【発明の属する技術分野】
この発明は組織を近接させて止血または止気を行うための装置に関し、特に生体適合性外科用発泡支持材（foam buttress)を身体組織に適用するステープル適用装置に関する。

【従来の技術および発明が解決しようとする課題】
外科手術時には外科用ステープルを用いて組織器官を近接させる必要がある。多くの場合、外科医はリニアカッターステープル適用装置を用いて肺、食道、胃、十二指腸その他の身体器官などの身体の器官および組織を縫合する。そのような装置は複数の横方向に離間したステープル列を切断された組織の反対側へ適用するものである。

そのようなステープル適用装置（ステープリング装置またはステープラともいう）の例が米国特許第４，６３３，８６１号および同第４，８９２，２４４号に記載されており、これらの開示を参照により本願に援用する。この外科用ステープル適用装置は１対の協働する長手の顎部材を備えている。顎部材の一つは少なくとも２つの横方向に離間したステープル列をもったステープルカートリッジを備え、他方の顎部材はカートリッジのステープル列と整列してステープルを閉じる押下部をもったアンビルを備えている。押ブロックが顎部材に沿って縦方向に前進して、アンビルに対してステープルを閉じるようにカートリッジからステープルを次々に射出し、顎部材間で把持された組織を通して横方向に離間したステープルラインを形成する。ナイフが押ブロックに伴われて顎部材に沿って前進して、先に形成されたステープル列間のラインに沿って組織を切断する。

組織上での手術時には、切断ラインに沿って開口した血管を閉じること（hemostasis：本願において「止血」という）が望ましい。また、肺などの気管系組織を近接させることを伴う手術においては、肺または気管もしくは気室の組織を封止して空気漏れを防止すること（pneumostasis：本願において「止気」という）が必要である。米国特許第５，２６３，６２９号は組織の少なくとも１つの面に隣接して配置されるプレジェット材（pledget ：ガーゼ）を利用することによりステープルラインに沿って止血を行う方法および装置を開示している。このステープルラインは組織および吸収性プレジェット材を貫通するように形成される。このプレジェット材は、ステープルラインに沿って圧力を実質的に均一に分配し、それにより切断された組織に沿って実質的な止血が生じるように選択される必要がある。これらのプレジェット材に好適な材料は無菌で吸収性の密に織った布である。プレジェット材は離間した超音波溶接または離間した接着剤結合によってステープル適用装置に固着される。

【課題を解決するための手段】
本発明は、プレジェット材すなわち支持材が、切断容易であって止血および止気のため良好な封止を行い、低温融解性または液状の生吸収性ポリマーによってステープル適用装置に剥離可能に付着することができる、柔軟で弾力性で生吸収性の発泡材料である改良された外科用ステープル適用装置を提供するものである。

本発明に係るステープル適用装置は、２つの離間したラインに設けられた複数の外科用ステープルを含有するとともに開口部をもった上面を有するステープルカートリッジを備え、前記ステープルは前記開口部を通して射出され、前記上面には弾力性で生吸収性の発泡材が剥離可能に付着されている。

また、本発明はステープル適用装置の顎部材に適合するように形作られた第１の面と第２の面を有する弾力性で生体適合性の発泡材に係るものである。発泡材の面は剥離性で生体適合性の接着剤を任意に有することができる。好ましくは、弾力性の発泡材および／または剥離性の接着剤が生吸収性である。

【発明の実施の形態】
図１に、概ね米国特許第４，６３３，８６１号および同第４，８９２，２４４号に開示された型の典型的な外科用ステープル適用装置（ステープリング装置またはステープラともいう）１０を示すが、該装置の特定の構造の詳細に関する更に詳細な説明についてはこれらの特許の開示を参照により本願に援用する。外科用ステープル適用装置１０は上方顎部材２０、発射手段３０、下方顎部材４０および下方顎部材４０内に受容されるステープルカートリッジ５０を備える。

発射手段３０は押ブロックおよび発射ウェッジ組立体３２とその間に位置するナイフ３４を備える。この発射ウェッジはステープルカートリッジ５０内に位置する縦方向スロットを通して移動する。カートリッジ５０は下方顎部材チャネル４４内に着脱可能に受容される。発射ノブ４２が発射手段３０を作動して、発射ウェッジ３２をステープルカートリッジ５０を通して移動させる。発射ウェッジ３２はカートリッジ５０を縦方向に通る際にステープル駆動装置（図示せず）と接触し、ステープル駆動装置が次にステープルカートリッジ５０内の開口部５３を通してステープル５１を射出する。

