Description
A型肝炎は、ポリオウィルスとの密接な関連性を有する微小なピコナウィルスに感染することによって引き起こされる急性疾患である。感染は、糞便/経口経路によって蔓延するので、これは衛生状態や衛生設備の水準が低い地域の風土病である。開発途上国への旅行者がA型肝炎に罹患する危険率は、腸チフスやコレラの罹患率と比べて遥かに大きい(それぞれの40倍、800倍)。
ウィルス自体は、直接的に細胞変性を起こさない。A型肝炎ウィルス(HAV)感染に起因する肝臓の損傷は、宿主の細胞毒性T−リンパ球によるウィルス感染細胞の破壊によって起こる。HAVの血細型はただ一つだけ存在し、感染は長期免疫を引き起こすが、これらの特徴は、A型肝炎の予防法を開発するに当たってのカギとなる。防御は、A型肝炎ウィルスが肝細胞内へ入り込むのを阻止する中和抗体によって達成される。精製されたヒト血清γ−グロブリンを使用する受動免疫は疾病に対して短期間の防御をもたらし、最近までは、これがA型肝炎を阻止する唯一の手段であった。
近年、HAVワクチンが開発されたが、不活化ワクチン及び弱毒化生ワクチンに開発が集中していた。両タイプのワクチンとも、組織培養細胞中で増殖させたHAVから調製される。HAVの複製は遅く、且つ、そのウィルスの大部分は細胞に結合したままであることから、ウィルスの収量は低く、商業的にあまり魅力のないものである。ウィルス収量が低いという問題は、タンパク質の大量生産が可能な遺伝子組み換え技術を使うことによって克服できるかも知れない。しかしながら、既知の中和エピトープは立体配置依存性を有しているので、HAVタンパクが正確に処理されて折り畳まれることを確実なものとすることが重要である。適切な免疫応答を引き出せる遺伝子組み替えHAV抗原を得ることは極めて難しいことが判明した。
アメリカン バイオジェネティック サイエンスズによって開発されたHAVカプシド調製物を非経口的に注射した時に、ある遺伝子組み替えHAV抗原によってHAVに対する防御をうまく誘導できることが判明した。アメリカン バイオジェネティック サイエンスズは、ワクシニアウィルス及びバキュロウィルス形質発現ベクターを用いることによって、真核生物細胞中で中身の無い空のHAVカプシドを生産することに成功した。遺伝子組み替えカプシドはモノクローナル抗体によって認識され、チンパンジーに非経口的に注射した時にはHAVに対する防御を誘導し、従来の手段によって得られるものと比べてかなり大量に生産される。このHAVカプシドは国際特許出願WO−A−9301279に開示されており、そこでの開示内容は、参考のためにここに収録されている。
多くのワクチン接種療法に伴う欠点は、ワクチン組成物をしばしば注射によって投与する必要があることであり、このことは多くの人々にとって潜在的な抑止効果を有する要因となり、とりわけ、療法の過程を完全なものとするために再度の又はブースターの注射が要求される場合に問題となる。この問題を克服する一方法として、ワクチン組成物を口腔粘膜又は鼻腔粘膜に投与する方法を挙げられるかも知れない。しかしながら、若干の他の抗原を用いて経口又は経鼻腔による免疫感作を調べてみたものの、血清抗体の誘導について、注射の免疫感作に比べてほんのわずかの効果しか通常は認められなかった。例えば、エム.エイチ.ショーグレンらによる論文、ワクチン第10巻補遺第1号第S135〜S137頁、1992年(M.H. Sjogren et al, Vaccine, Vol. 10, Suppl. 1, S135-S137, 1992)には、経口又は筋注経路によるA型肝炎弱毒化生ワクチンの投与が記載されている。筋注投与が良好な血清抗体応答を引き出したのに対して、経口投与に対する抗体応答は、いかなる投与量においても観察されなかった。
市販で入手可能な経粘膜ワクチンがほんのわずかしかないと言う事実は、そのようなワクチンを開発するのに伴う問題が存在することを示している。非生存可溶性抗原、特にオボアルブミン(OVA)やキーホール リンペット ヘモシアニン(KLH)のように免疫学者が伝統的に使用しているものは、経粘膜免疫原性に乏しい。そのような抗原は、全ての応答を誘導するのに大用量を必要とする。しかしながら大用量は、その後に引き続いて行われる抗原への非経口的暴露に対して、個体の免疫寛容を引き起こす可能性もある。これは、経口寛容として知られている状態である。コレラ毒素(CT)、大腸菌熱不安定性毒素(LT)、或いはこれらの毒素中の非毒性の結合部分(CT−B及びLT−B)のようないくつかの微生物構成成分は強力な分泌性及び循環性抗体を引き出す強力な経粘膜抗原であることが見い出だされたが、そのような分子が良好な経粘膜抗原であることの理由は、充分に解明されていない。重要であるかも知れない特性の一つは、これらの分子が一定の表面受容体を介して粘膜上皮細胞へ結合する能力である。しかしながら、抗原が有する真核生物細胞への結合能とその経粘膜免疫原性の間の関連性は必ずしもあるわけではないことが、他者の研究において判った。この例では、どのようにHAVが体内に入り込むのか、分子レベル又は細胞レベルでは判らず、また、特定の受容体が関係しているのか否かも判らない。
