Abstract
Description
a)脂質マトリックスを有する、カルボプラチン、シスプラチン、オキサリプラチン、またはネダプラチン等の白金薬剤の群から選択される少なくとも1つの抗腫瘍剤を含み、PEG修飾賦形剤等の1つ以上の賦形剤および/または担体を任意に含む、一連の密封包装された単回治療用量の吸入用乾燥粉末と、
b)一連の使い捨て乾燥粉末吸入装置と、を含み、
i)密封包装された単回治療用量それぞれは、閉鎖状態および開放状態を有し、
ii)使い捨て乾燥粉末吸入装置それぞれは、閉鎖位置および任意で開放位置を有し、
iii)密封包装された単回治療用量それぞれは、前記一連の使い捨て乾燥粉末吸入装置のうちの1つの使い捨て乾燥粉末吸入装置に閉鎖状態で収容されるように提供され、
iv)使い捨て乾燥粉末吸入装置それぞれは、密封包装された単回治療用量それぞれが前記開放状態になる前またはひとたび前記開放状態になると、前記閉鎖位置に存在するように設けられている、吸入化学療法用キットである。
簡潔に言えば、塊状の粉末(Shanghai Jinhe Bio-technology Co., Ltd., Shanghai, PRC)からの未処理シスプラチン微粒子を、最初に、50mLのイソプロパノールに懸濁させ、5%w/vの懸濁液を得た。次に、高速(24,000rpmにおいて10分間)(X620モーターとT10の分散シャフト、Ingenieurburo CAT M. Zipperer GmbH, Staufen, Germany)および高圧での均質化(EmulsiFlex-C5高圧ホモジナイザー、Avestin Inc., Ottawa, Canada)(各10回のプレミリングサイクルにおいては、5,000psiの次に10,000psi;20回のミリングサイクルにおいては、20,000psi)によって、懸濁液のサイズ減縮を行った。熱交換器を均質化バルブに接続し、F32-MA冷却サーキュレータ(Julabo GmbH, Seelbach, Germany)を用いて-15℃に維持した。レーザー回折によってシスプラチン微粒子の粒径分布(PSD)を測定するために、プロセスの最後にアリコートを懸濁液から除去した(下記参照)。
シスプラチン(Umicore, Hanau-Wolfgang, Germany)を、最初に50mLのエタノールに懸濁させ、5%w/vの濃度の懸濁液を得た。次に、高速(24000rpmにおいて10分間)(X620モーターとT10分散シャフト、Ingenieurburo CAT M. Zipperer GmbH, Staufen, Germany)、およびEmulsiFlex-C3高圧ホモジナイザー(Avestin Inc., Ottawa, Canada)を用いた高圧での均質化(40サイクルにおいて20,000PSI)により、懸濁液のサイズ減縮を行った。これらのサイクルは、処理された懸濁液を試料タンク(閉ループ)に直接再循環させることによって行った。HPHはサンプル温度を増加させるので、すべての操作はサンプル温度を15±1℃に維持しながら、均質化バルブの前方に配置された熱交換器を使用して行った。
動物試験はすべて、医学部(プロトコル424Nと585N)およびマイクロスコープ・分子イメージングセンター(プロトコルCMMI 2017 01)のULBの倫理委員会によって承認された。
同所性肺内移植手順は、以前に記載されたプロトコルの適合を使用して実施した(Wauthoz et al., Nanomedicine 2015)。簡潔に言えば、麻酔下で、6~7週齢のBALB/cマウスを、皮膚の切開および皮下筋肉の切開後に、140万個のM109-HiFR-LUC2細胞を含む、葉酸を含まないRPMI/Matrigel(登録商標)の50:50v/v混合物20μLを、肋間に注射することによって移植した。異なる組織切開部を3-0シルク(Ethicon(登録商標)、St-Stevens-Woluwe, Belgium)で縫合した。完全に回復するまでマウスを念入りに観察した。シスプラチンに対するM109モデルの治療反応を、インビボ光学イメージング(生物発光)および生存分析によって評価した。生物発光は、生きているM109-HiFR-LUC2腫瘍細胞の数に正比例する。生物発光は、PhotonImager RT(Biospace Lab, France)を使用して、従来の供給元のプロトコルを使用して、腫瘍細胞移植後19日目まで行った。マウスの体重を週3回測定し、実験開始時から体重減少が20%を超えた場合、または最後の測定時から体重減少が15%を超えた場合、またはマウスが呼吸困難になった場合に屠殺した。これらの場合において、マウスの死亡は、翌日に生じたものとして通知された。
