Abstract
Description

tdsRNA
本開示は、部分的に、治療用dsRNA(tdsRNA)と称されるdsRNAに関し、その好ましい実施態様は、リンタトリモド(たとえば、AMPLIGEN(登録商標));強固なdsRNA;ミスマッチdsRNA;または本開示に記載された手順の下で生成されたdsRNAである。tdsRNAは、以下の特性を有する。

投与期間は、通常、約1日、2日間、3日間、4日間、5日間、6日間、7日間、8日間、9日間、10日間、11日間、12日間、13日間、14日間、15日間、16日間、17日間、18日間、19日間、20日間、21日間、22日間、23日間、24日間、25日間、26日間、27日間、28日間、29日間、30日間、1週間、2週間、3週間、4週間、1か月、1か月以上、1年、または1年以上である。
特定の態様では、tdsRNAは、約0.1 mg/kg、約0.2 mg/kg、約0.3 mg/kg、約0.4 mg/kg、約0.5 mg/kg、約0.6 mg/kg、約0.7 mg/kg、約0.8 mg/kg、約1 mg/kg、約2 mg/kg、約3 mg/kg、約4 mg/kg、約5 mg/kg、約6 mg/kg、約7 mg/kg、約8 mg/kg、約9 mg/kg、約10 mg/kg、約15 mg/kg、約20 mg/kg、約25 mg/kg、約30 mg/kg、記載された値の間の任意の値、または記載された値の間の任意の範囲の用量で投与される。範囲の例は、4~20 mg/kgの間またはbetween 6~20 mg/kgの間であろう。
本明細書で使用される「治療する」、「治療すること」、「治療される」または「治療」という用語は、目的が症状を排除または軽減することである治療的処置を意味する。有利なまたは望ましい臨床結果には、症状の排除、症状の緩和、状態の程度の減少、状態の安定化(すなわち、悪化しない)状態、状態の進行の遅延または低速化が含まれるが、これらに限定されない。

序論
二重盲検、無作為化、プラセボ対照試験における、選択的TLR3アゴニストであるリンタトリモドの分析は、運動トレッドミル負荷(ETT)によって測定される疲労の軽減において統計的有意性(p <0.05)を実証した。リンタトリモドは、AMPLIGEN(登録商標)としても商標登録されており、本開示では、AMPLIGEN(登録商標)とリンタトリモドは同じ意味を有する。運動トレッドミル負荷(ETT)の主要評価項目は、研究に登録する前の筋痛性脳脊髄炎/慢性疲労症候群の症状の持続時間の関数として再検討されている。
208人の対象(n=208)の治療企図(ITT)集団を、2つのサブセットに分けた。第1のサブセットには、ME/CFS症状のある対象が含まれ、これらの対象はまた、約24時間以上続くPEM、2~8年間のME/CFS症状の持続期間を有し、この第1サブセットには合計75人の対象が存在する(n=75)。第2のサブセットには、第1のサブセットに適合しない対象が含まれる。たとえば、第2のサブセットのメンバーは、ME/CFS症状の持続期間が2年未満または8年を超えるメンバーであり、この第2のセットには合計133人の対象(n=133)が存在する。

208人の対象の治療企図集団では、有意に高いパーセンテージのリンタトリモドを投与された患者(39%)とプラセボ患者(23%)が、25%以上の運動トレッドミル持続時間を改善した(p= 0.013)。ベースラインの筋痛性脳脊髄炎/慢性疲労症候群(ME/CFS)の症状持続期間が2~8年間の患者のサブセット(すなわち、レスポンダーサブセット)の場合、リンタトリモド対プラセボ患者の51.2%対17.6%が、それぞれ、運動持続時間25%以上改善した(p=0.003)。プラセボ調整改善は、2~8年間のサブセットの33.6%と比較して、治療企図集団では15.9%であった。2~8年間のサブセットの運動期間と垂直上昇におけるプラセボ調整改善パーセンテージは、治療企図集団で見られたものの2倍以上であった。2年未満+8年を超えるサブセット(非レスポンダーサブセット、n=133)は、プラセボと比較して、リンタトリモドに対して臨床的に有意な運動トレッドミル負荷(ETT)応答を示さなかった。
リンタトリモドの二重盲検、無作為化、プラセボ対照試験からの運動トレッドミルデータの分析により、2~8年間の症状発現ウィンドウを有し、治療企図(ITT)集団と比較して2倍以上の運動応答を示すME/CFS患者のサブセットが特定された。運動持続時間を25%以上改善したこれらの重度の衰弱した患者の大多数(51.2%)で、身体能力の実質的な改善が見られた。運動持続時間の25%以上の改善は、この試験集団の生活の質の臨床的に有意な改善と関連した。リンタトリモドは、重度のME/CFSに苦しむこの患者集団で一般的に忍容性が良好であった。
一本鎖ポリ(I)およびポリ(C12U)の合成は、それぞれのヌクレオチド出発物質(ポリ(I)の場合はイノシン;ポリ(C12U)の場合はシチジン(C)およびウリジン(U))からのポリヌクレオチドの酵素的ポリヌクレオチド合成から始まった。次に、抽出と沈殿のステップを繰り返して、残留不純物を除去した。生成物を含む反応溶液を限外濾過により濃縮し、フェノールで抽出した。濃縮および抽出された溶液を、沈殿させ、溶解し、水性エタノール(50:50)から再沈殿させた。

