Description

【発明の詳細な説明】 産業上の利用分野 本発明はプロテーゼの関節、即ち損傷した関節の代替
をし且つ元の関節で可能であつたと同じ運動を回復する
ことを可能にする装置に関する。
従来の技術 人体には、例えば手を開閉するために、骨を互いに対
して枢動させる関節を構成する多数の連結部が骨の間に
あり、例えば指の二つの指節骨の間にある。しかしなが
ら、時折、事故又はなんらかの病気の後、関節が破壊さ
れて関節の枢動をもはや不可能にする。これらの運動を
回復するために、医師は傷付いた関節をプロテーゼの関
節と取替えることからなる手術を行つている。
取替えられるべき関節の寸法及び形状に実質的に適合
する多数の型のプロテーゼの関節が存在し、そのなかに
は二つの指節骨の間又は中手骨及び指節骨の間に配置さ
れるような小さい寸法の関節のためのプロテーゼの関節
が含まれる。小さい関節で使用する一つの形式は、三つ
の機能部分、実際にはヒンジ部分で連結される二つの端
部に実質的に分割される可撓性エラストマー材料の単一
の部片で構成される。二つの端部は連結すべき二つの骨
に埋没されるように設計され且つヒンジ部分は二つの骨
の間に配置される。ヒンジ部分は比較的可撓性のある材
料で作られるので、二つの骨をヒンジの平面中に配置す
ることは達成され得る。ある場合には、この構造は骨の
表面との長い接触の後に摩耗を受けることがある。
発明が解決しようとする課題 この不都合を改善すること、及び長期使用に耐えるこ
とができ、製作が容易であり且つ比較的安価であり、且
つ骨関節の全ての寸法に即座に適合され得る関節プロテ
ーゼを得ることは本発明の目的である。
課題を解決するための手段及び作用 後に説明する本発明の実施例に用いられている符号を
参考のために付記して記述すると、本発明は、二つの骨
のごとき要素を連結するための関節プロテーゼであっ
て、 二つのカラー10,11を有する第一の回転軸受部品６が
端部に配設されている体部４を含む第一の幹部分２であ
って、前記二つのカラー10,11は互いに隔離され且つ軸
線方向に整列しているとともに、前記二つのカラー10,1
1の内側端の二つの対向していてほぼ平行な面13,14によ
って境が定められた空間12を画定している、前記第一の
幹部分２と、 第二の回転軸受部品７が端部に配設されている体部５
を含む第二の幹部分３と、 一つの軸線８のまわりで前記第一及び第二の回転軸受
部品6,7が互いに他に対して回転し得るようにこれら第
一及び第二の回転軸受部品6,7を連結する連結装置30,31
とを含み、 前記第二の回転軸受部品７は前記空間12内に適合し得
る平面18,19を有する部分17を含んでいる、 関節プロテーゼにおいて、 前記平面18,19を有する部分17は各側部に円筒状の凹
部21,22を有し、該凹部21,22の底部23,24は前記連結装
置30,31によって加えられる圧力負荷に反作用するスラ
スト面を形成しており、前記第二の回転軸受部品７が前
記空間12内に適合された時、前記円筒状の凹部21,22の
回転軸線が前記二つのカラー10,11の穴15,16の回転軸線
８と一致することを特徴とする。
本発明の他の特徴及び利点は添付図面に従う以下の説
明から明らかになるが、図面の全ては例示的なものであ
つて本発明の範囲を制限するものではないと考えるべき
である。
実施例 第１図及び第２図に関節プロテーゼの同じ実施例が示
される。このプロテーゼは二つの幹部分2,3を具備す
る。幹部分2,3のそれぞれは細長い形の体部4,5と、該体
部の各端部に回転軸受６及び７とをそれぞれ有する。二
つの軸受６及び７は単一の軸線８の周りに枢動するよう
に一方を他方に対して回転させることによつて協働する
ようになつている。
更に、プロテーゼ１は二つの軸受を連結する装置９を
含む。この連結構造は安定されて維持されるが軸８の周
りに互いに相対的に自由に回転する作用をする。第１図
及び第２図は二つの軸受部分６及び７及び連結装置９を
例示する実施例を示す。軸受の一方（軸受６）は幹４と
一体の二つのブツシユ又はカラー10及び11を含み、その
カラーはそれらの間に二つの実質的に平行な面13及び14
によつて区画される凹部を構成する空間12を画成する。
カラー10,11のそれぞれは側部から側部まで円筒状の穴1
5及び16を含み、該穴は回転可能な軸37及び38の軸受を
形成する。
この実施例で、第二の軸受７は空間12に適合する平ら
な部分17からなり、それにより部分17は空間の内側で枢
動することができ、それにより平らな部分17を区画する
二つの垂直方向に平行な面18,19を上記した面13,14上に
滑動させる。更に、面18,19のそれぞれは円筒状の凹部2
1,22を有し、その軸線20は同一であり且つその直径は最
適な結果のためには二つの穴15,16の直径と垂直方向に
同一であるべきである。
好適な実施例では、円筒状の凹部21,22は窪んでいる
が横切つておらず、このためそれぞれは底部23,24を有
し、該底部は後述する連結装置によつて加えられる圧力
負荷に反作用する表面として作用する。
