本開示は、IL-15を発現する遺伝子改変された免疫細胞に関し、特に、本開示は、改変されたIL-15を発現し、細胞内に保持する並びにCD16の高親和性バリアント及びCAR(キメラ抗原受容体)の少なくとも1つをさらに発現するNK細胞に関する。

NK-92細胞は、レシピエントにおける同種異系細胞に対して明らかな毒性はなく、様々な腫瘍細胞に向けて比較的広範囲の細胞障害活性を有するので、細胞ベースの療法の種々の側面において望ましい。さらに、NK-92細胞は、比較的単純な方法で培養することができ、よって、養子癌免疫療法に魅力的な選択肢を提示することができる。あいにく、NK-92細胞の増殖及び機能は、IL-2に極めて依存しており、これは、NK-92細胞が大規模なスケールで必要とされる場合、費用を増加させる。このようなサイトカインの必要と関連する問題を回避するために、NK-92細胞は、IL-2を発現し、細胞内に保持するように形質転換された(たとえばExp Hematol 33:159-164を参照されたい)。このような改変された細胞は、実際に、外来性サイトカインから独立するようになったが、種々の不利益が残った。とりわけ、このような改変された細胞から放出されるIL-2は、このような細胞が哺乳動物において使用される場合、インビボにおいてIL-2媒介性の影響を増加させることがあり、これは、IL-2が腫瘍微小環境において免疫抑制を刺激する場合(典型的に、骨髄由来抑制細胞(MDSC)及び調節性T細胞(Treg)の成長及び増殖を介して)、特に害となる。
別の例では、NK-92細胞は、pcDNA3発現ベクターの中にクローニングされたcDNAからIL-15を発現するようにトランスフェクトされ(たとえばHaematologica,2004;89:338-347を参照されたい)、そのようにトランスフェクトされた細胞は、培養上清中に高レベルのIL-15を継続的に産生し、これにより、有意に、より急速に、低用量のIL-2又はIL-15による刺激に応答して、細胞を増殖させると考えられた。さらに、長期培養中の細胞の累積数もまた、非トランスフェクト細胞よりもかなり大きかった。しかしながら、このような細胞がインビボにおいて使用される場合、高レベルの分泌されたIL-15は、臨床上問題になり得る。
同様に、NK-92細胞は、IL-15の組換えネイティブ形態を発現するように、ウイルストランスフェクション系を使用して形質転換された(たとえばCancer Immunol Immunother(2012)61:1451-1461を参照されたい)。このような組換え細胞は、外来性サイトカインの非存在下において成長することができ、組換えCARを発現したが、トランスフェクション効率は、比較的低く、細胞内に産生されるIL-15の量は、比較的低く、培養培地の中に分泌されるIL-15の量は、低かった。さらに、組換え細胞の細胞毒性は、親NK-92細胞株と比較して、低下した。とりわけ、同様のIL-15がプラスミドから発現される場合、そのように生成されたNK-92細胞は、外来性成長因子から完全には独立しておらず、したがって、このような組換え細胞のインビボにおける使用を制限した。
したがって、たとえ種々の改変された免疫細胞、特に、改変されたNK細胞が、当技術分野において知られていても、それらはすべて又はほぼすべて、種々の不利益を被っている。その結果、目標とされる細胞毒性を維持しながら、望ましい成長特性を呈する、改善された、改変されたNK細胞を提供する必要がある。
NK細胞が、IL-15又はそのバリアントを細胞内に発現して、分泌するか又は提示するように遺伝子改変され、改変されたNK細胞を外来性サイトカインから独立させる、及び改変されたNK細胞の近くの他の免疫コンピテント細胞の刺激/活性化を可能にする、種々の組換え細胞、組成物、及び方法が、本明細書において開示される。
本発明の主題の一態様において、発明者らは、遺伝子改変されたNK細胞及びこのような細胞を作製するための方法を企図し、遺伝子改変されたNK細胞は、erLSP-IL-15をコードする第1のセグメントを含む組換え核酸を含む。最も典型的には、NK細胞は、NK-92細胞であり、組換え核酸は、DNA(たとえば線状プラスミド)である。好ましくは、必ずしもではないが、erLSP-IL-15中のIL-15部分は、コドン最適化ヒトIL-15配列を含む。
さらなる実施形態において、組換え核酸は、CD16若しくは高親和性CD16をコードする第2のセグメントをさらに含む並びに/又はキメラ抗原受容体をコードする第3のセグメントをさらに含んでいてもよい並びに/又は免疫刺激をもたらす、チェックポイント阻害を妨げるタンパク質、免疫抑制に関与するサイトカインに結合する/サイトカインを阻害するタンパク質、及び/若しくはIL-15受容体アルファ鎖をコードする第4のセグメントをさらに含んでいてもよい。組換え核酸が、(a)改変されたNK細胞を外来性サイトカインから独立させるのに及び(b)改変されたNK細胞の近くの他の免疫コンピテント細胞の刺激/活性化を可能にするのに十分な量で、erLSP-IL-15の発現を引き起こすのに十分な強度を有するプロモーターを含むこともまた、企図される。任意選択で、改変されたNK細胞は、CD16を介して細胞につながれた抗体を含んでいてもよい。