上方顎部材２０はラッチピン３５により下方顎部材４０に旋回自在に接続されており、ラッチピン３５はラッチ部材３８に係るスロット３６内に受容されて、両顎部材を共にその長さに沿った中間位置においてラッチする。図２に示すようなラッチ位置と図１に示すようなラッチ解除位置との間のラッチ部材３８の移動により、両顎部材２０および４０が互いに近づく方向、または遠ざかる方向に移動する。

図１と図３に、概ね米国特許第４，６３３，８６１号および同第４，９８２，２４４号に開示された型の、複数の外科用ステープル５１を含有する使い捨て式ステープルカートリッジ５０の好適な実施形態を示す。カートリッジ５０は好ましくは２対の離間した平行なステープルラインを備えている。カートリッジ５０は、本発明の好適な実施形態によれば、開口部５３を通してステープルが射出される開口部５３を有する上面５７を覆うような位置関係で剥離可能に（低温融解性または液状の生吸収性ポリマー５４により）付着された弾力性で生吸収性の発泡材５２の片（ストリップともいう）を備え、この発泡材の片は、低温融解性または液状の生吸収性ポリマー５４によって設けられた複数の離間した剥離可能な結合部または連続的なコーティングによってステープルカートリッジ５０の縦方向縁端部に固定される。

図１および図７乃至図１０において、上方顎部材２０の前方部分は、均一に離間したステープル形成ポケット６３の縦方向の列を備えたアンビル部６０を備える。使い捨て式のアンビル先端部６２がアンビル部６０の前端に着脱可能に取り付けられ、その一部分がアンビル部６０の前端の後方に受容される。アンビル先端部６２は顎部材が中空筒状の身体器官または組織部分の小開口に挿入されるのを容易にするための先導テーパ状部分６４を備える。アンビル先端部６２はまた、アンビル部６０内に延びる１対の離間した長手の内側壁部６６と、アンビル部６０の側面に沿って延びる１対の離間した長手の外側壁部６８を備える。発泡材の片５２は、ステープル適用装置がステープル部位から取外された後にステープルとともに当該発泡材片が残留できるようにする多様な手段により、アンビル先端部６２に（接着剤または機械手段によって）剥離可能に固定することができる。本発明の好適な実施形態によれば、発泡材の片は、低温融解性または液状の生吸収性ポリマー５４などのような剥離性の接着剤により形成された複数の離間した結合部または連続的なコーティングによって、外側壁部６８の底面に固定される。

本発明のさらに別の実施形態によれば、ステープル適用装置の両顎部材２０および４０上で適合（fit)するスリーブに発泡材を設けることもできる。スリーブの適切な設計は米国特許第５，５０３，６３８号、同第５，７０２，４０９号、および同第５，７６６，１８８号に記載されており、これらの開示を参照により本願に援用する。止血または止気のために患者内に留まるスリーブの部分に発泡材を使用すればよいのであって、スリーブの他の部分は他の適切な材料により製造してよい。

本発明の好適な実施形態によれば、発泡材片５２は好ましくは弾力性で生吸収性の発泡材により製造される。この発泡材はステープルラインに沿って均一に圧力を分配して、切断された組織に沿って実質的な止血または止気を生じさせる。また、この発泡材は薄弱なまたは病的な組織の場合にもステープルをその上に保持するための媒体となる。また、この発泡材は衝撃を吸収し、外傷を減少する。

支持材（buttress）として使用されるための適切な発泡材は、その発泡材が圧縮性負荷を分配するとともに組織の厚さのばらつきを補償することができ、それにより止血および止気のための有効なガスケットとして作用するように弾力性で柔軟であることが必要である。

