Abstract
【課題】緑内障または病因性眼圧を局所的に治療するための組成物を提供する。【解決手段】Wnt5a特異的阻害薬を含み、単位剤形の形態である、緑内障を治療するための眼科用製剤であって、該Wnt5a特異的阻害薬が、低分子干渉ペプチド、低分子阻害剤または遺伝子編集組成物である、眼科用製剤とする。前記低分子干渉ペプチドがt-ブチルオキシカルボニル修飾Wnt5a由来ヘキサペプチド(Box5)であり、前記低分子阻害剤が6,7-ジヒドロ-10α-ヒドロキシラジシコールであり、前記遺伝子編集組成物がCRISPR遺伝子編集組成物であることが好ましい。【選択図】なし

緑内障は、米国国内で300万人を超える人々、世界全体では6000万人が罹患している大きな健康問題の1つである。2040年には、世界全体で1億1180万人が緑内障に罹患すると推定されている。緑内障の主要な危険因子の1つとして、眼圧上昇(IOP)が挙げられる。眼圧上昇は視神経に障害を与える可能性があり、治療を行わなければ、永久的に失明することもある。現在、緑内障を治癒させる方法はない。既存の点眼薬または経口薬は、効果が限られており、多くの副作用を伴う。また、手術は、瘢痕化や線維化が生じて奏功しないことも多い。

房水は、前眼房および後眼房を満たす無色透明の液体である。房水は、後眼房の毛様体で産生され、前房隅角から線維柱帯とシュレム管を通る通常の経路で流出するとともに、ぶどう膜強膜流出路という副経路でも流出する。正常な眼では、房水の産生と流出の動的平衡が保たれ、IOPが正常範囲に維持されている。
シュレム管(SC)は、前眼房の虹彩角膜角に存在する輪状の管である。シュレム管は、通常の房水流出システムの一部をなしており、ヒトでは眼から流出する房水の70~90%がシュレム管を通って流出する。シュレム管の内皮細胞からなる内層は、房水流出抵抗を示す主要な部位の1つで、IOPの主要な決定因子である。年齢とともに、あるいは病的な状況下でシュレム管の抵抗が増大すると、IOPが上昇し、不可逆的な視神経障害や視力喪失を伴う緑内障へと至る。したがって、シュレム管は緑内障治療の重要なターゲットである。我々は、最近、シュレム管が、リンパ管形成の主たる制御遺伝子であるProx-1を発現していることを初めて証明した(Truong TN, Li H, Hong YK, Chen L. Novel characterization and live imaging of Schlemm’s canal expressing Prox-1. PLoS One. 2014; 9(5):e98245)。
Wnt5aはWntファミリーに属しており、Wntファミリーには、哺乳動物で同定されている複数のリガンドや複数の受容体が含まれる。
本明細書にて、我々は、Wnt5aがシュレム管で発現していること、そしてその発現量がずり応力の変化に応じて制御されることを開示する。
さらに、Wnt5aを阻害することで、in vivoで効果的にIOPを降下させることが可能であることも開示する。
本発明は、緑内障または病因性眼圧を局所的に治療する方法および組成物を提供する。
一態様において、本発明は、緑内障または病因性眼圧を治療する方法であって、治療を必要とする眼にWnt5a阻害薬を局所投与する工程を含む方法を提供する。
いくつかの実施形態において、
-前記投与工程は、点眼剤による送達、または前房内注射、結膜下注射もしくは硝子体内注射による送達を含み、
-前記阻害薬は、抗体、siRNA、低分子干渉ペプチドおよび低分子阻害剤から選択され、
-前記阻害薬は、アデノ随伴ウイルス(AAV)またはレンチウイルスなどのウイルスベクターで送達され、かつ/または
-前記投与は外用であり、前記阻害薬は、外用眼科用のゲル剤、軟膏剤、懸濁剤または液剤の形態で投与される。
別の一態様において、本発明は、緑内障または病因性眼圧を治療するための、単位剤形の形態であるWnt5a特異的阻害薬の眼科用製剤であって、該Wnt5a特異的阻害薬が抗体、siRNA、低分子干渉ペプチドおよび低分子阻害剤から選択される眼科用製剤を提供する。
いくつかの実施形態において、
-前記製剤は、眼科用潤滑剤などの外用眼科用のゲル剤、軟膏剤、懸濁剤または液剤の形態であり、
-前記剤形は、前記阻害薬を含有するコンタクトレンズ、点眼剤、デポ剤またはボーラス剤であり、
-前記製剤は、点眼剤ディスペンサーに充填されており、
-前記製剤は、前房内注射用、結膜下注射用または硝子体内注射用のシリンジに充填されており、かつ/または