凹部21,22はそれらの回転中心軸線20が部分７を空間1
2中へ挿入した時に穴15,16の軸線８と一致するように設
計される。
図示した実施例では、二つの軸受を連結する機構９は
二つのスナツプばめキヤツプ30,31からなり、各キヤツ
プは三つの構成部品、即ちボス32と、ボスと一体にあり
且つ穴15,16及び凹部21及び22に適合する直径を有する
ヒンジピン部分（すなわち、シヤフト部分）33と、上記
した二つのカラー10,11の周囲の突出肩部又は唇部35及
び36にスナツプばめするように設計されたスナツプばめ
形式の保持リング34とを有する。
動作可能の関節プロテーゼ１は上述した構成部品を用
いて軸受７が軸受６の空間12中へ挿入されるように二つ
の幹部分２及び３を配置することによつて構成され得
る。二つのキヤツプは端部37,38が凹部21,22の底部23,2
4に当接するまで軸部分33を穴15,16中へ滑動させ得るよ
うに設計される。軸部分は各保持リング34の内方肩部3
9,40が適合する肩部35及び36上に押付けられる前にこの
当接を生じさせるに充分なだけ長い。しかしながら、キ
ヤツプは比較的弾性的な材料から作られているので、キ
ヤツプは変形することができ且つ肩部39,40は突出唇部3
5,36にスナツプばめされる。固有の弾性は軸受6,7の間
を強固に結合する。
この関節プロテーゼの実施例はそれが安定であり且つ
いろいろな関節に適合し得るので特に有利である。接合
される骨の幅に依存して、単一のレバー割りつけが主に
ボス32の厚さに関していくつかの形式のキヤツプと共に
使用されることができるので、キヤツプ端部41,42は第4
A図及び第4B図に関して説明されるように関節を充分に
保護するために骨の横方向縁部の突出部と整合されるこ
とができる。
上述したように、二つの幹2,3は軸受6,7によつて互い
に相対的に枢動するように設計される。両方の軸受の回
転は滑動表面対13,18又は14,19の一方の間へ低摩擦座金
50を挿入することによつて容易にされ得る。座金は軸受
７の二つの面の一方の形状に適合し、それによりそれは
二つの軸受の間に挟まれてそれらの相対的回転のための
潤滑材として作用する。
レバー、キヤツプ及び座金を適用可能時に生体劣化及
び／又は埋没体の構成部品間の寄生化学反応を防止する
に適合可能な材料から作ることは有利である。次の材料
の部品は幹及びキヤツプのためにそれぞれ特に選択され
ている。即ち、コバルトクロム／ポリエチレン、チタン
／ポリエチレン、セラミツク／ポリエチレン、セラミツ
ク／セラミツク、TCF/TCF（登録商標名）、TCF/セラミ
ツク及び複合体／複合体材料。
幹2,3の体部4,5は接合される骨に固定されるスリーブ
中に挿入されるように設計される。各スリーブは空所を
特徴とし、その中で二つのレバー2,3の体部4,5は多分非
常に小さい横方向遊びをもつて単一の自由度（即ち並進
運動）で運動し得る。このために、空所の断面は第３図
に示す形状から最良の結果として選択される幹の断面と
密接に適合する。
体部60は四つの縁部61,62,63,64を有する四角形断面
を有する。体部65は八つの縁部66を有する多角形断面を
有する別の実施例からなる。第三の実施例67は楕円形断
面を有し、それはそれがその周囲68に鋭い縁部をもたず
且つスリーブ内でのクリープを防止する限りにおいて最
適な形状である。体部がスリーブに対して単一の自由度
（即ち並進運動）だけをもたねばならない時、幹は回転
を許す形状であつてはいけない。
第4A図及び第4B図は二つの骨101及び102（例えば手の
指103の二つの骨）を連結するための関節プロテーゼの
典型的な応用例である。
空所106及び107は指骨の自由端104,105から切削され
る。関節プロテーゼ１の幹2,3の二つの体部4,5の断面と
密接に適合する断面を有する空所110及び111を有するス
リーブ108及び109はこれらの空所中に固定される（例え
ばねじ止め、接着等によつて）。第4A図は二つの幹の軸
受6,7の連結を例示し、キヤツプ30,31は軸受６のカラー
10,11にプレスばめ及びスナツプばめされる。
その上、この図は両方のカラーの端部41,42が少なく
とも軸線８に沿つて二つの指節骨の横方向縁部の突出部
と垂直方向に整合されていることを示している。若干の
場合には、これらのキヤツプは周辺の連続性を備えるよ
うに修正されることができる。このようにして、レバー
2,3及び一連の異なる長さ及び形状についての一つの構
造によつて、どのような幅の骨に対する関節プロテーゼ
を構成し、それによりこれまで利用されているプロテー
ゼと比較してプロテーゼの低コスト化に寄与することは
可能である。
この構造は、特にそれが手の二つの指骨間の関節とし
て使用される時に本発明による関節プロテーゼに別の重
要な利点を付与する。そのプロテーゼを手に埋没する
時、手の他の活力ある要素（即ち神経、腱等）は無傷の
ままであるにちがいない。腱は二つの指節骨104,105の
間の通糸のように作用する。これは、そのプロテーゼを