本発明において使用するための適切な発泡材は生体適合性の弾性ポリマー（エラストマー）により製造され、好ましくはこのポリマーは生吸収性でもある。適切な生吸収性エラストマーの例は米国特許第５，４６８，２５３号に記載されており、その開示を参照により本願に援用する。好ましくは、生吸収性で生体適合性のエラストマーは脂肪族ポリエステルに基づくものであり、その例としてはε−カプロラクトンとグリコーリド（glycolide)との弾性共重合体（好ましくはε−カプロラクトン対グリコーリドのモル比が約３５：６５乃至約６５：３５、より好ましくは４５：５５乃至３５：６５である）、ε−カプロラクトンとＬ−ラクチド、Ｄ−ラクチド、その混合物または乳酸などのラクチド（lactide)との共重合体（好ましくはε−カプロラクトン対ラクチドのモル比が約３５：６５乃至約６５：３５、より好ましくは４５：５５乃至３０：７０、または約９５：５乃至約８５：１５である）、ｐ−ジオキサノン（すなわち１，４−ジオキサン−２−オン）とＬ−ラクチド、Ｄ−ラクチド、その混合物または乳酸などのラクチドとの弾性共重合体（好ましくはｐ−ジオキサノン対ラクチドのモル比が約４０：６０乃至約６０：４０である）、ε−カプロラクトンとｐ−ジオキサノンとの弾性共重合体（好ましくはε−カプロラクトン対ｐ−ジオキサノンのモル比が約３０：７０乃至約７０：３０である）、ｐ−ジオキサノンとトリメチレンカーボネートとの弾性共重合体（好ましくはｐ−ジオキサノン対トリメチレンカーボネートのモル比が約３０：７０乃至約７０：３０である）、トリメチレンカーボネートとグリコーリドとの弾性共重合体（好ましくはトリメチレンカーボネート対グリコーリドのモル比が約３０：７０乃至約７０：３０である）、トリメチレンカーボネートとＬ−ラクチド、Ｄ−ラクチド、その混合物または乳酸などのラクチドとの弾性共重合体（好ましくはトリメチレンカーボネート対ラクチドのモル比が約３０：７０乃至約７０：３０である）、およびこれらの混合物から成る群より選択されたものがあるが、これらには限らない。これらの弾性ポリマーは、２５℃における０．１グラム／デシリットル（ｇ／ｄＬ）のポリマーのヘキサフルオロイソプロパノール（ＨＦＩＰ）溶液において決定される固有粘度が約１．２ｄＬ／ｇ乃至約４ｄＬ／ｇ、好ましくは約１．２ｄＬ／ｇ乃至約２ｄＬ／ｇ、さらに最も好ましくは約１．４ｄＬ／ｇ乃至約２ｄＬ／ｇである。

好ましくは、エラストマーは高い伸長率（％）と低い弾性率を示す一方で、良好な引張り強さと良好な復元特性を有する。本発明の好適な実施形態によれば、発泡材を構成するエラストマーは２００％より大きい、好ましくは５００％より大きい伸長率を示す。また、当該エラストマーは約８０，０００ｐｓｉ（５，６２０ｋｇ重／ｃｍ2 ）より小さい、好ましくは約４０，０００ｐｓｉ（２，８１０ｋｇ重／ｃｍ2 ）より小さい弾性率（ヤング弾性率）を示す。当該生吸収性エラストマーの弾性の程度を測定する特性が達成される一方で、約５００ｐｓｉ（３５．１ｋｇ重／ｃｍ2 ）より大きい、好ましくは約１，０００ｐｓｉ（７０．３ｋｇ重／ｃｍ2 ）より大きい引張強さ、ならびに約５０ポンド／インチ（８．９ｋｇ重／ｃｍ）より大きい、好ましくは約８０ポンド／インチ（１４ｋｇ重／ｃｍ）より大きい引裂強さを維持する。

これらの弾性ポリマー（エラストマー）は、凍結乾燥、超臨界溶媒発泡（すなわちＥＰ ４６４，１６３ Ｂ１に記載されるような方法）、ガス注入押出し、ガス注入成形、抽出可能物質（すなわち塩類、糖または他の当業者に知られた手段）を用いた注型成形などにより発泡処理することができる。現在のところ凍結乾燥により生吸収性で生体適合性のエラストマーを製造することが好ましい。弾性ポリマーを凍結乾燥して発泡材を形成するための適切な方法の一つは後述の実施例２に記載する方法である。さらに、発泡材に医薬活性化合物を組み込んで患者を治療することができ、この医薬活性化合物としては抗生物質、抗真菌剤、止血剤、抗炎症剤、（細胞）増殖因子などがあるが、これらには限らない。

