Abstract

(57)【要約】 【課題】 人工関節の寿命に有害な剪断応力を大きく減
少させ、応力遮断現象を大幅に緩和し、骨とステムとの
間に間隙が形成されるのを抑制することにより、摩耗粒
子の浸透による骨の融解を最小限に押さえるセメント非
使用の人工関節ステム用金属ジャケットを提供するこ
と。 【解決手段】 人体の骨体腔に形成された孔内に長手方
向に挿入されるセメント非使用の人工関節のステム２１
の少なくとも一部分を覆うように構成し、骨３１が接触
成長しながら金属ジャケットに噛み合うように表面処理
された金属層又は金属ワイヤを外表面にして成るセメン
ト非使用の人工関節ステム用金属ジャケット１０が提供
される。

Description

【発明の詳細な説明】
【０００１】
【発明の属する技術分野】本発明はセメント非使用の人
工関節ステム用金属ジャケットに関し、特に、骨内に摺
動挿入されてこの骨に固定される人工関節のステムを覆
うように構成されたセメント非使用の人工関節ステム用
金属ジャケットに関するものである。また、本発明は上
記のように構成されたセメント非使用の人工関節ステム
用金属ジャケットがステムに覆われた人工関節に関する
ものである。
【０００２】
【従来の技術】一般的に、例えば、人工股関節は寛骨臼
部分（acetabulumの部分）と大腿骨部分（femurの部
分）とに区分されており、寛骨臼及び大腿骨部分はそれ
ぞれ金属、セラミック又はプラスチック材になってい
る。
【０００３】人体の大腿骨は骨幹端部の軟らかい海綿骨
及び骨幹部の固い皮質骨から形成されている。従って、
人工関節を人体内に挿入しようとする場合、その隣接部
位にある大腿骨の骨髄を掘り出してそこに人工関節のス
テム（stem）を挿入した後、セメント（cement）で固着
させたり、表面に多孔質、又は表面層が形成されたステ
ムを挿入して骨が成長しながら粗い表面層に生理学的に
噛み合うことができるようにするセメント非使用方法を
使用している。
【０００４】上記のような方法のうち、セメントを使用
する人工関節に関する技術は、本発明の発明者の大韓民
国特許公告公報第８５−１８１４号（発明の名称：捩じ
れ防止の人工股関節）に記載されている。
【０００５】図１を参照して、その記述内容を簡単に説
明すると次の通りである。
【０００６】人工関節１は上部から頭部２、頸部３、カ
ラー部４及びステム５が一体に形成されている。ここ
で、カラー部４の先端部は大腿骨の皮質骨の内側上端に
密着するように湾曲されており、ステム５はその上部断
面が円形に近い楕円形になっており、下部に近づくにつ
れて円形になるようになっている。このようにステム５
を形成することは、関節の垂直上部から加えられる圧縮
力と、頭部２から関節のステム５に垂直方向に加えられ
る横力によって生ずる回転を防止するためである。
【０００７】そして、このようなステム５の上部外側面
には、手術後、任意の方向に回転トルクが発生しても関
節が大腿骨内で回転することを防止できるように適当な
厚さの羽根７が長手方向に延びており、このような羽根
７の中央には固定孔８が形成されている。
【０００８】また、羽根７の内側には多数の鉄線溝がス
テム５の長手方向に適当な間隔をおいてそれぞれ形成さ
れており、このような鉄線溝には鎖型鉄線９が挟み込ま
れて巻かれている。このように鎖型鉄線９が巻かれた部
分を含んだステム５の表面にはセメント６が適当な厚さ
でコーティング処理されている。このようにセメント６
を予めコーティング処理することは、手術時に使用され
るセメントとの接合を容易にし、セメントの使用を減ら
して養生時に発生する熱量を減らすためである。
【０００９】このように構成された人工関節を人体の腸
骨内に挿入するためには、まず、人工関節１のステム５
が挿入され得るように骨体腔にリーミングが行われ、こ
こにセメントを適当量注入する。そうした後、セメント
が注入された部位にステム５を挿入すると、ステム５の
表面に予めコーティングされたセメント６の層に接合さ
れてステム５が大腿骨の内部に一体に堅固に固着され
る。
【００１０】しかし、上述の人工関節は大腿骨と骨体腔
とがセメントによって結合されるため、その結合が脆弱
になるという短所がある。
【００１１】このような問題点を解決するために、大腿
骨とステムとを結合すると、骨がステムの多孔の又は粗
い表面内で成長し、又は、接触成長しながらステムに生
理学的に噛み合うことができるように構成した。
