Abstract
Description
本発明者らは、特にRNA干渉(RNAi)を使用した、PMP22を標的とするアンチセンスRNAが、シャルコー・マリー・トゥース1Aの治療に非常に効率的であることを見出した。本発明者らは、実際に、PMP22を標的とする小分子干渉RNA(siRNA)を静脈内経路を介して投与することにより、PMP22がこれらのマウスにおいて1.5倍過剰発現されるシャルコー・マリー・トゥース1A病のマウスモデルにおいて、野生型マウスと同一の移動運動及び力の回復が可能となることを示した(実施例を参照されたい)。

本発明は、上で規定されるPMP22タンパク質をコードするmRNAを標的とするアンチセンスRNA、特に上で規定されるPMP22タンパク質をコードするmRNAを標的とするsiRNA、及び薬学的に許容される担体を含む又はそれからなる医薬組成物にも関する。
対象は、人間又は非ヒト哺乳動物であり得る。
本明細書において、「シャルコー・マリー・トゥース1A病の治療」によって、該疾患の進展を少なくとも部分的に食い止めること、又は該疾患を逆転させることを意味する。

本発明は、シャルコー・マリー・トゥース1A(CMT-1A)の治療における使用のための、PMP22タンパク質をコードするmRNAを標的とするアンチセンスRNAにも関する。
配列番号1は、配列番号12のアンチセンス鎖を含むsiRNAのセンス鎖配列である。
siRNA及び化学修飾
センス及びアンチセンスsiRNA鎖の設計配列を、Eurogentec社、Franceから購入した。それらを合成し、次いでマトリックス支援レーザー脱離/イオン化飛行時間型質量分析(MALDI-TOF MS)によって特徴付けし、逆相高速液体クロマトグラフィー(RP-HPLC)によって精製した。ヌクレアーゼに対する安定化を提供する2つの3'突出2'デオキシヌクレオチド残基を有する19merとして、一本鎖RNAを合成した。機能性を保つために、ジベンゾシクロオクチン(DBCO)反応基を、N-(ヘキサメチレニル)-6-オキソヘキサンアミドスペーサー(C6)を介して各siRNA配列のセンス鎖の5'末端に導入した。RNA一本鎖からsiRNAを生成するために、等モル量のセンス及びアンチセンス鎖の両方を、アニーリングバッファー[30mM HEPES-KOH(pH7.4)、2mM酢酸Mg、100mM酢酸K]中で95℃で3分間アニールさせ、次いで、-20℃で保管する前に室温で45分間インキュベートした。
基礎レベルのPMP22遺伝子発現を回復させるために、PMP22に対する8つのsiRNA(Table 2(表2)及びTable 3(表3)を参照されたい)及びsiRNA対照(siRNA CTRL)スクランブル配列を、3つの異なる方法(Taferソフトウェア、Thermofisher社ソフトウェア、及びReynold法)を使用することによって設計した。siRNAを、リポフェクタミンiMAX(登録商標)を使用することによって、PMP22を内在的に発現するシュワン細胞モデル(MSC 80)にトランスフェクトした。PMP22発現を減少させるsiRNAの能力を、24時間、48時間、及び72時間の時点で試験した。PMP22及びP0遺伝子の両方の発現を、リアルタイムPCRによって査定した。次いで、P0レベルに影響を及ぼすことなく(P0脱調節はCMT-1病に関与すると記載されている)、長期持続効果を有して、約50%のPMP22発現を阻害し得る最良のsiRNA配列を、25nM、50nM、及び100nMの種々の濃度で試験して、PMP22及び細胞生存率(MTT試験)を減少させるその能力をチェックした。スクランブルsiRNAを対照として使用した。
スクアレンを、以前に記載されるように(Massaad-Massadeら、Bioconjugate Chem.、2018、29 (6)、1961〜1972頁、DOI:10.1021/acs.bioconjchem.8b00205)、ジベンゾシクロオクチンのヒュスゲン環化付加(銅不含のクリックケミストリー)によってsiRNAに結合した。簡潔には、オリゴヌクレオチドのセンス鎖を、siRNAに対する市販の修飾であるジベンゾシクロオクチン残基によって5'末端で修飾した。スクアレンは、アジド官能基を運んでいる。コンジュゲートをHPLCによって精製し、MALDI-TOFF質量分析によって特徴付けした。