本発明の主題のさらなる態様において、発明者らはまた、本明細書において記載される遺伝子改変されたNK細胞と組み合わせて薬学的に許容され得るキャリアを含む医薬組成物をも企図する。最も典型的には、このような組成物は、患者への注入のために製剤化され、及び、投与単位当たり少なくとも1×109細胞を含んでいてもよい。
したがって、発明者らはまた、癌の治療における、本明細書において記載される遺伝子改変されたNK細胞の使用をも企図する。このような使用は、スタンドアローンの治療としての細胞の注入であってもよいが、他の企図される使用はまた、TMEに侵入する薬物の投与、免疫抑制を低下させる薬物の投与、免疫コンピテント細胞を刺激する薬物の投与、癌ワクチン組成物の投与、及び/又は免疫応答の維持を助け且つ記憶細胞の発達を促進する薬物の投与をも含む。
様々な目的、特徴、態様及び利点は、以下の好ましい実施形態の詳細な説明と共に、同様の数字が同様の構成要素を表す添付の図面からより一層明らかになるであろう。
発明者らは、これまでに、組換えCD16及び/又はCARの細胞毒性及び機能的発現を維持しながら、IL-2/IL-15から独立した成長及び刺激をもたらすのに十分な組換えIL-15の量を産生する改変されたNK細胞を、生成することができることを発見した。さらに、少なくともいくつかの実施形態において、企図される改変されたNK細胞は、サイトカインの非存在下において成長及び増殖を可能にするのに十分な細胞内IL-15(特にerLSP-IL-15)を産生するだけでなく、このような細胞がTME中に存在する場合に腫瘍微小環境(TME)において免疫刺激(又は免疫抑制の転換)を支援する量でIL-15の分泌をも可能にする。したがって、本発明の主題によるNK細胞は、CD16及び/又はCARを介しての組換え標的細胞毒性のための手段を提供しながら、培養及び増殖の簡略化を有利に可能にする。
内質保持配列と共にIL-2を発現した、以前に調製されたNK-92誘導体は、骨髄由来抑制細胞(MDSC)及び調節性T細胞(T-reg)などのような免疫抑制細胞の成長及び増殖を支持するように思われた。これらの負の調節因子は、免疫細胞、特にaNK、haNK、及びt-haNK並びにドナーNK細胞のいかなる抗腫瘍効果をも弱め得る。他方、組換えIL-15は、免疫活性細胞の機能のみを支持し、erIL-2の負の影響がない。
しかしながら、IL-15及びそのバリアントの発現は、NK細胞、特にNK-92細胞の成長及び/又は機能に悪影響を与えかねないことを理解されたい。特に、CD16及び/又はCARなどのような標的NK細胞にさらに所望される組換えタンパク質を有害に妨げることなく、細胞増殖及び活性を支持する量で、IL-15及びそのバリアントの生物学的活性形態を発現することも、期待することができない。さらに、免疫刺激レベルを超えるIL-15及びそのバリアントの過剰発現及び分泌は、このような細胞のレシピエントにおいて全身性副作用をもたらし得る。様々な観点から見て、好ましい改変されたNK細胞は、全身性副作用を回避するが、TME内で、改変された細胞及び免疫細胞について所望される有益な効果、成長、及び刺激特性を維持するのに十分に低いIL-15を産生し、分泌するであろう。とりわけ、発明者らは、これまでに、自己分泌方式で(すなわち、外来性サイトカインを必要とすることなく)成長及び増殖を刺激するために細胞内に形成され、保持されたIL-15とTME内の他の免疫コンピテント細胞に対して免疫刺激効果をもたらす分泌されたIL-15との間に、望ましいバランスを有するNK細胞を、調製することができることを発見した。
適したNK細胞に関して、NK細胞は、遺伝子改変されたNK細胞を受けるであろう対象由来の自家NK細胞であってもよいことが、一般に企図される。このような自家NK細胞は、全血から単離されてもよい又は当技術分野においてよく知られている方法を使用して前駆細胞若しくは幹細胞から培養されてもよい。しかしながら、NK細胞が、自家である必要はなく、同種異系若しくは異種NK細胞であってもよいこともまた、理解されたい。本発明の主題の特に好ましい態様において、遺伝子操作を受けるNK細胞は、NK-92細胞又はその誘導体である。たとえば、本発明の主題の特に好ましい一態様において、遺伝子操作を受けたNK細胞は、少なくとも1つのキラー細胞免疫グロブリン様受容体(KIR)の発現の低下又は消滅を有するように改変されたNK-92誘導体であり、これは、典型的に、このような細胞を恒常的に活性化させる(阻害の欠如又は低下を介して)。