［式中、Ｒ11は−ＣＲ12Ｈ−，−（ＣＨ2 ）3 −Ｏ−，−ＣＨ2 −ＣＨ2 −Ｏ−ＣＨ2 −，−ＣＲ12Ｈ−ＣＨ2 ，−（ＣＨ2 ）4 −，−（ＣＨ2 ）z −Ｏ−および−（ＣＨ2 ）z −Ｃ（Ｏ）−ＣＨ2 −；Ｒ12は水素またはメチル；ｚは１乃至７の範囲の整数；およびｙは約１０乃至約２０，０００の範囲の整数である］のポリマーにより製造される脂肪族エステル単独重合体（ホモポリマー）および共重合体（コポリマー）；粘性ＰＥＧ（ポリエチレングリコール）液体および低温融解性ＰＥＧ固体（室温で固体であって約６０℃よりも低い温度で融解するもの）の混合物；可塑剤（例えばグリセリンなど）と混合されて粘着性の接着剤を形成する生体適合性の単糖類、二糖類および多糖類（例えばペクチンなど）；および可塑剤（例えばグリセリンなど）と混合されて粘着性の接着剤を形成する生体適合性のタンパク質（例えばゼラチン）などがある。

室温で半結晶性固体または粘着性液体である多くの無毒性で生吸収性の脂肪族エステルポリマーが剥離性の接着剤として使用することができる。この発明の剥離性接着剤は一般に体温（３７℃）で流動可能なものとして特徴付けられ、好ましくは室温（２５℃）で流動する。最も好ましくはこれらの液体は該ポリマーの移動を防止するために低い降伏点を有する。適切な粘着性で液状の共重合体の例が１９９６年１１月６日提出の米国特許出願第０８／７４６，１８０号に含まれており、その開示を参照により本願に援用する。さらに、米国特許第５，５９９，８５２号に記載されるような粘着性の微粒子分散液(microdispersion）を使用することもでき、その開示を参照により本願に援用する。特に、約６５モル％乃至約３５モル％の範囲のε−カプロラクトン繰返し単位、トリメチレンカーボネート繰返し単位、エーテルラクトン（本発明の例として１，４−ジオキセパン−２−オン、１，５−ジオキセパン−２−オンおよびこれらの誘導体等がある）繰返し単位またはこれらの組合せから構成され、該重合体の残部が複数の第２ラクトン繰返し単位である液状共重合体が好ましい。この第２ラクトン繰返し単位は、グリコーリド（例えばグリコール酸）繰返し単位、ラクチド（例えば乳酸）繰返し単位、１，４−ジオキサノン繰返し単位、６，６−ジアルキル−１，４−ジオキセパン−２−オン繰返し単位、これらの組合せおよびこれらの混合物から成る群より選択される。さらにε−カプロラクトン、トリメチレンカーボネートまたはエーテルラクトンは共重合して液状共重合体としてもよい。また、微粒子状固体として使用するために好適なポリマーは、ポリ（ε−カプロラクトン）、ポリ（ｐ−ジオキサノン）またはポリ（トリメチレンカーボネート）などの単独重合体、ε−カプロラクトンとトリメチレンカーボネートとの共重合体、ε−カプロラクトンと複数の第２ラクトン繰返し単位との共重合体などを含む生吸収性ポリマーである。この第２ラクトン繰返し単位は、グリコーリド（例えばグリコール酸）繰返し単位、ラクチド（例えば乳酸）繰返し単位、１，４−ジオキサノン繰返し単位、１，４−ジオキセパン−２−オン繰返し単位、１，５−ジオキセパン−２−オン繰返し単位およびこれらの組合せから成る群より選択することができる。ε−カプロラクトンの共重合体は好ましくは９９モル％乃至７０モル％の範囲で構成され、該重合体の残部は複数の第２ラクトン繰返し単位である。これらのポリマーは線状、分岐状、または星分岐状であってよく、ブロック共重合体または三元共重合体（terpolymer）であってよく、セグメント化したブロック共重合体または三元共重合体であってよい。これらのポリマーは組織における炎症反応を生じさせる未反応モノマーを実質的に除去するために精製される。

剥離性接着剤として使用される好適な液状共重合体は、約６５モル％乃至約３５モル％の範囲のε−カプロラクトンまたはエーテルラクトン繰返し単位から構成され、該共重合体の残部はトリメチレンカーボネート繰返し単位である。また、適切な三元共重合体（terpolymer）の例としては、ε−カプロラクトン繰返し単位が約３５乃至約６５モル％であるポリ（グリコーリド−ε−カプロラクトン−ｐ−ジオキサノン）およびポリ（ラクチド−ε−カプロラクトン−ｐ−ジオキサノン）から成る群より選択される三元共重合体がある。好適なのはε−カプロラクトン繰返し単位が４０乃至６０モル％の範囲にある三元共重合体である。