1nモルの、siRNA PMP22の5'末端修飾されたセンス鎖DBCO-C6(DNAse/RNAse不含水中1mg/mL)と、50nモルのSQ-N3(DMSO中1mg/mL)とを、DMSO(286μL)及びアセトン(65μL)を含有するガラスバイアル中で混合した。次いで、溶液を撹拌下で室温にて12時間インキュベートして、siRNA PMP22-SQバイオコンジュゲートを獲得した。翌日、過剰のアセトンを30分間の窒素フローの下で除去し、その後に凍結乾燥工程が続いた。
波長が190〜800nmに及ぶフォトダイオードアレイ検出器、ポンプ、及ぶ手動注入器を備えたthermoscientifc社高速液体クロマトグラフィーシステム(UltiMAte3000)でHPLC精製を実施した。固定相は、プレカラム(Hamilton社)で保護された、非多孔性アルキル化ポリスチレンジビニルベンゼンカラム(Hamilton社PRP-3 10μm、4.6×250mm、PEEK、Ref:79574)からなった。Thermofisher社Chromeleonソフトウェアを、1.2mL/分の流速及び100μLの注入容量を用いたデータ収集に使用した。移動相A及びBの勾配を適用した。移動相Aは、5%アセトニトリル、水90%とともに0.2M TEAA(5%)、pH7.0から構成され、一方で移動相Bは、5% TEAA、5%の水とともに95%アセトニトリルからなった。精製のために適用された勾配は以下のとおりであった:0〜8分、B相の0%〜24%の線形勾配;8〜16分、B相の24%〜90%の線形勾配;16〜18分、B相の90%〜100%の線形勾配;18〜30分、B相の100%;30〜32分、B相の100%からA相の100%への線形勾配;及び32〜42分、100%のA相での再平衡化。siRNA-SQバイオコンジュゲートを、手動のピーク回収によって精製した。2.4mLの画分容量に相当する画分を2分間回収し、次いで凍結乾燥した。すべての凍結乾燥siRNA画分を、DEPC処理水中で再構成した。
MALDI-TOF/TOF UltrafleXtreme質量分析計(Bruker Daltonics社、Bremen)をすべての実験に使用した。マススペクトルを線形陽イオンモードで獲得した。レーザー強度をイオン生成閾値のすぐ上に設定して、著しいピークの広がりのない、考え得る最も高いシグナル対ノイズ(S/N)比を有するピークを獲得した。すべてのデータを、FlexAnalysisソフトウェアパッケージ(Bruker Daltonics社)を使用して処理した。
メーカーのプロトコールに従って、及び1本のRNA鎖からのsiRNAの生成に関して前に言及されたのと同じように、siRNA PMP22及びsiRNA Ctの両鎖のアニーリングを、SQへのセンス鎖のバイオコンジュゲーションの後に実施した。正確には、等モル量のsiPMP22C6-SQバイオコンジュゲート及びアンチセンスsiPMP22の両方を、アニーリングバッファー[30mM HEPES-KOH(pH 7.4)、2mM酢酸Mg、100mM酢酸K]を用いて混合し、95℃で3分間インキュベートし、次いで、-80℃で保管する前又は直接沈降の前に室温で45分間インキュベートした。同じプロトコールを実施して、siRNA Ct-SQバイオコンジュゲートを獲得した。
ナノ粒子(NP)のsiRNAPMP22-SQ及びsiRNA Ct-SQを、アセトン:水(1:2)中でのナノ沈降によって調製した。撹拌下で、一方の相を他方にゆっくりと添加し、すなわち10nモルのsiRNA-SQを1mlのDEPC処理水に溶解し、撹拌下で、500mlのアセトンの上に液滴を上手に添加した。次いで、2つの溶液を5分間撹拌下に保ち、その後、窒素流動を使用してアセトンを完全に蒸発させて、10μM濃度の純粋なsiRNA-SQナノ会合物の水性懸濁液を獲得した。
