動物
C57BL/6系統において作製されたオトフェリンノックアウト(Otof-/-)マウス(Roux I. et al, Cell, 127, 277-289 (2006))は、均一なFVB遺伝的バックグラウンドを得るために、FVBマウスと10世代以上戻し交配された。これは、C57BL/6バックグラウンドとは異なり、このバックグラウンドは生後10か月以内の安定した聴覚閾値に関連しているためである(Kommareddi, P., et al. J Assoc Res Otolaryngol 16, 695-712 (2015))。組換えAAV2ベクターは、FVB遺伝的バックグラウンドにおけるOtof-/-マウスに送達された。すべての手順及び動物の取り扱いは、Institut National de la Sante et de la Recherche Medicale、パスツール研究所及びNIHの福祉ガイドラインに準拠し、カリフォルニア大学サンフランシスコ校で承認されたプロトコル要件に準拠した。手術前に、塩酸ケタミン(Ketaset、100mg/kg)、塩酸キシラジン(Xyla-ject、10mg/kg)及びアセプロマジン(2mg/kg)の混合物の腹腔内注射により、マウスを麻酔した。つま先をつまむことに対する深部組織の反応をモニターすることにより、麻酔深度をチェックした。手術前及び手術後24時間ごとに一週間、マウスにカルプロフェン(2mg/kg)を皮下注射して炎症及び痛みを軽減させた。手術後の苦痛及び異常な体重減少の兆候について動物をモニターした。

1x非必須アミノ酸、10%ウシ胎児血清(Gibco)及びペニシリン-ストレプトマイシン(Pen/Strep、Invitrogen)を補充したダルベッコ改変イーグル培地中でポリリジンコーティングカバースリップ上の6ウェルプレートにおいてHEK293細胞を増殖させた。翌日、以前に記載されたように細胞を感染させた(Lopes V.S. et al, Gene Ther 20, 824-833 (2013))。簡単に説明すると、AAV2-Otof組換えウイルス(各ベクターに10000ゲノム含有粒子/細胞)のいずれか1種又は両方を含む無血清培地200μL中で、75%コンフルエンスの細胞を含むカバースリップを37℃、5%CO2でインキュベートした。2時間後、1mLの完全培地を添加した。翌日、培地を交換し、細胞をさらに48時間インキュベートした。細胞を4%パラホルムアルデヒドの0.1Mリン酸緩衝生理食塩水(PBS)溶液、pH7.4により、4℃で2時間固定し、PBSにより3回リンスし、0.25%TritonX-100及び5%正常ヤギ血清を含むPBSにより室温で1時間インキュベートした。次いで、細胞を、オトフェリンのN末端部分(アミノ酸196~211)及びC末端部分(アミノ酸1848~1978)に対してそれぞれ作製された、既に特徴付けられているウサギポリクローナル抗体、14cc及びC19(Roux I. et al, Cell, 127, 277-289 (2006))(希釈1:200)とともに4℃で一晩インキュベートした。サンプルをPBSにより2回リンスし、Cy3コンジュゲートヤギ抗ウサギIgG二次抗体(Life Technologies、希釈率1:2000)を含む液と室温で2時間インキュベートした。次いで、サンプルをPBSにより2回リンスし、4’,6-ジアミジノ-2-フェニルインドール(DAPI)により染色して細胞核を可視化し、Fluorsave溶剤(Biochem Laboratories、フランス)の滴下によりスライドガラスにマウントし、オリンパス共焦点免疫蛍光顕微鏡により観察した。
以前に記載されたように、ウイルスを左蝸牛に送達した(Akil O. et al, Neuron 75, 283-293 (2012))。麻酔をかけたOtof-/-マウスに対して、正円窓膜を介してP10、P17又はP30の蝸牛の鼓室階にAAV2-Otofベクターペアを注射した。耳には背側切開を介してアプローチした(Duan M et al, Gene Ther 11 Suppl 1, S51-56 (2004))。18Gの針によってブラ(bulla)に小さな穴を開け、鉗子によって拡張した。正円窓膜をホウケイ酸毛細管ガラスピペットにより穏やかに穿刺し、次いで、それを取り除いた。外リンパの流出が止まったら、AAV2-Otof NT(6.3x1012vg/mL)及びAAV2-Otof CT(4.5x1012vg/mL)のベクターのペアを含む固定容量(2μL)を、細いガラスマイクロピペット(外側の先端の直径は10μm)により鼓室階に1分間かけて注入した。ピペットを引き抜いて、隙間を筋膜及び脂肪組織により迅速に塞いだ。創傷は、6-0吸収性クロミック縫合糸(Ethicon)により層状に縫合された。