さらに、液体および固体ポリエチレングリコール（ＰＥＧ）の混合物も剥離性接着剤として使用することができる。液体ＰＥＧは分子量が約２００乃至約６００であってよい。固体ＰＥＧは分子量が約３４００乃至約１０，０００であってよい。本発明の範囲を何ら限定するものではないが、一般には低分子量液体ＰＥＧが固体ＰＥＧを可塑化して固体ＰＥＧに粘着性を賦与するものと考えられる。そのために該組成物の大部分は固体ＰＥＧである必要があり、好ましくは該組成物の約５０乃至約８０重量％が固体ＰＥＧである。一例として、分子量４００の液体ポリエチレングリコール（ＰＥＧ４００）を分子量約２，０００の固体ポリエチレングリコール（ＰＥＧ２０００）と混合して使用することができる。ＰＥＧ４００対ＰＥＧ２０００の比率は約４０：６０乃至約３０：７０の範囲とすることができる。これらの混合物は、液体ＰＥＧと固体ＰＥＧを加熱水浴中で定常的に攪拌して固体融解物と透明液体溶液が形成されるまで混合することによって形成することができる。これらの溶液を放冷した後に粘着性について試験して本発明において使用される所望の粘着性が得られれば、その得られた混合物を使用することができる。

組織部分を両顎部材間にクランプした後に、発射ノブ４２を前進させることによりステープル適用装置１０を発射して、押ブロックおよびナイフ刃組立体３０を作動する。発射ウェッジ３２がステープルカートリッジ５０を通して先端方向に前進してステープル駆動装置と係合し、開口部５３を通して２対の離間した平行なステープルラインにおいてステープル５１を次々に駆動する。ステープル５１はアンビル部６０に係る対応するステープル形成ポケット６３と接触して、ほぼＢ字形状または平坦形状のステープルを形成する。図４を参照すると、形成されるステープルが組織部分７０および７２および発泡材５２の片を貫通する。同時に、ナイフ刃３４がアンビル部６０およびステープルカートリッジ５０内に形成された縦方向スロットを通して先端方向に前進して、２対の離間した平行なステープルラインの間で両顎部材間に把持された当該組織部分を切断する。

実施例１
発泡支持材接着剤用の５０：５０ポリ（ε−カプロラクトン−グリコーリド）の合成
５７．０７グラムのε−カプロラクトン（０．５モル）、５８．０４グラムのグリコーリド（０．５モル）、５．６８ｍＬ（６０ミリモル／モル）のジエチレングリコール、および６７μＬ（４５Ｋ（カタール）／１モノマー／触媒比）の０．３３Ｍのスズ（ＩＩ）オクトエートのトルエン溶液を、窒素雰囲気グローブボックス内で、清浄で火炎乾燥した２５０ｍＬの一口丸底フラスコに加えた。フラスコの内容物を室温で十分な真空下で一晩乾燥した。一晩乾燥後、このフラスコを火炎乾燥したステンレス鋼製の機械的攪拌器に備え付け、さらに３０分間真空乾燥し、窒素ガスをパージし、重合の間、窒素ガス雰囲気下で保持した。このフラスコを油浴に移した。油浴の温度を８０℃に昇温し、８０℃で半時間保持してグリコーリドを完全に溶解させた。次に油の温度を１９０℃に昇温して１８時間保持した。フラスコを油浴から取り出して室温まで放冷した。機械的攪拌器を磁気的攪拌棒に置き換えた後に、８０℃で十分な真空下で１６時間攪拌して当該共重合体から揮発分を除去した。最終的な当該共重合体の特性を１Ｈ−ＮＭＲおよびＨＦＩＰ中の固有粘度（２５℃で０．１ｇ／ｄＬの濃度）により調べた。ＮＭＲの結果は、ポリグリコーリド４７．３％、ポリカプロラクトン５２．３％、残留グリコーリドモノマー０．２％未満、残留カプロラクトンモノマー０．２％未満であった。固有粘度（ＩＶ）は０．１９であった。