NK-92細胞は、珍しい受容体発現プロファイルを呈し、比較的多数の活性化(たとえばNKp30、NKp46、2B4、NKGD、E、CD28)受容体を発現する。逆に、NK-92細胞はさらに、阻害受容体をほとんど発現せず(たとえばNKGA/B、低レベルのKIR2DL4、ILT-2)、通常のNK細胞上にクローン発現される大部分のキラー細胞抑制受容体(KIR)を欠く。加えて、NK-92は、パーフォリン-グランザイム細胞溶解経路に関与する比較的高レベルの分子及び腫瘍壊死因子(TNF)-スーパーファミリーメンバーFasL、TRAIL、TWEAK、TNF-アルファを含む追加の細胞毒性エフェクター分子を発現し、代替機構を介して死滅させることができる能力を示す。さらに、NK-92細胞はまた、免疫エフェクター細胞調節に関係する他の分子をも発現する(CD80、CD86、CD40L、TRANCE)が、NK死滅における関連性は、はっきりしない。しかしながら、とりわけ、これらの特に望ましい形質は、本明細書において記載される改変によって悪影響を与えられないであろう。実際に、少なくともいくつかの実施形態において、1つ又はそれ以上の上記のアクチベータータンパク質及び/又はエフェクタータンパク質は、下記により詳細に述べられるように、IL-15の細胞内発現に応答して過剰発現されてもよい。
さらに、適したNK細胞は、MHCクラスI分子との相互作用を低下させる又は消滅させるなどのために突然変異させた1つ又はそれ以上の改変されたKIRを有していてもよい。当然、1つ若しくはそれ以上のKIRをさらに欠失させてもよい又は発現が抑制されてもよい(たとえばmiRNA、siRNA等を介して)ことに留意されたい。最も典型的には、1つを超えるKIRを、突然変異させる、欠失させる、又は発現停止させるであろう、また、特に企図されるKIRは、2つ又は3つのドメインを有し、短い又は長い細胞質側末端を有するものを含む。様々な観点から見て、改変された、発現停止させた、又は欠失させたKIRは、KIR2DL1、KIR2DL2、KIR2DL3、KIR2DL4、KIR2DL5A、KIR2DL5B、KIR2DS1、KIR2DS2、KIR2DS3、KIR2DS4、KIR2DS5、KIR3DL1、KIR3DL2、KIR3DL3、及びKIR3DS1を含むであろう。このような改変された細胞は、当技術分野においてよく知られているプロトコールを使用して調製されてもよい。その代わりに、このような細胞はまた、NantKwest(URL www.nantkwest.comを参照されたい)から、aNK細胞(活性化ナチュラルキラー細胞)として市販で得られてもよい。このような細胞は、次いで、下記にさらに論じられるように、IL-15又はそのバリアントを発現するように、追加で遺伝子改変されてもよい。
本発明の主題の別の好ましい態様において、遺伝子操作を受けたNK細胞はまた、高親和性Fcγ受容体(CD16)を発現するように改変されるNK-92誘導体であってもよい。Fcγ受容体の高親和性バリアントの配列は、当技術分野においてよく知られており(たとえばBlood 2009 113:3716-3725を参照されたい)、生成及び発現の方法はすべて、本明細書における使用に適していると考えられる。このような受容体の発現は、患者の腫瘍細胞(たとえばネオエピトープ)、特定の腫瘍型(たとえばher2neu、PSA、PSMA等)に対して特異的である又は癌(たとえばCEA-CAM)と関連する抗体を使用する腫瘍細胞の特異的な標的化を可能にすると考えられる。好都合なことに、このような抗体は、市販で入手可能であり、細胞(たとえば、Fcγ受容体に結合した)と併せて使用することができる。その代わりに、このような細胞はまた、NantKwestから、haNK細胞(「高親和性ナチュラルキラー細胞)として、商業的に得られてもよい。このような細胞は、次いで、下記にさらに論じられるように、IL-15又はそのバリアントを発現するように、さらに遺伝子改変されてもよい。
その代わりに又は追加で、遺伝子操作を受けたNK細胞はまた、キメラ抗原受容体(CAR)を発現するように遺伝子操作を受けてもよい。特に好ましい態様において、CARは、腫瘍関連抗原、腫瘍特異的抗原、及び癌ネオエピトープに対して結合特異性を有するscFv部分又は他の外部ドメインを有する。したがって、容易に理解されるように、適したCARは、第1、第2、及び第3世代のCARを含む(たとえばImmunotherapy(2015)7(5):487-497を参照されたい)。前に述べたように、このようなキメラT細胞受容体を発現するようにNK細胞に遺伝子操作を受けさせるための数多くの方法があり、方法はすべて、本明細書における使用に適していると考えられる。その代わりに、このような細胞はまた、NantKwestから、taNK細胞(「標的活性化ナチュラルキラー細胞」)として市販で得られてもよい。このような細胞は、次いで、下記にさらに論じられるように、IL-15又はそのバリアントを発現するように、さらに遺伝子改変されてもよい。
