Abstract
Wnt5a 억제제의 제제를 필요로 하는 눈에 국소적으로 투여함으로써, 녹내장 또는 병원성 안압을 치료한다.
Description
본 발명은 녹내장 또는 병원성 안압을 국소적으로 치료하기 위한 방법 및 조성물에 관한 것이다.
본 발명은 미국 국립 보건원이 허여한 허가 번호 EY017392 및 EY028995 하에 정부의 지원으로 이루어졌다. 정부는 본 발명에서의 특정한 권리를 갖는다.
녹내장은 3백만명 이상의 미국인과 전세계 6천만명이 걸리는 주요 건강상의 문제이다. 2040년에는 전세계 1 억 1,180 만명이 이 질환에 걸릴 것으로 추정된다. 이 질환의 주요 위험 인자는 안압 상승(increased intraocular pressure, IOP)이며, 이는 시신경을 손상시키고 치료를 받지 않으면 영구적인 실명을 초래할 수 있다. 현재는 녹내장에 대한 어떤 치료법도 없다. 기존의 점안액 또는 경구 약물은 많은 부작용으로 인해 효능이 제한적이며, 수술은 종종 흉터 형성 및 섬유 형성과 함께 실패한다.
수양액(aqueous humor)은 눈의 전방 및 후방(anterior and posterior chambers of the eye)을 채우는 투명한 무색의 액체이다. 이 액은 섬모체에 의해 후방에서 생성되며, 섬유 주대 및 쉴렘관(Schlemm's canal)을 통한 통상적인 경로와, 포도막공막 유출(uveoscleral outflow)을 통한 비통상적인 경로를 통해서 전방각을 나온다. 정상의 눈에서는, 수양액의 생성과 배출 사이에 동적인 균형이 존재하여 IOP를 정상 범위로 유지한다.
쉴렘관 (SC)은 홍체각막각(iridocorneal angle)으로 안구 전방에 위치된 원주 채널이다. 이는 통상적인 수양액 유출계의 일부이며, 인간의 눈에서 배출되는 전체 수양액의 70-90%를 차지한다. 쉴렘관의 내피 세포 내벽은 수양액 배수에 내성이 있는 주요 부위 중 하나이며, IOP의 주요한 결정요인이다. 나이가 들거나 병리학적 상황 하에서 관의 저항이 증가하는 경우, IOP가 상승하여 되돌릴 수 없는 시신경 손상 및 시력 상실과 함께 녹내장으로 이어진다. 따라서, 이는 녹내장 치료의 중요한 목표이다. 최근에, 본 발명자들은 쉴렘관이 림프 형성의 주요 제어 유전자인 Prox-1을 발현하는 것에 관한 첫 번째 증거를 제공했다 (Truong TN, Li H, Hong YK, Chen L. Prox-1을 발현하는 쉴렘관의 새로운 특성화 및 라이브 촬상(Novel characterization and live imaging of Schlemm's canal expressing Prox-1), PLoS One. 2014; 9(5):e98245).
Wnt5a는 포유 동물에서 리간드 및 수용체를 포함하는 Wnt 패밀리에 속한다.
여기서, 본 발명자들은 Wnt5a가 쉴렘관 상에서 발현되고, 그의 발현은 가파른 스트레스 변화에 대응하여 조절되는 것을 개시한다. 또한, Wnt5a를 억제함으로써, 생체 내에서 IOP를 효과적으로 저하시킬 수 있다.

구현예에서 :

구현예에서 :
- 제제는 국소 안과용 겔, 연고, 현탁액 또는 용액, 예컨대 안과용 윤활제의 형태이고;
- 투여 형태는 억제제-탑재(loaded) 콘택트 렌즈, 점안액, 데포(depot) 또는 볼러스(bollus)이며;
- 제제는 점안액 디스펜서에 포장되고;
- 제제는 전방안내 주사, 결막하 주사 또는 안내 주사용으로 구성된 주사기에 탑재되고; 및/또는
- 제제는 부형제, 및/또는 안과적으로 적합한 선명도(clarity), pH 완충성, 긴장성(tonicity), 점도, 안정성 및 무균성으로 이루어진 군에서 선택된 바와 같이, 눈에 직접 국소적으로 전달하기에 적합한 특징을 추가로 포함한다.