実施例２
カプロラクトン−グリコーリド発泡材製造のための凍結乾燥プロセス
この実施例はカプロラクトン−グリコーリド共重合体から凍結乾燥発泡材を製造するための好適な方法を説明するものである。

溶液製造：適当量（１６．５グラム）のポリマー（米国特許第５，４６８，２５３号に記載された方法と同様に製造された公称４０／６０ＣＡＰ／ＧＬＹ）を、１，４−ジオキサン溶媒（１４８ｍｌ）に添加し、溶解する（１０重量％溶液になる）まで約５０℃で約５時間攪拌した。当該溶液を超粗多孔性フィルタ（キムフローフリッテドディスク付キンブル、キマックス・ブッフナー漏斗（Kimble, Kimax Buchner funnel with Kimflow fritted disc）、１５０ｍｌ容量、超粗多孔性、あるいは均等物）を通して冷時濾過した。

凍結乾燥：適当量の溶液（厚さ約０．０３０インチ（約０．０７６ｃｍ）とするため３１．０ｇ）を、直径約８．５インチ（約２２ｃｍ）の特製平底ガラス皿（「光学的」に平らなガラス底および管から構成される）に移した。当該溶液を含有する皿を使用前に腐蝕剤で清浄化しシラン処理した。当該溶液を含有する皿を２０℃に維持された予め冷却した凍結乾燥器の棚に置いた。次に棚温度を−５℃に冷却することによって当該溶液を凍結させた。３０分後、凍結乾燥器内を真空に引いた（一例では１時間程度）。１，４−ジオキサンの大部分を除去するために、真空下−５℃での一次乾燥が２時間必要であった。この一次乾燥期の終了時に、真空度は通常約１０ｍＴｏｒｒ（約１．３Ｐａ）以下に達する。次の二次乾燥期は１０ｍＴｏｒｒ（約１．３Ｐａ）の真空下、二段階で行った。最初の段階では棚温度を５℃に昇温して１時間保持し、次の段階では温度を２０℃に昇温してさらに１時間保持した。

二次乾燥期の終了時に、チャンバーを室温にし、真空状態に窒素を導入した。皿から発泡材を取り出してプラスチック袋に入れ、窒素雰囲気下で保存した。

以上述べたように、本発明の支持材は、弾力性の発泡材から構成されているので、圧縮性負荷を分配するとともに組織の厚さのばらつきを補償することができ、それにより止血および止気のための有効なガスケットとして作用する。従って、本発明の支持材は、織布等から構成される従来のプレジェット材に比べて封止および衝撃特性に優れている。また、本発明のステープル適用装置は、組織にステープルを適用する際に当該弾力性の発泡材から構成された支持材を使用するものであるので、ステープルラインに沿ってより均一に圧力を分配して良好な止血または止気を行うことができる。

Claims (23)

  1. ２つの離間したラインに設けられた複数の外科用ステープルを含有するとともに開口部をもった上面を有するステープルカートリッジを備え、前記ステープルは前記開口部を通して射出され、前記上面には弾力性で生吸収性の連続気泡（ open cell ）発泡材が剥離可能に付着されており、当該連続気泡発泡材が前記カートリッジの上面と接触する実質的に封止された少なくとも１つの面を有し、かつ当該連続気泡発泡材の面が当該発泡材を加熱することにより封止されたものである、組織の止血または止気用装置。
  2. 前記弾力性で生吸収性の連続気泡発泡材がε−カプロラクトンとグリコーリドとの共重合体から成る請求項１記載の装置。
  3. 前記弾力性で生吸収性の連続気泡発泡材が生吸収性で剥離性の接着剤によって前記ステープルカートリッジの上面に剥離可能に付着された請求項１記載の装置。