細胞が、癌関連抗原又は癌関連抗原に向けて特異性を有する抗体に向けて親和性を有するように操作を受ける場合、知られている癌関連抗原はすべて、使用に適切であると見なされることが企図される。たとえば、癌関連抗原は、CEA、MUC-1、CYPB1等を含む。同様に、細胞が、癌特異抗原又は癌特異抗原に向けて特異性を有する抗体に向けて親和性を有するように操作を受ける場合、知られている癌特異抗原はすべて、使用に適切であると見なされることが企図される。たとえば、癌特異抗原は、PSA、Her-2、PSA、ブラキウリ等を含む。細胞が、癌ネオエピトープ又は癌ネオエピトープに向けて特異性を有する抗体に向けて親和性を有するように操作を受ける場合、ネオエピトープを同定するための知られている方法はすべて、適した標的を導くことが企図される。たとえば、ネオエピトープは、第1のステップにおいて、腫瘍生検材料(又はリンパ生検材料又は転移部位の生検材料)及び適合する正常組織(すなわち、同じ患者由来の非罹患組織)の全ゲノム解析によって、そのように得られたオミックス情報の同期比較を介して、患者腫瘍から同定されてもよい。そのように同定されたネオエピトープは、次いで、ネオエピトープの抗原提示の可能性を増加させるために、患者のHLA型への適合について、さらにフィルタ処理にかけることができる。最も好ましくは、このような適合は、インシリコで(in silico)行うことができる。
発現に適したIL-15配列に関して、IL-15は、哺乳動物IL-15配列、最も好ましくはヒトIL-15配列であることが一般に好ましい(たとえばUniProt identifier P40933を参照されたい)。さらに、適したIL-15配列は、種々のバリアントを含むこと並びに特に企図されるバリアントは、長いシグナルペプチド(LSP)及び短いシグナルペプチド(SSP)を有するものを含むことに留意されたい。LSPバリアントは、典型的に、48アミノ酸のシグナルペプチドを有し、転写物は、典型的に、316塩基の5’-非翻訳領域(UTR)、486塩基のコード化配列、及び長さが約400塩基であるC末端3’-UTR領域を含む。SSPバリアントは、典型的に、代替スプライシングに基づく21アミノ酸の短いシグナルペプチドを有する(エキソン4A及び5によってコードされる)。とりわけ、両方のアイソフォームは、同じ成熟タンパク質を産生するが、細胞輸送は別個である。より詳細には、IL-15 LSPアイソフォームは、ゴルジ装置、初期エンドソーム、及び小胞体中に検出され、分泌され、膜に結合した形態で存在することができる。他方、IL-15 SSPアイソフォームは、分泌されず、細胞質及び核に制限されているように見え、ここで、そのアイソフォームは、細胞周期の調節に関与していると思われる。さらに企図されるIL-15バリアントは、スーパーアゴニストバリアント、特にIL-15 N72D突然変異体を含み、これらは、下記に論じられるように、追加のシグナルペプチド及び/又は輸送シグナルを含んでいても、含んでいなくてもよい。
明らかなシグナル伝達の差に基づいて、発明者らは、したがって、適切な発現レベル及び細胞内の分布及び分泌される量をそのように達成するために、組換えIL-15に対する種々の改変の使用を企図する。その目的で、発明者らは、組換えIL-15に融合することができる種々のシグナル伝達部分の使用を企図する。たとえば、IL-15又はIL-15バリアントが、エンドソーム及びリソソーム区画に搬出されるべきである場合、CD1bリーダーペプチドなどのようなリーダーペプチドは、細胞質から(新生)タンパク質を隔離するために使用されてもよい。さらに又は代替的に、標的化プレ配列及び/又は標的化ペプチドを使用することができる。標的化ペプチドのプレ配列をN末端及び/又はC末端に付加して、典型的には、6~136個の塩基性及び疎水性アミノ酸を構成することができる。ペルオキシソーム標的化の場合、標的化配列は、C末端にあり得る。他のシグナル(例えば、シグナルパッチ)を使用し得、これらには、ペプチド配列において切り離されており、適切なペプチドフォールディング時に機能するようになる配列エレメントが挙げられる。加えて、グリコシル化のようなタンパク質修飾は、標的化を誘導することができる。本発明者らは、他の好適な標的化シグナルの中でも、ペルオキシソーム標的化シグナル1(PTS1)、C末端トリペプチド及びN末端の近くに位置するノナペプチドであるペルオキシソーム標的化シグナル2(PTS2)を企図している。