麻酔をかけたOtof+/+マウス、Otof-/-マウス及びレスキューされたOtof-/-マウスに、以前に記載されたように、防音チャンバー内において、様々なタイムポイントで聴覚検査を実施した(Akil O. et al, Neuron 75, 283-293 (2012))。塩酸ケタミン(Ketaset、100mg/mL)及び塩酸キシラジン(Xyla-ject、10mg/mL)の混合物の腹腔内注射によりマウスに麻酔をかけ、次いで、必要に応じて初期用量の5分の1を注射した。体温を加熱パッドにより維持し、記録中は直腸プローブによりモニターした。聴性脳幹反応(ABR)は、TDT BioSig IIIシステム(Tucker Davis Technologies)と、マウスの頭皮に配置された3つの皮下針電極(頭頂に1つ、左耳の耳介の下に1つ(参照電極)及び反対側の耳の下(接地電極))とを使用して記録された。音刺激は、クリック音(5ms持続時間、31Hz)と8kHz、16kHz及び32kHzにおけるトーンピップ(10ms持続時間、コサイン二乗形(cosine squared shaping)、21Hz)とであった。それらは無響条件で伝達され、左耳から片耳の応答が記録された(記録の間、右耳はブロックされた)。各音刺激について、脳波(EEG)活動は、フィルタリング(0.3~3kHz)を使用して25kHzのサンプリングレートにおいて20ミリ秒間記録された。512刺激及び1000刺激のEEG波形は、それぞれクリック音及びトーンバーストに対して平均化された。試験された最大強度(86dB SPL(音圧レベル))から5dB音圧レベル(SPL)間隔で音刺激強度を減少させた。目視検査時にI~V波のABRピークが明確且つ繰り返し存在する最低刺激レベルとして、聴覚閾値を定義した。保存された波形のオフライン分析によって、これらの閾値の評価を確認した。ABR I波の潜時は、クリック音刺激とI波のピーク振幅との間の時間間隔として測定された。さらに、レスキューされたOtof-/-マウスと野生型マウスとの比較に関して、ABRトレースにおけるI波ピーク振幅の値を、対照の野生型マウスの平均値(100%と見なす)に対して正規化した。
マウス蝸牛を4%パラホルムアルデヒドの0.1M PBS(pH7.4)溶液により灌流し、同じ固定液中で4℃2時間インキュベートした。蝸牛をPBSにより3回リンスし、5%エチレンジアミン四酢酸(EDTA)の0.1M PBS溶液と4℃で一晩インキュベートすることによりカルシウムを除去した。蝸牛感覚上皮(コルチ器)を表面標本に顕微解剖し、0.25%TritonX-100及び5%正常ヤギ血清を含むPBS(ブロッキングバッファー)により室温で1時間プレインキュベートし、一晩4℃で一次抗体によりインキュベートした。以下の抗体:ウサギ抗オトフェリンC末端部分(C19、1:250希釈)1、マウス(lgG1)抗CtBP2/リブアイ、マウス(lgG2a)抗グルタミン酸受容体サブユニットA2(ミリポア、1:200希釈)及びウサギ抗GFP(Invitrogen、A11122;1:250希釈)を使用した。サンプルをPBSにより3回リンスし、適切な二次抗体:Alexa Fluor488標識抗マウスlgG1、Alexa Fluor568標識抗マウスlgG2a(Life Technologies、1:1000希釈)又はAtto Fluor647標識抗ウサギIgG(Sigma、1:200希釈)とインキュベートした。サンプルをPBSにより3回洗浄し、細胞核を染色するためにDAPIを使用して、1滴のFluorsaveによりスライドガラス上にマウントした。高解像度の対物レンズ(開口数1.4、60x油浸対物レンズ)を備えたLSM 700共焦点顕微鏡(Zeiss、Oberkochen、ドイツ)によって、コルチ器の蛍光共焦点zスタックを取得した。0.2μmのzステップにより512x512又は1024x1024ラスター(x及びyにおいてピクセルサイズ=0.036μm)で画像を取得した。最大20の共焦点画像のzスタックの3Dレンダリングによって、オトフェリン及びシナプスリボンを生成する内有毛細胞(IHC)を計数した。オトフェリン導入遺伝子を発現するIHCの割合を計算するために、オトフェリンを産生するIHCの総数を、DAPI染色された細胞核によって特定されたIHCの総数により割った(最低150回の連続分析)。
