Description
本発明は、off-the-shelf幹細胞および免疫細胞、それを含む薬学的組成物に関する。
癌(悪性腫瘍)は、現代社会で死亡率1位を占める主要な疾病で、現在まで多くの研究にもかかわらず、画期的な治療法がない実情である。癌の治療において、抗癌剤のような化学療法剤を用いた治療はある程度効果をあげているが、癌の多様な発症機序と抗癌剤耐性発現によって多くの研究が要求されている。
最近数十年間、診断と治療技術の発達で癌治療に対して制限的ではあるが治療率の向上と機能的保存という肯定的な結果を得たが、多くの進行性癌において5年生存率は5~50%にとどまっている。このような癌は、攻撃的侵襲、リンパ節転移、遠隔転移と二次癌の発生が特徴といえるが、一部の癌においては様々な研究と治療にもかかわらず、過去20年間の生存率が大きく変わっていない状態である。最近では、このような癌に対する分子生物学的な接近によって治療効果を高めようとする試みが多くなり、癌の増殖、転移と細胞死と関連する標的治療に対する研究が活発に進められている。
ヒトT細胞療法は、患者における癌細胞を標的化して死滅させるために、豊富化または変形されたヒトT細胞に依存する。T細胞が特定の癌細胞を標的化して死滅させる能力を増加させるために、T細胞を特定の標的癌細胞と指示する構築物を発現するようにT細胞を操作する方法が開発されてきた。特定の腫瘍抗原と相互作用することができる結合ドメインを含むキメラ抗原受容体(CAR)および操作されたT細胞受容体(TCR)は、T細胞が特定の腫瘍抗原を発現する癌細胞を標的化または死滅させることを可能とする。
このような背景下において、本発明者は免疫拒絶反応なしに安定的に癌治療に使用できる細胞毒性を示すT細胞を開発するために、鋭意研究努力した結果、B2M、CIITA、TCR、PD1およびCTLA4遺伝子の発現が抑制され、CD24およびCD19CAR遺伝子の発現水準が増進された細胞毒性T細胞(cytotoxic T cell)に効果的な癌治療活性を示すことを確認し、さらにB2M、CIITA遺伝子の発現が抑制され、CD24遺伝子の発現水準が増進された幹細胞から誘導した免疫細胞が効果的な癌治療活性を示し、NK細胞による攻撃を回避して免疫拒絶反応が少なくなることを確認することによって、本発明を完成した。

以下、本発明を詳細に説明する。
本発明は、B2M(Beta-2 microglobulin)遺伝子の発現が抑制され、CD24(cluster of differentiation 24)遺伝子を発現する分化全能性幹細胞に関するものである。
B2M遺伝子は、HLA class Iの役割を行う遺伝子であって、B2M遺伝子の発現が抑制/欠失すると、HLA class Iの活性が阻害/欠乏する。
CD24遺伝子は、新規のDon’t eat me geneであって、HLA class Iの活性が欠乏した細胞は移植後にnatural killer cellの攻撃を受けるが、前記Don’t eat me遺伝子はnatural killer cellの攻撃を防御する。
本発明の分化全能性幹細胞は、B2M遺伝子の発現が抑制され、CD24遺伝子を発現することによって免疫拒絶反応を引き起こさないことができる。
遺伝子の発現抑制は発現抑制または欠失を意味するものであって、これは当分野で公知の方法によるものであってもよく、例えばCRISPRなどを使用することができる。
CRISPRによる場合、B2M遺伝子の発現抑制のために、例えば配列番号1のsgRNAを使用することができる。
CD24遺伝子は、内在的または外因的に発現するものであり得、その発現が増加したものであり得る。外来遺伝子が導入された場合、例えばヒト由来の遺伝子であり得、具体的には配列番号6(NM_013230)の配列からなる遺伝子であり得る。
遺伝子の導入は、当分野で公知の方法によることができ、例えば電気穿孔法などを使用することができるが、これに制限されるものではない。
遺伝子の導入時には、様々なベクターを使用することができ、これは公知のプラスミド、ウイルス性ベクター、非ウイルス性ベクターなどを含む。
本発明の分化全能性幹細胞は、胚性幹細胞または誘導多能性幹細胞(iPSC)であり得る。