それゆえに、我々の知る限りでは、与えられた抗原が良好な経粘膜免疫原性を有しているか否かをある程度の確信を持って予測する方法は、現在のところ存在しない。
上述した空の遺伝子組み替えHAVカプシドが、経粘膜投与とりわけ鼻腔内投与された時に、血清抗HAV抗体を誘導するのに有能なことが、ここで判明した。そして、HAVカプシド調製物を鼻腔内投与し、続いて1回目及び2回目のブースターを投与した時に、大部分の動物において抗HAVへのセロコンバージョンが観察され、これは、皮下注経路によるAlumアジュバント存在下での抗原投与に匹敵した。
従って、第1点目の発明は、患者の粘膜表面に投与してA型肝炎に対する血清イムノグロブリンG抗体の生産を誘導する経粘膜ワクチン組成物を製造するための、A型肝炎ウィルスカプシド又はその経粘膜免疫原性フラグメント又は経粘膜免疫原性エピトープの使用を提供する。
第2点目の発明は、粘膜表目へ適用するためのワクチン組成物であって、A型肝炎ウィルスカプシド又はその経粘膜免疫原性フラグメント又は経粘膜免疫原性エピトープ、及び薬剤学的に許容される担体から成ることを特徴とするワクチン組成物を提供する。
また、さらに発明は、哺乳動物(例えばヒト)のような宿主中でA型肝炎ウィルスに対する血清イムノグロブリンG抗体の生産を誘導する方法であって、有効量のA型肝炎ウィルスカプシド又はその経粘膜免疫原性フラグメント又は経粘膜免疫原性エピトープを宿主の粘膜表面へ直接投与することを特徴とする誘導方法を提供する。
本発明の経粘膜供給組成物は、例えば、口腔粘膜、胃腸粘膜、及び呼吸器(例えば、鼻腔や気管支)粘膜等の1又はそれ以上への供給用に処方することができる。
組成物を呼吸器(例えば、鼻腔や気管支)粘膜へ供給しようとする場合には、典型的には、エアゾル剤又は点鼻剤として投与される水溶液、又は吸入剤用の乾燥粉末に処方される。
点鼻剤として投与される組成物は、そのような組成物中に通常含まれているタイプの賦形剤を1又はそれ以上含有していてもよく、例えば、そのような賦形剤には、保存剤、粘度調整剤、張度調整剤、緩衝剤などがある。本発明のワクチン組成物は、胃腸管の粘膜表面へ抗原を供給させる組成物の形態をとっていてもよい。そのような組成物は、胃液による抗原の衰退を阻止する手段を用いることによって、例えば、カプセルに入っているワクチン組成物を防御性基質や公知のタイプの被覆物の内部に入れることによって提供できる。
患者へのA型肝炎ウィルスカプシド投与量は、典型的には、免疫応答を引き出すのに必要な濃度とした時に患者に毒性を示さないような量が選ばれる。例えば、投与されるカプシドの濃度は、宿主体重1kg当たり1.0mg〜100mgの範囲内にあってもよい。
本発明を、以下の実施例に記載され且つ添付図面に示されている特定の実施態様を参考にして、さらに詳しく説明する。実施例は単に本発明を説明するためだけのものであり、いかなる点においても発明の範囲を限定する意図を持ってはいない。
【図面の簡単な説明】
図1は、鼻腔内、経口、及び皮下注の各経路によってHAVカプシドを3回投与した後の個体別及び平均の抗HAV血清力価を示したものである。
遺伝子組み替えHAVカプシドのサンプルは、アメリカン バイオジェネティック サイエンスズ、レイナーズ ジャーム フリー ビルディング、P.O.ボックス 1001 ノートル デイム、インディアナ 46556、USA(American Biogenetic Sciences, Reyniers Germ Free Building, P.O. Box 1001 Notre Dame, Indianna 46556, USA)から入手した。サンプルは、ワクシニア−HAV感染ベロ細胞(Vero cell)から調製されたものである。細胞はNP40を用いて溶解され、トリクロロトリフルオロエタンを用いて抽出された。