この調査は、Pereaら(Hum Mol Genet 10、1007〜1018頁、2001)によって確立されたCMT-1Aのマウスモデルにおいて、インビボで最初に実施された。このモデルでは、PMP22の過剰発現は、末梢神経のシュワン細胞で特異的に生じ、CMT-1A病の原因となる脱髄を引き起こす。トランスジェニックマウスを、B6及びCBAという2つの遺伝的バックグラウンドにおいて、卵母細胞の復活(reviviscence of oocyte)後に、TAAM CNRSから購入した。3ヶ月齢の時点で、分子及び行動調査をJP18 B6及びCBAで実施して、それぞれPMP22のそれらの遺伝子発現、運動機能及び感覚機能を調査した。このマウスモデルは、PMP22遺伝子の1つの余分なコピーを含む。すべてのデータを、B6又はCBAバックグラウンドにおけるCMT-1AマウスとWTマウスとの間で比較した。JP18マウスモデルに関しては、マウスpmp22 cDNAをPhCMV*-1プロモーターの制御下に導入し、それ故、マウスは一生を通じてpmp22を過剰発現した。
JP18B6マウスを、比較としての野生型群に加えて、それぞれ5匹の3群に分けた。1つの群に5%デキストロース溶液のビヒクルを与え、2番目をsiRNACTRL-SQ NPで処理し、3番目をsiRNAPMP22-SQ NPで処理した。すべての処理を、1週間あたり2回、注射あたり0.5mg/kgの間隔で、2.5mg/Kgの累積用量の眼下IV注射によって投与した(合計5回の処理)。処理の終わりに、マウスを屠殺し、座骨神経を更なる調査のために採取した。すべての動物実験は、French Ministry of Higher Education and Research(Ministere de I'Enseignement Superieur et de la Recherche;MESR、APAFIS#I 0131-2016112916404689 vl 6)に登録された施設内動物実験倫理委員会(CEEA)及び研究評議会によって承認され、欧州共同体によって確立された条件(2010/63/UE指示)下でのフランスの法律及び規制に従って実行された。検証は、倫理基準に従って及びヘルシンキ宣言に従って行われた。動物の苦痛を最小限に抑えるあらゆる努力がなされた:処理の投与は、イソフルラン麻酔の下で実施され、動物は頸椎脱臼によって屠殺された。すべての動物は、滅菌した層流ケージシステムで飼育された。食糧、水、及び寝床は、ケージ内に置かれる前に滅菌された。食糧及び水は、自由裁量で与えられた。
梁歩行試験:3cmの直径、70cmの長さ、及び平面からおよそ30cm上方にある桿を有するプラットフォームにマウスを置いた。桿の一方の末端に、動物を収容する安全プラットフォームを設定した。まずマウスを順応させ、次いで梁を渡るために訓練し、その後、横断するのにかかる時間、速さ、停止の回数、及び左又は右ヒンダウ(hindaw)の誤り/スリップの回数を分析のために記録した。処理を始める前及び実験の終わりに、セッションあたり3回の試行に対して動物を記録した。動物あたり3回繰り返される行動課題を、高解像度デジタルカメラを使用して記録した。
アメリカ神経筋電気診断医学協会(American Association Of Neuromuscular And Electrodiagnostic Medicine)のガイドラインに従って、標準的EMG装置(Natus/EMG)を用いて試験を行った。イソフルラン吸入によって麻酔を実施し、マウスを純酸素中1.5〜2%イソフルランを含有する誘導チャンバーに置いた。全手順の間、フェイスマスクにより麻酔を同じレベルで維持した。マウスを加熱パッド上のそれらの正面側に置いて、それらの体温を34〜36℃に維持した。複合筋活動電位を記録するために、刺激針電極を座骨神経ノッチレベルに挿入し、陽極電極を尾の上側基部に挿入し、一方でレセプター針又は記録針を腓腹筋の内側部に挿入した。8mAの最大上矩形波パルスを刺激針を通じて送達し、筋肉を通じて大きさとして記録した。感覚神経伝導速度の測定のために、レセプター針から2〜2.5cmの距離を有して尾の長さの2/3に配置された刺激針を通じて、尾神経の複数回刺激を送達した。刺激電極とレセプター電極の中間に接地電極を挿入した。感覚神経伝導速度を、刺激の待ち時間、及び刺激電極とレセプター電極の間の距離から算出した。
