【発明の属する技術分野】
本発明はセメント非使用の人工関節ステム用金属ジャケットに関し、特に、骨内に摺動挿入されてこの骨に固定される人工関節のステムを覆うように構成されたセメント非使用の人工関節ステム用金属ジャケットに関するものである。また、本発明は上記のように構成されたセメント非使用の人工関節ステム用金属ジャケットがステムに覆われた人工関節に関するものである。

【従来の技術】
一般的に、例えば、人工股関節は寛骨臼部分（ａｃｅｔａｂｕｌｕｍの部分）と大腿骨部分（ｆｅｍｕｒの部分）とに区分されており、寛骨臼及び大腿骨部分はそれぞれ金属、セラミック又はプラスチック材になっている。

人体の大腿骨は骨幹端部の軟らかい海綿骨及び骨幹部の固い皮質骨から形成されている。従って、人工関節を人体内に挿入しようとする場合、その隣接部位にある大腿骨の骨髄を掘り出してそこに人工関節のステム（ｓｔｅｍ）を挿入した後、セメント（ｃｅｍｅｎｔ）で固着させたり、表面に多孔質、又は表面層が形成されたステムを挿入して骨が成長しながら粗い表面層に生理学的に噛み合うことができるようにするセメント非使用方法を使用している。

上記のような方法のうち、セメントを使用する人工関節に関する技術は、本発明の発明者の大韓民国特許公告公報第８５−１８１４号（発明の名称：捩じれ防止の人工股関節）に記載されている。

図１を参照して、その記述内容を簡単に説明すると次の通りである。

人工関節１は上部から頭部２、頸部３、カラー部４及びステム５が一体に形成されている。ここで、カラー部４の先端部は大腿骨の皮質骨の内側上端に密着するように湾曲されており、ステム５はその上部断面が円形に近い楕円形になっており、下部に近づくにつれて円形になるようになっている。このようにステム５を形成することは、関節の垂直上部から加えられる圧縮力と、頭部２から関節のステム５に垂直方向に加えられる横力によって生ずる回転を防止するためである。

そして、このようなステム５の上部外側面には、手術後、任意の方向に回転トルクが発生しても関節が大腿骨内で回転することを防止できるように適当な厚さの羽根７が長手方向に延びており、このような羽根７の中央には固定孔８が形成されている。

また、羽根７の内側には多数の鉄線溝がステム５の長手方向に適当な間隔をおいてそれぞれ形成されており、このような鉄線溝には鎖型鉄線９が挟み込まれて巻かれている。このように鎖型鉄線９が巻かれた部分を含んだステム５の表面にはセメント６が適当な厚さでコーティング処理されている。このようにセメント６を予めコーティング処理することは、手術時に使用されるセメントとの接合を容易にし、セメントの使用を減らして養生時に発生する熱量を減らすためである。

このように構成された人工関節を人体の腸骨内に挿入するためには、まず、人工関節１のステム５が挿入され得るように骨体腔にリーミングが行われ、ここにセメントを適当量注入する。そうした後、セメントが注入された部位にステム５を挿入すると、ステム５の表面に予めコーティングされたセメント６の層に接合されてステム５が大腿骨の内部に一体に堅固に固着される。

しかし、上述の人工関節は大腿骨と骨体腔とがセメントによって結合されるため、その結合が脆弱になるという短所がある。

このような問題点を解決するために、大腿骨とステムとを結合すると、骨がステムの多孔の又は粗い表面内で成長し、又は、接触成長しながらステムに生理学的に噛み合うことができるように構成した。

即ち、ステムの表面に多孔質（ｐｏｒｏｕｓ）を形成したり、微細な石で打撃（グリットブラスト（ｇｒｉｔ ｂｌａｓｔ））して粗い表面層を形成し、このように表面が形成されたステムを先に説明したように骨体腔に挿入して施術した。そうすると、骨が漸進的に成長し、その骨が多孔質、又は粗い表面層に生理学的に結合されてステムが大腿骨に固着される。

上記のように、生理学的な方法でステムを大腿骨に固着するセメント非使用の人工関節は、セメント使用の人工関節に比べて生理学的に活動的な、若い患者のために強い界面を確保できるという長所がある。しかし、このようなセメント非使用の人工関節もこれを使用しているうちに、骨体腔の間隔が数ミリメートルも大きくなることによって関節ステムの骨体接触成長過程が中断されるという恐れが生じる、即ち、不適切なリーミングが生じる恐れがある。この不適切なリーミングによって人工関節のステムが骨から分離される危険も生じ得る。

