径)を大きくすることができる拡張装具を含むステントアセンブリーを提供する。適切な拡張装具の一例は膨らますことができるバルーン(風船)である。

ステントスリーブで使用される形状記憶合金は、一般にニッケルーチタンベースの二元合金が可能である。適切な二元合金は、ニッケル含有量が少なくとも約50 at.%、好ましくは少なくとも約50.5 at.%の合金である。ニッケル含有量は52 at.%未満が有用で、好ましくは約51 at.%未満である。ステントスリーブは、第3添加物および第4添加物を有する合金を含むNiーTiベースの合金から作ることができる。この合金に添加できる元素の例としてFe、Co、Cr、Al、CuおよびVが挙げられる。添加元素は、約10 at.%まで、好ましくは、約5 at.%までの量で含有することができる。

ステントスリーブは例えばスリットあるいは大きな開口を有する目の粗い格子構造物を有する。格子構造のステントスリーブは管を切削することによって作ることができる。またステントスリーブは、ワイヤーが交差する場所に適切な接合技術を使用してワイヤーからも作ることができる。この方法の変形工程(ステップ)は、格子の形状を変えることによってステント装具の横寸法を短縮するのが望ましい。その変形はステント装具に圧縮力を、通常、横方向(径方向)にかけて行なうことができる。これにより格子構造の開口を形成するコンポーネントアームの曲げ変形の結果として、ステント装具の横寸法を短縮することになる。コンポーネントアームの幅は、通常、約1.0mm未満であることが多く、好ましくは約0.8mm未満、より好ましくは約0.5mm未満、特に約0.25mm未満である。そのアームが、力をかける方向に平行でない場合には、アームが交差する場所のアーム間で曲げが生じる。一方、コンポーネントアーム自身は略真っ直ぐのままになることが多い。格子構造が複数のダイヤモンド形状もしくは菱形状の開口を形成するステントスリーブでは、そのダイヤモンド形状もしくは菱形状の開口を偏平にする変形も含まれる。格子構造物の開口を定めるアームが力をかける方向に平行かあるいはほとんど平行である場合、隣接するアームが、交差する場所の間のアームを曲げる変形が生じることがある。

拘束要素はステントスリーブを嵌め込み可能なスリーブの形にすることができる。そのような拘束スリーブは、連続シートを押し出し成形するか折り曲げてできるような連続外面を有することがきる。その他、拘束要素は、メッシュあるいは孔あきシートの形で孔が開いていてもよい。拘束要素は独立したコンポーネントである必要はない。拘束要素は、例えば、ステントスリーブの周囲に巻かれたストリップあるいはワイヤーから作ることができるだろう。拘束要素はステントスリーブの一部として設けられるかも知れない。例えば、拘束要素をワイヤでステントスリーブ内に形成してもよく、このような拘束要素はステント装具の拡張時に塑性変形することができる。

【発明の実施の形態】
図1は本発明のステント装具に使用できるステントスリーブを示す。ステントスリーブはアーム2で形成される目が粗い格子構造物を有する。アーム間の開口4はそのスリーブの厚みを貫通して広がっている。スリーブは、例えばYAGレーザー切削工具を使用して管をを切削することによって形成される。(このスリーブはNiを約50.8 at.%含有するニッケルーチタンの二元合金から作られる。)このスリーブの横方向の寸法はスリーブの変形により変えることができる。この変形により、アームが交差する位置でアームを曲げる結果として開口4の形状に変化が生じる。管から作ると、スリーブは約4mmの内部横寸法(内径)がある。管の肉厚、従って、アーム2の各々の厚みは約0.3mmである。

図2はスリーブを変形した後の図1に示したスリーブを示す。その変形によりスリーブの横寸法の短縮する。その結果、図1に示したようにスリーブの開口4は実際上スリット6になる。このようにスリーブは、スリーブの対向する両端に長さ方向の力をかけることにより変形することができる。スリーブは、変形が弾力的に生じる条件で図1に示された形態から図2に示された形態に変形される。次に、このスリーブは、内腔に適切に配置されると図1に示した形態に戻ることができる。

図3と図4は、血管のような内腔10に配置されている本発明のステント装具8を示す。ステントスリーブ12は、ステントが図2に示した形態から図1に示した形態まで外側に広がることができる最大の横寸法(最大径)を制限する拘束手段(要素)14によって図2に示した変形した形態に保持される。拘束手段14は約0.1mmの肉厚の医療グレードのポリエチレンから作られたスリーブからなる。この肉厚のスリーブを使用することにより、ステント装具を少なくとも部分的に偏平にしながら、拘束手段がステントスリーブの最大の横寸法(最大径)を制限する。そのように拘束されると、ステントスリーブを有するステント装具は、カテーテルを伴うような従来の技術を使用して内腔に沿って移動させることができる。

