Description
本発明は、心臓線維芽細胞の増殖を抑制する方法、および心線維症を抑制する方法に関するものであり、有効成分として、カルボスチリル誘導体を使用する方法である。
発明の背景
I.カルボスチリル類
下記の一般式(1)で示されるカルボスチリル誘導体:
[式中、Rは、フェニル環に置換基として低級アルコキシ基を有していてもよいベンゾイル基であり、カルボスチリル骨格の3位および4位の炭素-炭素結合は単結合または二重結合である。]
、およびその塩は、当分野では良く知られている(米国特許第4,415,572号、これは、その全体が引用により、ここに引用される)。
これらのカルボスチリル類は、モデル系において、心拍数や心筋の酸素消費にほとんど影響を及ぼすことなく、心筋収縮を増強する経口変力剤として見いだされ(Feldman等、N.Engl.J.Med.,329:149-155(1993))、うっ血性心不全の患者の治療に有効である(米国特許第4,415,572号;およびHori等、Jpn.Circ.J.,50:659-666(1986))。
いくつかの研究によって、上記カルボスチリル類が、うっ血性心不全の患者の血行動態指数や運動能力を改善することが実証されている(Inoue等、Heart Vessels,2:166-171(1986);Sasayama等、Heart Vessels,2:23-28(1986);およびFeldman等、Am.Heart.J.,116:771-777(1988))。さらに、日本と米国の双方で実施されたマルチセンター・ランダム化プラシーボ統制試験は、これらのカルボスチリル類が、うっ血性心不全患者の生活の質を改善し、死の危険を減らすことを実証した(OPC-8212マルチセンター・リサーチ・グループ、Cardiovasc.Drugs Ther.,4:419-425(1990);Feldman等、Am.J.Cardiol.,68:1203-1210(1991);およびFeldman等、N.Engl.J.Med.,329:149-155(1993))。
これらのカルボスチリル類の、変力特性に関する作用メカニズムは、カリウム電流の減少(Iijima等、J.Pharmacol.Exp.Ther.,240:657-662(1987))、ホスホジエステラーゼの穏やかな阻害、および内向きのカルシウム電流の増加(Yutani等、J.Cardiovasc.Pharmacol.,13:812-819(1989);およびTaira等、Arzneimittelforschung,34:347-335(1984))を含む。しかし、死亡率の低下に最も有効なカルボスチリル類の用量(一日あたり、60mg)は、血行動態効果を全くあるいはほとんど示さず、該薬物が、陽性変力作用よりむしろ別のメカニズムを介して死亡率を減少することが示唆される(Feldman等、N.Engl.J.Med.,329:149-155(1993);およびPacker、N.Engl.J.Med.,329:201-202(1993))。
上記カルボスチリル類は、リポポリサッカライドに刺激された末梢血単核細胞(PBMC)によって産生されるTNF−α及びIL−6(Maruyama等、Biochem.Biophys.Res.Commu.,195:1264-1271(1993);およびMatsumori等、Circul.,89:955-958(1994))、およびIL−1を含む種々のサイトカイン類の産生を用量依存的に阻害すること、それによって心臓移植において、移植片の拒絶が抑制されることが知られている(米国特許第5,521,185号)。
さらに、それらは、CFU-Cの減少と関連する可逆的好中球減少症を誘発できる(Feldman等、Am.Heart J.,116:771-777(1988);OPC-8212マルチセンター・リサーチ・グループ、Cardiovasc.Drags,Ther.,4:419-425(1990);Feldman等、Am.J.Cardiol.,68:1203-1210;およびFeldman等、N.Engl.J.Med.,329:149-155(1993))。
