神経セロイドリポフスチン症(NCL)は、希少でかつ遺伝性の神経変性障害の群である。それらは、最もよく見られる神経遺伝性貯蔵症であると見なされており、自家蛍光性脂肪色素に似たセロイド及びリポフスチンの蓄積が患者において見られる。NCLは、筋肉の緊張または痙攣の異常な増加、失明または視覚の問題、認知症、筋肉協調運動の欠如、知的障がい、運動障害、発作、及び不安定な歩行を含めた、変動的であるが進行性の症状を伴う。この疾患の頻度は、12,500個体あたりおよそ1人である。主な3つのタイプのNCLがある:成人型(クーフス病またはパリー病);若年型;及び遅発性乳児型(ジャンスキー-ビールショウスキー(Jansky-Bielschowsky)病)。神経セロイドリポフスチン症(NCL)は、もともと、彼らの発症の年齢及び臨床症状(本明細書において記される)によって定義された。しかしながら、それらは、その後より新しい分子知見に基づいて再分類されており、それは、臨床表現型によって以前に示唆されていたよりも、種々の遺伝的バリアントに対するはるかに多くの重複の証拠を提供している。

AAV.hTPP1coを、インビトロでの機能的TPP1の発現について試験した。HEK293細胞を6ウェルプレートに播種した。約90%コンフルエンスに達した時点で、細胞に5μgのAAV.hTPP1coベクターまたはDNAなしをトランスフェクトした。72時間後、細胞培養物の上清を収穫し、TPP1活性/酵素アッセイのために処理した。

ソソーム蓄積及び脳における炎症、広範囲にわたる軸索変性)、及び寿命の短縮(約6ヶ月)を含めた、ヒトにおけるCLN2病を反映する。Tpp1tm1Plobマウスモデルは、数々の臨床病期プログラムの前臨床試験プログラムに組み入れられている(ClinicalTrials.gov識別番号:NCT00151216、NCT01414985、NCT01161576;Passini et al.,2005;Passini et al.,2006;Sondhi et al.,2007;Sondhi et al.,2008)。しかしながら、Tpp1tm1Plobマウスモデルは市販されておらず、ゆえに、AAV9.CB7.hCLN2に対する前臨床試験プログラムに組み入れられるのは実現可能ではなかった。
ーロンにおける好酸性貯蔵物質、皮質におけるグリオーシス、及び脳の多数の領域における広範囲にわたるアストロサイトーシスを含めた、神経病理の数々の測定が観察された。5.5ヶ月齢の時点で、脳全域の広範囲にわたるかつ著しいアストロサイトーシスに加えて、錐体皮質ニューロン及び脳幹大型ニューロンにおいて貯蔵病理が著しかった。貯蔵物質は、特に脳幹において、蛍光灯(染色なし)の下で容易に明らかであった。
1×1011GCのAAV.hTPP1coベクターを注射された5匹のTPP1 KOマウスの体重を、注射後の約14日ごとにモニターした(図11に示される線によってつながれた閉じた四角形)。PBSのみを注射された3匹のTPP1 KOマウスが対照として働いた(図11における線によってつながれた閉じた丸)。ゆえに、調査の間に死んだ第四のTPP1 KOマウスはここから除外された。注射後4日目の時点での体重は、処理されたマウスと対照との間で同等であったが、一方で1×1011GCのAAV9.CB7.CI.hCLN2co.RBGでの処理は、KO雌における体重増大を助長するように見えた。
PBS(図10、KO+PBS、n=4)または1×1011GC AAV9.CB7.CI.hCLN2co.RBG(図10、KO+1e11、n=5)をICV注射されたTPP1 KO雌の血清におけるTPP1酵素活性を測定した。実施例3に記載される
、注射なしの(図10、WT、n=2)または1×1011GC AAV9.CB7.CI.hCLN2co.RBG(図10、WT+1e11、n=2)をICV注射された野生型雄マウスを、精確な比較のために同時に試験した。
PBSまたは1×1011GC AAV9.CB7.CI.hCLN2co.RBGでのICV注射の60日後、動物を安楽死させ、脳、肺、肝臓、筋肉、及び腎臓などの組織を収穫した。脳サンプルを切片化し、さらなる組織学的分析のために採取した。

PBS(n=4)または1×1011GCのAAV9.CB7.CI.hCLN2co.RBG(n=5)をICV注射されたTPP1 KO雌は、下記のプロトコールに従ってロッキングロータロッド試験を受けた。