본 발명은 본원에서 언급한 특정 구현예의 모든 조합을 포함한다. 방법은 특정 구현예를 포함하여 개시된 모든 조성물로 실시될 수 있다.

개시한 Wnt5a 억제 방법은 유전자 조작 및/또는 소 간섭 RNA (small interfering RNA, siRNA), 항체, 소분자 등의 투여일 수 있으며, 이들 중 다수는 Applied Biological Materials Inc.(ABM, Richmond BC), Life Technologies (ThermoFisher Scientific), Sigma-Aldrich 등과 같은 공급원으로부터 상업적으로 입수할 수 있다. 상기 방법은 안압을 낮추고 녹내장을 예방 또는 치료하기 위해 단독으로, 및/또는 녹내장을 예방하거나 치료하기 위한 점안액, 약물, 레이저, 이식 장치 및 수술 등의 다른 치료적 접근법과 조합하여 사용할 수 있다.
전형적인 실시예
Wnt5a는 배양 중인 인간 1차 SC 세포 및 생체 내 생쥐 SC에서 발현된다. 정량적 실시간(real-time) PCR 분석에 의해 분석된 바와 같이, Wnt5a 발현은 가파른 스트레스 변화로 조절된다. 또한, 본 발명자들은 인간 SC 세포에서 Wnt5a 발현이 SC 세포 기능에도 영향을 주는 Wnt5a-특이적 siRNA에 의해서 하향 조절될 수 있음도 입증한다. SC-특이적 Wnt5a 유전자에 있어서, 녹내장 모델에서 유도된 조건부 유전자 결핍 생쥐(conditional knockout mice)의 IOP 상승은 한배 새끼 대조군(control littermates)과 비교하여 상당히 감소된다. 유전자 결핍 생쥐와 한배 새끼 대조군 간의 기준선 IOP에서 유의미한 차이는 발견되지 않았다. 연구한 모든 시점에서 IOP 상승을 갖는 한배 새끼 대조군과 비교하여, Wnt5a 유전자 결핍 생쥐는 초기(24 시간 이내)에만 상승된 IOP를 보였지만, 이후의 시점에서는 그렇지 않았으며, 이는 Wnt5a 개입으로 지속 불가능한 IOP 증가를 나타낸다. 또한, 본 발명자들은 wnt5a 개입이 망막 신경 섬유층의 보호 및 SC 투과성의 증가에 효과적인 것도 입증하며, 이는 안구 고압을 관리하는 통상적인 유출 시스템을 통한 수성 이동의 향상을 위한 표적이다(예, Tam et al., Scientific Reports 7:40717, DOI: 10.1038/srep40717). 이들 실험은 Wnt5a가 녹내장 관리에 효과적인 치료 표적임을 입증한다. 이러한 결과는 Huang 등의 방법을 채용하는 CRISPR 유전자 편집에 의한 Wnt5a의 선택적 억제에 의해 추가로 입증된다(Nature Communications, 2017; 8 (1) DOI: 10.1038/s41467-017-00140-3).