実施例3に記載されるプロトコルを、さらなる群を用いて実施した。6~10匹のマウスの3群を比較した:
(i)未処置の陰性対照、
(ii)1週間サイクルで、シスプラチン1.5mg/kgのIV注射を2サイクル行ったマウス(1週目と3週目に1サイクル1回投与、2週目は休薬)
(iii)1週間サイクルで、シスプラチン0.5mg/kgの用量の実施例1で調製したシスプラチンDPI、およびシスプラチン1.5mg/kgのIV注射を併用で処置したマウス(1週目と3週目に1サイクル3回投与、2週目は1週間休薬)。
本実施例の目的は、シスプラチン乾燥粉末吸入(CIS DPI)と従来の全身化学療法(すなわち、白金ベースのダブレット(Ptダブレット))との相加/相乗効果を実証することである。Ptダブレットは、一次処置、二次処置および後続処置として化学療法により現在処置されているステージIIIおよびIVの非小細胞肺癌(NSCLC)患者に広く使用されている。
-生物発光による経時的な腫瘍増殖(インビボ光学イメージング、生物発光シグナルは、インビボでの生きているM109腫瘍細胞の数に正比例する)。生物発光分析を、3日目(無作為化)から38日目まで週2回行った。
-カプランマイヤー生存曲線および生存期間の中央値(50%マウスが肺腫瘍および/または転移により死亡した期間に相当)を含む生存解析。
(i)生理食塩水を静脈内注射(IV)した未処置の陰性対照、
(ii)1週間サイクルで、1.5mg/kgのシスプラチンおよび10mg/kgのパクリタキセルから構成される白金ダブレットを2サイクル連続してIV注射したマウス(1サイクルあたり単回IV注射)、
(iii)1週間サイクルでシスプラチン0.5mg/kgの用量のCIS DPI(実施例2)と白金ダブレットを2サイクル連続して併用処置したマウス(1サイクルあたり3回のCIS DPI投与およびPtダブレットの単回IV注射)。
<白金ダブレットに対する応答>
大部分のマウス(7/8、88%)は、白金ダブレットに応答した。しかし、21日目からの腫瘍の再増殖が観察されたように、この応答は一時的な応答であった。
腫瘍細胞の減少(一時的な生物発光値<100%-基準日である3日目の腫瘍サイズ(すなわち、治療前の腫瘍サイズ))は、4/8(50%)のマウスにおいて観察された。
<白金ダブレット+CIS-DPIに対する応答>
すべてのマウス(8/8、100%)が併用処置に応答し、6/8(75%)のマウスでは3日目の基準値と比較して腫瘍細胞の減少が認められた(値<100%)。
本実施例の目的は、単剤療法(すなわち、~30%のステージIVのNSCLC(PD-L1バイオマーカー発現>50%)における現在の標準治療であるiv化学療法と併用しない)としてCIS DPIと免疫チェックポイント阻害剤との相乗効果を実証することである。

(i)陰性対照、すなわち、生理食塩水iv注射+抗βGALラットIgG2a腹腔内(ip)注射(抗PD1のアイソタイプ対照、BioXCell, West Lebanon, NH, USA)+DPIビヒクル(すなわち、シスプラチンを含まない粉末、賦形剤のみ)、
(ii)下記のスキームに従って、1週間サイクルで抗PD1抗体(RMP1-14、BioXCell, West Lebanon, NH, USA)のip注射を1サイクル行ったマウス、
(iii)下記のスキームに従って、1週間サイクルで0.5mg/kgの用量のシスプラチンDPI(実施例2)と抗PD1抗体(RMP1-14、BioXCell, West Lebanon, NH, USA)を1サイクル併用処置したマウス。
<陰性対照>
腫瘍増殖がコントロールされておらず、連続的に腫瘍増殖が続き、3日目から21日目の間に腫瘍細胞数が500倍超増加した。
<免疫療法に対する応答>
3/8のマウス(37.5%)は抗PD1単剤療法に応答したが、長い応答者は観察されなかった。これらの3匹の応答者については、投与後14日目から腫瘍の再増殖が認められた。
<シスプラチンDPIと免疫療法との併用に対する応答>