PBS(n=4)または1×1011GCのAAV9.CB7.CI.hCLN2co.RBG(n=5)をICV注射されたTPP1 KO雌は、下記のプロトコールに従って加速式ロータロッド試験を受けた。
実験を実施して、様々な用量でのAAV.hTPP1co注射の効力及び毒性を査定した。TPP1ノックアウト(KO)マウス及び野生型マウスを、食糧/水が自由に提供される12時間の明/12時間の暗サイクルで維持した。1ヶ月齢のTPP1 KOマウスに、3×109(低用量)または3×1011(高用量)GCの、上で記載されるAAV.hTPP1coを脳室内投与により注射した。TPP1 KO及び野生型マウスにPBSも注射し、対照として働いた。各群は、10匹の雄及び10匹の雌動物を含有した。試験されたマウスの生存及び体重をモニターし及び記録した。脳、CSF、血清、及び肝臓からのサンプルのTPP1酵素活性を測定した。リポフスチン自家蛍光、及びGFAP発現によって査定されるアストロサイトーシスなどの脳組織学を評価した。運動協調性アッセイなどの機能試験及び行動試験を実施した。さらに、抗TPP1免疫応答も評価した。
生存を毎日モニターし、動物を、それらが重量の20%を喪失した場合に安楽死させた。生存曲線を図4Aにプロットし、一方で各試験群に関する生存週中央値を算出し、図4Bに列挙した。高用量で処理された動物の死亡は、1匹の雌を例外として、観察期間中に検出されなかった。低用量で処理されたすべての動物及び未処理KOは死んで発見された。雄KOマウスの生存週中央値は15であり、一方で雌KOマウスは、24週の生存時間中央値を示した。3×109GCのAAV.hTPP1coで処理されたKOマウスにおいて、雄の生存期間中央値は16週であり、一方で雌の1匹は19週であった(図4B)。
NCLはCNS疾患であり、ニューロンの死は運動機能に影響を及ぼす。疾患表現型を評価するために、マウスを、実施例4に記載されるようにロータロッドで運動協調性について試験した。マウスを、1ヶ月齢から3週間ごとにこのプロトコールで訓練した。ロータロッド上で過ごした時間で秒で測定される落下するまでの潜時を記録した。療法の90日後に開始する試験の3日目における試行3の結果を図6にプロットした。未処理と高用量で処理されたKOとの間に有意な差が観察され、一方で、WTと高用量で処理されたKOとの間で同じ身体能力が観察された。これらの結果は、高用量療法が、TPP1 KOマウスにおける疾患表現型を補うことを立証した。
上で記載されるAAV.hTPP1coの毒性及び酵素補正を評価するために、TPP1に対する免疫応答を評価した。ICV注射後70日の時点で、本実施例において上で記載される3.3ヶ月齢のマウスから血を抜き取った。産生された血清を抗TPP1抗体の存在について試験して、TPP1タンパク質に対する免疫応答を評価した。
温で2時間のインキュベーション後、バッファーBでの5回の洗浄をプレート洗浄器で実施した。
種々の臓器におけるTPP1酵素活性を評価するために、ならびに罹患組織(大部分は脳)における酵素補正を査定するために、TPP1酵素活性を、実施例2及び4に記載されるように測定した。実施例4に記載されるように、皮質、ならびに側脳室(LV)、海馬、及び視床の一部を含む区画(図9においてLV区画と名前を付されている)などの脳組織を収穫し、切開し、及び処理した。TPP1活性は、3ヶ月齢の雄及び雌TPP1 KOマウスにおいて観察されず、ゆえに、KOにおいて残存TPP1が完全にないことを示すベースラインとして用いられ、一方で1ヶ月齢の野生型マウスは、正常でかつ検出可能なレベルのTPP1活性を示した(図8)。野生型マウスならびにPBSのみを注射されたKOにおいて、小脳及び肝臓を含めた他の臓器において、同様の結果が観察された(図9)。比較のために、上で記載される3×1011GCのAAV.hTPP1coで処理されたTPP1酵素活性も測定し、図9にプロットした。試験された3つの異なる臓器において(LV区画、小脳、及び肝臓)、高用量ベクターで処理されたKOは、野生型よりも多くのTPP1活性を提供し、一方でPBSを注射されたKOのTPP1活性レベルは低いままであり、ゆえに図9に示されるすべてのサンプルから差し引かれ、hAAV.hTPP1coで処理されたKOマウスにおける細胞内酵素補正の成功及び健常な表現型が立証された。