加えて、上記カルボスチリル類は、アポトーシス(プログラムされた細胞死)の制御、並びにガン治療、腫瘍転移の抑制、及びRNAウイルス複製の阻害に有効であることが見いだされている(1993年4月30日に出願された米国特許出願第07/989,028号,これは欧州特許公開第0552373号に相当する。双方とも、引用により、全てがここに引用される;Nakai等、Jpn.J.Cancer Res.,Abstract、Proc.Jpn.Cancer Assoc.,第581頁(1993)、およびMaruyama等、Biochem.Biophys.Res.Comm.,195:1264-1271(1993))。
上記カルボスチリル類は、DNAウイルス複製の阻害に有効であり、抗RNAウイルス化合物と共に用いられると、RNAウイルス複製の阻害に相乗効果をもたらす(米国特許第5,504,093号)。
さらに、上記カルボスチリル類は、ほ乳類の細胞において、ヌクレオシドやヌクレオベース、例えばアデノシンの輸送を用量依存的に阻害するのに有効であり、またヌクレオシドアナログ、特にAZTのリン酸化の増強に有効であることが見いだされた(米国特許第5,504,093号)。別のヌクレオシド輸送阻害剤であるジピリダモールは、高濃度(治療的範囲外)でのみ(10-100μM)、アデノシンの輸送を阻害する(Scholtissek等、Biochem.Biophys.Acta,158:435-447(1968);およびPlagemann等、J.Membr.Biol.,81:255-262(1984))。
ジピリダモールは、細胞内へのアデノシンの輸送を阻害することにより、局所的にアデノシン濃度を増加させることが提案されている(Plagemann等、Biochem.Biophys.Acta,947:405-443(1988))。アデノシンは、アデニル酸シクラーゼの活性化、又はホスホジエステラーゼの阻害のいずれかを介して、心筋細胞内のcAMPの増加(Fox等、Ann.Rev.Biochem.,47:655-686(1978);およびTakeya等、Drug Res.,34:364-370(1984))、細胞表面膜上の特異的なアデノシン受容体へのその結合を介して、冠状動脈の拡張(Fox等、Ann.Rev.Biochem.,47:655-686(1978))、脳血流量の増加(Heistad等、Am.J.Physiol.,240:775-780(1981))、TNF−α産生の減少(Parmely等、J.Immunol.,151:389-396(1993))、および血小板凝集の減少(Dawicki等、Biochem.Pharmacol.,34:3965-3972(1985))を誘導することが知られている。
カルボスチリル類によって生じるアデノシン輸送の阻害は、それらの作用の別の新規な側面との結びつきを示すものと考えられる。すなわち、上記カルボスチリル類はアデノシン輸送の阻害によりアデノシンの血中濃度を上昇し、これにより、うっ血性心疾患(Feldman等、N.Engl.J.Med.,329:149-155(1993);およびPacker、Nngl.J.Med.,329:201-202(1993))、もしくはTNF−α産生の減少(Maruyama等、Biochem.Biophys.Res.Comm.,195:1264-1271(1993);およびMatumori等、Circul.,89:955-958(1994))における、ベスナリノンの治療上の利点がいくつか説明されるかもしれない。
カルボスチリル類は、運動誘発性虚血を、心拍数や収縮期血圧を変化させることなく改善し、虚血の進行を妨げることが見いだされている(Kinosita等、Respir.Circ.,36:1199-1203(1988))。心筋虚血の程度は血流量に依存するので、冠血流量の増加は心筋虚血の軽減を意味するかもしれないが、梗塞のサイズは、冠血管拡張能によって決められない。これは、心筋虚血の間中、冠状動脈が完全に閉塞されるためである。心筋梗塞の進行は、ATPの消耗速度、及び虚血間の側副血流量の程度、また血小板や好中球の活性化、Ca2+やカテコールアミンの過負荷、及び酸素由来のフリーラジカルの発生に起因している。従って、冠血管拡張による心筋虚血の軽減は、必ずしも梗塞サイズの限定を意味するものではない。