加えて、エンドソーム及びリソソームへのタンパク質のソーティングは、タンパク質の細胞質ゾルドメイン内のシグナルによっても媒介され得、典型的には短い線状配列を含む。いくつかのシグナルは、チロシンベースのソーティングシグナルと呼ばれ、NPXY又はYXXφコンセンサスモチーフに適合する。ジロイシンベースのシグナルとして知られる他のシグナルは、[DE]XXXL[LI]又はDXXLLコンセンサスモチーフに適合する。これらのシグナルは、すべて膜の細胞質面と周辺に関連するタンパク質コートの成分によって認識される。YXXφ及び[DE]XXXL[LI]シグナルは、アダプタータンパク質(AP)複合体AP-1、AP-2、AP-3及びAP-4によって特徴的な微細特異性で認識されるが、DXXLLシグナルは、GGAとして知られているアダプターの別のファミリーによって認識される。また、空胞タンパク質のソーティング及びエンドソーム機能に関連するFYVEドメインを加えることができる。さらなる態様において、エンドソーム区画は、ヒトCD1テール配列を用いて標的化することもできる(たとえば、Immunology,122,522-531を参照されたい)。たとえば、リソソームの標的化は、LAMP1-TM(膜貫通)配列を使用して達成され得るが、リサイクリングエンドソームは、CD1aテール標的配列を介して標的化され得、ソーティングエンドソームは、CD1cテール標的配列を介して標的化され得る。
細胞質ゾル区画への輸送又は細胞質ゾル区画内での保持は、必ずしも1つ以上の特定の配列エレメントを必要とするものではない。しかしながら、少なくともいくつかの態様では、膜固着タンパク質又は膜固着タンパク質の膜固着ドメインなどのN-又はC-末端細胞質保持シグナルを加えて、タンパク質が細胞質ゾルに面している細胞内に保持されるようにし得る。たとえば、膜固着タンパク質としては、SNAP-25、シンタキシン、シナプトプレビン(synaptoprevin)、シナプトタグミン、小胞関連膜タンパク質(VAMP)、シナプス小胞糖タンパク質(SV2)、高親和性コリン輸送体、ニューレキシン、電位依存性カルシウムチャネル、アセチルコリンエステラーゼ及びNOTCHが挙げられる。

加えて、IL-15又はIL-15バリアントが、IL-15受容体アルファサブユニットと同時発現されてもよいこともまた、企図される。このような同時発現は、受容体アルファ鎖とIL-15又はIL-15バリアントの結合又は他の関連を可能にし、IL-15若しくはIL-15バリアントを安定化する及び/又は改変されたNK細胞の表面上でのIL-15若しくはIL-15バリアントの分泌若しくは輸送/提示を支援すると考えられる。同様に、種々の他のタンパク質(CD16及び/又はCAR以外が、IL-15又はIL-15バリアントと同時発現されてもよく、適した同時発現されたタンパク質は、種々の免疫調節化合物、特に、チェックポイント阻害を妨げる化合物(たとえば、PD1、PD-L1、CTLA4等に対するscFv)、免疫刺激物質(たとえばIFN-γ、IL-12、IL-21等)、及び/又は免疫抑制に関与するサイトカイン(たとえばTGF-β、IL-8等)に結合する/を阻害する化合物を含む。
容易に理解されるように、IL-15又はIL-15バリアント及び他の同時発現されるタンパク質は、組換え核酸上にコードされ、これは、1つ又はそれ以上の組換えRNA分子又はDNA分子であってもよい。しかしながら、最も典型的には、組換え核酸は、多シストロン性DNA構築物、好ましくはプラスミドである。しかしながら、種々の他の構築物もまた、本明細書における使用に適していると考えられ、ウイルスベクター(ウイルスとして又は他の方法を介してトランスフェクトされてもよい)、線状DNA、キャリアに結合したDNA(たとえば、弾道(ballistic)トランスフェクションのための)等を含む。
核酸の特定の形状及びタイプとは無関係に、改変されたNK-92細胞は、(a)そのようにトランスフェクトされた細胞を外来性サイトカインから独立させる及び(b)トランスフェクトされた細胞の近くの(典型的にTME内の)他の免疫コンピテント細胞の刺激/活性化を可能にするのに十分な量のIL-15又はIL-15バリアントを発現することが、一般に好ましい。たとえば、分泌された又は細胞外の(若しくは細胞外に提示された)組換えIL-15又はIL-15バリアントは、細胞中に産生された全IL-15又はIL-15バリアントの5~10%の間又は10~20%の間又は20~30%の間又は30~50%の間を占めるであろうということが企図される。したがって、また、様々な観点から見て、分泌された又は細胞外の(若しくは細胞外に提示された)組換えIL-15又はIL-15バリアントは、約20~60pg/mlの間又は約60~80pg/mlの間又は約80~100pg/mlの間又は約100~150pg/mlの間又は約150~200pg/mlの間又はそれ以上の量で存在してもよい。他方、細胞内に保持されたIL-15又はIL-15バリアントは、約100~150pg/mlの間又は約150~250pg/mlの間又は約250~500pg/mlの間又は約500~750pg/mlの間又はそれ以上量で存在するであろうということが企図される。したがって、特に好ましい改変されたNK-92細胞は、自律的な成長を支持し、TMEにおける免疫コンピテント細胞を刺激するのに並びにCD8+T細胞記憶の確立及び維持を刺激するのに十分な量で、組換えIL-15又はIL-15バリアントを産生するであろうが、このような量は、このような細胞を受けている患者において全身性の有害事象を引き起こすには不十分であることに留意されたい。
