Abstract
Description
a)前記対象の少なくとも1つの膝において関節裂隙幅(JSW)を測定するステップ、
b)ステップa)の結果からプラセボ効果のリスクを予測するステップ
を含む、方法を対象とする。
a)前記対象からの試料から、CTX-II及び/又はProC2からなる群から選択されるバイオマーカーの少なくとも1つの量を決定するステップ、
b)前記対象の少なくとも1つの膝において関節裂隙幅(JSW)を測定するステップ、
c)ステップa)及びb)の結果から前記活性化合物を用いる治療に対する前記対象の感受性を予測するステップ
を含み、ステップa)及びb)がいずれかの順序で行われる、
方法を対象とする。
a)前記対象からの試料から、CTX-II及び/又はProC2からなる群から選択されるバイオマーカーの少なくとも1つの量を決定し、かつ前記対象の少なくとも1つの膝において関節裂隙幅(JSW)を測定するステップであって、これらのタンパク質の少なくとも1つの量及びJSWの幅(width if the JSW)が、前記治療に対して感受性又は非感受性であることについての対象のリスクに関して予測する、前記ステップ、ならびに
b)感受性対象を前記治療、又は臨床試験のために好適であるとして選択するステップ
を含む、方法もまた記載される。

ii.3.5±2SDmmより高いJSWと共に350±2SDng/mmolより低いCTX-II及び/もしくは4.2±2SDng/mLより低いProC2又は
iii.3.5±2SDmmより低いJSWと共に350±2SDng/mmolより高いCTX-II及び/もしくは4.2±2SDng/mLより高いProC2
を有する対象を選択するステップ、
c)前記選択された対象の関節内に前記活性化合物を投与するステップ
を含む、方法もまた目的とする。

1)軟骨障害を有する対象においてプラセボ応答を予測する他に、2)変形性関節症、軟骨傷害、関節軟骨に影響する骨折又は関節軟骨に対する影響力を有する外科的処置(例えば、マイクロフラクチャー)などの軟骨障害を有する対象の治療のための活性化合物治療の臨床的有効性(特に、軟骨修復に関して)を予測する必要性が存在する。臨床試験の管理を最適化するため又は前記対象の治療を最適化するために、特に軟骨修復に関して、1)プラセボ応答及び/又は2)活性化合物治療に対する所与の対象の応答の予測因子として使用され得るマーカーを同定することが重要である。そのような予測バイオマーカーは、1)プラセボ効果を示すリスクがある対象の群及び/又は2)治療に対して低感受性もしくは反対に感受性(例えば、中もしくは良感受性)もしくはさらには高感受性のいずれかである高リスク群を同定するために使用されてもよい。例えば、変形性関節症を有する1の対象が、治療に対して中応答(又は中感受性である)について高リスクであることが既知である場合、医師は、例えば、前記対象に投与される活性化合物の用量をより高めるため及び/又はより長い期間で薬物を投与するために投薬レジメンを適用させるように決定してもよい。そのような予測的情報は、特に、1患者に適用される投薬レジメンもしくは治療の長さ又は必要な場合(例えば、活性化合物治療があまり効率的でないと予測される場合)の関節置換手術の時期に関して、医学的決定をガイドするために臨床的に有用であり得る。

主要エンドポイントに関する処置効果を、因子としてのベースライン値、処置群、時間、及び国ならびに相互作用としての時点による処置(treatment-by-time point)を含む、ベースラインからの絶対的変化に関する反復測定分散分析(ANOVA、SASにおけるPROC MIXEDを使用)を使用して用量範囲を通じて評価した。主要有効性解析は、線形用量関係性及び2年時の全体的な処置効果を試験することからなるものであった。有意性レベルは、両方の試験について両側で5%に設定した。一対比較(スプリフェルミン対プラセボ、ならびにスプリフェルミン用量及びレジメン群の間)をこのモデリングフレームワークの文脈内で行った。各一対比較について、処置の間の差異ならびに対応する95%信頼区画(CI)及びp値を提示する。主要エンドポイントのために使用した同じANOVAモデルを使用して、各時点における及び経時的なMRIエンドポイント、WOMACエンドポイント(合計、疼痛、機能、及び硬直スコア)、ならびにX線エンドポイントなどの連続的な二次エンドポイントに関する処置効果を評価した。ロジスティック回帰を使用して、OMERACT-OARSIレスポンダー率などの二元的有効性エンドポイントに関する処置効果を評価した。各一対比較についての点推定値ならびに対応する95% CI及びp値を提供する。
