Description

【発明の詳細な説明】 〔産業上の利用分野〕 本発明は、背柱の体内固定に用いる肉茎ねじに関し、
特に、小形で、調節可能で隣接背椎に適用できる肉茎ね
じに関する。
〔従来の技術〕
体内背柱固定装置が背柱骨折の治療や背柱変形の修正
に用いられている。一般に、このような装置は、長い軸
を有するねじを含み、そのねじは肉茎に押し込まれ、次
に背柱に大体平行に走る棒または板に締付けられる。ね
じは、背柱に留め具として働き、ねじの軸の棒または板
に対する位置を調節することによつて、背柱（またはそ
の断面）を所望の形状に固定できる。
肉茎ねじを棒に取付ける方法は、いろいろと考えられ
た問題であつたので、この目的を達成する非常に様々な
クランプや腕木がある。しかし、従来の提案は、構成が
複雑で嵩ばりすぎる装置を含んでいたので、棒／ねじ組
立体を隣接背椎に取付けることは、不可能でなければ困
難であつた。さらに、多くの装置の場合、棒は、すべて
のねじを挿入する前に一つ以上のねじに固着されなけれ
ばならない。ある装置では、棒は、ねじを切られてい
て、棒とねじの相対位置を変えるのを困難にしている。
〔発明の解決しようとする課題〕
本発明は、種々の寸法に容易に調節でき、隣接背椎に
使用するのに十分に小形でかつねじを背柱内に置いた後
に棒を受けて調節のできる肉茎ねじを提供することを目
的としている。
〔課題を解決するための手段〕 本発明によれば、肉茎ねじが一端に骨に挿込むための
ねじのついた軸と、他端に棒を受けるように構成された
ヨークまたはトラフを備えた締付け手段とを具備してい
る。ヨークの前端の内部は、挿入された棒を押すように
構成された縁をもつた尖頭の形になつているのが最も好
ましい突起と、ヨークにあつて挿入された棒を尖頭に押
付けて締付ける手段とを形作るように成形されているの
が好ましい。
締付け手段は、ヨーク内に滑り込ませて取付けられ棒
を尖頭または突起に押付けるねじ手段を有するブロツク
またはパネルであるのが好ましい。ねじ手段は、尖頭ま
たは突起の各側に１本づつの２本のねじを備え、どちら
かのねじを進めることによつて棒を傾けることができる
ようにしてあるのが好ましい。
ヨークは、みぞを備えているのが好ましく、ブロツク
は、ヨーク内のみぞとぴつたり合つて、ブロツクがヨー
クの中に滑り込めるようにする見合いのみぞを備えてい
る。ソケツトと見合いの突起がヨークとブロツクとのみ
ぞに設けられて、ブロツクがヨークに挿入された棒にし
つかり締付けられるときブロツクがヨークの中で滑らな
いようにするのが好ましい。
〔実施例〕
図面を参照すると、第１図は、本発明によるねじであ
る。総括的に10として表わされたねじは、12におけるよ
うに一端にねじを切られ、骨の中に導入するためにやや
尖らされているのが好ましい軸11を備えている。反対端
にはヨークまたはトラフ13がある。ヨークの前端は、14
におけるように棒15を受けるために湾曲している。棒15
はなめらかでもよい、すなわちねじやまがつていなくて
もよい。第２図および第３図にもつと明瞭に示されてい
るように、ヨークの前内壁16は、棒15がもたれることの
できる尖頭または突起17を形作るために内方へテーパが
ついている。ヨークの側壁18は、後方アンダカツト部分
20を有し、パネルまたはブロツク21を受けるみぞまたは
長穴19を備えている。４図および第５図に示されている
ように、ブロツク21は、ヨークのアンダカツト部分とあ
りつぎを形成する見合いのアンダカツト部分26をもつた
ランド29を備えている。
ブロツク21は、第３図に示されているように、棒15に
接触し、棒をヨークの前内側表面に形成された尖頭17に
押しつけるようにブロツクを通して進めることのできる
２本の止めねじ22および23を備えている。止めねじ22、
23の外端は、適当なレンチで操作できるように六角形の
頭にしてもよい。ブロツク21の厚さは、ヨーク19の深さ
よりかなり小さいので、ブロツクはヨークにゆるくはま
る。しかし、止めねじ22、23が、第３図に示されている
ように、挿入された棒を圧迫するように進められると、
ブロツクは、ランド29のアンダカツト部分26がヨークの
アンダカツト部分20に係合するように押しかえされる。
ヨークのみぞ19のアンダカツト部分20の内部は、小さ
な突起24（第２図および第３図）を備えていてもよく、
見合いのソケツト25がブロツク21に設けられて止めねじ
22、23を進めて、棒に押しつけ、ブロツクをヨークのみ
ぞ19の後部に押しかえすと突起24がソケツト25の中には
まつてソケツト内でブロツクの滑るのを妨げるようにす
る。
使用時には、ねじは、従来の教示に従つて案内穴を設
けるためにキルシユネル針金を用いて肉茎に挿入でき
る。ソケツト30を肉茎ねじを操作するのを助けるために
ねじヨークの側壁に設けてもよい。２本以上のねじを所
望の点に取付けた後に、棒をねじのヨークの開放後端を
通して挿入できる。次に、ブロツク21をみぞ19に落し込
んで、止めねじ22、23を締める。棒とヨークの内側との
間の接点が尖頭17にあるので、どちらかの止めねじを進
めることによつて棒を肉茎ねじに対して傾けることがで
きる。これは、とりわけ、背柱の変形を修正するために
背柱自体を所望通りに一直線にするために用いることが
できる。
〔発明の効果〕
第６図は、本発明による肉茎ねじを用いる２本の固定
棒を適用した一連の胸椎を示している。肉茎ねじのヨー
ク端の横寸法は、かなり小さく、例えば、12×12mmの程
度のものである。従つて、第６図に示されているよう
に、本発明による肉茎ねじを隣接背椎27、28に干渉なし
に挿入することが可能である。さらに、肉茎ねじと棒と
の間の角度を２本の止めねじの釣合いをとることによつ
て調節できるので、棒が非常に容易に所定の位置に納つ
た後に棒の角度を調節することが可能である。