다음에, 본 발명자들은 IOP를 감소시키기 위해 Wnt5a siRNA 억제제 치료의 효능을 입증하는 실험 프로토콜을 개발했다. 이들 프로토콜을 위해, Wnt5a-특이적 siRNA를 상업적으로 입수했다(human WNT5A siRNA, Life Technologies; Anastas, et al. J. Clin. Investig. 2014, 124, 2877-2890). 하나의 프로토콜에서, siRNA의 결막하 주사는 Yuen 등, 에 기재한 바와 같이 실행되었다(2014, Invest Ophthalmol Vis Sci. 2014;55:3320-3327). 생쥐를 무작위로 선택하여, 2주 동안 일주일에 2회 5 uL (0.2 lg/uL) siRNA의 결막하 주사를 맞혔다. 제2 프로토콜에서, siRNA의 전방내 주사는 Tam 등,에 기재한 바와 같이 실행되었다(2017, Scientific Reports 7, 40717). 복강내 주사로 생쥐를 마취시키고, 동공을 확장시킨다. 먼저, 당김식 뭉툭단 미세 유리 바늘(pulled blunt-ended micro-glass needle)을 사용하여 각막에 구멍을 뚫고 수양액을 빼낸다. 천공 직후, 10 ㎕ 주사기에 부착된 당김식 뭉툭단 미세 유리 바늘을 천공을 통해 삽입하고, 1 ㎍ siRNA를 함유하는 1.5 ㎕의 PBS를 전방(anterior chamber) 내에 투여한다. 반대쪽 눈에는 동일한 농도의 스크램블된 siRNA를 함유하는 1.5 ㎕의 액으로 동일한 주사를 맞힌다. 이들 실험은, 결막하 주사 또는 전방안내 주사에 의해 국소적으로 전달된 Wnt5a-특이적 억제제 siRNA가 병원성 IOP에 대한 효과적인 치료법임을 입증한다.
점안액에 의해 전달된 siRNA가 IOP에 주는 영향을 평가하기 위해, 본 발명자들은 Martinez 등의 방법에 기반한 추가 프로토콜을 개발했으며(Mol Ther. 2014 Jan;22(1):81-91), 여기서 뉴질랜드 흰 토끼는 연속으로 4일의 기간에 걸쳐 20nmol/일의 siRNA 또는 인산 완충 식염수(phosphate-buffered saline, PBS)를 국소적으로 투여받았다. 처리된 눈은 비히클-치료된 군과 비교할 때, 상당한 IOP 감소를 나타낸다. IOP에 대한 siRNA의 효과는 첫 번째 투여 2일 후에 검출될 수 있으며, 마지막 투여 후 ~2 일까지 기저 수준 아래로 값이 유지된다. 또한, 본 발명자들은 녹내장에서 관찰된 것과 같은 병리학적 조건에서 Wnt5a siRNA의 IOP- 저하 효과를 평가하기 위해 경구 수분 과적 모델(oral water overloading model)을 뉴질랜드 흰 토끼에 적용했다. 처음에 4가지 상이한 용량의 siRNA(10 nmol, 20 nmol, 40 nmol 및 60 nmol/눈/일)를 고혈압 유도 48시간, 24시간 및 2시간 전에 총 3회 투여했다. 모든 치료는 양쪽 눈에서, 고혈압 유도 전 및 경구 과적 후 매 20분에서 120 분까지 측정된 IOP에 적용된다. 결과 분석은, Wnt5a siRNA가 시험한 모든 용량에서 IOP의 상승에 대해 상당한 보호를 제공함을 입증한다.
IOP 상에서 Wnt5a siRNA의 효능 및 특이성을 확인하기 위해, 더 많은 군의 동물들을 연속 4일 동안 40 nmol/눈/일의 용량으로 처리하고; 넷째 날에, 수분 적하에 의해 안구 고압을 유도한다. 대조군 결과는, 수분 적하가 PBS로 처리된 동물에서 고혈압 유도 후 처음 1시간 동안 IOP의 증가를 유발했음을 입증한다. 각 시점에서 IOP 값을 비교함으로써 수행한 분석은, siRNA에 의한 치료가 PBS-치료 동물과 비교하여 처음 1시간 내에 ΔIOP 값을 상당히 감소시켰음을 나타낸다. 스크램블된 서열 siRNA에 의한 치료는 IOP에 영향을 주지 않으므로, 그 효과는 특이적이다.
다음에, 본 발명자들은 IOP를 감소시키기 위해 Wnt5a-특이적 항체 억제제 치료의 효능을 입증하는 실험 프로토콜을 개발했다. 이들 프로토콜은 다른 Wnt5a 항체(예, Hanaki et al., Mol Cancer Ther 11(2) Feb 2012; He et al. Oncogene. 2005, 24 (18): 3054-3058)를 사용할 수 있지만, 2가지 상이한 항체: 토끼 정제 면역 글로불린에서 생성된 항-인간 WNT5A 항체, 완충 수용액(Sigma-Aldrich SAB1411396); 및 생쥐 클론 6F2에서 생성된 항-인간 WNT5A 모노클로날 항체, 복수 액(ascites fluid) (Sigma-Aldrich SAB5300183)을 채용한다. 생쥐 및 토끼 모델(상기)을 사용하고, 이들 실험은, 점안액에 의해 국소적으로 전달된 Wnt5a-특이적 항체 억제제가 병원성 IOP에 대한 효과적인 치료법임을 입증한다.