WOMACは、臨床OA研究において症状改変を評価するために使用される検証された道具である。この臨床スコアは1981年に開発され、臨床研究者及び規制当局の両方により有効な道具としてみなされている。WOMACは、股関節及び膝OAにおける臨床研究において広く使用されており、大規模に検証されてきた。
X線により測定されるJSWにおける変化は、OAにおける有効性研究における使用のための欧州医薬品庁及び米国食品医薬品局により承認された認識されるエンドポイントである。JSWは、標準化された技術を使用して測定した。X線はまた、実施例2においてKL等級を評価するために使用された。
DBPC処置フェーズの主要エンドポイントは、mITTにおける2年時のqMRIにより評価された場合の全大腿脛骨関節における軟骨厚のベースラインからの変化であった。全大腿脛骨関節の軟骨厚は、2つのやり方で算出した:
1.平均軟骨厚(総体積を総表面積で割ったもの)、
2.総軟骨厚(内側及び外側区画における軟骨厚の合計)。
骨及び関節組織ターンオーバーの他に滑膜炎症の血清学的及び尿生化学的マーカーを評価した。軟骨代謝の潜在的なバイオマーカーは、コラーゲンII型プロペプチドのネオエピトープ(proC2)及びII型コラーゲン架橋C-テロペプチド(C-telopeptide cross-linking of type II collagen;CTX-II)を含むがこれらに限定されなかった。全身バイオマーカー評価用の血液及び尿試料を以下の時点:0週目(スプリフェルミンの1回目の注射の前)、26週目、54週目、80週目及び104週目に収集した。注射も投与された時点について、試料を注射の前に収集した。滑液試料をこれらの時点において収集した。これらの試料は、i.art.注射手順の部分として、注射のために使用されたものと同じ針を使用して注射の直前に採取した。尿収集のために、2回目の朝の排尿を得た。
- 化合物の投与と関連付けられる血清及び尿バイオマーカーにおけるベースラインからの変化。
- 処置に対する応答又は疾患進行と関連付けられるベースラインタンパク質マーカー(応答はMRI及び/又は質問票により評価した)。
本実施例における処置として使用されたFGF-18化合物はスプリフェルミン(「定義」のセクションにおいて定義される)である。2つの強度のスプリフェルミンが研究用に供給された:30μg及び100μg。スプリフェルミンは、3mLのガラスバイアル中の白色無菌のフリーズドライ粉末として供給された。各バイアルは、31.5μg又は105μgのいずれかのスプリフェルミン活性物質を含有し、これらの量は5%の過剰を含んでおり、これは、0.9%w/vの塩化ナトリウム注射液(本明細書において「生理食塩水溶液」と称される)を用いた再構成後にそれぞれ30μg又は100μgのスプリフェルミン活性物質の抽出を可能とするものであった。製剤の賦形剤は、リン酸ナトリウム緩衝液(pH7.2)、水酸化ナトリウム、O-リン酸、スクロース、及びポロキサマー188であった。全ての処置群について、投与された体積は2mLであった。

・群3(4サイクルスプリフェルミン30μg/注射;以後スプリフェルミン30μgと称する):111人の対象。
・群4(2サイクルのプラセボと交互の2サイクルスプリフェルミン100μg/注射;以後スプリフェルミン/プラセボ100μgと称する):110人の対象。
・群5(4サイクルスプリフェルミン100μg/注射;以後スプリフェルミン100μgと称する):110人の対象。
研究は、化合物Xの評価のための二重盲検、無作為化、プラセボ対照及び多施設の臨床試験であった。この研究のプラセボアームに約600人の患者が登録された。50~80歳の、少なくとも1つの膝の有痛性OAを有するが、それ以外は良好な全般的健康状態の患者を採用した。経口薬物である化合物X、プラセボを同一の包装において供給した。包含されるために、患者はOAの診断用のACR基準を満たす必要がった。両方の膝を研究の間に評価したが、主要な有効性エンドポイントの評価のために無作為化の前に標的膝を同定した。標的膝は、前の月のほとんどの日に有痛性である必要があった。追加的に、患者は、以下の基準:50歳より高い年齢、30分未満で続く朝の硬直の経験、又は膝捻髪音の少なくとも1つを満たす必要があった。標的膝は、X線で測定された内側脛骨大腿関節におけるJSW≧2.0mmを有し、かつ2又は3のKLスコアを有する必要があった。WOMACにおいて、患者は、疼痛について≧150mm及び/又は機能について≧510mmのスコアであった。両方の膝が基準を満たす場合、2のKLスコアを満たす膝として標的膝を定義した。両方の膝が2のKLスコアを有する場合、(80mmより低い限り)最も高い疼痛VASを有する膝を選択した。