本発明の分化全能性幹細胞は、CIITA(class II、major histocompatibility complex、transactivator)、PDCD1(Programmed cell death protein 1)、CTLA4(cytotoxic T-lymphocyte-associated protein 4)およびTCR(T-cell receptor)からなる群から選択された少なくとも1つの遺伝子の発現がさらに抑制されたものであり得る。
CIITAは、HLA class IIの役割を行う遺伝子であって、CIITA遺伝子が抑制/欠失すると、HLA class IIの活性が阻害/欠乏する。前記遺伝子が抑制されることによって免疫拒絶反応がより抑制される。
PDCD1は、T cell inhibitory receptorの一種であって、癌細胞がT cellのPD1に結合すると、T細胞の機能が抑制される。前記遺伝子が抑制されることによって、前記幹細胞から分化した免疫細胞の活性が癌細胞によって抑制されることを防止することができる。
CTLA4は、T cell inhibitory receptorの一種であって、癌細胞がT cellのCTLA4に結合するとT細胞の機能が抑制される。前記遺伝子が抑制されることによって、前記幹細胞から分化した免疫細胞の活性が癌細胞によって抑制されることを防止することができる。
TCRは、T cell antigen recognition receptorの役割を行う遺伝子である。前記遺伝子が抑制されることによって、前記幹細胞から分化した免疫細胞が移植片対宿主病(Graft-versus-host disease、GVHD)を誘発することを防止することができる。TCRは、例えばTRA(T cell receptor Alpha)であり得る。
遺伝子の発現抑制は、発現抑制または欠失を意味するものであって、これは当分野で公知の方法によるものであってもよく、例えばCRISPRなどを使用することができる。
CRISPRによる場合、CIITAの発現抑制のために配列番号2のsgRNA、PDCD1の発現抑制のために配列番号3のsgRNA、CTLA4の発現抑制のために配列番号4のsgRNA、TCRの発現抑制のために配列番号5のsgRNAを使用することができる。
本発明の分化全能性幹細胞は、癌細胞表面抗原特異的CAR(Chimeric Antigen Receptor、キメラ抗原受容体)を発現するものであり得る。その場合、これから分化した免疫細胞が抗癌活性を示すことができる。
CARは、癌細胞表面抗原に特異的なものであって、癌細胞表面抗原の種類は制限されず、適用しようとする癌腫、その特定の癌細胞表面の抗原であり得る。例えばCD19であり得る。
CARは、適用しようとする癌細胞表面抗原に特異的な抗原結合ドメインを有するものであれば、その世代、各ドメインの種類、配置、配列などは制限されずに使用することができる。
CD19 CARの場合の具体的な例としては、抗原結合ドメインの配列はFMC63クローン配列からなるものであってもよく、例えば全配列は配列番号7の配列からなるものであってもよい。
本発明の分化全能性幹細胞は、前述の様々な組み合わせの遺伝子の発現が抑制されたものであり得、CD24および/または癌細胞表面抗原特異的CARを発現するものであり得る。
具体的な例を挙げると、B2M、CIITA、PDCD1およびCTLA4遺伝子の発現が抑制され、CD24遺伝子および癌細胞表面抗原特異的CARを発現するものであり得、ここでTCR遺伝子の発現がさらに抑制されたものであり得る。TCRは、例えばTRAであり得る。
また、本発明は、前記分化全能性幹細胞から分化した免疫細胞に関するもの

前記幹細胞から分化した免疫細胞は、前記幹細胞が発現する遺伝子を発現し、発現が抑制された遺伝子の発現が抑制されたものであり得る。これによって、免疫拒絶反応を引き起こさないことができる。
また、癌細胞表面抗原特異的CARを発現する場合、抗癌活性を示すことができる。
免疫細胞は、例えば細胞毒性T細胞(cytotoxic T cell)、NK(Natural kill)細胞または大食細胞(macrophage)であり得る。
また、本発明は、前記免疫細胞を含む癌の予防または治療用薬学組成物に関するものである。
