Description
動脈瘤の修復のための構成物および方法が記載される。特に、完全またはほぼ完全に吸収性である血管閉塞部材が、これらの部材を作製する方法および使用する方法と同様に開示される。
動脈瘤は、可能性のある破裂、凝固、または解離による健康への危険を与える、(風船のような)血管の拡張である。脳での動脈瘤の経路は脳卒中を引き起こし、腹部の動脈瘤はショックを引き起こす。脳動脈瘤は一般的に、発作および出血の結果として患者に検出され、有意な罹患率または死亡率という結果となる。
従って、本発明は、新規の閉塞構成物ならびにこれらの構成物を使用および作製する方法を含む。
閉塞性(例えば、閉塞性)構成物が記載される。閉塞性構成物として、例えば完全にまたは実質的に吸収性の血管閉塞部材が挙げられる。さらなる物質はまた、これらの閉塞性構成物において使用され得る。本明細書中に記載される構成物は血管および神経血管指標として使用され、特に動脈瘤(例えば、小さい直径で、曲線状)を処置するのに有用であり、そうでなければ、血管系(例えば、脳動脈瘤)にアクセスするのが困難である。これらの血管閉塞部材を作製および使用する方法もまた本発明の局面である。
記載される吸収性血管閉塞部材を作製する方法がまた提供される。本明細書中に記載の血管閉塞部材が、吸収性材料を第一らせん状(代表的に、コイル状)に巻くことによって代表的に形成され、次いで第一らせん状は、第二形態に巻かれる。代表的に、第二形態は、送達カテーテルから取り出された場合、体腔を実質的に充填する三次元形状を形成する形態である。望ましくは、血管閉塞部材は、血管腔(例えば、動脈瘤、または、おそらく瘻孔)内にぴったり適合するために適切なサイズおよび形状である。
本明細書中に記載される塞栓構成物は、しばしば、以下に概説される手順を用いて、選択された部位に導入される。この手順は、種々の疾患の処置において使用され得る。例えば、動脈瘤の処置において、動脈瘤自体は、本明細書中に記載される構成物で(部分的にかまたは完全に)充填される。
本発明者らは、0.007’’ODステンレス鋼マンドレル上に巻き付けられた(PTFEチュービングを有してかまたは有さずに)、0.0055’’PGA単一フィラメント(0.0055’’、Biogeneral Fiber Technology、バッチ番号2005−154)からの吸収性PGA材料で形成された一連の第1コイルを製造した。第1コイルを、上記の手順を用いて巻き付け、そして100℃の温度で10分間焼き鈍しした。十分に成形した、取り外しの容易な第1コイルを形成した。
第2構成を有する一連の吸収性血管閉塞部材もまた製造した。第1コイルを、本質的に実施例1に記載されるように製造し、次いで、第1コイルを、0.004’’ODステンレス鋼マンドレルの周囲に、PTFEチュービングを有してかまたは有さずに、第2構成へと巻き付けた。第2の形状を、165℃の温度で、約15分と約30分との間焼き鈍しした。例えば、図面に示されるような、第2構成を有する吸収性血管閉塞部材を形成した。
次いで、実施例2に記載されるように製造された第2構成を有する吸収性血管閉塞部材を、伸展耐性となるように改変した。詳細には、その縫合材料(非吸収性の、ポリプロピレン単一フィラメント縫合糸)が、ねじ巻きすることによって、および/または第2構成物の2つの位置ではんだ付けすることによって取り付けられるように、コイルを改変した。その後、縫合糸含有吸収性血管閉塞部材を、展開のためにプッシャーワイヤに取り付けた。
Claims (25)
- 吸収性の血管閉塞部材であって:
(a)吸収性材料であって、三次元構成物を有し、該三次元構成物は二つの末端と該二つの末端の間を延びる長手方向軸の外面を有し、該吸収性材料はコアワイヤに取り外し可能に連結される、吸収性材料;および
(b)該吸収性材料の少なくとも2つの位置に固定して取り付けられた1つ以上の伸展耐性部材、
を備え、
該1つ以上の伸展耐性部材は、該吸収性材料の該長手方向軸の外面のほとんどまたは全てをおおって延び、;そして/または
該伸展耐性部材は該吸収性材料から形成され、