カルボスチリル類は、また冠状動脈疾患をもつ患者の、運動中のST機能低下を改善することが示されている(Kinosita等、上記文献)。心電図のST−Tレベルの変化は、Ca2、K+、H+、及びNa+の細胞内および外のバランス、心臓の回旋運動、心室壁運動、及び虚血の存在によるものである(Nobel等、Cardiovasc.Res.,12:13-17(1978))。従って、その中には、該薬物が心筋虚血を改善しうるという示唆はない。たとえ、虚血領域の心筋の機能が、該薬物によって生じる冠血管拡張により改善されたとしても、虚血の軽減は梗塞サイズの縮小を示すわけではない。これは心筋壊死が、複数の冠血流量-非依存性病因によるためである。
上記カルボスチリル類は、最近、虚血性心疾患に苦しむ被験者の、梗塞サイズの縮小に有効であることが見いだされた(1995年12月12日に出願された米国特許出願第08/570,767号)。
II.線維症
上記のように、カルボスチリル類はアポトーシスを制御する。その制御は、薬物誘発肝炎やウイルス性肝炎の患者に見いだされ、これにより肝炎における治療効果が現れ、肝細胞の線維化が予防される(1993年4月30日に出願された米国特許出願第07/989,028、これは欧州特許公開第0552373号に対応する)。しかし、このような二次的な線維化の抑制、すなわち実質細胞の破壊による線維化の抑制は、多くの心疾患に対して実行可能な治療法ではない。そのため、線維芽細胞に直接効果のある方法が捜し求められている。
種々の心疾患において、線維症と心不全の過程との関連を示す証拠が存在しており、その中には容積と圧力の過負荷の両者に関連するものが含まれている(Maron等、Am.J.Cardiol.,35:725-739(1975);Schwarz等、Am.J.Cardiol.,42:661-669;Fuster等、Circ.,55:504-508(1976)、Bartosova等、J.Physiol.,200:285-295(1969);Weber等、Circ.,83:1849-1865(1991);Schaper等、Basic Res.Cardiol.,87:S1303-S1309(1992);Boluyt等、Circ.Res.,75:2203-32(1994);およびBishop等、J.Mol.Cell.Cardiol.,22:1157-1165(1990))。心不全の場合、線維症は、線維芽細胞数と基質沈着の両者の増加を伴い(Morkin等、Am.J.Physiol.,215:1409-1413(1968);Skosey等、Circ.Res.,31:145-157(1972);およびBooz等、Cardiovasc.Res.,30:537-543(1995))、この状態の進行において線維芽細胞の重要性が示唆される。
心臓の線維芽細胞は、心臓の拡張機能に対する作用、及び間接的に収縮期の機能不全を誘発または潜在化する心筋細胞に対する作用によって心不全を生じる間質タンパク質(insterstitial proteins)、及び他の心筋成分の主要な合成源である(Hess等、Circ.,63:360-371(1981);Villari等、Am.J.Cardiol.,69:927-934(1992);Villari等、JACC,22:1477-1484(1993);Brilla等、Circ.Res.,69:107-115(1991);およびSabbah等、Mol.&Cell Biochem.,147:29-34(1995))。
間質のコラーゲン合成は、過負荷状態でおこり得る線維芽細胞の伸張や変形によって、刺激されると考えられている(Desrosiers等、Annales de Chirurgie,49:768-774(1995);Sadoshima等、J.Bio.Chem.,267:10551-10560(1992);Carver等、Circ.Res.,69:116-122(1991);およびButt等、Ann.N.Y.Acard.Sci.,752:387-393(1995))。線維芽細胞に関しては、未だイオンチャンネル活性における伸張媒介変化は実証されていないが、この現象は他の細胞では認められている(Ruknudin等、Am.J.Physiol.,264:H960-H972(1993))。本発明は、ベスナリノンのイオンチャンネル活性が、心疾患の線維芽細胞の活性に対する作用を調節すると仮定した。