6匹のOtof+/+マウス及びP10においてレスキューされた6匹のOtof-/-マウスの左蝸牛から、トータルmRNAを抽出した(Trizol、Invitrogen)。逆転写(RT)は、oligo dTプライマー及びsuperscript II RNaseH-(Invitrogen)を使用して42℃で50分間行った。2マイクロリットルのRT反応生成物を35サイクル(94℃で30秒間、60℃で45秒間、72℃で60秒間)及び72℃で10分間の最終伸長からなるポリメラーゼ連鎖反応(PCR;TaqDNAポリメラーゼ、Invitrogen)に使用した。オトフェリンcDNA(GenBankアクセッション番号NM_001100395.1)の、AAV-Otof NTインサートとAAV-Otof CTインサートとの間の接合部を包含する2676bp中間フラグメント(ヌクレオチド27~2702)を増幅するように、PCRプライマーのペア(フォワードプライマーTGTCTCAGAGCTCCGAGGCA(配列番号14)及びリバースプライマーATCGTGGAGGAGGAACTGGGCA(配列番号15))を設計した。PCR産物を、0.5mg/mL臭化エチジウムを含む2%アガロースゲルでの電気泳動により精製し(Qiaquickゲル抽出キット、QIAGEN)、配列決定し(Elim Biopharmaceuticals)、オトフェリンcDNA配列との配列同一性を確認した。
データを平均±標準偏差(SD)として表す。ノンパラメトリックなマンホイットニーU検定により、すべての統計分析を実施した。統計的有意性は、図に以下のように示されている。n.s.、有意差なし;*、p<0.05;**、p<0.01;***、p<0.001。
キメラCMV-ニワトリβ-アクチンプロモーターの制御下で緑色蛍光タンパク質(GFP)遺伝子を発現するように、AAV2ベースのベクターを設計した。この発現カセットは、AAV2キャプシドの4つの表面チロシン(Y)残基がフェニルアラニン(F)残基に置換されている、AAV2 quadY-Fキャプシドにパッケージされており、網膜における遺伝子導入の効率を高めることが示されている(Petrs-Silva H. et al, Molecular therapy : the journal of the American Society of Gene Therapy 19(2):293-301 (2011))。組換えウイルスは、P2において5匹の野生型マウスの左蝸牛に正円窓膜を通じて注射された。注射の3週間後の感覚上皮のGFP免疫染色により、IHCを含む様々なタイプの細胞の形質導入を明らかにした。IHCの形質導入率は78±6%(平均±SD)であり、これらの細胞に治療遺伝子を送達するためのこのAAV血清型の適合性を実証した(図示せず)。マウスオトフェリンcDNAのコード配列を5’フラグメント(Otof NT、ヌクレオチド1~2448)と3’フラグメント(Otof CT、ヌクレオチド2449~5979)とに分割し、それらのそれぞれを組換え誘導性のブリッジ配列を有するAAVベクターに挿入した。(Ghosh A. et al, Hum Gene Ther 22(1):77-83 (2011); Dyka FM et al, Hum Gene Ther Methods 25(2):166-177 (2014))。AAV-Otof NTの組換えベクターはcDNAの5’部分に続いてスプライスドナー部位を有し、AAV-Otof CTの組換えベクターはスプライスアクセプター部位に続いてcDNAの3’部分を有する(方法及び図1を参照)。これらの組換えベクターのそれぞれは、AAV2 quadY-Fキャプシドにパッケージされた。HEK293細胞をAAV-Otof NT、AAV Otof CT又はこれらの両方の組換えウイルスに感染させ、48時間後にオトフェリンに対して免疫染色を行った。タンパク質のC末端部分又はN末端部分に対して作製された2種の異なる抗体を使用して(Roux I, et al, Cell 127:277-289 (2006))、同じ結果が得られた。オトフェリンは両方のウイルスに同時に感染した細胞でのみ検出されたため、2種のベクターが組換えされ、逆方向末端反復を介してコンカテマーを生じ、得られた転写産物を正しくスプライシングしてタンパク質を産生することができたことを示した(図1)。