【００１２】即ち、ステムの表面に多孔質（porous）を
形成したり、微細な石で打撃（グリットブラスト（grit
blast））して粗い表面層を形成し、このように表面が
形成されたステムを先に説明したように骨体腔に挿入し
て施術した。そうすると、骨が漸進的に成長し、その骨
が多孔質、又は粗い表面層に生理学的に結合されてステ
ムが大腿骨に固着される。
【００１３】上記のように、生理学的な方法でステムを
大腿骨に固着するセメント非使用の人工関節は、セメン
ト使用の人工関節に比べて生理学的に活動的な、若い患
者のために強い界面を確保できるという長所がある。し
かし、このようなセメント非使用の人工関節もこれを使
用しているうちに、骨体腔の間隔が数ミリメートルも大
きくなることによって関節ステムの骨体接触成長過程が
中断されるという恐れが生じる、即ち、不適切なリーミ
ングが生じる恐れがある。この不適切なリーミングによ
って人工関節のステムが骨から分離される危険も生じ得
る。
【００１４】ステムの頭部に作用する垂直力が骨の界面
に剪断力として作用して骨に形成される繊維性組織膜
（fibrous tissue membrane） と骨との間に有効ジョイ
ント空間（effective joint space） を形成する。そう
すると、プラスチックカップから発生した磨耗粒子など
が有効ジョイント空間に侵入して骨の融解（osteolysi
s）を促進させる短所がある。
【００１５】また、ステムの全表面を多孔質にする場合
には、垂直力の大部分が下方から伝達され、大腿骨の上
方では応力がかからない応力遮断現象（stress shieldi
ng）により大腿骨の上方部分が骨再吸収を行う。そし
て、ステムの上方部分だけを多孔質化する場合には下方
部分が互いに結合されなくなり、微細運動を起こして痛
みを誘発する問題点が発生する。
【００１６】なお、セメント非使用の人工関節に異常が
発生して再手術（revision surgery） をする場合に
は、ステムと大腿骨とを結合することが、骨が金属ステ
ムの多孔質表面への内成長による生理学的な結合である
ため、ステムの除去時に非常に難しくなる。
【００１７】
【発明が解決しようとする課題】本発明は先に説明した
ような従来の技術の問題点を解決するために案出された
ものであって、その課題は、大腿骨の骨体腔内に摺動挿
入されて人工関節を大腿骨に固定するための人工関節の
ステムの表面を覆って、人工関節の寿命を短くする剪断
応力を大きく減少させ、且つ応力遮断現象を大幅に緩和
して大腿骨とステムとの間に間隙が形成されるのを抑制
し、磨耗粒子が間隙に侵入して骨を融解するのを最小限
に押さえるようにしたセメント非使用の人工関節ステム
用金属ジャケットを提供することにある。
【００１８】また、本発明の他の目的は、上記のように
構成されたセメント非使用の人工関節ステム用ジャケッ
トがステムに覆われた人工関節を提供することである。
【００１９】
【課題を解決するための手段】上記の目的を達成するた
めに、本発明に基づくセメント非使用の人工関節ステム
用金属ジャケットは、人体の骨体腔に形成された孔内に
長手方向に挿入された後、セメント非使用の人工関節ス
テムの少なくとも一部分を覆うことができるように形成
されており、金属ジャケットの外表面に骨が成長しなが
ら容易に噛み合うことができるように表面が粗い金属層
を有している。
【００２０】そして、本発明によれば、前記金属層はジ
グザグ状に、又は蛇行して巻かれた針金から形成されて
いるのが望ましい。
【００２１】また、本発明によれば、前記金属層は外表
面上に数多くの小さい孔を形成した薄膜金属バッグによ
って形成される。
【００２２】なお、本発明によれば、金属層はその外表
面に多くの皺が形成された薄膜金属バッグで形成しても
よい。
【００２３】さらに、本発明によれば、前記ジャケット
の内面には摩耗性に優れた合成樹脂層が形成されている
ことが望ましい。
【００２４】また、上記のような目的を達成するため、
本発明に基づく人工関節は、一体の頭部、頸部及びステ
ムを有し、このステムに人体の骨頭及び骨体腔の一部分
をリーミングしてこれらに形成された孔内に挿入し、こ
のステムの少なくとも一部分を覆い、骨が接触成長しな
がら骨との間に機械的に噛み合う粗い金属外表面を有す
る金属層が設けられ、ステムが下方に摺動挿入されたと
きにステムの挿入端と金属ジャケットの内端との間に空
間を残して該ステムを内部に固定するセメント非使用の