-すべてのマウス(7/7、100%)は最初の10日目までの腫瘍増殖に応答し、続いて腫瘍サイズの急激な減少と17日目までの腫瘍増殖の制御がすべてのマウスで観察された。
-17日目まで(すなわち、処置期間)、抗PD1療法に対するシスプラチンDPIの不可逆的な相加効果は有効である。
6~10匹のマウスの3群を比較したことを除いて、実施例3に記載のプロトコルを再現した:
(i)未処置の陰性対照、
(ii)2週間サイクルで抗PD1抗体(RMP1-14、Bioceros, Utrecht, The Netherlands)を腹腔内注射(IP)したマウス(1週目は4回投与、2週目は休薬)
(iii)3週間サイクルでシスプラチン0.5mg/kgの用量のシスプラチンDPIと、1.5mg/kgのシスプラチンのIV注射との併用処置を行い(1週目と3週目は3回投与、2週目は休薬)、2週間サイクルで抗PD1抗体(RMP1-14、Bioceros, Utrecht, The Netherlands)を腹腔内注射(IP)したマウス(1週目は4回投与、2週目は休薬)で処置したマウス。
6~10匹のマウスの3群を比較したことを除いて、実施例3に記載のプロトコルを再現した:
(iv)未処置の陰性対照、
(v)2週間サイクルで抗PD1抗体(RMP1-14、Bioceros, Utrecht, The Netherlands)を腹腔内注射(IP)したマウス(1週目は4回投与、2週目は休薬)
(vi)3週間サイクルでシスプラチン0.5mg/kgの用量のシスプラチンDPIと、細胞毒性用量(cytotoxic dose)のカルボプラチンIVとの併用処置を行い(1週目と3週目は3回投与、2週目は休薬)、2週間サイクルで抗PD1抗体(RMP1-14、Bioceros, Utrecht, The Netherlands)を腹腔内注射(IP)したマウス(1週目は4回投与、2週目は休薬)で処置したマウス。
本実施例の目的は、~60%のステージIVのNSCLC(PD-L1バイオマーカー発現<50%)における現在の標準治療であるiv化学療法(すなわち、全身白金ダブレット)と併用したときの、シスプラチンDPIと免疫チェックポイント阻害剤(すなわち、抗PD1免疫療法)との相乗効果を実証することである。

(iii)下記のスキームに従って、1週間サイクルで、0.5mg/kgの用量のシスプラチンDPI(実施例2)と、抗PD1抗体(RMP1-14、BioXCell, West Lebanon, NH, USA)と、10mg/kgのパクリタキセル(すなわち、タキソール)のiv注射と、を1サイクル併用処置したマウス。
(iv)1週間サイクルで、0.5mg/kgの用量のシスプラチンDPI(実施例2)と、抗PD1抗体(RMP1-14、BioXCell, West Lebanon, NH, USA)と、1.5mg/kgのシスプラチンおよび10mg/kgのパクリタキセルから構成される白金ダブレットのiv注射と、を1サイクル併用処置したマウス。
<陰性対照>
腫瘍増殖がコントロールされておらず、連続的に腫瘍増殖が続き、3日目から21日目の間に腫瘍細胞数が500倍超増加した。
<抗PD1+白金ダブレット>
7/13のマウス(54%)は、一時的な生物発光値が100%未満だった(これは、最初の処置(投与)前と比較して腫瘍細胞数の減少を示す)。
<抗PD1+タキソール+シスプラチンDPI>
7/9のマウス(78%)は、生物発光値が100%未満だった(これは、3日目と比較して腫瘍細胞数の減少を示す)。
<抗PD1+白金ダブレット+シスプラチンDPI>
9/11のマウス(82%)は、生物発光値が100%未満だった(これは、3日目と比較して腫瘍細胞数の減少を示す)。
環境汚染に関して、本発明のキットを従来のキットと比較した。
Claims (36)
- a)脂質マトリックスを有する、カルボプラチン、シスプラチン、オキサリプラチン、またはネダプラチン等の白金薬剤の群から選択される少なくとも1つの抗腫瘍剤を含み、PEG修飾賦形剤等の1つ以上の賦形剤および/または担体を任意に含む、一連の密封包装された単回治療用量の吸入用乾燥粉末と、
b)一連の使い捨て乾燥粉末吸入装置と、を含み、
i)密封包装された単回治療用量それぞれは、閉鎖状態および開放状態を有し、
ii)使い捨て乾燥粉末吸入装置それぞれは、閉鎖位置および任意で開放位置を有し、