本発明において、予想外にもカルボスチリル類が、心臓線維芽細胞の増殖阻害に有用であり、また心線維症の抑制に有用であることが発見された。
ベスナリノンの肝硬変の予防効果という最終結果に類似するものの、心臓の線維芽細胞に対するベスナリノンの効果は、次の3つの理由で従来技術から予測されなかった。
(1)2つの反応の基礎を成すメカニズムは全く別物であり、関連性がないと思われる。肝炎は、肝細胞の壊死に対する二次的反応として線維症を導く。すなわち、ベスナリノンは、炎症性(及び線維症性)肝細胞壊死の代わりに、非炎症性肝細胞アポトーシスを引き起こすことで、この状況において線維症を抑制するようである。本発明において、ベスナリノンの有利な作用は、直接線維芽細胞に及び、実質細胞には及ばないことである。
(2)肝細胞と心臓線維芽細胞に対する有効投与量は有意に異なる。ほとんど予期せぬことに、心臓線維芽細胞に対する有効濃度は、肝細胞に対するものより2.5-25倍低いことが、本発明において見いだされた(すなわち、心臓線維芽細胞には4.0-40ng/mLであるのに対し、肝細胞では1.0μm/mL以上が必要である)。
(3)肝細胞の反応はベスナリノン濃度に正比例するが、心臓線維芽細胞は全く逆の様式で反応する。すなわち、その反応は用量に逆比例の関係にあり、事実、心蔵線維芽細胞は、肝細胞に影響を及ぼす高い投与量に対して非感受性である。
発明の要約

【図面の簡単な説明】
図1は、正常動物(●)から単離した心臓線維芽細胞、及び慢性大動脈弁閉鎖不全症の動物(▽)から単離した心臓線維芽細胞のベスナリノン処理用量反応曲線を示す。図1において、各点は任意のある濃度における全実験の平均であるが、全ての濃度が、同時にまたは同じ時間で試験されたわけではない。データは、ベスナリノン(スルホレン中)で処理した培地と、ベスナリノンで処理してないが、スルホレンを含む対照培地(matching culture)の全細胞数を比べ、比率として表す。非パラメータデータに対するマン-ホイットニー・ログ・ランクト・テスト(Mann-Whitney log ranked test)を用いた統計的分析を生データについて行ったので、単に明確にする目的で、3次回帰線を付した。

発明の詳細な説明
ここで用いられる″増殖″とは、複製および生存を意味する。従って、一般式(1)で示されるカルボスチリル誘導体は、本発明において、心臓の線維芽細胞の複製および生存の抑制に有効であることが見いだされた。
一般式(1)において、フェニル環に低級アルコキシ基および置換基を有していてもよいベンゾイル基には、1〜3個の直鎖状または分岐状のC1-6のアルコキシ基で置換されたフェニル環を有するベンゾイル基、例えばベンゾイル、2-メトキシベンゾイル、3-メトキシベンゾイル、4-メトキシベンゾイル、2-エトキシベンゾイル、3-エトキシベンゾイル、4-エトキシベンゾイル、4-イソブトキシベンゾイル、4-ヘキシルオキシベンゾイル、3,4-ジメトキシベンゾイル、3,4-ジエトキシベンゾイル、3,4,5-トリメトキシベンゾイル、2,5-ジメトキシベンゾイル、等が含まれる。
本発明の有効成分化合物(1)の中で、3,4-ジヒドロ-6-[4-(3,4-ジメトキシベンゾイル)-1-ピペラジニル]-2(1H)-キノリノン、即ち、ベスナリノンが最も好まい。
上記カルボスチリル類は、慣用されている酸と容易に塩を形成する。そのような酸として、例えば硫酸、硝酸、塩酸および臭化水素酸のような無機酸、;例えば酢酸、p-トルエンスルホン酸、エタンスルホン酸、シュウ酸、マレイン酸、フマル酸、クエン酸、コハク酸、安息香酸のような有機酸が挙げることができる。これらの塩は、本発明において、一般式(1)の遊離化合物と同じように、有効成分として使用され得る。
心臓線維芽細胞の増殖阻害方法において、有効量は、処理前の細胞の生物学的状態、すなわち、正常か異常かにより異なる。一般的には、心臓線維芽細胞と接触して、その増殖を阻害する式(1)の化合物の量は、約10-8〜10-7Mである。