-前記製剤は、眼への直接的な外用による送達に適した添加剤および/または特性をさらに含み、該添加剤および/または該特性は、眼科用に適した透明性、pH緩衝性、等張性、粘性、安定性および無菌性からなる群から選択される。
本発明には、本明細書に記載の個々の実施形態のすべての組み合わせが包含される。本発明の方法は、特定の実施形態を含む、開示されたあらゆる組成物を用いて実施してもよい。

本開示のWnt5a阻害方法は、遺伝子操作、および/または低分子干渉RNA(siRNA)、抗体、低分子などの投与でありうる。低分子干渉RNA(siRNA)、抗体、低分子などの多くは、Applied Biological Materials社(ABM、ブリティッシュコロンビア州リッチモンド)、Life Technologies社(ThermoFisher Scientific社)、Sigma-Aldrich社などの供給者から市販されている。上記方法は、眼圧の降下および緑内障の予防もしくは治療を目的として単独で使用してもよく、かつ/または緑内障の予防もしくは治療を目的として、点眼剤、投薬、レーザー、移植デバイスおよび手術などの他の治療法などと組み合わせて使用してもよい。
典型的な実施例
Wnt5aは、培養したヒト初代SC細胞での発現、またin vivoでもマウスのSCでの発現が確認されている。Wnt5aの発現は、ずり応力の変化に応じて制御されていることが、定量的リアルタイムPCRアッセイによる解析で明らかになっている。また、我々は、Wnt5aに特異的なsiRNAが、ヒトSC細胞のWnt5a発現をダウンレギュレートする作用を有するとともに、SC細胞の機能にも影響を与えることを確認している。SC特異的にWnt5a遺伝子を欠損させた条件付ノックアウトマウスでは、その同腹子であるコントロールマウスと比べて、緑内障モデルで誘導されるIOPの上昇が有意に抑制される。このノックアウトマウスと同腹子コントロールマウスとの間でベースラインのIOPに有意差はなかった。同腹子コントロールマウスでは、すべての測定時点でIOPの上昇が認められたが、Wnt5aノックアウトマウスでIOPの上昇が認められたのは初期の時点(24時間以内)のみであり、それ以降の時点ではIOPの上昇は認められなかった。このことから、Wnt5a介入により、IOPの上昇が持続できなくなることが示唆される。また、我々は、高眼圧症の制御において、wnt5a介入が、網膜神経線維層の保護およびSC透過性(上述した通常の流出システムによる水分移動を促進させるためのターゲット)の増加に有効であることも確認している(例えば、Tamら, Scientific Reports 7:40717, DOI: 10.1038/srep40717)。これらの実験は、Wnt5aが緑内障を制御するための有効な治療標的であることを示している。さらに、このような結果は、Huangら(Nature Communications, 2017; 8 (1) DOI: 10.1038/s41467-017-00140-3)の方法を用いて行ったCRISPR遺伝子編集によるWnt5aの選択的阻害によっても示されている。
次に、我々は、Wnt5a阻害薬としてのWnt5a siRNAの投与によるIOP降下効果を確認するための実験プロトコールを開発した。このプロトコールの実施に際して、Wnt5a特異的siRNAは市販のもの(ヒトWNT5A siRNA,Life Technologies社;Anastasら,J.Clin.Investig.2014,124,2877-2890)を入手した。1つのプロトコールでは、Yuenら(2014, Invest Ophthalmol Vis Sci. 2014;55:3320-3327)に記載の方法に従って、siRNAを結膜下注射する。siRNAまたはコントロールを投与するマウスをランダムに選択し、5μL(0.2lg/μL)の用量を、週2回、2週間結膜下注射する。別のプロトコールでは、Tamら(2017, Scientific Reports 7, 40717)に記載の方法に従って、siRNAを前房内注射する。マウスを腹腔内注射で麻酔して、瞳孔を拡大させる。まず、先端が平坦な引き伸ばしたガラス製マイクロ針で角膜に穴をあけて房水を抜く。穿孔後直ちに、先端が平坦な引き伸ばしたガラス製マイクロ針を10μLシリンジに取り付けて、穿孔した穴に挿入し、1μgのsiRNAを含むPBSを前眼房に1.5μL注入する。反対側の眼には、同じ濃度のスクランブルsiRNAを含むPBSを同様に1.5μL注入する。これらの実験から、Wnt5a阻害薬として結膜下注射または前房内注射により局所的に送達されたWnt5a特異的siRNAが、病因性IOPの治療に有効であることが確認された。