水相を濃縮し、次いでバイオゲル A−15(Biogel A-15)カラムを用いて等電点クロマトグラフを行った。空のカプシドを含有するフラクションを集め、濃縮した。ホルマリンを加えて生き残っているワクシニアウィルスを全て不活化した。以下に示したサンプル中HAVカプシド含有量は、ELISA単位(EU)で表わした。EU値は、スミスクライン ビーチャム(Smith Kline Beecham)から入手したA型肝炎ウィルスのサンプルに基づいて規格化した。受領したサンプルは、1μl当たり52ELISA単位(EU)のHAVカプシドを含有していた。サンプルの蛋白含有量は30mg/mlであり、その中の100ng/mlがHAV抗原であろうと概算された。
上記したタイプの遺伝子組み替えHAVカプシドは、国際特許出願WO−A−9301279(PCT/US92/05714)で述べられている方法に従って調製することができる。
マウスを、異なる投与量のHAVを用いる経口投与及び鼻腔内投与(I/N)を行って免疫感作した。いくつかのマウス群に与えた材料中には、少量(1μgm)のコレラ毒素(CT)を含有させ、アジュバントとして作用させた。CTは最も強力な経粘膜アジュバントとして知られていることから、使用に供された。別のマウスの一群は、アジュバントである水酸化アルミニウムに吸着させたHAVを用いる注射を行って免疫感作し、これをポジティブコントロールとした。各群は、下記の表1に述べた通りである。

免疫感作は、下記の表2に述べた方法によって行った。
抗HAV応答は、以下に示した捕獲ELISA技法(Capture ELISA technique)を利用して分析した。既知のA型肝炎患者から採ったヒト回復期ポリクローナル血清を、96穴プラスチックプレートに塗布した。ポリクローナル血清はHAVカプシドを捕獲し、これをプレートに結合させる。それからマウス血清を、プレートに結合したHAVカプシドと共にインキュベートし、HAVに対するマウス血清の反応性を標識化抗マウス抗体を使用して測定した。分析のためのプロトコール(手順)は次の通りである。
プロトコール(別段の記載がない限り、全ての容量は50μL/穴)
1) PBSで1:25000に希釈したヒト捕獲抗体を、コスター(Costar)EIAプレート(Cat no:3590)に塗布し、4℃でオーバーナイト。
2) プレートを、リン酸緩衝溶液/トゥイーン(0.05%)(PBST)で3回洗浄。
3) PBST中1%BSA(Sigma、 Cat no:A7888)溶液(200μl/穴)、1時間、37℃でブロック。
4) プレートを、PBSTで3回洗浄。
5) プレートを、PBST(0.1%BSA)中にHAVを含有するサンプル(0.1EU/μl)で被覆し、2〜3時間、37℃。
6) プレートを、PBSTで3回洗浄。
7) プレートを、PBSTで希釈したマウス血清と共に2〜3時間、37℃でインキュベート。
8) プレートを、PBSTで3回洗浄。
9) PBSTで1:1000に希釈した抗マウスIgG(ヤギ、Sigma、Cat no:B7022)と共に、1〜2時間、37℃でインキュベート。
10) プレートを、PBSTで3回洗浄。
11) PBSTで1:1000に希釈したストレプタビジン−パーオキシダーゼ(Streptavidin-peroxidase)(Dako、Cat no:P397)と共に、1〜2時間、37℃でインキュベート。
12) プレートを、PBSTで3回洗浄。
13) 基質として、リン酸−クエン酸緩衝液に溶解したOPD(Sigma、Cat no:P8287)を加え、30分間、37℃でインキュベート。
14) 基質反応の停止(3M H2SO4後、発色を判断した。
ヒト血清とHAVカプシドサンプルは、ABSから供給された。
捕獲抗体とHAVカプシドサンプルの最適な状態を選び、S/N比を最小限にした。
ヒト血清捕獲抗体に対するマウス血清の反応性に起因すると考えられる高いバックグラウンド・ノイズと言う問題に直面した。ABSが推奨する希釈率以上に捕獲抗体を希釈することによって、このバックグラウンドを減少することができた。
カプシドの捕獲に関しては、1:1000〜1:25000に希釈した捕獲抗体を用いると、同じ結果が得られた。
血清IgG抗HAV応答は、下記の表3に示した通りである。