一般式(1)の化合物およびその塩は、一般に、それ自体、慣用されている薬学的製剤に製剤化されうる。そのような製剤は、慣用の充填剤、増量剤、結合剤、湿潤剤、崩壊剤、界面活性剤、滑沢剤等の希釈剤または賦形剤を用いて調製される。これらの薬学的製剤は、治療の目的に応じて選択される種々の投与形態を有することができ、その典型的な例としては、錠剤、丸剤、粉末剤、液剤、懸濁剤、エマルジョン、顆粒剤、カプセル剤、坐剤、注射剤(溶液、懸濁液など)、および眼科用液剤である。
錠剤の製造には、この分野で周知の担体を広く用いることができる。例えばラクトース、シュクロース、塩化ナトリウム、グルコース、尿素、デンプン、炭酸カルシウム、カオリン、結晶セルロースおよびケイ酸といった溶剤または賦形剤;水、エタノール、プロパノール、単シロップ、グルコース溶液、デンプン溶液、ゼラチン溶液、カルボキシメチルセルロース、シェラック、メチルセルロース、リン酸カリウムおよびポリビニルピロリドンといった結合剤;乾燥デンプン、アルギン酸ナトリウム、粉末寒天、粉末ラミナラン、炭酸水素ナトリウム、炭酸カルシウム、ポリオキシエチレンソルビタン脂肪酸エステル、ラウリル硫酸ナトリウム、ステアリン酸モノグリセリド、デンプンおよびラクトースといった崩壊剤;シュクロース、ステアリン、カカオバターおよび水素添加油といった崩壊阻害剤;第4級アンモニウム塩およびラウリル硫酸ナトリウムといった吸収促進剤;グリセロールおよびデンプンといった湿潤剤または保湿剤;デンプン、ラクトース、カオリン、ベントナイトおよびコロイドシリカといった吸着剤;精製タルク、ステアリン酸塩、粉末ホウ酸およびポリエチレングリコールといった滑沢剤を用いることができる。必要があれば、錠剤は、さらに慣用のコーティングを施してもよく、例えば糖衣錠、ゼラチンコートされた錠、腸溶錠、フィルムコート錠、二重コートまたは多層錠を挙げることができる。
丸剤の製造には、当分野で周知の担体が広く使用され得る。例としては、グルコース、ラクトース、デンプン、カカオバター、硬化植物油、カオリンおよびタルクといった溶剤および賦形剤;粉末アラビアガム、粉末トラガントガム、ゼラチンおよびエタノールといった結合剤;ラミナラン(laminaran)および寒天といった崩壊剤である。
坐剤の製造には、周知の担体が広く使用され得る。例としては、ポリエチレングリコール、カカオバター、高級アルコール類、高級アルコールエステル類、ゼラチンおよび半合成グリセリド類を挙げることができる。
注射剤の調製に、溶液または懸濁液は好ましくは滅菌され、好ましくは血液と等張であり、そのような投与形態に調製するために、当分野で慣用されているあらゆる希釈剤が使用され得る。従って、例えば、水、エチルアルコール、プロピレングリコール、エトキシ化イソステアリルアルコール、ポリオキシル化メソスチアリルアルコール及びポリオキシエチレンソルビタン脂肪酸エステルを挙げることができる。この場合、薬学的製剤は、塩化ナトリウム、グルコースまたはグリセロールを、等張液を与えるために十分な量で、含むことができる。慣用の溶解補助剤、緩衝液、無痛化剤、又は局所麻酔剤等を添加することができる。
さらに、必要であれば、薬学的製剤は着色料、保存剤、香料、矯味剤、甘味剤など、並びに他の薬物を含むことができる。
本発明で用いられる薬学的製剤中の有効成分化合物の割合は重要ではなく、広範囲から適切に選択することができる。しかし、一般に、その割合は、約1.0-70重量%、好ましくは約1.0-30重量%の範囲から推奨的に選択される。
本発明の薬学的製剤の投与経路は重要でなく、投与形態、患者の年齢、性別およびその他の要因、並びに治療される心線維症の重篤度に応じて選択される。従って、例えば、錠剤、丸剤、液剤、懸濁剤、エマルジョン、顆粒剤またはカプセル剤である場合は、その製剤は経口的に投与される。注射可能な溶液は、単独で、またはグルコースやアミノ酸等を含む非経口的注入に慣用される液体と混合して、経静脈的に投与される。必要であれば、これらの溶液は筋肉内、皮内、皮下または腹腔内経路により同じように投与されても良い。坐剤は直腸に投与され、眼用溶液は点眼ローションである。
前述の薬学的製剤の投与量は投与法、患者の年齢、性別およびその他の背景要因、疾患の重篤度、並びに治療される疾患に依存するが、一般には、経口的に約5-60mg/日で投与するか、または血漿レベルで同等の静脈投与が推奨される。