前記免疫細胞は、癌細胞表面抗原特異的CARを発現するものであり得る。
CARに使用することができる抗原結合ドメインの種類が制限されず、適用しようとする癌腫に合う抗原結合ドメインを使用することができるため、本発明の薬学組成物は、様々な癌種に対する抗癌活性を示すことができる。一例として、固形癌または血液癌であってもよく、他の例として、膵臓癌、乳癌、前立腺癌、脳腫瘍、頭頸部癌腫、黒色腫、骨髄腫、肝癌、胃癌、結腸癌、骨癌、子宮癌、卵巣癌、直膓癌、食道癌、小腸癌、肛門付近癌、結腸癌、卵管癌腫、子宮内膜癌腫、子宮頸部癌種、膣癌腫、陰門癌腫、ホジキン病、膀胱癌、腎臓癌、輸尿管癌、腎臓細胞癌腫、腎臓骨盤癌腫、中枢神経系腫瘍などの固形癌になり得、また他の例として、白血病などの血液癌になり得る。
本発明の薬学組成物は、薬学組成物の製造で通常的に使用する薬学的に許容可能な担体、賦形剤または希釈剤を追加で含むことができ、前記担体は、非自然的担体(non-naturally occuring carrier)を含むことができる。前記担体、賦形剤および希釈剤としては、ラクトース、デキストロース、スクロース、ソルビトール、マンニトール、キシリトール、エリスリトール、マルチトール、デンプン、アカシアゴム、アルギン酸塩、ゼラチン、リン酸カルシウム、ケイ酸カルシウム、セルロース、メチルセルロース、微晶質セルロース、ポリビニルピロリドン、水、メチルヒドロキシベンゾエート、プロピルヒドロキシベンゾエート、タルク、ステアリン酸マグネシウムおよび鉱物油が挙げられる。
また、前記薬学組成物は、それぞれ通常の方法により、錠剤、丸剤、散剤、顆粒剤、カプセル剤、懸濁剤、内用液剤、乳剤、シロップ剤、滅菌された水溶液、非水性溶剤、懸濁剤、乳剤、凍結乾燥剤、経皮吸収剤、ゲル剤、ローション剤、軟膏剤、クリーム剤、貼付剤、カタプラズマ剤、ペースト剤、スプレー、皮膚乳化液、皮膚懸濁液、経皮伝達性パッチ、薬物含有包帯または坐剤の形態に剤形化して使用することができる。具体的に、剤形化する場合、通常使用する充填剤、増量剤、結合剤、湿潤剤、崩壊剤、界面活性剤などの希釈剤または賦形剤を使用して調製することができる。経口投与のための固形製剤には、錠剤、丸剤、散剤、顆粒剤、カプセル剤などが含まれるが、これに制限されない。このような固形製剤は、少なくとも1つ以上の賦形剤、例えば、デンプン、炭酸カルシウム、スクロース(sucrose)、ラクトース(lactose)、ゼラチンなどを混合して調製することができる。
また、単純な賦形剤以外にステアリン酸マグネシウム、タルクなどのような潤滑剤も使用することができる。経口投与のための液状物、リキッドパラフィン以外に様々な賦形剤、例えば湿潤剤、甘味剤、芳香剤、保存剤などを添加して調製することができる。非経口投与のための製剤には、滅菌された水溶液、非水性溶剤、懸濁剤、乳剤、凍結乾燥製剤および坐剤を含む。非水性溶剤および懸濁溶剤としては、プロピレングリコール、ポリエチレングリコール、オリーブオイルなどの植物性オイル、オレイン酸エチルのような注射可能なエステルなどを使用することができる。坐剤の基剤としては、ウイテプゾール、マクロゴール、ツイン61、カカオ脂、ラウリン脂、グリセロゼラチンなどを使用することができる。
本発明の薬学的組成物は、薬学的に有効な量で投与する。前記用語、「薬学的に有効な量」とは、医学的な治療に適用可能な合理的なベネフィット/リスクの比率であって、疾患を治療するのに十分な量を意味し、有効な用量水準は、個体の種類および重症度、年齢、性別、薬物の活性、薬物への敏感度、投与時間、投与経路および排出比率、治療期間、同時に使用される薬物を含めた要素、並びにその他の医学分野によく知られている要素に応じて決定されることができる。例えば、前記薬学組成物は、1日に0.01~500mg/kg、具体的に10~100mg/kgの用量で投与することができ、前記投与は1日1回または数回に分けて投与することもできる。
前記薬学組成物は、個別の治療剤として投与するか、または他の治療剤と併用して投与することができ、従来の治療剤とは順次的または同時に投与することができる。そして単一または多重投与されることができる。前記要素をすべて考慮して、副作用なしに最小限の量で最大の効果を得ることができる量を投与することが重要であり、これは当業者によって容易に決定することができる。