脳におけるAAV.hTPP1coのICV注射の生物活性及び効力を査定するために、試験されたマウスの脳サンプルを、活性化アストロサイト/アストロサイトーシスのマーカーであるGFAPに対して免疫組織化学により染色した。画像は示されず。1ヶ月齢の野生型マウスの皮質及び視床の両方ともGFAPを示さなかったが、KO動物の対応するサンプルは、1ヶ月齢の時点でアストロサイトーシスを有することが観察され、TPP1変異に起因したニューロンにおける脂質の蓄積によってアストロサイトが活性化され、そのようなアストロサイトーシスの進行は1ヶ月齢またはそれよりも早くに開始することが示された。KOマウスにおいて、成長すると、アストロサイトーシスは増加し続けた。3ヶ月齢の時点で、さらに上昇したレベルのGFAPが、KOマウスにおける皮質及び視床の両方において検出された。図4に示される生存曲線において、KO動物の死亡は3.5ヶ月齢で開始することが観察され、それは、NCL患者及び動物モデルの脳の種々の部分において観察される活性化アストロサイトとニューロンの死との間の報告されるつながりと合致している。ゆえに、このニューロン喪失が、KOにおいて観察される振戦及び運動協調性異常という疾患表現型を付与した。この結論は、実施例4に記載されるようにPBSを注射され及び死んで発見された3.5ヶ月齢のKOマウスの側脳室に近接した海馬区画におけるGFAP免疫染色により観察された大規模なアストロサイトーシスによってさらに支持された。
簡潔には、マスターセルバンク(MCB)からのHEK293細胞に3種のプラスミドをトランスフェクトして、パッケージされたベクターゲノムを産生する:CLN2発現カセットを含有するプラスミド、ならびにAAV9カプシドタンパク質及び複製エレメントをコードする2種のヘルパープラスミド。ウイルスベクター産物を含有する細胞培養上清を収穫し、浄化し、タンジェンシャルフロー濾過(TFF)によって濃縮し、その後にアフィニティー及びアニオン交換(AEX)クロマトグラフィーが続く。AEX画分をプールし、TFFを繰り返して、精製された原薬(bulk drug substance)(BDS)を産生し、最終製剤バッファーで濃度を調整し(希釈またはTFFによる濃縮)、その後に滅菌濾過及びバイアル内への充填が続くことによって、そこから薬物製品が産生される。AAV9.CB7.hCLN2 BDS及び最終薬物製品(FDP)の製造工程は、安全性、同一性、品質、純度、ならびにBDS及びFDPの効能を確保するための現行適正製造基準(cGMP)を用いて、受託製造機関(複数可)(CMO)で実施される。
細胞培養及び収穫製造工程は、4つの主な製造ステップ:細胞の播種及び拡張、一過性のトランスフェクション、ベクター収穫、ならびにベクター浄化、を含む。これらの工程ステップは、概略工程フロー図に描かれている。

提案される調査では、AAV9.CB7.hCLN2を、2×1010GC/g脳質量~最大実現可能用量(MFD)の7.5×1011GC/g脳質量(解剖学的制約に起因して)の用量レベル域で、4週齢のマウスにICV注射により投与する。動物の半分を、対照動物において予想される神経病理及び死亡と一致する注射後60日の時点で屠殺し、TPP1酵素活性、リソソーム貯蔵物質の蓄積及び貯蔵病理、ならびにアストロサイトーシスを含めた生物活性について評価する。動物の残りを、疾患表現型(臨床観察時の振戦及び/または歩行異常によって規定される)の発症までの時間について評価し、対照動物において100%の死亡率が予想される時点を超える注射後最高で210日間の長期生存について追跡する。
AAV9.CB7.hCLN2に対する提案される前臨床試験プログラムは、CLN2病のTpp1m1jマウスモデルにおいて行われるハイブリッド薬理学/毒物学調査を含む。複数の用量レベルでICV注射によりAAV9.CB7.hCLN2を投与されたマウスを、以下のように複数の時点で安全性について評価する。

当であると見なされる、なぜなら、i)C57Bl/6マウスは、薬理学査定において用いられるTpp1m1jマウスモデルのバックグラウンド系統であり;ii)マウスにおけるICV注射及び大型動物におけるIC注射の後の、AAV9ベクターに基づく産物の生体内分布及び導入遺伝子発現プロファイルの同等性が確立されており;iii)健常マウスにおいて安全性査定(例えば、連続出血)を実施する実現可能性は、Tpp1m1jマウスなどにおける動物取扱懸念または環境刺激誘発性致死性発作に対する感受性によって限定されず;ならびにiv)健常マウスにおけるGLP準拠の安全性/毒物学調査の実行は、実現可能であり、十分に確立されており、かつ統計学的に堅牢であるためである。