人工関節ステム用金属ジャケットを備えている。
【００２５】
【発明の実施の形態】以下、本発明に基づくセメント非
使用の人工関節ステム用金属ジャケット及びこれを備え
た人工関節の望ましい実施形態を、添付した図面を参照
して詳細に説明する。
【００２６】図２は本発明の一実施形態によるセメント
非使用の人工関節ステム用ジャケットの構成を説明する
ための斜視図であり、図３は図２に示されたセメント非
使用の人工関節ステム用ジャケットのＡ−Ａ線矢視断面
図である。そして、図４は図２に示されたジャケットが
人工関節ステムに挿入されている状態を示した側面図で
あり、図５は図４に示された人工関節を人体の骨体腔に
挿入した状態を示した縦断面図である。
【００２７】図２及び図３に示すように、本発明のセメ
ント非使用の人工関節ステム用金属ジャケット１０は、
これに人工関節ステムを挿入する場合、ステムを囲んで
いる形状、即ち、バッグ形状に形成されている。そし
て、ステムをその外周面が金属ジャケット１０の内周面
にしっくりと嵌合して金属ジャケット１０内に収容する
ために、金属ジャケット１０を、これにステムが整合し
て滑り込むように形成する。
【００２８】また、金属ジャケット１０はステムを挿入
する場合、ステムの下端部が金属ジャケット１０の下端
部に触れないようにステムの長手方向の長さより少し長
く形成され、ステムの下端部と金属ジャケット１０の下
端部との間に所定の空間が形成されるように構成されて
いる。このように所定の空間を形成するように構成する
ことは、ステムが下方へ挿入される時、金属ジャケット
１０が破損されることを事前に予防するためである。
【００２９】そして、金属ジャケット１０は、ステムと
当接する内部に、ステムが上下に摺動する事によって生
ずる摩耗を最小限に押さえるように超高密度ポリエチレ
ン（ultrahigh molecular weight polyethylene） 、又
はこれに類似する合成樹脂層１１が形成される。このよ
うな金属層は表面がグリットブラスト加工されるかプラ
ズマ溶射されるかして粗くされるか、機械的に粗くされ
た（grit blasted, plasma sprayed or mechanically r
oughened）チタニウム、又はこれに類似した合金製の針
金をジグザグに、又は蛇行するように巻いて形成したも
のである。このように、金属針金１２をジクザクに、又
は蛇行させて巻くことによって金属ジャケットの表面を
形成することは、頭部からの垂直力を骨体腔に圧縮応力
で転移させ、骨の接触成長と共に界面の噛み合いが確保
され得るように金属ジャケット１０の表面を粗くし、表
面積を大きくするためであり、且つ、フープ応力が大腿
部にもっとよく転移され得るようにするためである。
【００３０】金属層は針金の代わりに、金属バッグの外
表面上に数多くの小さい孔を形成するか、数多くの皺を
形成して金属バックの外表面を粗くすることもできる。
このような孔又は皺を形成する理由は、金属バッグの粗
い外表面を放射状に膨らまして、大腿部へのフープ応力
転移を改善するためである。
【００３１】以下に、先に説明したように構成された本
発明の金属ジャケット１０を使用して人工関節を人体に
挿入する工程を説明する。
【００３２】図４に示すように、まず、本発明の金属ジ
ャケット１０に、ステム２１の表面にセメント層がコー
ティングされていない従来の人工関節２０のステム２１
を挿入する。そうすると、ステム２１の下端部と金属ジ
ャケット１０の下端部との間に所定の空間を残して、ス
テム２１が金属ジャケット１０内に嵌装される。
【００３３】その後、このようにステム２１上に金属ジ
ャケット１０が嵌装された人工関節２０を人体の骨体腔
内に挿入するが、そうするためには図５に示されたよう
に、まず、人工関節２０のステム２１が挿入された金属
ジャケット１０が挿入され得るように骨３１に骨体腔３
２をリーミングし、そこに金属ジャケット１０が挿入さ
れた人工関節２０を挿入する。そうすると、骨３１が針
金１２に接触成長して針金１２を巻いて形成した金属ジ
ャケット１０が骨３１と生理学的に噛み合う。即ち、ス
テム２１は金属ジャケット１０に対して下方に摺動して
いき、最終的に骨３１と金属ジャケット１０とが堅固に
結合される。
【００３４】先に説明した本発明のジャケットはあらゆ
る種類の人工関節（股関節、膝関節、肩関節など）に使
用することができる。