また、前記薬学組成物は、目的とする方法に応じて経口投与するか、または非経口投与(例えば、静脈内、皮下、腹腔内または局所に適用)することができ、投与量は、患者の状態および体重、疾病の程度、薬物形態、投与経路および時間に応じて異なるが、当業者によって適切に選択されることができる。
実施例
以下、本発明を具体的に説明するために実施例を挙げて詳細に説明する。
1.Wild type iPSCの製作
皮膚細胞にreprogramming geneを導入してiPSCを製作した。
具体的には、野生型iPSCを製造するために、ヒト線維芽細胞(Human dermal fibroblasts;hDF)にreprogramming gene (Oct-4、Sox-2、c-Myc、Klf-4)を発現するレトロウイルスまたはプラスミドを導入した。MOI 5-20のレトロウイルスをspinfection方法でhDFに導入した。プラスミドは電気穿孔法でhDFに導入した。Reprogramming geneが導入されたhDFを3週間一般的なiPSC培養条件で培養し、最終的に未分化iPSC群集を形成した。未分化iPSC群集をピペットを用いてメカニカルにピッキングし、継代培養によって増殖させて、その後遺伝子編集実験に使用した。本発明においては、主にレトロウイルスを用いて製作したiPSCを使用して遺伝子編集実験を進行した。使用した配列は以下の通りである。
2.B2M-/-CIITA-/-PDCD1-/-CTLA4-/-iPSCの製作
前記Wild type iPSCにnon-viral plasmidで4種のsgRNA/Cas9を電気穿孔法で導入した。
B2Mのノックアウトのために配列番号1のsgRNA、CIITAのノックアウトのために配列番号2のsgRNA、PDCD1のノックアウトのために配列番号3のsgRNA、CTLA4のノックアウトのために配列番号4のsgRNAを使用した。
15個のコロニーのうちの1つがB2M-/- CIITA-/- PDCD1-/- CTLA4-/-であることを確認し、これは遺伝子編集以後にも典型的な未分化コロニー形態を維持し(図1)、そのgenotypeは図2の通りである。
B2M-/-CIITA-/-iPSC genotypeは図3の通りである。
3.Novel Don’t eat me遺伝子のスクリーニング
Novel Don’t eat me遺伝子をスクリーニングするために、CD24、HLA-G(B2M linked)、PSG9(Pregnancy-specific beta-1-glycoprotein 9)遺伝子をレトロウイルス ベクター(pMX)にクローニングした。HLA class I null (B2M-/-)iPSC由来分化細胞にそれぞれのレトロウイルスをspinfectionで導入した(MOI=10)。qRT-PCR技法によって、対照群対比50倍以上のそれぞれの遺伝子が過剰発現されたことを確認した。この細胞をNK細胞(Natural killer cell)と反応させて細胞毒性の程度を比較した。使用した配列は以下の通りである。
これによって、従来公知のDon’t eat me遺伝子であるHLA-Gと同様の水準の機能を示すCD24遺伝子を選別した(図4)。
B2M-/-細胞(HLA class Iノックアウト細胞)は、NK細胞の攻撃を受けて死ぬことになる(Death rate=約50%)。しかし、B2M-/-細胞に伝統的なDon’t eat me遺伝子であるHLA-Gを過剰発現させると、NK細胞がHLA-Gと相互作用して該当細胞を攻撃しなくなる。従って、cell death rateが約60%に低下することになる(Death rate=約20%)。本発明者は、CD24をB2M-/-細胞に過剰発現させてHLA-Gとほぼ同じcell death減少効果を確認した。これは、NK細胞にCD24と結合するinhibitory receptorが存在するためであると判断される。
4.B2M-/- CIITA-/- PDCD1-/- CTLA4-/- CD24+ CD19CAR+ iPSCの製作