KL3富化及びCTX-II平衡化設計は、少なくとも3年目まで症候的利益の検出を可能にすることを実施例は明確に強調した(図8を参照)。したがって、薄い軟骨(3.5±2SDmmより低い又はそれに等しい)及び低いCTX-II(350±2SDng/mmolより低い又はそれに等しい)は、プラセボ応答の上昇を軽減するための臨床試験のための最良の研究集団のようである。注記として、参加希望者全員のデータの使用、即ち、患者の損失が無いことが可能なのは自明であり、それは単に層化、患者の管理の事項である。同様に、実施例セクションの結果に基づいて、ProC2レベルがCTX-IIレベルの代わりに使用され得る。そのような場合、薄い軟骨及び低いProC2(4.2±2SDng/mlより低い又はそれに等しい)は、プラセボ応答の上昇を軽減するための臨床試験のための等しく適切な研究集団である。

- 臨床試験におけるリスクを低減する方法であって、前記臨床試験が、活性化合物を用いる対象における軟骨障害の治療に関し、前記方法は、
a)前記対象からの試料から、CTX-II及び/又はProC2からなる群から選択されるバイオマーカーの少なくとも1つの量を決定するステップ、
b)前記対象の少なくとも1つの膝において関節裂隙幅(JSW)を測定するステップ、ならびに
c)ステップa)及びb)の結果から前記活性化合物を用いる治療に対する前記対象の高い、中間の、低い又は無しの感受性を予測するステップ
を含み、
ステップa)及びb)がいずれかの順序で行われる、

a)前記対象からの試料から、CTX-II及び/又はProC2からなる群から選択されるバイオマーカーの少なくとも1つの量を決定し、かつ前記対象の少なくとも1つの膝において関節裂隙幅(JSW)を測定するステップであって、これらのタンパク質の少なくとも1つの量及びJSWの幅が、前記治療に対して感受性又は非感受性であることについての前記対象のリスクに関して予測する、前記ステップ、ならびに
b)前記治療又は臨床試験のために好適であるとして前記感受性対象を選択するステップ
を含む、方法。 - 350±2SDng/mmolより高いCTX-II、及び/又は4.2±2SDng/mLより高いProC2ならびに3.5±2SDmmより高いJSWを示す前記対象を活性化合物を用いる前記治療から除外するステップを含む、請求項4に記載の方法。
- a)350±2SDng/mmolより低いCTX-II、及び/もしくは4.2±2SDng/mLより低いProC2ならびに3.5±2SDmmより低いもしくはそれに等しいJSW、
b)350±2SDng/mmolより低いCTX-II、及び/もしくは4.2±2SDng/mLより低いProC2ならびに3.5±2SDmmより高いJSW、又は
c)350±2SDng/mmolより高いCTX-II及び/もしくは4.2±2SDng/mLより高いProC2ならびに3.5±2SDmmより低いJSW
を示す前記対象を活性化合物を用いる前記治療のために包含するステップを含む、請求項5に記載の方法。 - a)350±2SDng/mmolより低いCTX-II、及び/もしくは4.2±2SDng/mLより低いProC2ならびに3.5±2SDmmより高いJSW、又は
b)350±2SDng/mmolより高いCTX-II及び/もしくは4.2±2SDng/mLより高いProC2ならびに3.5±2SDmmより低いJSW
を示す前記対象を、代替的な投薬レジメンによる活性化合物を用いる前記治療のために包含するステップを含む、請求項6に記載の方法。 - 軟骨障害を有する対象を活性化合物を用いて治療する方法であって、
a)前記対象からの試料から、CTX-II及び/又はProC2からなる群から選択されるバイオマーカーの少なくとも1つの量を決定し、かつ前記対象の少なくとも1つの膝において関節裂隙幅(JSW)を測定するステップであって、前記量及び幅が、前記活性化合物を用いる治療に対して良感受性であることについての前記対象のリスクに関して予測する、前記ステップ、
b)以下:
i.350±2SDng/mmolより低いCTX-II、及び/もしくは4.2±2SDng/mLより低いProC2及び3.5±2SDmmより低いもしくはそれに等しいJSW、
ii.350±2SDng/mmolより低いCTX-II及び/もしくは4.2±2SDng/mLより低いProC2及び3.5±2SDmmより高いJSW、又は
iii.350±2SDng/mmolより高いCTX-II及び/もしくは4.2±2SDng/mLより高いProC2及び3.5±2SDmmより低いJSW
を有する前記対象を選択するステップ、ならびに
c)前記選択された対象の関節内に前記活性化合物を投与するステップ
を含む、方法。 - 軟骨障害を有する対象を活性化合物を用いて治療する方法であって、