CMT-1A動物(siRNAで処理された又は処理されていない)の座骨神経組織構造を、有髄又は無髄軸索の数及び髄鞘の厚みがg比を判定することによって査定される電子顕微鏡法によって調査した。次いで、軸索あたりのミトコンドリアの数を算出して、軸索罹患を査定した。RT-qPCR及びウェスタンブロット実験を実施して、CMT-1Aマウスの座骨神経におけるPMP22発現に対するsiRNAの効果を評価した。PMP22発現を対照レベル(siRNA PMP22で処理されたWT動物又はCMT-1Aマウス)に対して正規化した。siRNA PMP22で処理された又は処理されていないCMT-1Aマウスの神経筋接合部(NMJ)を免疫組織化学によって分析する。CMT-1Aにおける筋肉の衰弱は軸索変性症に起因しており、筋肉の除神経及び萎縮をもたらす。
放射性標識された遊離した又はSQコンジュゲートしたsiRNA PMP22(siRNA PMP22-SQ 32P NP)を、CMT-1Aを持つトランスジェニックマウスに静脈内注射する。種々の時点で臓器及び血液を採取し、遊離対siRNA PMP22-SQ NPの濃度を、gカウンターによって放射能をカウントすることによって判定した。また、siRNA PMP22は所望の組織内にいったん蓄積されると分解されないことが「Radio-HPLC」分析によって立証される。薬物動態パラメーターを算出する(例えば、血漿中半減期及びクリアランス)。特筆すべきは、スクアレンの50%を上回る割合がLDL及びHDLリポタンパク質によって輸送され、それ故、siRNA PMP22-SQ NPと血液構成成分(すなわち、LDL、HDL、VLDL、血球等)との考え得る相互作用を、Sawleら(Journal of lipid research 43、335〜343頁、2002)によって以前に記載されるように評価する。
すべてのデータを平均±標準偏差(SD)として提示した。ノンパラメトリックなクラスカル-ウォリス分析、それに続くダン検定、又はAnova、それに続くボンフェローニ検定を使用し、GraphPad Prismを使用して複数の処理を比較した。p<0.05を統計的に有意なレベルと見なした。
MSC-80細胞におけるPMP22 mRNA発現に対するsiRNA PMP22の効果
まず、mRNA PMP22発現に対するsiRNA PMP22(1〜8)の効果を調査した。それを、未処理の細胞へのsiRNA対照であるスクランブル配列に対する効果と比較した。クラスカル-ウォリス、それに続くダン検定を使用することによって、siPMP6を除いて、すべてのsiRNAが、PMP22発現を24時間及び48時間有意に下方調節することが見出された。トランスフェクションの72時間後、siPMP4、5、7、及び8は、対照と比較した場合に、PMP22遺伝子発現を50%減少させ得た(図1を参照されたい)。加えて、トランスフェクションの72時間後、siPMP7は、PMP22の遺伝子発現を減少させ得るだけでなく、そのタンパク質発現も減少させ得ることが見出された(図2を参照されたい)。
その後、72時間及び96時間のMSC80細胞のトランスフェクションによるPMP22及びP0の遺伝子発現に対する、種々の濃度(25nM、50nM、及び100nM)のsiPMP7の効果を調査して、ミエリン圧縮及び細胞生存率にPMP22とともに関与するタンパク質であるP0発現に影響を及ぼすことなくPMP22発現を正常化し得る最適な濃度を判定した。本発明者らの結果は、最適な濃度は50nMであることを示している。siPMP7(50nM)は、72及び96時間の間、PMP22の遺伝子発現をそれぞれ50%減少させ、一方でそれはP0遺伝子発現に影響を及ぼさなかった(図3を参照されたい)。また、MTTアッセイによる細胞生存率試験は、siPMP7 50nMが、MSC80生存率に有意な効果を有しないことを示している(図4を参照されたい)。siRNA対照(siCt)は、調査した2つの遺伝子の発現及び細胞生存率を改変しない。