点眼剤として送達されたsiRNAがIOPに与える効果を評価するために、我々は、Martinezら(Mol Ther. 2014 Jan;22(1):81-91)の方法に準じて、さらなるプロトコールを開発した。ニュージーランド白ウサギに、20nmol/日のsiRNAまたはリン酸緩衝生理食塩水(PBS)を4日間連続で外用投与する。siRNAを投与した眼では、ビヒクル投与群と比較してIOPの有意な降下が認められる。IOPに対するsiRNAの効果は、初回投与から2日後の時点で確認することができ、最終投与から2日後程度までベースラインレベルを下回るIOP値が維持される。また、我々は、緑内障で観察されるような病態におけるWnt5a siRNAのIOP降下効果を評価するために、ニュージーランド白ウサギで経口水過負荷モデルを作製した。まず、4種類の用量のsiRNA(10nmol、20nmol、40nmolおよび60nmol/片眼/日)を、高眼圧を誘導する48時間前、24時間前および2時間前の合計3回投与する。いずれの投与も両眼に対して行い、IOPの測定は、高眼圧誘導前と経口過負荷後に行い、経口過負荷後の測定は20分毎に120分後まで行う。結果を解析したところ、Wnt5a siRNAは、投与したいずれの用量においてもIOPの上昇を顕著に抑えることが示された。
IOPに対するWnt5a siRNAの効果および特異性を確認するために、1群の動物数を増やし、用量を40nmol/片眼/日として4日間連続で投与する。投与4日目に水負荷により高眼圧を誘導する。コントロールの結果から、PBSを投与した動物では、高眼圧誘導から初めの1時間の間に水負荷によってIOPが上昇することが確認された。各測定時点でのIOP値を比較して解析したところ、siRNAの投与により、初めの1時間の間に、PBSを投与した動物と比較してΔIOP値が有意に低下した。スクランブル配列siRNAの投与はIOPに何ら影響を与えなかったことから、この効果は特異的な効果である。
次に、我々は、Wnt5a阻害薬としてのWnt5a特異的抗体の投与によるIOP降下効果を確認するための実験プロトコールを開発した。このプロトコールでは、ウサギ精製免疫グロブリンとして得られた抗ヒトWNT5A抗体(緩衝水溶液)(Sigma-Aldrich SAB1411396)と、マウスクローン6F2で産生された抗ヒトWNT5Aモノクローナル抗体(腹水)(Sigma-Aldrich SAB5300183)の2種類の抗体を使用するが、その他のWnt5a抗体、例えばHanakiら, Mol Cancer Ther 11(2) Feb 2012;およびHeら, Oncogene. 2005, 24 (18): 3054-3058に記載されている抗体も使用可能である。上記のマウスとウサギのモデルの両方を使用して行った実験から、Wnt5a阻害薬として点眼剤の形態で局所的に送達されたWnt5a特異的抗体が、病因性IOPの治療に有効であることが確認された。
IOPおよび緑内障のその他のパラメータ、例えば、角膜浮腫、網膜神経節細胞(RGC)死、RNFL(網膜神経線維層)の菲薄化などに対するWnt5a中和抗体の治療効果を評価するために、例示的なモデルシステムにおいて、野生型正常マウスの右眼(OD)で高眼圧を誘導し、Wnt5a中和抗体を投与した。コントロール群では各マウスの右眼のIOPが有意に上昇したが、Wnt5a抗体を投与した眼のIOPはそれより有意に低く、ベースラインレベルに維持されていた。また、光干渉断層計(OCT)を用いてin vivoで中心角膜厚を測定したところ、Wnt5a介入により、角膜浮腫が抑制されていた。IOPが上昇した後、コントロール群では角膜厚が増大したが、Wnt5a抗体を投与した眼では角膜厚の増大は認められなかった。また、Wnt5a抗体を投与した眼では、Wnt5a介入により、RGC死とともにRNFL菲薄化も抑制された。RGC死とRNFL菲薄化の抑制は、それぞれ免疫染色とOCTによって確認した。これらの結果から、緑内障マウスモデルにおいて、外用によるWnt5a抗体介入により、IOPが有意に降下し、角膜と網膜が保護されることが確認された。