CLN2病を治療するための、CNSにおける増加するTPP1酵素活性に対する科学的論拠
CLN2病に対する治療としてTPP1酵素活性の増加を標的にすることに対する科学的論拠は、CLN2病の治療におけるERT、及びBrineura(登録商標)の投与を用いた前臨床及び臨床経験によって支持される(Katz et al.,2014;Brineura(登録商標)、FDA承認審査概要;Brineura(登録商標)EPAR;Schulz et al.,2016)。永久埋め込み型デバイスを介したBrineura(登録商標)の側脳室への隔週注入は、CLN2病を有する患者における運動機能を安定させるとFDAによって判定され、一方でEMAは、言語技能にもプラスの影響があると判定した。Brineura(登録商標)のCSF及びリソソーム半減期は、それぞれ7時間及び11.5日間であると推定され;結果として、上昇したTPP1酵素活性レベル(及びおそらく臨床的有益性)を維持するためにBrineura(登録商標)の反復注入が必要である。ゆえに、ERTの反復投与に伴う重い患者負担及び罹患率なく、CNSにおいて永続性がありかつ長期にわたるTPP1酵素活性を提供し得る新しい療法に対する満たされていない必要性が残る。
CNSを標的にし得るAAVベクターに基づく産物の能力は、単一遺伝子CNS疾患の数々のモデルにおいて立証されており、第一世代のAAVベクターに基づく産物を評価するいくつかの早期ヒト試験は、脳へのベクター送達の安全性を示している(Bartus

during a single administration procedure via intracistemal(IC) injection)」と題されたFIH臨床試験において。当該試験は、2つのパートから構成される。
2つの患者集団を自然病歴コホートと比較する。これら2つの集団を組み合わせる主な理由は、一般的なCLN2病の希少性、及びBrineura(登録商標)の近年の承認後のIT ERTナイーブの患者のさらなる低下に起因して、試験登録を促すためである。
TPP1活性の大幅でかつ持続的な増加は、神経行動学的機能における長期安定化を予測する可能性が高い。種々の量のTPP1を発現するマウスCLN2変異体を評価した調査からのデータに基づき、正常の3~6%のTPP1酵素活性レベルが、疾患の発症を劇的に遅らせ及び生存率を増加させるのに十分であるように見える(Sleat et al.,2008)。CLN2病を有する患者からの付加的な臨床データは、CLN2におけるある特定の変異が、TPP1酵素活性の不完全な喪失(少なくとも末梢組織における)及びその後長引く表現型につながることを示唆する(Sleat et al.,1999;Bessa et al.,2008;Schulz et al.,2013)。さらに、永久埋め込み型デバイスを介したCSFへの隔週注入として投与される、組み換えTPP1(Brineura(登録商標)、セルリポナーゼアルファ、BioMarin Pharmaceuticals)を用いたERTは、i)FDAによる96週間の調査期間にわたる運動機能の衰退;ならびにii)EMAによる48週間の調査期間にわたる運動及び言語機能の両方の衰退、を安定化させると判定された。AAV9.CB7.hCLN2は、CNSにおけるTPP1の効率的でかつ持続的な発現のために設計され、CFSへのAAV9.CB7.hCLN2の1回限りの送達は、TPP1酵素の長期供給源を提供し、ならびに満たされていない高い必要性を有するこの希少遺伝子疾患における疾患の進行及び神経認知の衰退を止める(または大幅に遅らせる)潜在性を有する。ゆえに、臨床データが、臨床上意義のあるCRSにおける安定化のより長期にわたる向上を示唆する、早期の時点でのCSFにおけるTPP1酵素活性の大幅でかつ継続的な測定結果を示す場合、将来的な販売申請におけるAAV9.CB7.hCLN2の効力への主要な支持を提供するこれらのデータの妥当性がFDAとともに議論される。