取付けた人が荷物を持上げる時、二つの指節骨がそれら
を離そうとし且つ腱によつて加えられる逆反作用に抗し
て作用する引張力の作用を受け、それにより体部4,5を
スリーブ108中で僅かに滑動させることを意味する。引
張力の作用を受けるが、体部はこの負荷をスリーブへ伝
達しないので、スリーブは以前に利用しているプロテー
ゼで時々起こるような骨から弛むことがない。この特徴
は本発明による関節プロテーゼにはるかに長い動作寿命
を与えること、及び骨が引抜負荷によつて劣化されない
ので以前のプロテーゼと比較して組織系統への起こり得
る作用をはるかに少なくさせることの助けになる。

Claims (9)

    (57)【特許請求の範囲】
  1. 【請求項１】二つの骨のごとき要素を連結するための関
    節プロテーゼであって、 二つのカラー（10,11）を有する第一の回転軸受部品
    （６）が端部に配設されている体部（４）を含む第一の
    幹部分（２）であって、前記二つのカラー（10,11）は
    互いに隔離され且つ軸線方向に整列しているとともに、
    前記二つのカラー（10,11）の内側端の二つの対向して
    いてほぼ平行な面（13,14）によって境が定められた空
    間（12）を画定している、前記第一の幹部分（２）と、 第二の回転軸受部品（７）が端部に配設されている体部
    （５）を含む第二の幹部分（３）と、 一つの軸線（８）のまわりで前記第一及び第二の回転軸
    受部品（6,7）が互いに他に対して回転し得るようにこ
    れら第一及び第二の回転軸受部品（6,7）を連結する連
    結装置（30,31）とを含み、 前記第二の回転軸受部品（７）は前記空間（12）内に適
    合し得る平面（18,19）を有する部分（17）を含んでい
    る、 関節プロテーゼにおいて、 前記平面（18,19）を有する部分（17）は各側部に円筒
    状の凹部（21,22）を有し、該凹部（21,22）の底部（2
    3,24）は前記連結装置（30,31）によって加えられる圧
    力負荷に反作用するスラスト面を形成しており、前記第
    二の回転軸受部品（７）が前記空間（12）内に適合され
    た時、前記円筒状の凹部（21,22）の回転軸線が前記二
    つのカラー（10,11）の穴（15,16）の回転軸線（８）と
    一致することを特徴とする関節プロテーゼ。
  2. 【請求項２】前記凹部（21,22）の各々は皿形の形状で
    ある、特許請求の範囲第１項記載の関節プロテーゼ。
  3. 【請求項３】前記連結装置がキャップ（30,31）を含
    み、各該キャップは、ボス（32）と、該ボス（32）と一
    体であるシャフト部分（33）と、該ボス（32）と一体で
    ある保持リング（34）とを含む三つの部分で成ってお
    り、前記保持リング（34）は内方肩部（39,40）を有
    し、前記カラー（10,11）の各々の外側端部の周囲に突
    出唇部（35,36）が一体に備えられ、前記内方肩部（39,
    40）と前記突出唇部（35,36）とは、前記シャフト部分
    （33）が前記カラーの穴内に挿入され且つ前記凹部（2
    1,22）の一方内に挿入された時に、前記保持リング（3
    4）の各々の前記内方肩部が前記カラーの前記突出唇部
    上に位置されるような形状にされている、特許請求の範
    囲第１項又は第２項記載の関節プロテーゼ。
  4. 【請求項４】前記シャフト部分（33）の長さは、前記内
    方肩部が前記突出唇部に達する直前に、前記シャフト部
    分の端部が前記凹部の底部に当接するような長さになっ
    ている、特許請求の範囲第３項記載の関節プロテーゼ。
  5. 【請求項５】前記第一及び第二の回転軸受部品の間に少
    なくとも一つの低摩擦座金（50）が配置されている、特
    許請求の範囲第１項から第４項までのいずれか一つに記
    載の関節プロテーゼ。
  6. 【請求項６】前記体部（4,5）の各々は細長い形状であ
    る、特許請求の範囲第１項から第５項までのいずれか一
    つに記載の関節プロテーゼ。
  7. 【請求項７】骨の空所に埋没されるようになったスリー
    ブ（108,109）を含み、該スリーブ（108,109）は前記体
    部（4,5）の横断面に相当する横断面を有していて、前
    記関節プロテーゼが埋没される時に、前記体部（4,5）
    を受け入れるようになっている、特許請求の範囲第１項
    から第６項までのいずれか一つに記載の関節プロテー
    ゼ。
  8. 【請求項８】前記体部は楕円形の横断面を有する、特許
    請求の範囲第１項から第７項までのいずれか一つに記載
    の関節プロテーゼ。
  9. 【請求項９】前記体部及び前記連結装置は、それぞれ、
    金属材料及びプラスチック材料を含む、特許請求の範囲
    第１項から第８項までのいずれか一つに記載の関節プロ
    テーゼ。