序論
重度のME/CFS対象は、臨床試験において、リンタトリモドの全身投与後、主要評価項目である運動トレッドミル負荷(ETT)の有意な改善を示した。この試験では、リンタトリモドを40週間、週2回投与し、対照対象にはプラセボを40週間投与した。
この試験は、ME/CFSにおけるリンタトリモドの安全性と有効性を評価するために実施された、同等の並行コホートによる前向き二重盲検試験であった。ME/CFS患者(本明細書では対象とも呼ばれる)の多くは、心機能の評価のために一般的に使用される標準的なブルースの最大下運動プロトコルを物理的に実行することがでなかった。患者の安全上の理由から、高齢者向けに設計されたブルースプロトコルにエネルギー要件が類似する修正ブルースME/CFSプロトコルが使用された。主要評価項目は、ベースラインから第40週までの運動トレッドミル負荷(ETT)における変化であった。

Toll様受容体(TLR)は、自然免疫の誘導による微生物病原体に対する防御の第一線として機能し、さらに、炎症性サイトカインによる部分的に媒介される特定の液性および細胞性免疫を提供するための適応免疫応答の誘導のための初期細胞編成(cellular orchestration)を提供する。それらは、宿主適応免疫応答システムの中心である樹状細胞(DC)において特に見出される。MyD88に依存しないTRIF経路を使用するTLR3を除いて、すべてのTLRは、MyD88に依存するシグナル伝達経路を使用する。自然免疫応答を開始する遺伝子発現の2つの他のdsRNA活性化誘導物質は、サイトゾルヘリカーゼである、MDA5およびRIG-1である。自然免疫および獲得免疫の誘導におけるリンタトリモド (ポリI:ポリC12U)活性は、TLR3の他のdsRNAアクチベーター(たとえば、ポリI:ポリC、ウイルスdsRNA)とは対照的に、TLR-3に制限される。リンタトリモドのTLR3への制限は、ヒトを含む霊長類における全身性サイトカイン誘導の欠如に起因する。ME/CFSにおいて観察された異常な免疫応答にとって重要なことは、上記の2つの非制限dsRNA TLR3アゴニストとは対照的に、リンタトリモドがヒト微小環境におけるTeff/Treg細胞の比率を増加させるという、癌における最近の独創的な観察である。

Claims (16)
- 筋痛性脳脊髄炎/慢性疲労症候群症状を有する対象を治療するための方法であって、
対象が、治療前に2~8年間のME/CFS症状の発症を示す標的対象であることを決定すること;および
有効成分として有効量の治療用dsRNA(tdsRNA)を含む医薬組成物を標的対象に投与すること;
を含む方法。 - 標的対象が、2~8年間のME/CFS症状の発症ならびに約24時間以上持続する運動後の倦怠感(PEM)の発症を示す、請求項1に記載の方法。
- 治療が、治療前と比較して、標的対象における治療後の少なくとも25%の臨床的に有意な量の増加によって、運動耐容能を増加させる、請求項1、または前記請求項のいずれかに記載の方法。
- 標的対象の少なくとも40%~50%以上が、少なくとも25%の運動耐容能の増加を示す、請求項1、または前記請求項のいずれかに記載の方法。
- 投与が、頬側;埋め込みによる;吸入による;点滴による;噴霧による;坐剤による;経腸的に;皮膚上;頭蓋内;皮内;筋肉内;鼻腔内;眼窩内;腹腔内;くも膜下腔内;気管内;静脈内;脳室内;膀胱内;経口;非経口;皮下;舌下;局所;経皮;経粘膜;またはそれらの組み合わせである、請求項1、または前記請求項のいずれかに記載の方法。
- 医薬組成物が、1週間に1~3回、1治療日あたり平均で約25~700ミリグラムのtdsRNAを提供する投薬量で、最長1ヶ月間;1ヶ月以上;最長1年;または1年以上、対象に投与される、請求項1、または前記請求項のいずれかに記載の方法。
- 医薬組成物が、1週間に1~3回、1治療日あたり平均で約25~700ミリグラムのtdsRNAを継続的に提供する投薬量で、最長1ヶ月間;1ヶ月以上;最長1年;または1年以上、対象に静脈内投与される、請求項1、または前記請求項のいずれかに記載の方法。
- 対象が、ヒトである、請求項1、または前記請求項のいずれかに記載の方法。
