Description
象牙質の過敏症は、過敏性の歯が温度および/または圧力の変化あるいは化学的な作用にさらされた時に生じる一時的に誘発される痛みの感覚である。歯の象牙質が摩擦や剥離にさらされた場合はいつでも、あるいは歯の細かい根の表面が歯周病により露出した場合に、過敏症が起こる。象牙質は歯にある骨様の物質で、通常は、ゴム質の上にあるエナメル質およびゴム質の下にあるセメント質により覆われている。エナメル質またはセメント質はう蝕、傷、疾病または他の原因により失われ、そのことにより象牙質が口中の外的環境にさらされる。一般的には、象牙質は、小管と呼ばれる導管を有し、それは象牙質外部と神経が存在する歯の内部との間の物質およびエネルギーの輸送を可能にしている。象牙質の過敏症に関する1の理論は、流体力学理論と呼ばれるものであり、これらの小管の外部刺激への曝露が神経の刺激を引き起こし、過敏性の不快感を生じさせうることを示している。流体力学理論は、過敏症は歯中の神経を刺激に対して敏感でなくすことにより、あるいは小管を遮断または閉塞させて神経が外部刺激にさらされるのを防止または制限することにより治療できることを示唆している。
本発明は、象牙質小管中における鉱物質の形成を誘導することができ、さらに、外的刺激に対する神経の過敏性を減じるための脱感作剤のごとき治療薬剤を輸送することができるリポソームを含む、象牙質の過敏症を治療するための経口的に許容される処方に関する。また本発明は、ある種のリポソームを含む組成物を用いる、象牙質の過敏症の治療方法も包含する。
本発明は、口腔の健康用途に適した担体ならびに象牙質の小管における鉱物質形成を誘導することができ、そのことにより小管の閉塞を達成する有効量のある種のリポソームを含む、過敏性の歯を治療するための組成物を提供する。リポソームはよく知られた物質であり、それらの調製は種々の両親媒性分子から行われる。リポソームは健康分野の種々の用途に、主に、種々の活性物質のビヒクルとして用いられているが、それらは歯用調合物には広く用いられていない。
脱感作剤としてのリポソームの脱感作特性を試験するために、米国特許第5589159号に記載された方法を用いてこれらの物質を含むいくつかの経口組成物を試験した。この方法において、冶金されたノコギリを用いて、う蝕および修復のないインタクトなヒト臼歯を歯の長軸に対して垂直に切片化して、厚さ約0.4ないし0.6mmの薄い切片または円板を得る。象牙質を有し、エナメル質のない切片を試験用に選択し、ついで、エチレンジアミン四酢酸溶液でエッチングして汚れた層を除去する。各円板をJ. Dent. Reaearch, 57:187 (1978)に記載されたスプリット−チャンバードデバイス(split-chambered device)中に固定する。該デバイスは、組織培養液の入った加圧液体リザーバーに接続される特別に気密性が高いチャンバーである。加圧窒素および二酸化炭素ガスを用いることにより、液を生理学的pHにすることができる。さらに正確さを期すために、円板をヒト唾液で湿らせて、口腔内条件とほぼ同じにする。該装置は、定規または他の測定装置上に固定されたガス毛細管を有している。気泡をガラス毛細管中に注入する。時間の関数としての気泡の置換を測定することにより、象牙質を通しての液体の流れを測定することができる。
本発明のリポソーム組成物のリポソームの鉱物質誘導能を評価するもう1つの方法は、リポソームの誘導による、刺激された細胞外液体(SBF,生理学的に血漿を代表するイオン組成を有する溶液)からのカルシウム枯渇をモニターすることである。SBF溶液は室温においていくつかの固体無機相に転移可能である。実験を行っている間に、添加リポソームを含むSBF試料は沈殿を形成させるが、リポソーム不含の対照試料は全く透明である。2つの40mLのSBF試料に0.5mLのアニオン性リポソーム0.15%溶液を添加した。処理から48時間後に、リポソームを添加した試料は沈殿を生じ、沈殿は上清と分離した。未処理SBF溶液および上清をカルシウムに関して分析した。最初のSBF溶液および上清は、それぞれ65ppmおよび44ppmのカルシウムを含んでいた。0.50x10−8モルのDOPAが溶液中に存在した場合、[2 DOPA:1 カルシウム]複合体が形成されたならば、SBFの遊離カルシウムは約2.5x10−3ppmだけ枯渇したであろう。実際のカルシウム枯渇は21ppmであった。このレベルは、有意な鉱物質形成が起こったことを示す。なぜなら表面だけを想定した場合よりも減価償却が有意に高いからである。
許容された調剤法に従って下記成分を混合することにより本発明の脱感作歯磨きを製造してもよい:

Claims (14)
- 適当な担体および有効量のアニオン性ジオレオイルホスファチジン酸(DOPA)リポソームを含む象牙質の過敏症を治療するための歯用組成物であって、該アニオン性ジオレオイルホスファチジン酸(DOPA)リポソームが象牙質小管中の鉱物質形成を誘導し、そのことにより小管を閉塞しうる点でユニークなものであり、該リポソームが象牙質小管よりも大きくない直径を有する結果、該小管中に浸透し、保持されるものである組成物。
- 該リポソームが2ミクロンよりも大きくない直径を有するものである請求項1記載の歯用組成物。
- 該リポソームがジオレオイルホスファチジン酸(DOPA)のカリウム塩を含むものである請求項1記載の歯用組成物。
- 該リポソームが0.1ないし20重量パーセントで存在している請求項1記載の歯用組成物。
- 該リポソームがさらに少なくとも1種の他の象牙質過敏症治療用薬剤を提供するものである請求項1記載の歯用組成物。
- 該他の薬剤が神経脱感作剤である請求項5記載の歯用組成物。
- 該神経脱感作剤が、少なくとも1種のカリウム塩、カプサイシン、オイゲノールおよびそれらの混合物からなる群より選択されるものである請求項6記載の歯用組成物。
- 該少なくとも1種のカリウム塩が、重炭酸カリウム、クエン酸カリウム、塩化カリウムおよび硝酸カリウムからなる群より選択されるものである請求項7記載の歯用組成物。
- 該神経脱感作剤のほかに口腔ケア処方において有用な少なくとも1種のさらなる治療薬剤を含む請求項6記載の歯用組成物。
- 生理学的に許容されるフッ化物イオン源をさらに含む請求項1記載の歯用組成物。
- 該生理学的に許容されるフッ化物イオン源が、フッ化第1スズ、フッ化ナトリウム、フッ化カリウムおよびそれらの混合物からなる群より選択されるものである請求項10記載の歯用組成物。
- さらに神経脱感作剤を含む請求項10記載の歯用組成物。
- 該神経脱感作剤が、少なくとも1種のカリウム塩、カプサイシン、オイゲノールおよびそれらの混合物からなる群より選択されるものである請求項12記載の歯用組成物。
- 該神経脱感作剤のほかに口腔ケア処方において有用な少なくとも1種のさらなる治療薬剤を含む請求項12記載の歯用組成物。