ステムの頭部に作用する垂直力が骨の界面に剪断力として作用して骨に形成される繊維性組織膜（ｆｉｂｒｏｕｓ ｔｉｓｓｕｅ ｍｅｍｂｒａｎｅ） と骨との間に有効ジョイント空間（ｅｆｆｅｃｔｉｖｅ ｊｏｉｎｔ ｓｐａｃｅ） を形成する。そうすると、プラスチックカップから発生した磨耗粒子などが有効ジョイント空間に侵入して骨の融解（ｏｓｔｅｏｌｙｓｉｓ）を促進させる短所がある。

また、ステムの全表面を多孔質にする場合には、垂直力の大部分が下方から伝達され、大腿骨の上方では応力がかからない応力遮断現象（ｓｔｒｅｓｓ ｓｈｉｅｌｄｉｎｇ）により大腿骨の上方部分が骨再吸収を行う。そして、ステムの上方部分だけを多孔質化する場合には下方部分が互いに結合されなくなり、微細運動を起こして痛みを誘発する問題点が発生する。

なお、セメント非使用の人工関節に異常が発生して再手術（ｒｅｖｉｓｉｏｎ ｓｕｒｇｅｒｙ） をする場合には、ステムと大腿骨とを結合することが、骨が金属ステムの多孔質表面への内成長による生理学的な結合であるため、ステムの除去時に非常に難しくなる。

【発明が解決しようとする課題】
本発明は先に説明したような従来の技術の問題点を解決するために案出されたものであって、その課題は、大腿骨の骨体腔内に摺動挿入されて人工関節を大腿骨に固定するための人工関節のステムの表面を覆って、人工関節の寿命を短くする剪断応力を大きく減少させ、且つ応力遮断現象を大幅に緩和して大腿骨とステムとの間に間隙が形成されるのを抑制し、磨耗粒子が間隙に侵入して骨を融解するのを最小限に押さえるようにしたセメント非使用の人工関節ステム用金属ジャケットを提供することにある。

また、本発明の他の目的は、上記のように構成されたセメント非使用の人工関節ステム用ジャケットがステムに覆われた人工関節を提供することである。

【課題を解決するための手段】
上記の目的を達成するために、本発明に基づくセメント非使用の人工関節ステム用金属ジャケットは、人体の骨体腔に形成された孔内に長手方向に挿入された後、セメント非使用の人工関節のステムの少なくとも一部分を覆うことができるように形成されており、金属ジャケットは一端が開口していてステムを受け入れるバッグ形状をなし、ステムの下端部とバッグの下端部との間に所定の空間が形成されていて、金属ジャケットの外表面に骨が接触成長しながら容易に噛み合うことができるように表面処理された金属層と、金属層の内面に耐摩耗性に優れた合成樹脂層とを有し、金属層はジグザグ状に又は蛇行して巻かれている金属針金から形成されている。

また、本発明は、金属針金から形成された前記金属層に代えて、数多くの小さい孔を形成した金属層から構成される。

また、本発明は、金属針金から形成された前記金属層に代えて、多くの皺が形成された金属層から構成される。

また、上記のような目的を達成するため、本発明に基づくセメント非使用の人工関節は、一体の頭部、頸部及びステムを有し、このステムが、人体の骨体腔の一部分をリーミングしてこの中に形成された孔内に挿入されるものであって、
このステムの少なくとも一部分を覆うように構成されるとともに、骨が接触成長しながら骨との間に機械的に噛み合う粗い金属外表面を有する金属層と、この金属層の内面に形成された耐摩耗性に優れた合成樹脂層とを有する金属ジャケットが設けられ、ステムが骨体腔に摺動挿入されたときにステムの下端部と金属ジャケットの下端部との間に空間を残して該ステムを骨体腔の内部に固定し、金属層がジグザグ状に又は蛇行して巻かれた金属針金から形成される。

【発明の実施の形態】
以下、本発明に基づくセメント非使用の人工関節ステム用金属ジャケット及びこれを備えた人工関節の望ましい実施形態を、添付した図面を参照して詳細に説明する。

図２は本発明の一実施形態によるセメント非使用の人工関節ステム用ジャケットの構成を説明するための斜視図であり、図３は図２に示されたセメント非使用の人工関節ステム用ジャケットのＡ−Ａ線矢視断面図である。そして、図４は図２に示されたジャケットが人工関節ステムに挿入されている状態を示した側面図であり、図５は図４に示された人工関節を人体の骨体腔に挿入した状態を示した縦断面図である。