したがって、機能的観点から、組換えIL-15又はIL-15バリアントは、他のNK細胞(たとえばTME中の自家NK細胞)、種々のT細胞等のエフェクター機能及び/又は増殖を刺激する又は増強する並びにTME中の細胞におけるJak/STATシグナル伝達を増強する又は引き起こすために存在するであろう。さらに、細胞内に保持されたIL-15又はIL-15バリアントの分画のおかげで、このような細胞はまた、下記にさらに詳細に記載されるように、外来性IL-2及び/又はIL-15が全く存在しない状況でも増殖することができるであろう。さらに企図される態様において、このような改変されたNK-92細胞はまた、改変されていないNK-92細胞と比較して、IL-12シグナル伝達に対する感受性が増加しており、これは、IFN-γのIL-4媒介性の抑制を低下させてもよく、これは、次に、TME中のTh1 T細胞の抑制を低下させてもよい。
企図される改変されたNK細胞の使用は、好ましくは、NK細胞の投与に応答する疾患の治療における、特に、種々の癌の治療におけるものである。容易に理解されるように、改変されたNK細胞は、単独の治療剤として又はより複雑なレジメンにおける薬剤として投与されてもよい。たとえば、改変されたNK細胞は、免疫療法戦略の一部であってもよく、その中で、腫瘍は、最初に、TMEに侵入する薬物(たとえばアブラキサン)により、免疫抑制を低下させる薬物(たとえばシトキサン)により、種々の免疫コンピテント細胞を刺激する薬物(たとえばALT-803)により、癌ワクチン組成物(たとえば、腫瘍新生抗原をコードする組換えAdV)により、及び/又は免疫応答を維持し且つ記憶細胞の発達を促進するのを助ける薬物(たとえば腫瘍標的IL-12)により治療されてもよい。例示的な適した治療レジメンは、本明細書において参照によって援用される国際公開第2018/005973号パンフレットにおいて論じられる。
適した投与量及び投与様式に関して、細胞の量及び注入のスケジュールは、NK細胞注入について知られている確立されたプロトコールに典型的に従うことが一般に好ましい。したがって、改変されたNK-92細胞の典型的な量は、5×107細胞/用量IV~5×108細胞/用量IVの間又は5×108細胞/用量IV~5×109細胞/用量IVの間又は5×109細胞/用量IV~5×1010細胞/用量IVの間、及び最も典型的には7×108細胞/用量IV~7×109細胞/用量IVの間(たとえば2×109細胞/用量IV)であろう。当然、改変されたNK細胞がCD16又はCD16の高親和性バリアント(たとえば158V)を発現する場合、細胞は、改変されたNK細胞に有利に結合するであろう1つ又はそれ以上の抗体と同時投与されてもよい(インビトロにおいて注入の前に又は順次、たとえば、細胞注入の前に抗体を投与する)ことを理解されたい。
以下の実施例は、代表的な手引きのみを提供し、本発明の主題を限定するものと理解されるべきではない。特に断りのない限り、すべての組換え発現構築物は、配列番号1として示されるIL-15のコドン最適化バージョンを使用し、これは、少なくともいくつかの実施形態において、高収率の組換えタンパク質をもたらした(データは示さず)。
実験の第1のセットにおいて、IL-15の3つのバリアントは、配列番号1を使用して作り出した:野生型IL-15に対応する長いシグナルペプチドのバリアント(LSP-IL-15)、ER-保持シグナルを有する長いシグナルペプチドのバリアント(erLSP-IL15)、及び代替のスプライスバリアントである短いシグナルペプチドのバリアント(SSP-IL-15)。より詳細には、一実施形態において、IL-15 LSPは、配列番号2において示されるヌクレオチド配列を有した;IL-15 LSPは、配列番号3のペプチド配列を有した;erIL-15 LSPは、配列番号4のヌクレオチド配列を有した;erIL-15 LSPは、配列番号5のペプチド配列を有した;IL-15 SSPは、配列番号6のヌクレオチド配列を有した;IL-15は、配列番号7のペプチド配列を有した。
プラスミドの設計及びトランスフェクション:

プラスミドはすべて、サンガーシークエンシングによって確認し、SalI制限酵素消化によって線状にし、カラム精製によって単離した。細胞株は、MaxCyte GTエレクトロポレーターを使用して、1μgの線状DNA/106細胞をエレクトロポレーション処理することによって作り出した(プログラムNK-92-2-OC)。pNEUkv1-CD16-IRES-[erIL-2]は、ポジティブコントロールとして同様に調製し、エレクトロポレーション処理した。この実験は、結果を確認するために、2回繰り返した。CD16及びerIL-15を同時発現するNK-92細胞株は、aNK(NK-92「野生型」)への以下の線状プラスミドのエレクトロポレーションによって作り出した:pNEUkv1-CD16(158V)-IRES-[LSP IL-15];pNEUkv1-CD16(158V)-IRES-[erIL-15];及びpNEUkv1-CD16(158V)-IRES-[SSP IL-15]。ポジティブコントロールは、pNeukv1-CD16-IRES-[erIL-2]によりトランスフェクトした。サンプルはすべて、いかなるサイトカインも存在しない状況で2週間、X-vivo-10/5%HS中でインキュベートした。
プラスミドのトランスフェクションが成功したことを決定するために、エレクトロポレーション処理された細胞を、いかなるサイトカインも存在しない状況で、2週間、5%HSを有するX-vivo-10中で成長させた。とりわけ、3つの試験したIL-15クローンのうちの1つだけが、再生に成功した:ER-保留シグナルを有する長いシグナルペプチド(erIL-15)。さらに、下記にさらに詳細に示されるように、erIL-15を発現するNK-92細胞(erLSP-IL15)は、すべての関連するNK表面マーカー及び細胞毒性を維持した。さらに、これらの細胞はまた、望ましい量の細胞外erLSP-IL15をもたらし、これは、おそらく、成熟IL-15からのer/LSPセグメントの除去の後に、免疫刺激についてのその生物学的機能を保持した。
図1からわかるように、erIL-15の発現は、aNK又はhaNK(CD16+、erIL-2)細胞と比較して、外来性サイトカインの非存在下において、erLSP-IL-15発現細胞の成長速度を変化させることはなかった。したがって、erLSP-IL-15の組換え発現は、十分な自己分泌シグナル伝達を有利にもたらし、細胞増殖を支持する。ER保持配列は、おそらく、細胞内では除去されないが、適切な機能をなお維持したので、このような結果は、予想外である。さらに、erLSP-IL-15の組換え発現はまた、改変されたN-92細胞の数多くの表現型マーカーにも悪影響を与えなかった。新たに生成された細胞株の免疫表現型検査からの例示的な結果を、図2に示す。ここで、試験したフローサイトメトリー表面受容体プロファイルは、haNK細胞、erIL2を同時発現するCD16発現NK-92細胞株と同一であった。
これに関連して、新たに作り出された細胞株が機能的に重要な表面タンパク質:CD16(Fc受容体)及び活性化受容体NKG2Dの完全な発現を維持したことを理解されたい。erIL-15発現細胞株の機能性を試験するために、また、erIL-2発現細胞株(haNK)との比較のために、細胞毒性アッセイを、様々な標的細胞株:K562、Raji、及びHL-60を使用して実行した。図3~5に示す結果は、erIL-15の導入が、haNK細胞と比較して、自発的な細胞毒性の特性を変化させなかったことを明らかに立証するものである。これは、新規な/異なる遺伝子の統合がNK-92細胞の細胞毒性機能に影響を与え得るので、予想外の発見である。とりわけ、図3~5からもわかるように、erIL-15発現細胞は、同じエフェクター細胞対標的細胞比で、erIL-2発現細胞より優れていた。
新たなerIL-15ベースのプラスミドを、CARの活性に影響を与えることなく、トリシストロン性(tricistronic)CAR発現プラスミドの中に組み込むことができるかどうかをさらに試験するために、NK-92細胞株を、CD19CAR、CD20CAR、又はCD33CARをコード化するトリシストロン性CAR_CD16_erIL-15プラスミドによるエレクトロポレーションによって生成した。erIL-15発現株の細胞毒性機能について、試験し、いくつかの標的細胞株:K562(CD19neg、CD20neg、CD33+)、SUPB15CD20(CD20+)、及びTHP-1(CD33+)に対する、対応するt-haNK細胞(erIL-2)の細胞毒性機能と比較した。図6~8は、例示的な結果を示す。ここで、上のパネルについて、K562に対する、トランスフェクトされた(トリシストロン性)細胞株の死滅は、試験したエフェクター:標的比のすべてで、優れた死滅を示す。新たな構築物が標準K562株(上のグラフ)の死滅に影響を与えないので、この結果は重要である。重要なことに、新たに作り出されたCARは、そうでなければ野生型NK-92による死滅に対して抵抗性であるCD20及びCD33発現標的細胞株を死滅させる。