該吸収性材料が、複数のらせん状の巻き、第1端、第2端、および該第1端と該第2端との間に管腔を有するらせん状コイルとして構成され、
該伸展耐性部材が、該らせん状コイルの内側または外側に取り付けられている、部材。 - 前記伸展耐性部材が、前記吸収性部材を改変することにより形成される、請求項1に記載の血管閉塞部材。
- 前記吸収性部材が、選択された位置で加熱またははんだ付けすることにより改変される、請求項2に記載の血管閉塞部材。
- 前記加熱またははんだ付けが、前記らせん状巻きの少なくとも2つを連結するよう作用する、請求項3に記載の血管閉塞部材。
- 前記吸収性材料が、ポリグリコール酸(PGA)、ポリグルコール酸/ポリ−L−乳酸コポリマー、ポリカプロラクトン、ポリヒドロキシブチレート/ヒドロキシバレレートコポリマー、ポリ−L−ラクチド、ポリジオキサノン、ポリカーボネート、ポリ無水物、コラーゲン、エラスチン、フィブリノーゲン、フィブロネクチン、ビトロネクチン、ラミニン、ゼラチン、およびそれらの混合物からなる群より選択される、請求項1〜4のいずれかに記載の血管閉塞部材。
- 前記伸展耐性部材が単一フィラメントである、請求項1〜5のいずれかに記載の血管閉塞部材。
- 前記伸展耐性部材が複数フィラメントである、請求項1〜5のいずれかに記載の血管閉塞部材。
- 展開先端をさらに備える、請求項1〜7のいずれかに記載の血管閉塞部材。
- 前記展開先端が、プッシャー上に電流を印加することにより、該プッシャーから離れるよう適合された電気分解的に切断可能な末端を備える、請求項8に記載の血管閉塞部材。
- 放射線不透過性材料をさらに含む、請求項1〜9のいずれかに記載の血管閉塞部材。
- 前記放射線不透過材料が、粉末タンタル、粉末タングステン、酸化ビスマス、および硫酸バリウムからなる群より選択される、請求項10に記載の血管閉塞部材。
- 生体活性材料をさらに含む、請求項1〜11のいずれかに記載の血管閉塞部材。
- 血管閉塞部材を製造する方法であって:
(i)吸収性材料を含む、ほぼ直線形の主要要素を調製する工程;
(ii)該主要要素をマンドレル上に巻き付ける工程;
(iii)該マンドレルおよび該主要要素を加熱して、三次元構成の該血管閉塞部材を製造する工程;および
(iv)該血管閉塞部材の2つ以上の位置を連結して、1つ以上の伸展耐性部材を形成する工程を包含する、方法。 - 前記血管閉塞部材が、らせん状コイルを含む三次元構成物を有する、請求項13に記載の方法。
- 前記血管閉塞部材が、円錐形状またはらせん形状を含む三次元構成物を有する、請求項13に記載の方法。
- 前記吸収性材料が、ポリグリコール酸(PGA)、ポリグルコール酸/ポリ−L−乳酸コポリマー、ポリカプロラクトン、ポリヒドロキシブチレート/ヒドロキシバレレートコポリマー、ポリ−L−ラクチド、ポリジオキサノン、ポリカーボネート、ポリ無水物、コラーゲン、エラスチン、フィブリノーゲン、フィブロネクチン、ビトロネクチン、ラミニン、ゼラチン、およびそれらの混合物からなる群より選択される、請求項13〜15のいずれかに記載の方法。
- 工程(iii)が、約165℃にて約15分間の加熱工程を包含する、請求項13に記載の方法。
- 前記連結が、前記2つ以上の位置の互いに対する加熱またははんだ付けにより形成される、請求項13に記載の方法。
- 前記伸展耐性部材が吸収性である、請求項13〜18のいずれかに記載の方法。
- 前記伸展耐性部材が非吸収性である、請求項13〜18のいずれかに記載の方法。
- 前記伸展耐性部材が単一フィラメントである、請求項19に記載の方法。
- 前記マンドレルがステンレス鋼を含む、請求項13〜21のいずれかに記載の方法。
- 前記血管閉塞部材がさらに、少なくとも1つの放射線不透過性材料を含む、請求項13〜22のいずれかに記載の方法。
- 前記放射線不透過材料が、粉末タンタル、粉末タングステン、酸化ビスマス、および硫酸バリウムからなる群より選択される、請求項23に記載の方法。
- 前記血管閉塞部材が、少なくとも1つの生体活性材料をさらに含む、請求項13〜24のいずれかに記載の方法。