表からわかるように、1回の投与後には、250EUの皮下注によって、及び500EUの鼻腔内投与によって免疫感作されたマウスの血清中に、抗HAV抗体が認められた。他のいかなる群にも応答は認められなかった。ブースティングは、皮下注群及び鼻腔内投与500群において応答を大きく高め、約10500及び約9500の力価をそれぞれにもたらした。極めて類似した応答が250EUプラスCTのI/N投与を受けたマウスにおいて見られ、低い値ではあるが測定可能な応答が250EUのI/N投与を2回受けたマウスのうちの半数の血清に認められた。この時点においては、経口的に免疫感作されたどのマウスにも血清応答は全く認められなかった。さらにブースティングすると、250EU、500EU、及び、250EU+CTの経口免疫感作マウス群のいくらかに、セロコンバージョンが引き起こされた。応答は後者になるほど強かった。3回投与の後、全てのI/N免疫感作マウスはセロコンバージョンし、応答の大きさは、CT無しにおいて投与量依存性であった。500EU、250EU+CTによるI/N免疫感作は、alumに吸着させたHAVの250EUを用いたS/C免疫感作による血清力価と比べてわずかに低いが、それに匹敵するほどの高い血清力価を引き起こした。
図1は、HAVカプシドを3回投与した後の個体別及び平均の抗HAV血清力価を示したものである。見て判るように、CTは、I/N及び経口投与による抗原での血清抗HAV応答を非常に増大させた。250EU+CTのI/N投与を受けたマウスでの力価は、250EU単独のI/N免疫感作によって誘導されるそれと比べて、各時点において10倍よりも大きいと評価された。250EUのCT併用とCT無しで経口的に免疫感作されたマウスにおいて、抗HAV力価について類似の差があった。CTは、セロコンバージョンしたマウスの数も増加させた。さらに、I/Nの250EU+CT群における各免疫感作後の力価は、alumに吸着させた250EUを皮下投与によって与えたマウスのそれらと極めて類似していた。
下記の表2は、第1回目及び第2回目のブースター投与後にセロコンバージョンしたマウスの数を群ごとに示したものである。セロコンバージョンは投与量及び投与経路への依存性を有していた。皮下注による免疫感作は、250EUの2回投与で全てのマウスをセロコンバージョンさせたのに対して、同様の投与でも、抗原を鼻腔内投与したマウスでは半数しかセロコンバージョンさせず、抗原を経口投与したマウスでは全くセロコンバージョンさせなかった。第2回目のブースト後には、2回投与後に全く応答を示さなかった125EU投与群を含むI/N群の全マウスがセロコンバージョンした。また、経口的に免疫感作されたマウスは、たとえCTを付加しても全てのマウスにセロコンバージョンを起こせなかったが、第2回目のブースト後に応答を開始した。
表4は、第1回目及び第2回目のブースター投与後にセロコンバージョンしたマウスの数を群ごとに示したものである。セロコンバージョンは投与量及び投与経路への依存性を有していた。皮下注による免疫感作は、250EUの2回投与で全てのマウスをセロコンバージョンさせたのに対して、同様の投与でも、鼻腔内処置したマウスでは半数しかセロコンバージョンさせず、経口処置したマウスでは全くセロコンバージョンさせなかった。第2回目のブースト後には、2回投与後に全く応答を示さなかった125EU投与群を含む鼻腔内投与群の全マウスがセロコンバージョンした。また、経口的に免疫感作されたマウスは、たとえCTを付加しても全てのマウスにセロコンバージョンを起こせなかったが、第2回目のブースト後に応答を開始した。
上述の実施例は、説明の手段として供されたに過ぎず、出願の範囲を限定する意図はなく、出願はここに添付したクレームによってのみ限定される。
Claims (5)
- 経粘膜ワクチン組成物が、A型肝炎ウィルス及び百日咳毒素のBサブユニットを含有する鼻腔組成物ではないことを条件とし、A型肝炎ウィルスカプシド又はその経粘膜免疫原性フラグメント又はその経粘膜免疫原性エピトープを、A型肝炎ウィルスに対する血清イムノグロブリンG抗体の生成を誘発する経粘膜ワクチン組成物として、ヒトを除く哺乳動物に使用する方法。
- A型肝炎ウィルスカプシドが遺伝子組み換え体である、クレーム1に記載の方法。
- 経粘膜ワクチン組成物が鼻粘膜又は気管支粘膜への供給用にまとめられている、クレーム1又は2に記載の方法。
- 前記組成物がエアゾル剤又は点鼻剤として投与される水溶液、又は吸入剤用の乾燥粉末に処方されている、クレーム4に記載の方法。
- 前記経粘膜ワクチン組成物がA型肝炎ウィルスカプシド又はその経粘膜免疫原性フラグメント又はその経粘膜免疫原性エピトープの経口供給用又は経胃腸供給用に構成されている、クレーム1又は2に記載の方法。