예시적인 모델 시스템에서, 야생형 정상 생쥐의 우안(OD)에 안구내 고압을 유도했고, IOP 상에서의 그들의 치료 효과 및 각막 부종(corneal edema), 망막 신경절 세포(retinal ganglion cell, RGC) 사멸 및 RNFL 얇아짐(thinning)을 포함한 녹내장의 다른 파라미터를 평가하기 위해 Wnt5a 중화 항체를 투여했다. 대조군과 비교하여, 생쥐의 우안에서 IOP가 유의하게 상승했고, Wnt5a 항체 치료된 눈에서 IOP는 상당히 저하되어 기준선 수준으로 유지되었다. Wnt5a 개입은 OCT에 의해 생체 내 중심 각막 두께로 측정한 바와 같이, 각막 부종을 감소시켰다. IOP가 증가된 후, 대조군에서 각막 두께의 증가가 관찰되었지만, Wnt5a 항체 처리된 눈에서는 관찰되지 않았다. Wnt5a 개입은 처리된 눈에서 RNFL 얇아짐(RNFL thinning)뿐만 아니라 RGC 사멸을 감소시켰다. 이들은 면역 염색 및 OCT에 의해 각각 검출되었다. 이들 결과는, 국소적인 Wnt5a 항체 개입이 녹내장의 생쥐 모델에서 IOP를 유의하게 저하시키고 각막 및 망막을 보호하는 것을 확인하였다.
다음에, 본 발명자들은 IOP를 감소시키기 위해 Wnt5a-특이적 길항제 펩티드 및 소분자 억제제 치료의 효능을 입증하는 실험 프로토콜을 설계했다. 이들 프로토콜은 Wnt5a의 강력한 길항제로서 기능하는(Jenei, et la., PNAS USA, 106 (46), 19473-8) t-부틸옥시카보닐-변형 Wnt5a-유도 헥사펩티드 (Box5), 및 비교적 낮은 독성과 우수한 안정성을 갖는 강력한 WNT-5A 발현 억제제인 6,7-디히드로-10알파-히드록시 라디시콜을 채용한다(Shinonaga et al. Bioorg Med Chem. 2009 Jul 1;17(13):4622-35). 다시 생쥐 및 토끼 모델(상기)을 사용하면, 이들 실험은, 점안액에 의해 국소적으로 전달된 Wnt5a-특이적 변형 펩티드 억제제 및 Wnt5a 발현의 소분자 억제제가 병원성 IOP에 대한 효과적인 치료법임을 입증한다.
Claims (10)
- 녹내장 또는 병원성 안압을 치료하는 방법으로서, Wnt5a 억제제를 필요로 하는 눈에 국소적으로 투여하는 단계를 포함하는 방법.
- 제1항에 있어서, 상기 투여 단계는 점안액에 의한, 또는 전안방내, 결막하 주사, 또는 안내 주사에 의한 전달을 포함하는 방법.
- 제1항 또는 제2항에 있어서, 상기 억제제는 항체, siRNA, 소 간섭 펩티드 및 소분자 억제제에서 선택되는 것인 방법.
- 제1항, 제2항 또는 제3항에 있어서, 상기 투여는 국소적이며, 상기 억제제는 국소 안과용 겔, 연고, 현탁액 또는 용액의 형태로 투여되는 방법.
- 항체, siRNA, 소 간섭 펩티드, 및 소분자 억제제로부터 선택되는 Wnt5a-특이적 억제제의 안과용 제형으로써, 녹내장 또는 병원성 안압을 치료하기 위한 단위 투여 형태의 제제.