a)前記対象からの試料から、CTX-II及び/又はProC2からなる群から選択されるバイオマーカーの少なくとも1つの量を決定し、かつ前記対象の少なくとも1つの膝において関節裂隙幅(JSW)を測定するステップであって、前記量及び幅が、前記活性化合物を用いる治療に対して中感受性であることについての前記対象のリスクに関して予測する、前記ステップ、
b)以下:
i.350±2SDng/mmolより低いCTX-II、及び/もしくは4.2±2SDng/mLより低いProC2及び3.5±2SDmmより高いJSW、又は
II.350±2SDng/mmolより高いCTX-II及び/もしくは4.2±2SDng/mLより高いProC2及び3.5±2SDmmより低いJSW
を有する前記対象を選択するステップ、ならびに
c)前記活性化合物のための通常の投薬レジメンと比較して前記活性化合物の用量が増加され、かつ/又は前記活性化合物がより長い治療期間にわたり投与される代替的な投薬レジメンにしたがって前記選択された対象の関節内に前記活性化合物を投与するステップ
を含む、方法。 - 臨床試験におけるリスクを低減し又は臨床試験において対象におけるプラセボ効果を決定する方法であって、前記臨床試験が、活性化合物を用いる対象における軟骨障害の治療に関し、前記方法が、
a)前記対象の少なくとも1つの膝において関節裂隙幅(JSW)を測定するステップ、
b)ステップa)の結果から前記対象のプラセボ効果のリスクを予測するステップ
を含み、3.5±2SDmmより高いJSWの存在がプラセボ効果を予測し、かつ3.5±2SDmmより低い又はそれに等しいJSWの存在が、プラセボ効果が無い又は低いことを予測する、
方法。 - 臨床試験におけるリスクを低減し又は臨床試験において対象におけるプラセボ効果を決定する方法であって、前記臨床試験が、活性化合物を用いる対象における軟骨障害の治療に関し、前記方法が、
a)前記対象からの試料から、CTX-II及び/又はProC2からなる群から選択されるバイオマーカーの少なくとも1つの量を決定するステップ、
b)前記対象の少なくとも1つの膝において関節裂隙幅(JSW)を測定するステップ、
c)ステップa)及びb)の結果から前記対象についてのプラセボ効果のリスクを予測するステップ
を含み、ステップa)及びb)がいずれかの順序で行われる、
方法。 - 1)350±2SDng/mmolより高いCTX-II(正常平均の>240~260%)及び/もしくは4.2±2SDng/mLより高いProC2(正常平均の>120~280%)ならびに3.5±2SDmmより高いJSW又は2)350±2SDng/mmolより低いCTX-II及び/もしくは4.2±2SDng/mLより低いProC2ならびに3.5±2SDmmより高いJSW又は3)350±2SDng/mmolより高いCTX-II及び/もしくは4.2±2SDng/mLより高いProC2ならびに3.5±2SDmmより低いもしくはそれに等しいJSWの存在がプラセボ効果を予測する一方、350±2SDng/mmolより低いCTX-II及び/又は4.2±2SDng/mLより低いProC2ならびに3.5±2SDmmより低い又はそれに等しいJSWの存在が、プラセボ効果が無い又は低いことを予測する、請求項11に記載の方法。
- 請求項1~12のいずれかに記載の方法を行うための手段及び使用のための指示を含む、キット。
- 軟骨障害を有する対象の治療における使用のための活性化合物であって、前記対象が、
a)350±2SDng/mmolより低いCTX-II及び/もしくは4.2±2SDng/mLより低いProC2ならびに3.5±2SDmmより低いもしくはそれに等しいJSW、又は
b)350±2SDng/mmolより低いCTX-II及び/もしくは4.2±2SDng/mLより低いProC2ならびに3.5±2SDmmより高いJSW、又は
c)350±2SDng/mmolより高いCTX-II及び/もしくは4.2±2SDng/mLより高いProC2ならびに3.5±2SDmmより低いJSW
を示すことを特徴とする、使用のための活性化合物。 - 前記活性化合物が、FGF-18化合物、BMP-2、BMP-7、GDF-5、FGFβ、FGF-9、SOX-9増進剤、TGFβ、Wnt阻害剤、抗MMP13阻害剤、抗ADAMTS4又は5阻害剤、カルシトニン及びこれらの任意のバリアント又は融合タンパク質からなる群から選択される、請求項1~12のいずれか1項に記載の方法、請求項13に記載のキット又は請求項14に記載の使用のための活性化合物。
- 前記軟骨障害が、変形性関節症、軟骨傷害、関節軟骨に影響する骨折又は関節軟骨に対する影響力を有する外科的処置、例えば、マイクロフラクチャーからなる群から選択される、請求項1~12のいずれか1項に記載の方法、請求項13のいずれか1項に記載のキット又は請求項14のいずれか1項に記載の使用のための活性化合物。