iii)密封包装された単回治療用量それぞれは、前記一連の使い捨て乾燥粉末吸入装置のうちの1つの使い捨て乾燥粉末吸入装置に閉鎖状態で収容されるように提供され、

- 前記多剤併用療法が、静脈内注射もしくは経口化学療法、免疫療法、標的療法、ホルモン療法、腫瘍切除手術、腫瘍を有する器官の一部または全部を除去するための切除手術、治癒手術、放射線療法およびそれらの組み合わせからなる群から選択される1つの療法と、追加療法としての少なくとも1つの吸入による化学療法とを含む、請求項14に記載の吸入化学療法用キット。
- 前記療法が、一連のサイクルの間に投与されるように提供される、静脈内注射または経口化学療法または免疫療法を少なくとも含み、サイクルそれぞれが投与期間および休薬期間を含み、前記少なくとも1つの吸入による化学療法が、前記療法前の期間および/または前記休薬期間中に投与されるように提供される、請求項15に記載の吸入化学療法用キット。
- 前記投与期間が、1日未満、好ましくは15分~6時間、好ましくは30分~4時間、例えば1~3時間にわたる期間であり、
前記休薬期間が2~5週間、好ましくは2~4週間、より好ましくは2,5~3,5週間、特に約3週間±2日にわたる期間である、請求項16に記載の吸入化学療法用キット。 - 前記少なくとも1つの吸入による化学療法が、前記療法前の期間および/または前記休薬期間中に、毎週少なくとも1つの密封包装された単回治療用量の抗腫瘍剤の割合で、好ましくは、前記療法前の期間および/または前記休薬期間中に、毎週少なくとも3つの密封包装された単回治療用量の抗腫瘍剤の割合で、より好ましくは、前記療法前の期間および/または前記休薬期間中に、毎週少なくとも5つの密封包装された単回治療用量の抗腫瘍剤の割合で、場合によっては、一日の用量として、前記療法前の期間および/または前記休薬期間中に、毎週少なくとも7つの密封包装された単回治療用量の抗腫瘍剤の割合で、任意選択で、前記療法前の期間および/または前記休薬期間中に均等に分散させた態様で投与されるように提供される、請求項14~17のいずれか1項に記載の吸入化学療法用キット。
- 前記多剤併用療法が、腫瘍切除手術、腫瘍を有する器官の一部もしくは全部を除去する切除手術、治癒手術、または放射線療法とそれに続く回復期間を少なくとも含み、前記少なくとも1つの吸入による化学療法が、前記療法の前、前記療法の間、または前記療法の後の期間に投与されるように提供される、請求項14または18に記載の吸入化学療法用キット。
- 前記少なくとも1つの吸入による化学療法が、前記療法前、療法の間、または療法の後の期間中に、毎週少なくとも1つの密封包装された単回治療用量の抗腫瘍剤の割合で、好ましくは、前記療法前、前記療法の間、または前記療法の後の期間中に、毎週少なくとも3つの密封包装された単回治療用量の抗腫瘍剤の割合で、より好ましくは、前記療法前、療法の間、または療法の後の期間中に、毎週少なくとも5つの密封包装された単回治療用量の抗腫瘍剤の割合で、場合によっては、一日の用量として、前記療法前、前記療法の間、または前記療法の後の期間中に、毎週少なくとも7つの密封包装された単回治療用量の抗腫瘍剤の割合で、任意選択で、前記療法前、前記療法の間、または前記療法の後の期間中に、均等に分散させた態様で投与されるように提供される、請求項14~17のいずれか1項に記載の吸入化学療法用キット。
- 多剤併用療法における、請求項1~13のいずれか1項に記載の吸入化学療法用キットの使用。
- 前記多剤併用療法が、静脈内注射もしくは経口化学療法、免疫療法、標的療法、ホルモン療法、腫瘍切除手術、腫瘍を有する器官の一部もしくは全部を除去するための切除手術、治癒手術、放射線療法およびそれらの組み合わせからなる群から選択される1つの療法と、追加療法としての少なくとも1つの吸入による化学療法とを含む、請求項21に記載の吸入化学療法用キットの使用。
- 前記療法が、一連のサイクルの間に投与されるように提供される、静脈内注射または経口化学療法または免疫療法を少なくとも含み、サイクルそれぞれが投与期間および休薬期間を含み、前記吸入による化学療法が、前記療法前の期間および/または前記休薬期間中に投与されるように提供される、請求項22に記載の吸入化学療法用キットの使用。
- 前記投与期間が、1日未満、好ましくは15分~6時間、好ましくは30分~4時間、例えば1~3時間にわたる期間であり、