図２及び図３に示すように、本発明のセメント非使用の人工関節ステム用金属ジャケット１０は、これに人工関節ステムを挿入する場合、ステムを囲んでいる形状、即ち、バッグ形状に形成されている。そして、ステムをその外周面が金属ジャケット１０の内周面にしっくりと嵌合して金属ジャケット１０内に収容するために、金属ジャケット１０を、これにステムが整合して滑り込むように形成する。

また、金属ジャケット１０はステムを挿入する場合、ステムの下端部が金属ジャケット１０の下端部に触れないようにステムの長手方向の長さより少し長く形成され、ステムの下端部と金属ジャケット１０の下端部との間に所定の空間が形成されるように構成されている。このように所定の空間を形成するように構成することは、ステムが下方へ挿入される時、金属ジャケット１０が破損されることを事前に予防するためである。

そして、金属ジャケット１０は、ステムと当接する内部に、ステムが上下に摺動する事によって生ずる摩耗を最小限に押さえるように超高密度ポリエチレン（ｕｌｔｒａｈｉｇｈ ｍｏｌｅｃｕｌａｒ ｗｅｉｇｈｔ ｐｏｌｙｅｔｈｙｌｅｎｅ） 、又はこれに類似する合成樹脂層１１が形成される。このような金属層は表面がグリットブラスト加工されるかプラズマ溶射されるかして粗くされるか、機械的に粗くされた（ｇｒｉｔ ｂｌａｓｔｅｄ， ｐｌａｓｍａ ｓｐｒａｙｅｄ ｏｒ ｍｅｃｈａｎｉｃａｌｌｙ ｒｏｕｇｈｅｎｅｄ）チタニウム、又はこれに類似した合金製の針金をジグザグに、又は蛇行するように巻いて形成したものである。このように、金属針金１２をジクザクに、又は蛇行させて巻くことによって金属ジャケットの表面を形成することは、頭部からの垂直力を骨体腔に圧縮応力で転移させ、骨の接触成長と共に界面の噛み合いが確保され得るように金属ジャケット１０の表面を粗くし、表面積を大きくするためであり、且つ、フープ応力が大腿部にもっとよく転移され得るようにするためである。

金属層は針金の代わりに、金属バッグの外表面上に数多くの小さい孔を形成するか、数多くの皺を形成して金属バックの外表面を粗くすることもできる。このような孔又は皺を形成する理由は、金属バッグの粗い外表面を放射状に膨らまして、大腿部へのフープ応力転移を改善するためである。

以下に、先に説明したように構成された本発明の金属ジャケット１０を使用して人工関節を人体に挿入する工程を説明する。

図４に示すように、まず、本発明の金属ジャケット１０に、ステム２１の表面にセメント層がコーティングされていない従来の人工関節２０のステム２１を挿入する。そうすると、ステム２１の下端部と金属ジャケット１０の下端部との間に所定の空間を残して、ステム２１が金属ジャケット１０内に嵌装される。

その後、このようにステム２１上に金属ジャケット１０が嵌装された人工関節２０を人体の骨体腔内に挿入するが、そうするためには図５に示されたように、まず、人工関節２０のステム２１が挿入された金属ジャケット１０が挿入され得るように骨３１に骨体腔３２をリーミングし、そこに金属ジャケット１０が挿入された人工関節２０を挿入する。そうすると、骨３１が針金１２に接触成長して針金１２を巻いて形成した金属ジャケット１０が骨３１と生理学的に噛み合う。即ち、ステム２１は金属ジャケット１０に対して下方に摺動していき、最終的に骨３１と金属ジャケット１０とが堅固に結合される。

先に説明した本発明のジャケットはあらゆる種類の人工関節（股関節、膝関節、肩関節など）に使用することができる。

以上に本発明のセメント非使用の人工関節ステム用ジャケット及びこれを備えた人工関節に対する技術思想を添付図面と共に述べたが、これは本発明の最も望ましい実施形態を例示的に説明したものであり、本発明を限定するものではない。また、この技術分野の通常の知識を持つ者であれば何人でも、本発明の技術思想の範疇を離脱しない範囲内で多様な変形及び変更を行うことができることは明白である。