本発明における心線維症は、心臓線維芽細胞の増殖、または心臓線維芽細胞による細胞外の基質タンパク質合成の活性化と関係する種々の疾患に帰することができる。これらの疾患は、本発明で有効に治療され得る。そのような疾患は大動脈および僧帽弁の弁膜性逆流を含む。加えて、多くの心疾患と関係付けられ、しばしば筋細胞および線維芽細胞の成分を含む心肥大が、本発明において効果的に治療される。

上記有効成分、アビセル、コーンスターチ、及びステアリン酸マグネシウムを混合し、共にすりつぶし、得られた混合物を糖衣錠用R10mmパンチ(dragee R10 punch)で圧縮成形する。このようにして得られた錠剤は、ヒドロキシプロピルメチルセルロース、ポリエチレングリコール6000、ひまし油およびメタノールから成るフィルムコーティング組成物でコートし、フィルムコート錠とする。

必要な場合には、アルコールを添加して粉末をペースト様の塊にする。次にコーンスターチを加え、顆粒が形成されるまで混合を続ける。その混合物をNo.10ふるいに通し、トレイの中に置いて、オーブンの中で、100℃を12-14時間維持して乾燥する。乾燥した顆粒をNo.16ふるいに通し、次に乾燥ラウリル硫酸ナトリウムと乾燥ステアリン酸マグネシウムを添加し、混合した後、その混合物を、錠剤機を使って、所望の大きさ、及び形状に圧縮する。
上記の核を、水分の吸収を防ぐため、ニスで処理し、タルクを振りかけ、次にアンダーコート層を付与する。ニスのコーティングは、内服用に十分な回数繰り返す。
さらに、アンダーコート層と平滑なコーティングを適用することにより、錠剤を完全に丸く滑らかにする。着色コーティングは所望の色が得られるまで行われる。乾燥後、コーティングされた錠剤はつや出しされ、均一な光沢の錠剤を得る。
以下の実施例は、説明のみを目的として記載するものであり、本発明の範囲の制限を何ら意図するものではない。
実施例
心臓疾患のモデル系としての組織培養は、良く確立されており(Douglas等、Research Animals and Concepts of Applicability to Clinical Medicine,編者.Gartner等、S.Karger、Basel,第196-204頁(1982);Zoller等、J.Molec.Cell.Cardiol.,26:627-638(1994);およびNi等、Cell.Biol.Toxicol.,8:1-ll(1992))、また組織培養細胞(特に線維芽細胞)は薬理研究において、毒性、用量反応曲線、および作用機作の確立に使われている。ウサギもまた心臓血管疾患のためのモデル系として、よく確立されている(Gardner等、Experimental Surgery and Physiology:Induced Animal Models of Human Disease,編者.Swindle等、Williams及びWilkins、Baltimore、第74-113頁(1988);Fizelova等、Res.Adv.Stud.Cardiol.Struc.Metab.,1:200-210(1972);Young等、Am.J.Noninvasive Cardiol.,4:145-138(1990);およびMagid等、Am.J.Physiol.,263:H226-233(1992))。
A.細胞の単離
前部で7つの細胞単離を行い、6つの心臓繊維芽細胞の初代細胞株を樹立した。これらの株のうち2つは正常のニュージーランド白色ウサギ(平均27週齢)から得、1つは疑似手術を行ったウサギ(225週齢;201週後に手術)から得、3つは外科的に大動脈逆流(AR)を誘導したウサギ(平均110週齢;93週後に手術)から得た。