Claims (12)

    (57)【特許請求の範囲】
  1. 【請求項１】骨に挿込むためにねじを切られている前端
    と棒を受けるヨークをもつた後端とを有する軸を備え、
    前記ヨークの前方壁が前記ヨークに挿入された棒を圧迫
    するように構成された縁をもつた尖頭を形作るように成
    形されており、さらに前記ヨーク内にあつて前記ヨーク
    に挿入された棒を前記尖頭に締付ける手段を備えた肉茎
    ねじ。
  2. 【請求項２】ブロツクと、前記ブロツクを前記ヨーク内
    に摺動させて取付ける手段と、前記ブロツクについてお
    り、前記ヨークに挿入された棒を締付ける手段とをさら
    に備えた請求項１に記載の肉茎ねじ。
  3. 【請求項３】前記ヨークの側面にあるみぞと、前記ヨー
    クの側面にあるみぞに適合するランドを有するブロツク
    と、前記ヨーク内にブロツクを摺動させて取付ける手段
    と、前記ブロツク内にあつて前記ヨークに挿入された棒
    を前記尖頭に押しつけるねじ手段とをさらに備える請求
    項１に記載の肉茎ねじ。
  4. 【請求項４】前記ねじ手段が２本の止めねじからなる請
    求項３に記載の肉茎ねじ。
  5. 【請求項５】前記ヨーク内の前記みぞと前記ブロツク内
    の前記ランドが互いにありつぎになるアンダカツト部分
    を有している請求項３に記載の肉茎ねじ。
  6. 【請求項６】前記アンダカツト部分内にあつて、前記ブ
    ロツクと前記ヨークとの間の相対運動を抑える手段をさ
    らに備える請求項３に記載の肉茎ねじ。
  7. 【請求項７】軸と、前記軸についており、前記軸の端に
    ヨークを備え、前記軸を前記軸の軸線に直角に走る棒に
    締付ける締めつけ手段と、前記ヨーク内に取外し自在に
    取付けられたパネルと、前記パネル内にあつて前記ヨー
    クに挿入された棒を前記ヨークの内部に押しつける手段
    とを備える肉茎ねじ。
  8. 【請求項８】前記ヨークの内側壁が前記ヨークに挿入さ
    れた棒を接触する内方に伸びる突起をもつている請求項
    ７に記載の肉茎ねじ。
  9. 【請求項９】前記パネルが前記突起のどちらの側でも前
    記ヨークの中に進められるように構成されたねじを有す
    る請求項８に記載の肉茎ねじ。
  10. 【請求項１０】肉茎ねじと棒とを具備し、前記肉茎ねじ
    は軸と前記軸に付いていて前記肉茎ねじを前記棒に固着
    する締付け手段とを備え、前記締付け手段が、前記軸の
    端にあるヨークと、前記ヨークに取外し自在に取付けら
    れたブロツクと、前記ブロツクにあつて前記棒を前記ヨ
    ークに挿入されたとき前記ヨークの内部に押しつける手
    段とを備えている背柱固定組立体。
  11. 【請求項１１】前記軸の前記ヨークから遠い端は、ねじ
    を切られており、前記棒はねじを切られていない請求項
    10に記載の背柱固定組立体。
  12. 【請求項１２】前記ヨークの内側壁が尖頭に形作られ、
    前記ブロツクが前記ブロツクに挿入された棒を前記尖頭
    に押しつけるねじ手段を備えている請求項10に記載の背
    柱固定組立体。