Tpp1m1jマウスにおけるパイロット用量域調査(調査#W2553)を含めた、いくつかのインビトロ及びインビボ前臨床研究及びパイロット調査を行って、臨床候補を選択し、AAV9.CB7.hCLN2の生物活性に関する予備的データを回収した(研究及びパイロット調査はINDに要約される)。調査#W2553では、1ヶ月齢のTpp1m1jマウスに、7.5×109GC/g脳質量(低用量;n=10/性別)または7.5×1011GC/g脳質量(高用量;n=10/性別)のいずれかの用量レベルで合計5μlの容量のAAV9.CB7.hCLN2をICV注射により投与した。ノックアウト(n=10/性別)及び野生型対照(n=10/性別)動物には、PBSをICV注射により投与した。動物を、注射後30週間、疾患の進行及び生存率についてモニターし;臨床観察、体重、ロータロッドに基づく神経行動学アッセイ(運動協調性及び運動学習)、ヒトTPP1導入遺伝子産物に対する液性免疫応答、脳及び肝臓におけるTPP1酵素活性、アストロサイトーシス、基質のリソソーム蓄積、ならびに生存率を含めた、安全性及び生物活性の予備的測定結果を複数の時点で回収した。

得るといういくつかの証拠があるが(Davidoff,2003)、とはいえこのパイロット調査における変動的なデータは、AAV9.CB7.hCLN2の生体内分布プロファイルまたは免疫原性における任意の性別特異的差異よりも、動物におけるヒトタンパク質に対する免疫応答の評価への課題(ならびに、ヒト臨床シナリオへのそれらの解釈可能性及び適用可能性における課題)をより例示する可能性が高い。それにもかかわらず、任意の潜在的な性別特異的差異、具体的には生物活性または免疫原性に対する任意の効果を、計画されたINDにより可能となるハイブリッド薬理学/毒物学調査においてさらに評価する。
Tpp1m1jマウスモデルにおいて行われたパイロット調査では、AAV9.CB7.hCLN2をICV注射により投与した。この投与の経路は、計画された臨床的IC投与経路とは異なるが、しかしながら、IC注射はTpp1m1jマウスにおいて実現可能ではない。IC注射処置の間、小容量のCSFを引き抜いて、CSFコンパートメントにおける圧力を減少させ及び大槽における正しい針の配置を確認する必要がある。マウスにおける比較的少ないCSF容量に起因して、正しい針の配置は、CSFを引き抜くことによって確認され得ない。ゆえに、注射前に筋膜と筋肉とを解離して硬膜を曝露する必要がある。この外科的処置は時間がかかり、マウスにおける処置誘発性運動機能障害及び/または病変の危険性を提示する。
al.,(2013)によって公開された調査では、IC(n=1)またはICV(n=2)注射のいずれかによる2×1013vg/動物のAAV9.GFPベクターのCSF送達後のイヌにおいて、AAV9ベクター生体内分布及びGFP mRNA発現が比較された。CNS及び体細胞器官におけるAAV9ベクター生体内分布及び導入遺伝子発現プロファイルについての比較分析は、いずれかの送達の経路を用いたベクター形質導入プロファイルの大きな差を示唆しなかった。
AAV9.CB7.hCLN2に対する提案される前臨床試験プログラムは、CLN2病のTpp1m1jマウスモデルにおいて行われる、INDにより可能となるハイブリッド薬理学/毒物学調査を含む。一般的に、ハイブリッド調査設計は、臨床背景における製品の安全性及び生物活性に影響を与え得る、標的患者集団の局所微小環境及び病態生理学的状態を再現する疾患背景における試験品目の安全性及び生物活性についての評価を容易にする。結果として、ハイブリッド調査において作出されるデータは、健常動物において行われる調査から獲得されるデータよりも関連している場合が多い。ゆえに、提案されるハイブリッド薬理学/毒物学調査から作出されるデータは、安全でかつ効力のある初回臨床用量レベルの選択に情報を与えるのに役立つ。
1)疾患の動物モデル:Tpp1m1jマウスモデルは、CLN2病の生物学的に関連したマウスモデルである。
2)試験品目:意図される臨床用製品及び製剤(バッファー/希釈剤)を評価する。
3)用量レベル域:2×1010GC/g脳質量~7.5×1011GC/g脳質量の値域内にある複数の用量レベルを評価して、最小有効用量(MED)及び無毒性量(no-observed-adverse-effect-level)(NOAEL)を特定する。パイロット調査において、7.5×109GC/g脳質量の用量レベルでAAV9.CB7.hCLN2を投与されたTpp1m1jマウスにおいて、生存率または生物活性の他の測定において有益性があるようには見えなかった。ICV投与経路(ROA)を利用するTpp1m1jマウスにおけるMFDは、7.5×1011GC/g脳質量である。