Mitra等、Am.J.Physiol.,249:H1057-H1060(1985)とTytgat、Cardiovas.Res.,28:238-283(1994)によって示された方法と類似したコラゲナーゼ灌流法(collgenase perfusion procedure)を用いて細胞を単離し、5.0%(v/v)ウシ胎児血清、5.0%(v/v)Nu-血清(Collaborative Biomedical Products、Becton Dickinson、Bedfold、MA)に加えて、抗生物質/抗菌物質(100ユニット/mLペニシリンG、100ユニット/mLストレプトマイシンおよび0.25ユニット/mLアンフォテリシンB(GIBCO BRL,Gaithersburg,MD))を含む最小必須培地からなる完全培地で、8継代まで増殖させた。
細胞は、Eghbali等、J.Mol.Cell Cardiol.,20:267-276(1988)に示されたように、ビメンチン、デスミン、平滑筋α-アクチン、および平滑筋α-ミオシンに対する抗体を用いた間接免疫蛍光顕微鏡法により、免疫学的に線維芽細胞として特徴づけられた。
B.用量反応曲線
後の全ての実験で用いるベスナリノンの有効濃度を決定するため、用量-反応曲線を構築した。より明確には、最初に1.0×10-4〜1.0×10-6Mの濃度のベスナリノンを評価した。その後、用量の範囲を1.0×10-9Mベスナリノン低濃度を含むように下方域に下げた。
正常心臓(n=2の心臓)から培養された線維芽細胞を用いた12実験、及びAR心臓(n=2の心臓)から培養された線維芽細胞を用いた7実験において、1.0×105細胞の全てがT25培養フラスコに置かれ、上記の完全培地中で37℃で終夜培養された。次に、ベスナリノンを様々の濃度で添加して、評価した。これらの全ての実験において、スルホランがベスナリノンを溶解するための溶剤として用いられた。そのため、培養液の全てが一定のスルホラン濃度(0.33%(v/v))となるように調整され、またスルホランは単独で全実験のネガティブコントロールとして用いられた。培養細胞は37℃で2度目の終夜培養をした後、トリプシン処理により回収した。全細胞数はトリパンブルーを用い、カウンティングして決定した。その結果を図1に示す。
図1に示したように、1.0-5.0×10-7Mのベスナリノン濃度では、正常の線維芽細胞の細胞数は、スルホラン単独コントロールより約55%少なかった(p=0.03)。より高い濃度では、細胞数は徐々に増加し、1.0×10-5Mのべスナリノン濃度で最大となり、わずかにコントロールよりも上回った。比較的低濃度のベスナリノンを添加した場合、AR培養細胞において、比較できる程、統計的に有意(p<0.02)な細胞数の減少が認められた。しかし、この効果は1.0×10-8Mのベスナリノンで認められ、正常細胞に対する最低有効濃度より1オーダー低い濃度であった。また、ARの研究では、細胞数は、調べたいずれの薬物濃度でも、スルホラン単独コントロールの細胞数に達することも、又それを越えることもなかった。
これらの結果は、正常心臓とAR心臓に由来する線維芽細胞による異なる反応に起因すると考えられる。低濃度のベスナリノンで処理した培養細胞と、全くベスナリノンで処理しなかった培養細胞との相違は、正常及びAR研究の双方で統計的に有意であり、用量-反応の関係は明らかであった。有意な用量反応関係の存在は、その薬物の効果が、単に偶然から生じたものではないことを示している。重要なのは、一日当たり60mgを経口的に接取したヒトのベスナリノンの血漿濃度はおよそ1.7×10-5Mであり、これは循環血漿濃度と組織液濃度におそらく違いがあることを説明している。このように、この結果は、線維芽細胞の生存能力が、治療上の経口用量で心筋中に存在する濃度に匹敵するベスナリノン濃度によって影響を受け得ることを示している。
C.長期の暴露
次に、3動物(2つは正常、1つはAR)から単離された線維芽細胞を使って実験を行った。この実験では、スルホラン溶解ベスナリノン1.0×10-6M含有、又は非含有のいずれかの完全培地で、全部で1.0×105細胞を含む培養を平行して、37℃で14日まで行った。ベスナリノンとコントロールの培養細胞を、毎日トリプシン処理により回収し、培養細胞当たりの生細胞数を求めた。その結果を図2に示す。