  7. 前記生吸収性で剥離性の接着剤がタンパク質および可塑剤である請求項３記載の装置。
  8. 前記弾力性で生吸収性の連続気泡発泡材が有効量の医薬活性化合物を含有する請求項１記載の装置。
  9. 前記剥離性の接着剤が粘着性の微粒子分散液である請求項３記載の装置。
  10. ステープル適用装置の顎部材に適合するように形作られた第１の面と第２の面を有する弾力性で生吸収性の発泡材であって、当該弾力性で生吸収性の発泡材が、連続気泡発泡材であり、脂肪族ポリエステルエラストマーから成り、かつ当該発泡材が当該発泡材を加熱することにより実質的に封止された少なくとも１つの面を有する、弾力性で生吸収性の発泡材。
  11. 前記脂肪族ポリエステルエラストマーが、ε−カプロラクトンとグリコーリドとの共重合体、ε−カプロラクトンとラクチド（Ｌ−ラクチド、Ｄ−ラクチド、その混合物または乳酸を含む）との共重合体、ｐ−ジオキサノンとラクチド（Ｌ−ラクチド、Ｄ−ラクチドおよび乳酸を含む）との共重合体、ε−カプロラクトンとｐ−ジオキサノンとの共重合体、ｐ−ジオキサノンとトリメチレンカーボネートとの共重合体、トリメチレンカーボネートとグリコーリドとの共重合体、トリメチレンカーボネートとラクチド（Ｌ−ラクチド、Ｄ−ラクチド、その混合物または乳酸を含む）との共重合体およびこれらの混合物から成る群より選択される請求項１０記載の弾力性で生吸収性の発泡材。
  12. 前記弾力性で生吸収性の発泡材が有効量の医薬活性化合物を含有する請求項１０記載の弾力性で生吸収性の発泡材。
  13. ε−カプロラクトンとトリメチレンカーボネートとの共重合体、ε−カプロラクトンとエーテルラクトンとの共重合体、トリメチレンカーボネートとエーテルラクトンとの共重合体、約６５モル％乃至約３５モル％の範囲のε−カプロラクトン繰 返し単位、トリメチレンカーボネート繰返し単位、エーテルラクトン繰返し単位またはこれらの組合せから構成され、該重合体の残部が複数の第２ラクトン繰返し単位である共重合体（前記第２ラクトン繰返し単位はグリコーリド繰返し単位、ラクチド繰返し単位、１，４−ジオキサノン繰返し単位、６，６−ジアルキル−１，４−ジオキセパン−２−オン繰返し単位、これらの組合せおよびこれらの混合物から成る群より選択される）、これらの組合せおよびこれらの混合物から成る群より選択される剥離姓の接着剤を前記第１の面にさらに備える請求項１０記載の弾力性で生吸収性の発泡材。
  14. 前記第１の面に剥離性で生吸収性の接着剤をさらに備え、当該剥離性で生吸収性の接着剤が液状および低温融解性ポリエチレングリコールの混合物である請求項１０記載の弾力性で生吸収性の発泡材。
  15. 前記弾力性で生吸収性の発泡材が有効量の医薬活性化合物を含有する請求項１４記載の弾力性で生吸収性の発泡材。
  16. 前記第１の面に剥離性で生吸収性の接着剤をさらに備え、当該剥離性で生吸収性の接着剤が糖類および可塑剤である請求項１０記載の弾力性で生吸収性の発泡材。
  17. 前記弾力性で生吸収性の発泡材が有効量の医薬活性化合物を含有する請求項１６記載の弾力性で生吸収性の発泡材。
  18. 前記第１の面に剥離性で生吸収性の接着剤をさらに備え、当該剥離性で生吸収性の接着剤がタンパク質および可塑剤である請求項１０記載の弾力性で生吸収性の発泡材。
  19. 前記弾力性で生吸収性の発泡材が有効量の医薬活性化合物を含有する請求項１８記載の弾力性で生吸収性の発泡材。
  20. 前記第１の面に剥離性で生体適合性の接着剤をさらに備え、当該剥離性の接着剤が粘着性の微粒子分散液である請求項１０記載の弾力性で生吸収性の発泡材。
  21. 前記弾力性で生吸収性の発泡材が有効量の医薬活性化合物を含有する請求項２０記載の弾力性で生吸収性の発泡材。
  22. 組織の近接用装置であって、２つの離間したラインに設けられた複数の外科用ステープルを含有するとともに開口部をもった上面を有するステープルカートリッジを備え、前記ステープルは前記開口部を通して射出され、前記上面には弾力性で生吸収性の発泡材が剥離可能に付着され、当該発泡材が実質的に封止された少なくとも１つの面を有しており、前記装置は前記発泡材の第１の面に剥離性で生吸収性の接着剤をさらに備え、当該剥離性で生吸収性の接着剤が液状および低温融解性ポリエチレングリコールの混合物である、装置。
  23. 前記発泡材が連続気泡発泡材である請求項２２記載の装置。