次に、我々は、Wnt5a特異的なアンタゴニストペプチドと低分子阻害剤との投与によるIOP降下効果を確認するための実験プロトコールをデザインした。このプロトコールでは、Wnt5aの強力なアンタゴニストとして作用する、t-ブチルオキシカルボニル修飾Wnt5a由来ヘキサペプチド(Box5)(Jeneiら, PNAS USA, 106 (46), 19473-8)と、比較的毒性が低く、安定性に優れた強力なWNT-5A発現阻害剤である、6,7-ジヒドロ-10α-ヒドロキシラジシコール(Shinonagaら, Bioorg Med Chem. 2009 Jul 1;17(13):4622-35)とを使用する。この場合も、上記のマウスとウサギのモデルの両方を使用して行った実験から、Wnt5a阻害薬として点眼剤の形態で局所的に送達された、Wnt5a特異的な修飾ペプチドとWnt5a発現の低分子阻害剤が、病因性IOPの治療に有効であることが確認された。
本発明は、以下の発明を含む。
[1]緑内障または病因性眼圧を治療する方法であって、治療を必要とする眼にWnt5a阻害薬を局所投与する工程を含む方法。
[2]前記投与工程が、点眼剤による送達、または前房内注射、結膜下注射もしくは硝子体内注射による送達を含む、[1]の方法。
[3]前記阻害薬が、抗体、siRNA、低分子干渉ペプチドおよび低分子阻害剤から選択される、[1]または[2]の方法。
[4]前記投与が外用であり、前記阻害薬が、外用眼科用のゲル剤、軟膏剤、懸濁剤または液剤の形態で投与される、[1]、[2]または[3]の方法。
[5]緑内障または病因性眼圧を治療するための、単位剤形の形態であるWnt5a特異的阻害薬の眼科用製剤であって、該Wnt5a特異的阻害薬が、抗体、siRNA、低分子干渉ペプチドおよび低分子阻害剤から選択される、眼科用製剤。
[6]外用眼科用のゲル剤、軟膏剤、懸濁剤または液剤の形態である、[5]の製剤。
[7]前記剤形が、前記阻害薬を含有するコンタクトレンズ、点眼剤、デポ剤またはボーラス剤である、[5]または[6]の製剤。
[8]点眼剤ディスペンサーに充填されている、[5]、[6]または[7]の製剤。
[9]前房内注射用、結膜下注射用または硝子体内注射用のシリンジに充填されている、[5]、[6]、[7]または[8]の製剤。
[10]眼への直接的な外用による送達に適した添加剤および特性をさらに含み、該添加剤および該特性が、眼科用に適した透明性、pH緩衝性、等張性、粘性、安定性および無菌性からなる群から選択される、[5]、[6]、[7]、[8]または[9]の製剤。
[1]緑内障または病因性眼圧を治療する方法であって、治療を必要とする眼にWnt5a阻害薬を局所投与する工程を含む方法。
[2]前記投与工程が、点眼剤による送達、または前房内注射、結膜下注射もしくは硝子体内注射による送達を含む、[1]の方法。

[6]外用眼科用のゲル剤、軟膏剤、懸濁剤または液剤の形態である、[5]の製剤。
[7]前記剤形が、前記阻害薬を含有するコンタクトレンズ、点眼剤、デポ剤またはボーラス剤である、[5]または[6]の製剤。
[8]点眼剤ディスペンサーに充填されている、[5]、[6]または[7]の製剤。

Claims (8)
- Wnt5a特異的阻害薬を含み、単位剤形の形態である、緑内障を治療するための眼科用製剤であって、該Wnt5a特異的阻害薬が、低分子干渉ペプチド、低分子阻害剤または遺伝子編集組成物である、眼科用製剤。
- 前記低分子干渉ペプチドがt-ブチルオキシカルボニル修飾Wnt5a由来ヘキサペプチド(Box5)であり、前記低分子阻害剤が6,7-ジヒドロ-10α-ヒドロキシラジシコールであり、前記遺伝子編集組成物がCRISPR遺伝子編集組成物である、請求項1に記載の眼科用製剤。
- 前記Wnt5a特異的阻害薬が、アデノ随伴ウイルス(AAV)またはレンチウイルスなどのウイルスベクターで送達されることを特徴とする、請求項1または2に記載の眼科用製剤。
- 外用眼科用のゲル剤、軟膏剤、懸濁剤または液剤の形態である、請求項1、2または3に記載の眼科用製剤。
- 前記剤形が、前記阻害薬を含有するコンタクトレンズ、点眼剤、デポ剤またはボーラス剤である、請求項1、2、3または4に記載の眼科用製剤。
- 点眼剤ディスペンサーに充填されている、請求項1、2、3、4または5に記載の眼科用製剤。
- 前房内注射用、結膜下注射用または硝子体内注射用のシリンジに充填されている、請求項1、2、3、4、5または6に記載の眼科用製剤。
- 眼への直接的な外用による送達に適した添加剤および特性をさらに含み、該添加剤および該特性が、眼科用に適した透明性、pH緩衝性、等張性、粘性、安定性および無菌性からなる群から選択される、請求項1、2、3、4、5、6または7に記載の眼科用製剤。
