Description
1) 先端部に側孔を有する可撓性のチューブ状ガイドを用いて生体内で処置するための方法であって、
口と肛門とのいづれか一方から、生体内の目標部位に先端部が至るまで前記チューブ状ガイドを挿入する工程と、
前記チューブ状ガイドを通して体内に内視鏡を挿入し、前記側孔から内視鏡先端を突出させて、目標部位を観察する工程と、
前記内視鏡で観察している目標部位に対して、前記チューブ状ガイドを通して体内に挿入された処置具を用いて処置する工程と、
からなる方法。
口と肛門との一方から、生体内の目標部位に先端部が至るまでチューブ状ガイドを挿入し、
このガイドの1つのルーメンを通して体内に内視鏡を挿入し、
この内視鏡による監視下で、内視鏡と他のルーメンとの少なくとも一方を通して体内に挿入した処置具を用いて体内で処置し、
前記他のルーメンから体内に挿入した処置具を用いて処置するときに、前記内視鏡の先端部を、チューブ状ガイドの先端部の近部に形成した側部孔(14)から突出させ、チューブ状ガイドの側方から処置部を監視する、方法。
Claims (10)
- 内視鏡先端部(22a)を有する内視鏡(22)を、体内に案内するガイド(12)であって、
口と肛門との少なくとも一方から、体内の目標部位まで挿入されるチューブ先端部と、体外に配置されるチューブ基端部と、前記チューブ先端部から前記チューブ基端部まで延びる内視鏡挿通孔(30)と、前記チューブ先端部から前記チューブ基端部まで延びる少なくとも1つのルーメンからなるガイド手段(32,34)とを有するチューブ状本体と、
前記チューブ先端部に形成され、前記内視鏡挿通孔(30)を通り挿入された前記内視鏡先端部(22a)を前記チューブ状本体の長手軸線方向の前方へ突き出し可能な先端開口を形成した先端面と、
前記先端面の先端開口領域を避けて前記チューブ先端部における基端側領域の側壁に開口され、前記内視鏡先端部(22a)を挿通可能であり、かつ内視鏡挿通孔(30)から前記チューブ状本体の外へ前記内視鏡先端部(22a)を突き出して前記チューブ先端部を迂回して前記チューブ先端部の外から前記先端面の前方へ誘導可能な側部孔(14)と、
を備えたガイド。 - 更に、前記チューブ先端部内で前記先端面の近傍に形成され、器具を収容可能な空間部を備え、前記内視鏡先端部(22a)を側部孔(14)から前記チューブ状本体の外へ突き出して前記チューブ先端部及び前記空間部を迂回して前記チューブ先端部の外から前記先端面の前方へ誘導するようにした請求項1に記載のガイド。
- 前記先端開口を有した先端面は、前記チューブ状本体の長手軸方向に対して傾斜しており、
この傾斜した先端面の、前記チューブ状本体の長手軸方向へ最も前方へ突き出した部分が位置する側と反対側に位置して前記側部孔(14)を配置した請求項1または請求項2に記載のガイド。 - 前記側部孔(14)と、ガイド手段(32,34)は、前記内視鏡挿通孔(30)に対し、互いに反対側に位置して配置された請求項1、請求項2または請求項3に記載のガイド。
- 前記ガイド手段は、把持鉗子(19)と保持具(18)とを含む処置具のうちから選択された少なくとも1つの処置具を挿通可能な軸方向孔を有する請求項1、請求項2、請求項3または請求項4に記載のガイド。
- 前記チューブ状本体は、少なくとも先端部が透明樹脂で形成されている請求項1乃至請求項5のいずれかに記載のガイド。
- 前記チューブ状本体は、少なくとも先端部と基端部とを除く部位が可撓性材料で形成されている請求項1乃至請求項6のいずれかに記載のガイド。
- 更に、内視鏡挿通孔(30)と、前記ガイド手段(32,34)とを内包する可撓性チューブを有する請求項1乃至請求項7のいずれかに記載のガイド。
- 前記内視鏡挿通孔(30)及び前記ガイド手段(32,34)を形成する第1領域(β)と前記収容空間部を形成する第2領域(α)との両領域に跨る領域に前記側部孔(14)を形成した請求項4に記載のガイド。
- 前記収容空間部を形成する共通ルーメンは、前記内視鏡挿通孔(30)の先端開口及び前記ガイド手段(32,34)を形成するルーメンの先端開口を含む大きさを有した内空を備えており、前記収容空間部は、処置器具の先端部を収容可能であるところの請求項2に記載のガイド。