図2に示すように、細胞数の減少を引き起こす濃度より高い濃度のベスナリノンに長期間曝すと、心臓線維芽細胞の生存能力が、経時的に影響を受けた。観察期間中(14日まで)、正常細胞の培養では、10-6Mのベスナリノン処理培地中の細胞数は、スルホラン単独に曝した場合と比べて減少していた。この実験では、6日までは、ベスナリノン含有または非含有の培養液は、類似の細胞数を含んでいた。それ以降、ベスナリノンで処理した培養液中の細胞数は、非処理のコントロールに比べて減少した。同様の結果が、AR細胞を使った実験でも観察された。この結果は、全てのケースで、3日又は4日毎に培地を交換したので、毒性の分解産物の蓄積によるものとはいえない。
D.細胞の形態
細胞の形態を位相差顕微鏡を用いて調べた。観察は用量反応と長期暴露の研究の双方における細胞について行った。
形態に、特記すべき影響はいずれの濃度でも認められなかった。
E.アポトーシス効果
いくつかのプロセスによって、ナノモーラー濃度のベスナリノンで短時間処理した培養液中、またはマイクロモーラー濃度で長期間処理した培養液中に認められる細胞数の減少の理由が説明できるかもしれない。当該知見に対する1つの可能な説明は、ベスナリノンが心臓線維芽細胞に有毒であることである。この説明が正しければ、ベスナリノンは完全に細胞を殺すか、又はアポトーシス(プログラムされた細胞死)を誘導するはずである。細胞数は、ベスナリノンに対して、より多くの用量でより多くが生存し、少ない用量でアポトーシスの形跡が認められるという2様式の反応を示すので、この効果はより多い用量で消失し、直接の毒性は用量に応答しないであろうと推論される。
アポトーシスは、1.0×106以上の細胞を含むコンフルエント培養物中で調べられた。この培養物を、5.0×10-5Mのベスナリノンで終夜処理し、トリプシン処理により回収し、ホルマリンで固定した後、商業的に入手可能なin situターミナル・デオキシヌクレオチジル・トランスフェラーゼ・ラベリング・キット(Oncor、Apoptag-Plus)を使って、型通りのDNA断片化(アポトーシスの証拠)を証明するため染色した。このアッセイで、生存細胞は赤く、アポトーシスの進行している細胞は黄緑色に見える。
正常線維芽細胞に対する最初の実験(3実験)により、ベスナリノンで処理した培養物は、アポトーシス細胞の数が未処理の細胞の約2倍であることが明らかとなった(6.1% 対 3.4%;p<0.1;n=3)。
F.要約
これらの結果は、ベスナリノンは、ヒトにおいて一般的に理解されている治療上の経口投与量で達成される濃度相当量、並びに他の者によって示された、培養中の肺癌細胞の増殖に影響を与える濃度よりも低い濃度で、正常心臓線維芽細胞の生存能力を弱めることを示している。これらの結果は、この効果は、ARの過酷な容量負荷に長期間曝された心臓から単離された線維芽細胞において、より顕著であることを示している。この実験は、この現象はより高い用量を長期間用いたときに観察され得ることも示している。ベスナリノンの線維芽細胞に及ぼす効果は、少なくとも一部は、アポトーシスの誘導によって媒介されていると考えられる。
線維芽細胞の生存能力は、比較的高濃度のベスナリノン(≧10-6M)に急激には影響されないことを示す結果は、培養において心臓以外の様々な線維芽細胞で認められる結果と一致する(Yoshinaka等、Exp.Cell Res.,219:21-28(1995))。しかし、これらの研究成果とは対照的に、心臓線維芽細胞のベスナリノンに誘導された形態変化は、位相差顕微鏡を用いて認められなかった。この顕微鏡では、細胞性ストレスファイバーへの変化が検出できる。
本発明は特定の実施態様を参照して詳細に述べてきたが、種々の変更および修飾が本発明の精神および範囲から逸脱することなく為され得ることは、当業者に明らかである。
Claims (5)
- 上記カルボスチリルが、3,4-ジヒドロ-6-[4-(3,4-ジメトキシベンゾイル)-1-ピペラジニル]-2(1H)-キノリノン、またはその薬学的に許容されるその塩である請求項1に記載の心臓線維芽細胞の増殖阻害剤。
- 上記カルボスチリルが、3,4-ジヒドロ-6-[4-(3,4-ジメトキシベンゾイル)-1-ピペラジニル]-2(1H)-キノリノン、またはその薬学的に許容される塩である請求項3に記載の心線維症治療剤。
- 上記心線維症が、大動脈弁逆流および僧帽弁逆流から成る群から選択される心疾患に帰因するものである、請求項3または4に記載する心線維症治療剤。