前記休薬期間が2~5週間、好ましくは2~4週間、より好ましくは2,5~3,5週間、特に約3週間±2日にわたる期間である、請求項23に記載の吸入化学療法用キットの使用。 - 前記少なくとも1つの吸入による化学療法が、前記療法前の期間および/または前記休薬期間中に、毎週少なくとも1つの密封包装された単回治療用量の抗腫瘍剤の割合で、好ましくは、前記療法前の期間および/または前記休薬期間中に、毎週少なくとも3つの密封包装された単回治療用量の抗腫瘍剤の割合で、より好ましくは、前記療法前の期間および/または前記休薬期間中に、毎週少なくとも5つの密封包装された単回治療用量の抗腫瘍剤の割合で、場合によっては、一日の用量として、前記療法前の期間および/または前記休薬期間中に、毎週少なくとも7つの密封包装された単回治療用量の抗腫瘍剤の割合で、任意選択で、前記療法前の期間および/または前記休薬期間中に均等に分散させた態様で投与されるように提供される、請求項23または24に記載の吸入化学療法用キットの使用。
- 前記多剤併用療法が、腫瘍切除手術、腫瘍を有する器官の一部もしくは全部を除去する切除手術、治癒手術、または放射線療法とそれに続く回復期間を少なくとも含み、前記少なくとも1つの吸入による化学療法が、前記療法の前、前記療法の間、または前記療法の後の期間に投与されるように提供される、請求項21または25に記載の吸入化学療法用キットの使用。
- 前記少なくとも1つの吸入による化学療法が、毎週少なくとも1つの密封包装された単回治療用量の抗腫瘍剤の割合で、好ましくは、毎週少なくとも3つの密封包装された単回治療用量の抗腫瘍剤の割合で、より好ましくは、毎週少なくとも5つの密封包装された単回治療用量の抗腫瘍剤の割合で、場合によっては、一日の用量として、毎週少なくとも7つの密封包装された単回治療用量の抗腫瘍剤の割合で、任意選択で、前記療法前、前記療法の間、または前記療法の後の期間中に、均等に分散させた態様で投与されるように提供される、請求項26に記載の吸入化学療法用キットの使用。
- 静脈内注射もしくは経口化学療法、免疫療法、標的療法、ホルモン療法、腫瘍切除手術、腫瘍を有する器官の一部もしくは全部を除去するための切除手術、治癒手術、放射線療法およびそれらの組み合わせからなる群から選択される少なくとも1つの療法と、追加療法としての少なくとも1つの吸入による化学療法とを含む、多剤併用療法において、少なくとも1つの吸入化学療法を投与する工程を含む、肺癌に対する治療的処置。
- 前記療法が、一連のサイクルの間に投与されるように提供される、静脈内注射または経口化学療法または免疫療法を少なくとも含み、サイクルそれぞれが投与期間および休薬期間を含み、前記吸入による化学療法が、前記療法前の期間および/または前記休薬期間中に投与されるように提供される、請求項28に記載の肺癌に対する治療的処置。

- 前記多剤併用療法が、腫瘍切除手術、腫瘍を有する器官の一部もしくは全部を除去する切除手術、治癒手術、または放射線療法とそれに続く回復期間を少なくとも含み、前記少なくとも1つの吸入による化学療法が、前記療法の前、前記療法の間、または前記療法の後の期間に投与されるように提供される、請求項28に記載の肺癌に対する治療的処置。
- 前記少なくとも1つの吸入による化学療法が、前記療法前、療法の間、または療法の後の期間中に、毎週少なくとも1つの密封包装された単回治療用量の抗腫瘍剤の割合で、好ましくは、前記療法前、前記療法の間、または前記療法の後の期間中に、毎週少なくとも3つの密封包装された単回治療用量の抗腫瘍剤の割合で、より好ましくは、前記療法前、療法の間、または療法の後の期間中に、毎週少なくとも5つの密封包装された単回治療用量の抗腫瘍剤の割合で、場合によっては、一日の用量として、前記療法前、前記療法の間、または前記療法の後の期間中に、毎週少なくとも7つの密封包装された単回治療用量の抗腫瘍剤の割合で、任意選択で、前記療法前、前記療法の間、または前記療法の後の期間中に、均等に分散させた態様で投与されるように提供される、請求項32に記載の肺癌に対する治療的処置。
- 前記肺癌が、例えば骨肉腫転移である肺転移等の任意の肺腫瘍である、請求項28~33のいずれか1項に記載の肺癌に対する治療的処置。
- 前記肺癌が小細胞肺癌である、請求項28~34のいずれか1項に記載の肺癌に対する治療的処置。
- 前記肺癌が非小細胞肺癌である、請求項28~35のいずれか1項に記載の肺癌に対する治療的処置。