【発明の効果】
上記に詳細に説明したように、本発明のセメント非使用の人工関節ステム用金属ジャケットを備えた人工関節を使用する場合には、骨と金属ジャケットとの間には主に圧縮力が作用し、繊維組織膜（ｆｉｂｒｏｕｓ ｔｉｓｓｕｅ ｍｅｍｂｒａｎｅ） が形成されるのを最大限に抑制して骨を活性化させ（固くし）、骨と金属ジャケットとの界面で骨に有害な剪断応力を大きく減少させ、これを有益な圧縮に変換させる。また、このような圧縮力によって骨とジャケットとによる間隙の生成を抑制し、摩耗粒子の侵入による大腿骨の融解を最小限にするという効果がある。
そして、本発明のセメント非使用の人工関節ステム用金属ジャケットを備えた人工関節を使用する場合には、垂直力が、大抵、骨たが応力（フープ応力（ｈｏｏｐ ｓｔｒｅｓｓ）） に転移されて大腿骨の上端部（ｐｒｏｘｉｍａｌ ｒｅｇｉｏｎ ｏｆ ｂｏｎｅ） でも生理学的に骨の活性化（Ｗｏｌｆｅ’ｓ ｌａｗ） を起こすのに十分な張力を引き起こして応力遮断現象を大幅緩和できるという効果もある。
また、本発明のセメント非使用の人工関節ステム用金属ジャケットは表面に巻かれた針金が正常的に骨と生理学的な噛み合いを確保しながらも、再手術時に金属針金の固有特性によって針金がプラスチックフィルムから容易に分離され、骨から糸を一巻ずつ解くように解かれて骨の損傷を最小限に押さえることができる利点がある。
さらに、本発明のジャケットはその内部に摩耗に強いポリエチレン層を形成することにより、ステムとの相互スライディング運動による摩耗粒子の発生を最小限に押さえることができるという効果もある。
さらにまた、本発明のジャケットは強度に優れており、厚さが均一なチューブ形態の層を確保することにより骨とステムとの直接的な接触を予防し、ステムが鉛直荷重によって下降する時、ジャケットの端部に空間を確保することによりジャケットの破断を防止する効果がある。

Claims (4)

  1. 人体の骨体腔に形成された孔内に長手方向に挿入されるセメント非使用の人工関節のステムの少なくとも一部分を覆うように構成されるとともに、一端が開口していてステムを受け入れるバッグ形状をなし、ステムの下端部とバッグの下端部との間に所定の空間が形成されており、骨が接触成長しながら噛み合うように表面処理された金属層と、この金属層の内面に形成された耐摩耗性に優れた合成樹脂層とを有し、前記金属層はジグザグ状に又は蛇行して巻かれている金属針金から形成されることを特徴とするセメント非使用の人工関節ステム用金属ジャケット。
  2. 人体の骨体腔に形成された孔内に長手方向に挿入されるセメント非使用の人工関節のステムの少なくとも一部分を覆うように構成されるとともに、一端が開口していてステムを受け入れるバッグ形状をなし、ステムの下端部とバッグの下端部との間に所定の空間が形成されており、骨が接触成長しながら噛み合うように表面処理された金属層と、この金属層の内面に形成された耐摩耗性に優れた合成樹脂層とを有し、前記金属層は多数の小さな孔が形成されていることを特徴とするセメント非使用の人工関節ステム用金属ジャケット。
  3. 人体の骨体腔に形成された孔内に長手方向に挿入されるセメント非使用の人工関節のステムの少なくとも一部分を覆うように構成されるとともに、一端が開口していてステムを受け入れるバッグ形状をなし、ステムの下端部とバッグの下端部との間に所定の空間が形成されており、骨が接触成長しながら噛み合うように表面処理された金属層と、この金属層の内面に形成された耐摩耗性に優れた合成樹脂層とを有し、前記金属層の外表面に多数の皺が形成されていることを特徴とするセメント非使用の人工関節ステム用金属ジャケット。
  4. 一体に形成された頭部、頸部及びステムを有し、人体の骨体腔の一部分をリーミングしてこの中に形成された孔内に該ステムを挿入して成る人工関節において、
    前記ステムの少なくとも一部分を覆うように構成されるとともに、骨が接触成長しながら噛み合うように粗く表面処理された金属層とこの金属層の内面に形成された耐摩耗性に優れた合成樹脂層とを有する金属ジャケットを備え、前記ステムを骨体腔に摺動挿入することによって前記ステムの下端部と金属ジャケットの下端部との間に空間を残して前記ステムを骨体腔の内部に固定し、前記金属層はジグザグ状に又は蛇行して巻かれた金属針金から形成されることを特徴とするセメント非使用の人工関節。