さらなる実験において、erLSP-IL-15細胞の回復力及び機能について評価する。たとえば、あらかじめ凍結させたerIL-2 CD19 t-haNK及びerIL-15 CD19 t-haNKを、解凍し、次いで、サイトカインなしのX-Vivo10 5%中で成長させる。両方の細胞株は、外因的に追加されるサイトカインの非存在下において、同等の生存率及び増殖特性を有することが期待される。
さらなる実験において、IL-15及びIL-2の分泌を、測定する。ここで、IL-15の分泌は、IL-2の分泌に等しいか、又は特に、細胞が、IL-15受容体アルファ鎖をも同時発現する場合、少なくとも5%若しくは少なくとも10%若しくは少なくとも20%若しくは少なくとも40%若しくはそれ以上、IL-2の分泌より優れていることが企図される。量的には、改変されたNK=92細胞は、少なくとも100pg/ml又は少なくとも150pg/ml又は少なくとも200pg/ml又はそれ以上のIL-15を分泌するということ、及び細胞内の量(溶解物によって決定)は、少なくとも150pg/ml又は少なくとも250pg/ml又は少なくとも500pg/ml又は少なくとも750pg/ml又はそれ以上であることが期待される。
さらに、本明細書において記載される改変されたNK-92細胞が、増殖及び培養の間に、集塊又は他の凝集を呈しないことが期待される。同様に、本明細書において提示される細胞(特に、細胞がCD16(好ましくは高親和性バリアント)及び/又はCARを発現する場合)は、標準的な成長アッセイを使用すると、aNK細胞又はhaNK細胞の倍加時間と比較して、実質的に同一の倍加時間を有するであろう(すなわち、15%以下又は10%以下又は5%以下の偏差)。同様に、erLSP-IL-15を発現する細胞は、haNK細胞及びt-haNK細胞と比較して、実質的に同様の量で(すなわち、15%以下又は10%以下又は5%以下の偏差)、機能的に活性な組換えCAR及び/又はCD16を典型的に発現するであろう。
同様に、erLSP-IL-15並びに組換えCAR及び/又はCD16を発現する細胞のK562に対する非特異的細胞毒性が、aNK細胞及びhaNK細胞と実質的に同じであることが期待される。上記にすでに示されるように、erLSP-IL-15及び組換えCARを発現するNK-92細胞によるSUP-B15のCAR媒介性の細胞死滅は、SUPB15 CD20+のADCC媒介性の細胞死滅であることが期待される。
機能的観点から、本明細書において提示される、企図される改変されたNK-92細胞は、標的刺激あり及びなしで同等の又は増強されたIFNγ分泌(又はサイトカインパネルIFNγ/IL-8/IL-10/ケモカイン)を有するであろう。さらに、本明細書において提示される、企図される改変されたNK-92細胞は、フローマーカー(flow marker)(NKp30、NKp44、NKp46、DNAM-1、NKG2D、NKG2A、NKG2C、CD94、CD96、TIGIT、PD-1、PD-L1、CTLA-4、TIM-3、LAG-3、FasL、TRAIL、T-bet、Eomes、グランザイムB、パーフォリン)の同等の又は増強された発現を有するであろうということ期待される。
さらなる企図される方法において、RNA seq解析は、発現パターンを同定するために実行し、これは、他の免疫コンピテント細胞の存在及び/又は活性の中での、腫瘍内の改変されたNK-92細胞の活性及び/又は存在を調査するために使用することができる。RNA seq解析は、活性化NK細胞を実質的に代表するRNAの発現パターンを提供することが期待される。

- 前記組換え核酸が、(a)前記改変されたNK細胞を外来性サイトカインから独立させるのに、及び(b)前記改変されたNK細胞の近くの他の免疫コンピテント細胞の刺激/活性化を可能にするのに十分な量で、erLSP-IL-15の発現を引き起こすのに十分な強度を有するプロモーターを含む、請求項11~17のいずれか一項に記載の方法。
- 前記erLSP-IL-15におけるIL-15部分が、コドン最適化ヒトIL-15配列を含む、請求項11~18のいずれか一項に記載の方法。
- CD16を介して前記細胞につながれた抗体をさらに含む、請求項11~19のいずれか一項に記載の方法。
- 請求項1~10のいずれか一項に記載の遺伝子改変NK細胞と組み合わせた薬学的に許容され得るキャリアを含む医薬組成物。
- 患者への注入のために製剤化された、請求項21に記載の医薬組成物。
- 投与単位当たり少なくとも1×109細胞を含む、請求項21に記載の医薬組成物。
- 癌の治療における、請求項1~10のいずれか一項に記載の遺伝子改変されたNK細胞の使用。
- 前記治療が、TMEに侵入する薬物の投与、免疫抑制を低下させる薬物の投与、免疫コンピテント細胞を刺激する薬物の投与、癌ワクチン組成物の投与、及び/又は免疫応答の維持を助け且つ記憶細胞の発達を促進する薬物の投与をさらに含む、請求項24に記載の使用。