4)脳室内(ICV)投与経路(ROA):IC及びICV経路間の類似性についての詳細な説明に関しては第3.4.2.5節をぜひ参照されたく、それは、計画された臨床シナリオへのマウスにおけるICV経路の適用可能性を支持する。IC注射処置の重大な安全性は、IND#17565の下で行われたNHP調査において以前に確立された。
5)予定された屠殺及び調査継続期間:Tpp1m1jマウスモデルにおける疾患表現型の発症(疾患発症は、歩行異常、振戦、発作、及び突然死によって特徴付けされる)、ならびに予想される神経病理及び早期死亡とそれぞれ一致するように、注射後60日及び90日の時点における予定された屠殺を選択した。自然病歴データに基づき、いかなるTp
p1m1j対照マウスも、注射後210日の時点における最終的な予定される屠殺までは生存しないと予想される。
6)薬理学/生物活性評価項目:生物活性を評価する定量的評価項目には、疾患表現型(臨床観察時の歩行異常及び/または振戦によって規定される)の発症までの時間、注射後60日の時点でのアストロサイトーシス補正、体重変化(週1回測定される)、注射後60日の時点での脳組織におけるTPP1酵素活性、及び注射後210日までの生存率、が含まれる。剖検(注射後60日)の時点で、リソソーム貯蔵補正の定性的組織学、ならびに大脳、小脳、肝臓、及び血清におけるTPP1酵素活性は、生物活性についての付加的な測定結果を提供する。パイロット調査からのデータは、神経行動学的測定(ロータロッドに基づくアッセイ)が、Tpp1m1jマウスモデルにおける利用性が限定されていることを示し;ゆえに、これらの査定はこの調査の設計に組み入れられていない。

8)免疫原性査定:液性免疫応答(hTPP1に対する血清抗体)を、ベースラインならびに注射後14、60、及び210日の時点で評価する。マウスにおける導入遺伝子産物に対する液性免疫応答は、NHPにおいて観察される応答と相関することが示されている(データ示さず)。しかしながら、細胞性免疫応答(すなわち、インターフェロンガンマT細胞ELISPOTアッセイ)は、アッセイをランするほど十分なPBMCを全血から単離することは技術的に実現可能でないため、マウスにおいてそれほど情報を与えない。この理由のために、マウスにおけるELISPOTアッセイは、脾細胞を用いて実施される必要があるが、しかしながら、NHPから単離された脾細胞におけるT細胞応答の評価を含む以前の調査からのデータは、同じNHP由来のPBMCにおいて獲得されたデータと相関しなかった。ゆえに、T細胞免疫応答の評価は調査設計に組み入れられない。

C57Bl/6マウスにおける潜在的GLP安全性/毒物学調査
AAV9.CB7.hCLN2に対する提案される前臨床試験プログラムは、CLN2病のTpp1m1jマウスモデルにおいて行われるハイブリッド薬理学/毒物学調査を含む。最高でMFDまでの複数の用量レベルでICV注射によりAAV9.CB7.hCLN2を投与されたマウスを、以下のように、生存中の複数の時点及び予定された屠殺の時点で安全性について評価する。

限られたCSFコンパートメント内の標的CNS組織へのベクターの直接的でかつ非侵襲的な送達を可能にする、AAV9.CB7.hCLN2に対する投与の経路(ROA)として大槽内(IC)注射を選択した(Hinderer et al.,2014b;Hinderer et al.,2017;IND#17565)。AAV9ベクター
に基づく産物の静脈内送達も、CNS内の細胞を標的にすることが示されているものの、CSFへの送達は、より低いベクター用量レベルで及び末梢臓器のより低い形質導入で、脳におけるより効率的な形質導入を達成することが示されている(Nathwani et al.,2011;Gray et al.,2013;Hinderer et al.,2014;Hinderer et al.,2014b)。
