Description
さらに、探知機管の配置場所に鑑みて、ステープル形成機構は、探知機管を跨ぐために断面がほぼU字形でなければならない。このために、作用フィンガは、形成プロセス中、外方に広がる傾向がある。
最初に、図1〜図3を参照すると、外科用ステープル留め器は、上半部10a及び下半部10bを備えた細長いハウジング、即ち、シャフト10を有し、これらの半部の間には、ステープル14を摺動自在に収容する長手方向チャネル12が形成されている。外科用ステープル留め器は、ステープル閉鎖アクチュエータ16及び血液探知機管又は部材18を更に有している。なお、内部構成部品が見えるようにするため、図1〜図6にはハウジングの下半部10bだけを示しており、図面には、本発明の実施形態が具体化されているハウジング10の自由前端部だけを示している。ハウジング10の後端部は、拳銃型握りを備え、以下に説明する種々の構成要素の運動は、カムシステムを介するトリガの作動によって行われる。かかる構成は、アイルランド国特許出願第S2000/0722号明細書に記載されており、かかる特許出願明細書に記載されている構成を、本発明の実施形態のステープル留め器を動作させるように容易に改造できる。
ステープル留め器の初期形態(図3参照)では、血液探知機管18は、その前端部がハウジング10の前端部を越えて突出した伸長位置にある。アクチュエータ16を後方に引っ込め、未変形のステープル14をアーム16aの直ぐ前のスロット20内に自由に収める。アンビルのブリッジ要素24cは、前方にステープルの後部14aと間隔をおいた状態でスロット20内に位置し、ハウジング10の前面と面一をなしている。全ての構成部品は、図3〜図6には示していないハウジングの上半部10aによって定位置に保持されている。
最後に、図6に示すように、血液探知機管18を更に引っ込めると、閉じた状態のステープル14が自由になり、突出しばね28により、閉じた状態のステープルを横方向ブリッジ要素24cに対して上方に押し、ステープルを放出する。図7で分かるように、ハウジングの上半部10aはその前端部に、閉じた状態のステープルのこの上方移動を可能にする凹部30を有している。
a.真直ぐな標準型の脚部付きステープルを用いることができる。このステープルは、一層予測可能な仕方で変形し、製造するのが安価であり、しかも、アンビル又はシャフト内で引っ掛かる恐れが小さい。
b.用いられるステープルが真っ直ぐなので、ステープルが開いて「いい加減な」ステープル効果を生じさせる恐れが小さい。
c.製造が容易になり且つ位置が一層よく分かるように血液流量を増大させる、大きな血液探知機管を用いることができる。
d.横方向ブリッジが設けられているので、強固で製造が容易なアンビルが得られる。
e.ステープル形成機構の形成構成部品相互間のブリッジが、ステープル成形中における広がりを防止する。
本発明は、本明細書に記載した実施形態には限定されず、本発明の範囲から逸脱することなく改造又は変形できる。
Claims (5)
- 自由前端部を備えた細長いハウジングと、後部及び2つの前方に向いた脚部を備え且つ前記ハウジング内でその自由前端部に向かって長手方向に摺動自在な外科用ステープルと、このステープルをハウジングの自由前端部に向かって駆動し、ステープルを曲げ、ステープルの脚部の自由端部を互いに近づけて、ステープルを閉じ、閉じられたステープルを放出するステープル発射機構と、を有する外科用ステープル留め器において、
更に、細長い探知機部材を有し、この探知機部材は、その前端部が前記ハウジングの自由前端部を越えて突出して人又は動物内の液体運搬管の傷孔部位に入り、それにより、前記ハウジングの自由前端部を傷孔部位に位置決めする初期伸長位置と、ステープルが閉じるのに合わせて前記探知機部材が前記ハウジング内に引っ込められる引込み位置との間で前記ハウジング内を長手方向に摺動自在であり、前記探知機部材は、その前端部から後方に前記探知機部材の長さの一部に沿って延びる長手方向スロットを有し、ステープルの後部は、ステープルの前方に向いた脚部が前記探知機部材の互いに対向する側部の上に且つその外部に配置された状態で前記スロット内に横方向に収納されることを特徴とする外科用ステープル留め器。 - 前記ステープル発射機構は、前記スロット内を前記ハウジングの長手方向に摺動自在であり且つステープルを前記ハウジングの自由前端部に向かって駆動するアクチュエータと、ステープルを変形させてこれを閉じるためにステープルがアクチュエータによって押し付けられるアンビルと、を有することを特徴とする請求項1に記載の外科用ステープル留め器。
- 前記アンビルは、ステープルの前方で前記探知機部材のスロットをの中を通って横方向に延びる横方向要素を有し、前記探知機部材の前端部は、前記探知機部材の引っ込み位置において、閉じられたステープルの放出を可能にするように前記横方向要素の後方に位置決めされることを特徴とする請求項2に記載の外科用ステープル留め器。
- 閉じられたステープルを放出するために、閉じられたステープルを前記横方向要素に対して上方に押す弾性付勢手段を更に有することを特徴とする請求項3に記載の外科用ステープル留め器。
- 前記探知機部材は、ガイドワイヤを収容し且つ血液をハウジングに沿って視覚化ポートまで後方に運ぶ中空管を有することを特徴とする請求項1〜4のいずれか1項に記載の外科用ステープル留め器。