Claims (12)
- DFNB9難聴に罹患している患者を治療するために、又はDFNB9変異を有する患者においてDFNB9難聴を予防するために使用するための、内有毛細胞における全長オトフェリンポリペプチド又はその機能的フラグメントの発現を可能にするベクターシステムであって、前記患者が、発達及び成熟した聴覚系を有するヒト、例えば、新生児、幼児、小児、ティーンエイジャー又は成人である、前記ベクターシステム。
- 前記オトフェリンポリペプチドが配列番号1の配列を有する、請求項1に記載の使用のためのベクターシステム。
- 前記全長オトフェリンポリペプチド又はその機能的フラグメントをコードするポリヌクレオチドを含む少なくとも1種のAAV粒子を含む、請求項1に記載の使用のためのベクターシステム。
- 前記ベクターシステムが、少なくとも2種のAAV粒子を含み、それらのうち、i)1種は、オトフェリンポリペプチド若しくはその機能的フラグメントのN末端部分をコードする部分的コード配列を含むポリヌクレオチドを含み、且つ、ii)別の1種は、オトフェリンポリペプチド若しくはその機能的フラグメントのC末端部分をコードする部分的コード配列を含むポリヌクレオチドを含む、請求項1に記載の使用のためのベクターシステム。
- 前記ベクターシステムが、少なくとも2種のAAV粒子を含み、
前記AAV粒子のそれぞれが、
a)前記ポリヌクレオチドの各末端に逆方向末端反復を含むとともに、前記逆方向末端反復の間に5’から3’に向かって、適切なプロモーターと、それに続く、オトフェリン遺伝子のN末端部分を含む部分的コード配列と、スプライスドナー部位とを含む第1のポリヌクレオチド、又は
b)前記ポリヌクレオチドの各末端に逆方向末端反復を含むとともに、前記逆方向末端反復の間に5’から3’に向かって、スプライスアクセプター部位と、オトフェリン遺伝子のC末端部分を含む部分的コード配列と、場合により、それに続くポリアデニル化配列とを含む第2のポリヌクレオチド
のいずれかを含み、
前記第1及び第2のポリヌクレオチドが、それぞれ、前記第1のポリヌクレオチド中の前記スプライスドナー部位の後に位置する組換え誘導配列、及び、前記第2のポリヌクレオチド中の前記スプライスアクセプター部位の前に位置する組換え誘導配列を含み、且つ、
前記第1及び第2のポリヌクレオチド中の前記コード配列が、組み合わさると、前記全長オトフェリンポリペプチド又はその機能的フラグメントをコードする、請求項1に記載の使用のためのベクターシステム。 - 前記オトフェリン遺伝子が配列番号2の配列を有する、請求項5に記載の使用のためのベクターシステム。
- 前記オトフェリン遺伝子の前記N末端部分が配列番号3の配列であり、前記オトフェリン遺伝子の前記C末端部分が配列番号4の配列である、請求項5に記載の使用のためのベクターシステム。
- 前記AAV粒子がAAV2血清型である、請求項3に記載の使用のためのベクターシステム。
- チロシンアミノ酸残基をフェニルアラニンアミノ酸残基に置換することによってキャプシドが修飾されているAAV2粒子を含む、請求項1に記載の使用のためのベクターシステム。
- 前記ヒト患者が、言語習得後にDFNB9難聴であると診断されている、請求項1に記載の使用のためのベクターシステム。
- 前記患者が、温度感受性変異、好ましくは、P.Q994VfsX6、P.I515T、p.G541S、PR1607W、p.E1804delから選択される温度感受性変異によって誘発されるDFNB9難聴に罹患しているティーンエイジャー又は成人である、請求項1に記載の使用のためのベクターシステム。
- 請求項1~9のいずれか一項に記載のベクターシステムと、薬学的に許容可能なビヒクルとを含む、DFNB9難聴に罹患している患者を治療するために、又はDFNB9変異を有する患者においてDFNB9難聴を予防するために使用するための医薬組成物であって、前記患者が、発達及び成熟した聴覚系を有するヒト、例えば、新生児、幼児、小児、ティーンエイジャー又は成人である、前記医薬組成物。