標的患者集団におけるIC注射後のAAV9.CB7.hCLN2の可能性のある生体内分布プロファイルは、類似のAAV9ベクターに基づく産物に対して作出された既存の生体内分布データから情報を得ている。例えば、類似のAAV9ベクターに基づく産物の生体内分布は、IC注射後最高で2年間、ネコ、イヌ、及びNHPを含めた複数の種において包括的に特徴付けされており、生体内分布についての最も信頼できる査定は、FDA/CBER/OTATの投入で設計されたGLP準拠のNHP調査(PTS#PS002653及びPTS#PS002919)において行われた。要約すると、これらの産物は、多様な動物種におけるIC注射の後、脳及び脊髄において広く分布し及び持続することが示された。複数の用量レベルを評価した場合、ベクター組織レベルはほぼ用量依存的であった。肝臓及び脾臓へのベクター分布は、概して、脳において観察されたものと同じぐらい高く、ときにはそれよりも高く;限定されたデータは、CNS発現に対する観察可能な効果はなかったものの、AAV9に対する既存の力価が末梢組織ベクター発現を低下させ得ることを示唆する。CNS及び肝臓以外の組織におけるベクター濃度は、調査内及び調査間の両方で非常に変動的であったが、評価されたすべての組織がベクターを含有した。さらに、ベクター発現は、NHPにおける生体内分布についての最も信頼できる査定において、投与後最高で6ヶ月間、意義があるほどに変化しなかった。
臨床開発プログラム及び計画された中核的臨床試験の概略
AAV9.CB7.hCLN2に対する提案される臨床開発プログラムは、2つのパートで行われる単一試験を含む(図26;図27)。詳細な梗概は第4節に提供されている。
ERTを中断するかを決定することにおいて同じ工程を経験する。安全性審査要因(SRT)が観察されない場合(表35)、第二の対象に対する最高で8週目の来院までかつ8週目の来院を含んで獲得されたすべての安全性データはIDMCによって評価される。任意の事象が停止規則の基準を満たす場合、すべての安全性データについての完全な審査が実施されるまで、任意の新たな対象の投薬は保留される。投薬コホートの終わりの時点で計画されるまたはSRTによって要求されるかどうかにかかわらない任意の所定のIDMC会議で、IDMCは、試験を停止する、現在の用量で付加的な対象に投薬する、次の投薬コホートに進む、またはより低い用量で進むことを勧告し得る。リアルタイムで、投薬された各対象の安全性及び忍容性を以下のように査定する。
得、疾患の進行を反映しない可能性があり;ゆえに、治療的介入(例えば、発作医薬の変更)及び付加的な試験を実施して、CRSスコアにおける可逆性を査定し得る。
・とりわけ遺伝子産物の完全な欠如をもたらし得る遺伝子変異を有する患者において、TPP1に対する潜在的に過剰な免疫応答を阻止するまたは最小限に抑えるため。そのような免疫応答は、導入遺伝子を発現する組織に対する過敏性反応または免疫介在性反応の危険性を増加させ得る。
・AAV9.CB7.hCLN2及び/またはIT ERTの生物活性を潜在的に低下させ得る、TPP1に対するNAbの形成またはTPP1に対するNAbの増加(既存のNAbを有する可能性が高いERT中の患者における)を阻止するため。

002);ならびにii)「レスキュー」IT ERT注入を有しない対象、として定義される応答者としてカテゴリー分けされる対象の割合を含めた、効力についての複数の測定が評価される。2組の患者集団(すなわち、IT ERTナイーブ及びIT ERT治療された)が、パートIIの拡張コホートにおいて計画され、IT ERTナイーブの対象は72週間追跡され、IT ERT治療された対象は80週間追跡され、組み合わせた2つの患者集団を自然病歴コホートと比較する。これら2つの集団を組み合わせる主な理由は、一般的なCLN2病の希少性、及びBrineura(登録商標)の近年の承認後のIT ERTナイーブの患者のさらなる低下に起因して、試験登録を促すためである。IT ERT治療された患者における任意の残存ERTの潜在的な影響、ならびにAAV9.CB7.hCLN2についての査定に対する潜在的交絡効果に起因して、「ウォッシュアウト」期間が調査設計に組み入れられる。CSF及び血漿におけるBrineura(登録商標)に関するPKデータは、CSF及びリソソームにおけるそれぞれおよそ7時間及び11.5日間の半減期を有して、明らかな蓄積またはPK時間依存性がないことを示唆し(Brineura(登録商標)EPAR)、ゆえに、IT ERTの中断後最高で4週間という提案される「ウォッシュアウト」期間は適当である。AAV9.CB7.hCLN2高用量群における組み合わせたIT ERTナイーブの対象及びIT ERT治療された対象と、外部の自然病歴対照群における最も近い1-1でマッチした対象とを比較することによって、応答者分析を用いて分析を行う。