一旦、内腔内に適切に配置されると、図3に示したステント装具は、例えば挿入プッシュロッドによって知られた技術を使用してカテーテルから出される。本発明の装具は、図5に示したように膨らませることができる適切なバルーン(風船)18により広げて内腔の内面に接触させることができる。バルーンの膨張により、図1に示した形態に弾力的に戻す方向に、ステントスリーブ12が外側に変形する。同時に拘束スリーブ14は塑性的に広げられる。

図6は挿入バルーンで拡張した後、内腔に取り付けられたステント装具8を示す。
図7は内腔内に配置された本発明に係る装具を備えた内腔20を示す。その装具は図1ないし図6を参照して上記説明した装具のようにステントスリーブを含む。拘束手段22は図3に示したシートから作られるのと違い不連続である。その拘束手段22はステントスリーブ周囲に広がる複数の略円周バンドを含む。そのバンドはステントスリーブ周囲に複数の別々のバンドとしてあるいはらせん状に巻かれた1本のバンドとして設けることができる。

なお本発明の好適な実施態様として、以下のものがある。
(A) (a)複合ステント装具を配置すべき内腔に対して外への力を作用させることができるように処理された形状記憶合金ステントスリーブと、(b)前記ステントスリーブが外側に広がることができる最大の横寸法を制限する拘束手段を含む複合ステント装具。
(1)前記ステントスリーブは、通常、前記拘束要素内に配置される実施態様(A)に記載の複合ステント装具。
(2)前記拘束要素は、ステントスリーブと少なくとも同じ長さである実施態様(1)に記載の複合ステント装具。
(3)前記拘束要素はポリマー材料を含む実施態様(A)に記載の複合ステント装具。
(B) (a)実施態様(A)に記載の複合ステントと、(b)ステント装具内に配置した場合に前記拘束要素の塑性変形で前記ステント装具を広げることができる拡張装具を含むステントアセンブリー。
(4)前記拡張装具は膨らますことができるバルーンを含む実施態様(B)に記載のステントアセンブリー。
(C) (a)前記ステント装具を配置すべき内腔に対して外への力を作用させることができるように処理された形状記憶合金ステントスリーブと、(b)前記ステントスリーブが外側に広がることができる最大の横寸法を制限する拘束手段を含む複合ステント装具を製造する方法であって、前記拘束要素の塑性変形により前記ステント装具を横に広げる工程を有する複合ステント装具を製造する方法。
(5)前記ステント装具は、内部に配置された拡張装具によって広げられる実施態様(C)に記載の方法。
(6)前記拡張装具は膨らますことができるバルーンを含み、前記バルーンを膨らませる工程を有する実施態様(5)に記載の方法。

【発明の効果】
以上説明したように本発明に係る複合ステント装具によれば、形状記憶合金の変形特性を利用し、しかも外面に拘束要素を設けているため、サイズのわからない内腔などに有効に利用できしかもその内腔を損傷させることがない。又、使用前のステント装具の変形を防止することができる。更に又、カテーテル等による送達時に、ステント装具とカテーテルとの間の摩擦を抑えると共に、ステント装置によるカテーテルの変形も抑えることができる。

【図1】本発明のステント装具に使用するためのステントスリーブの等尺図であり、適切な形態で製造し、しかしステントスリーブに所望の変形挙動を与えることになる熱処理あるいは機械的処理の前のステントスリーブを示す。

Claims (6)

  1. (a)複合ステント装具を配置すべき内腔に対して外向きの力を作用させることができるように処理された形状記憶合金ステントスリーブと、(b)拘束手段であって、当該拘束手段に対して外向きの力を及ぼす前記ステントスリーブの少なくとも一部をカバーし、拡張具による当該拘束手段の塑性変形が生じるまで、前記ステントスリーブが広がって血管の内腔に力を及ぼすことを防止するのに十分な力を有する拘束手段とを含む複合ステント装具。
  2. 前記ステントスリーブは、前記拘束要素内に配置される請求項1に記載の複合ステント装具。
  3. 前記拘束要素は、ステントスリーブと少なくとも同じ長さである請求項1または2に記載の複合ステント装具。
  4. 前記拘束要素はポリマー材料を含む請求項1〜3のいずれかに記載の複合ステント装具。
  5. (a)請求項1に記載の複合ステントと、(b)ステント装具内に配置した場合に前記拘束要素の塑性変形で前記ステント装具を広げることができる拡張装具を含むステントアセンブリー。
  6. 前記拡張装具は膨らますことができるバルーンを含む請求項5に記載のステントアセンブリー。