【００３５】以上に本発明のセメント非使用の人工関節
ステム用ジャケット及びこれを備えた人工関節に対する
技術思想を添付図面と共に述べたが、これは本発明の最
も望ましい実施形態を例示的に説明したものであり、本
発明を限定するものではない。また、この技術分野の通
常の知識を持つ者であれば何人でも、本発明の技術思想
の範疇を離脱しない範囲内で多様な変形及び変更を行う
ことができることは明白である。
【００３６】
【発明の効果】上記に詳細に説明したように、本発明の
セメント非使用の人工関節ステム用金属ジャケットを備
えた人工関節を使用する場合には、骨と金属ジャケット
との間には主に圧縮力が作用し、繊維組織膜（fibrous
tissue membrane） が形成されるのを最大限に抑制して
骨を活性化させ（固くし）、骨と金属ジャケットとの界
面で骨に有害な剪断応力を大きく減少させ、これを有益
な圧縮に変換させる。また、このような圧縮力によって
骨とジャケットとによる間隙の生成を抑制し、摩耗粒子
の侵入による大腿骨の融解を最小限にするという効果が
ある。そして、本発明のセメント非使用の人工関節ステ
ム用金属ジャケットを備えた人工関節を使用する場合に
は、垂直力が、大抵、骨たが応力（フープ応力（hoopst
ress）） に転移されて大腿骨の上端部（proximal regi
on of bone） でも生理学的に骨の活性化（Wolfe's la
w） を起こすのに十分な張力を引き起こして応力遮断現
象を大幅緩和できるという効果もある。また、本発明の
セメント非使用の人工関節ステム用金属ジャケットは表
面に巻かれた針金が正常的に骨と生理学的な噛み合いを
確保しながらも、再手術時に金属針金の固有特性によっ
て針金がプラスチックフィルムから容易に分離され、骨
から糸を一巻ずつ解くように解かれて骨の損傷を最小限
に押さえることができる利点がある。さらに、本発明の
ジャケットはその内部に摩耗に強いポリエチレン層を形
成することにより、ステムとの相互スライディング運動
による摩耗粒子の発生を最小限に押さえることができる
という効果もある。さらにまた、本発明のジャケットは
強度に優れており、厚さが均一なチューブ形態の層を確
保することにより骨とステムとの直接的な接触を予防
し、ステムが鉛直荷重によって下降する時、ジャケット
の端部に空間を確保することによりジャケットの破断を
防止する効果がある。

    【特許請求の範囲】
  1. 【請求項１】 人体の骨体腔に形成された孔内に長手方
    向に挿入されるセメント非使用の人工関節ステムの少な
    くとも一部分を覆うように構成されており、骨が針金に
    接触成長しながら噛み合うように表面処理された金属層
    を外表面に形成して成ることを特徴とするセメント非使
    用の人工関節ステム用金属ジャケット。
  2. 【請求項２】 前記金属層はジグザグ状に、又は蛇行し
    て巻かれている金属針金から形成されることを特徴とす
    る請求項１に記載のセメント非使用の人工関節ステム用
    金属ジャケット。
  3. 【請求項３】 前記金属層は、多数の小さな孔が形成さ
    れて表面処理された薄膜金属バッグを含むことを特徴と
    する請求項１に記載のセメント非使用の人工関節ステム
    用金属ジャケット。
  4. 【請求項４】 前記金属層は、その外表面上に多数の皺
    が形成されて表面処理された薄膜金属バッグを含むこと
    を特徴とする請求項１に記載のセメント非使用の人工関
    節ステム用金属ジャケット。
  5. 【請求項５】 前記金属ジャケットの内面に摩耗性に優
    れた合成樹脂層が形成されていることを特徴とする請求
    項１乃至４の何れかの１に記載のセメント非使用の人工
    関節ステム用金属ジャケット。
  6. 【請求項６】 一体に形成された頭部、頸部及びステム
    を有し、人体の骨体腔の一部分をリーミングしてこの中
    に形成された孔内に該ステムを挿入して成る人工関節に
    おいて、 前記ステムの少なくとも一部分を覆い、骨が接触成長し
    ながら噛み合う粗い金属外表面に有する金属層が形成さ
    れ、前記ステムを摺動挿入することによって該ステムの
    挿入端と金属ジャケットの内端との間に空間を残して該
    ステムを内部に固定するセメント非使用の人工関節ステ
    ム用金属ジャケットを備えたことを特徴とする人工関
    節。