マクネマー正確検定を実施して、マッチした集団を分析する。自然病歴対照群からの1-1でマッチした対象を、ベースラインCLN2スコア、±6ヶ月齢、及び共通の遺伝子型に基づいて選択する。1つを上回る数のマッチが生じる場合、以下の順序(1)詳細な遺伝子型及び(2)性別で、付加的な基準に対してさらにマッチさせることによって選択を狭める。この分析を、IT ERTナイーブの対象に対して72週の時点で、及びIT ERT治療された対象に対して80週の時点で実施する。ベースラインCRS(組み合わせた運動及び言語ドメイン)スコアは、AAV9.CB7.hCLN2群に対するAAV9.CB7.hCLN2注射前である。自然病歴対照群におけるマッチした対象に対する72週を通じたデータを分析に用いる。
Brineura(登録商標)中核的調査における対象は、実演査定のみを通じて前向きに臨床医によって評定された、という評定の異なる方法;ならびにii)CLN2 CRS尺度が、自然病歴調査及びBrineura(登録商標)中核的調査において、それぞれおよそ12週間及び8週間ごとに査定された、という査定の異なるスケジュール、の両方をめぐるものであった。本発明者らは、機関の審査から、自然病歴調査及びBrineura(登録商標)中核的調査において用いられたCLN2尺度への評定の相違があったように、映像比較可能性調査からの知見も、CLN2運動-言語総スコアにおける言語項目の使用を支持しなかったと理解する。言語項目とのより大きな不一致があることが観察された。
・一貫性のある言語ドメイン評定により、各調査内及び調査間の運動-言語総スコアの解釈及び直接比較が容易になり得るように、自然病歴調査において用いられる言語ドメイン評定についての元の記述テキストを用いる。
・評価は、複数の独立した盲検検査官による、映像査定の複数の無作為審査を組み入れ、各対象に対する複数の審査のうちの1つのみが、正しい/本当の時系列で完了する(他の審査は、映像査定の無作為に並べ替えた順序で実施される(及び破棄される))。このようにして、縦断的でかつ個体間のスコアリングが、いかなる潜在的バイアスも導入せずに、同じ検査官によって実施される。
FDAは、希少疾患に対する薬物開発を促す複数の機会を提供し、産業界に対するFDAドラフトガイダンス:Expedited Programs for Serious Conditions-Drugs and Biologies-2014において欠損遺伝子の標的化に具体的に対処している。このガイダンスは、希少遺伝子疾患に対する製品の登録を支持するのに十分であり得る評価項目に関連したいくつかの洞察も提供し、「一部のよく理解された酵素欠損に関して、欠損した酵素の補充は臨床的有益性を確実に予測する」と述べている。これは、導入遺伝子発現活性を確実に示す遺伝子療法が、21 CFR 314、subpart Hの下での加速した承認のための基準を満たし得ることを示唆する。さらには、この主張に基づき、欠損した酵素レベルが公知の病因に
つながり、かつそれらの補正または正常化が、臨床上意義のある効果と関連付けられる疾患に対して、早期承認の評価項目に対する論証がなされ得る。
バッテン病の一形態であるCLN2病は、可溶性酵素トリペプチジルペプチダーゼ-1(TPP1)をコードする遺伝子における変異によって引き起こされる神経変性リソソーム貯蔵障害である。当該疾患は、小児期中期までの死亡前に、発作及び運動失調を有する、2~4歳での早期発症を特徴とする。組み換えTPP1(Brineura(登録商標)、BioMarin Pharmaceuticals)を用いた酵素補充療法(ERT)が近年承認され、永久埋め込み型デバイスを介した側脳室への隔週注入として投与されている。組み換え酵素の短い半減期に起因して、反復注入が必要である。ゆえに、反復
投与に伴う重い負担なく、患者において持続的なTPP1酵素活性を提供し得る療法に対する重大な満たされていない必要性が残る。
TPP1m1JKOマウスは、右脳室に3×1011GCのAAV9-hCLNcoの単回投薬を受け;ビヒクルを注射されたWT同腹仔及びTPP1m1Jマウスを対照として用いた。記録は処理の2週間後に開始し、屠殺または予定外の死亡まで続いた。ICV注射後16週の調査終結時に、剖検を実施し、脳及び肝臓をサンプリングした。