B2M-/- CIITA-/- PDCD1-/- CTLA4-/- iPSCにretroviralヒトCD24(pMX CD24;配列番号6、NM_013230)、2世代lentiviral CD19(FMC63 clone)CAR (pHR CD19 CAR;配列番号12)を導入し、24個のsingle cell由来コロニーを独立的に培養した後、CD24、CD19 CARの細胞内の導入可否をPCRで検証した。
24個のコロニーのうちの18個がCD24+ CD19 CAR+ iPSCであることを確認した(図5)。
5.B2M-/- CIITA-/- PDCD1-/- CTLA4-/- TRA-/- CD24+ CD19 CAR+ iPSCの製作
T CELL RECEPTOR(TCR)は、TRA、TRB遺伝子で構成される。B2M-/- CIITA-/- PDCD1-/- CTLA4-/- CD24+ CD19 CAR+ iPSCにTRA knockoutのためのCRISPR/Cas9を電気穿孔法で導入してTRA-/-を追加した。このために配列番号5のsgRNAを使用した。結果的に、B2M-/- CIITA-/- PDCD1-/- CTLA4-/- TRA-/- CD24+ CD19 CAR+ iPSCが製作された。
6.NK細胞のheterogenicityの確認
Novel Don’t eat me遺伝子であるCD24の機能に対するメカニズムを分析するために、PBMCとNK細胞で発現するinhibitory receptorをFACSで分析した。結果的に、PBMCとNK細胞は、HLA-Gと結合するreceptorであるCD85jを発現した。また、これとは別にCD24と結合するreceptorであるSiglec-10を発現した。まとめると、HLA class I null細胞に過剰発現されたCD24は、NK細胞のSiglec-10 receptorと結合してdon’t eat me遺伝子として機能するものと説明することができる。
Claims (13)
- B2M(Beta-2 microglobulin)遺伝子の発現が抑制され、CD24(cluster of differentiation 24)遺伝子を発現することを特徴とする分化全能性幹細胞。
- CIITA(class II、major histocompatibility complex、transactivator)、PDCD1(Programmed cell death protein 1)、CTLA4(cytotoxic T-lymphocyte-associated protein 4)およびTCR(T-cell receptor)からなる群から選択された少なくとも1つの遺伝子の発現が抑制された請求項1に記載の分化全能性幹細胞。
- 癌細胞表面抗原特異的CAR(Chimeric Antigen Receptor、キメラ抗原受容体)を発現する請求項1に記載の分化全能性幹細胞。
- B2M、CIITA、PDCD1およびCTLA4遺伝子の発現が抑制され、CD24遺伝子および癌細胞表面抗原特異的CARを発現する請求項1に記載の分化全能性幹細胞。
- TCRの発現が抑制された請求項4に記載の分化全能性幹細胞。
- 前記TCRは、TRA(T cell receptor Alpha)である請求項1に記載の分化全能性幹細胞。
- 前記癌細胞表面抗原は、CD19(Cluster of Differentiation 19)である請求項1に記載の分化全能性幹細胞。
- 請求項1~7のいずれか1項に記載の幹細胞から誘導される免疫細胞。
- 細胞毒性T細胞(cytotoxic T cell)、NK(Natural kill)細胞または大食細胞である請求項8に記載の免疫細胞。
- 請求項3に記載の免疫細胞を含む癌の予防または治療用薬学組成物。
- 前記癌は、固形癌または血液癌である請求項10に記載の薬学組成物。
- 前記固形癌は、膵臓癌、乳癌、前立腺癌、脳腫瘍、頭頸部癌腫、黒色腫、骨髄腫、肝癌、胃癌、結腸癌、骨癌、子宮癌、卵巣癌、直膓癌、食道癌、小腸癌、結腸癌、卵管癌腫、膣癌、陰門癌、ホジキン病、膀胱癌、腎臓癌、輸尿管癌、腎臓細胞癌、腎臓骨盤癌、中枢神経系腫瘍およびこれらの組み合わせで構成された群から選択された癌腫である請求項11に記載の薬学組成物。
- 前記血液癌は、白血病である請求項11に記載の薬学組成物。