バイオコンジュゲートsiRNA PMP22-SQ及びsiRNA CTRLを、図12に詳述される反応の最適化条件のおかげで90%を上回る割合の産出量を有して、Cu不含クリックケミストリーによって獲得した。siRNA PMP22-SQに関しては、HPLCによって同定され且つMALDI-TOF MSによって分析された回収副産物(bi-product)は、バイオコンジュゲートが7628の予想分子量を有することを示した(データ示さず)。結果として生じたバイオコンジュゲートをアンチセンス鎖とアニールさせ、RNAse不含水中でナノ沈降させた。溶液はチンダル効果を生み出し、ナノ粒子の形成を示唆した。動的光散乱分析(DLS)及びcryoTEM画像は、良好な多分散指数(0.14〜0.2)を有する約180nmの安定なナノ粒子の形成を30日間示し、IV注射に適した均質な溶液を反映した(図4を参照されたい)。siRNA CTRLに関しては、バイオコンジュゲーション及び産物の回収の後にMALDI-TOF MSによって同定された分子量は7621ダルトンであった。DLS測定は、siRNA CTRL-SQ NPのサイズが、1ヶ月の期間にわたって安定であること(サイズ:0日目に255±2及び30日目に238±4)、及び多分散指数(0及び30日目にそれぞれ0.15±0.02及び0.1±0.01)を示した。
i)siRNA PMP22のスクアレノイル化(squalenoylation)が、SQへのバイオコンジュゲーション後に約100%の産出量、及びHPLC精製後に約85%の産出量を生み出したこと;
ii)結果として生じたNPが再現性があり且つ1ヶ月にわたって安定であること;並びに
iii)NPがインビトロで効率的であり:リポフェクタミンを用いてトランスフェクトした場合に、それらは、むき出しのsiRNA PMP22と同程度にPMP22を阻害し、且つ細胞生存率に効果を有しなかったこと
を示した。
まず、B6及びCBAバックグラウンドにおけるトランスジェニックPMP22マウスをそれらの対応する野生型マウスと比較することによって、座骨神経でのPMP22の発現を試験した。B6及びCBAバックグラウンドにおける両PMP22マウスに関して、本発明者らは、野生型と比較してPMP22 mRNA発現の増加を見出した。P0発現は、両鎖において同じレベルで見出された(図7A及び図7B、並びに図8A及び図8Bを参照されたい)。
非常に興味深いことに、B6バックグラウンドでのJP18で行われた梁歩行試験は、siRNA PMP22-SQナノ粒子により処理されたマウスによって道を渡るのに費やされた時間が、未処理の野生型マウスのものに匹敵することを示した(図8C;図8Dを参照されたい)。ビヒクル(5%デキストロース)で又はsiRNA対照-SQナノ粒子で処理されたマウスは、時間の差を示さなかった。CBAマウスでの予備的結果は、siRNA PMP22-SQナノ粒子で処理された群に対して同じ結果を示した(図10を参照されたい)。
マウスの歩行運動活性を、処理の前及び終了後に、2つの補完的試験:梁歩行及びロコトロニック試験によって試験した。また、握力を同じ条件下で調査した。本発明者らは、siRNA PMP22-SQナノ粒子で処理されたJP18(シングルトランスジェニック)及びJP18/JY13(ダブルトランスジェニック)マウスが、野生型マウスと同様の歩行運動能力、並びに未処理のマウス(5%デキストロース)及びCt-SQナノ粒子で処理されたマウスよりも有意により優れた歩行運動能力を有することを見出した(図13及び図14)。
siPMP22-SQ NPは、単一及びダブルトランスジェニックマウスの両方において、処理の3週間後に、複合筋活動電位(CMAP)(図15A及び図15Cを参照されたい)及び感覚神経速度(図15B及び図15Dを参照されたい)を回復させる。
siPMP22-SQ NPは、単一及びダブルトランスジェニックマウスの両方において、g比を改変しなかった(図16及び図17を参照されたい)。
次いで、梁歩行、ロコトロニック、及び握力試験に対するsiPMP22-SQ NPの長期持続効果を分析した。ダブルトランスジェニックJP18/JY13 B6マウスはサイクルあたり2.5mg/Kgの2サイクルの処理を受けた。処理を2サイクルの間に21日間停止して、再発を調査した。マウスを週1回追跡して、回復及び再発期間を分析した。
CMT-1A病は、シャルコー・マリー・トゥース(CMT)病態の最もよく見られるタイプであり、CMTの症例の40〜50%を占める。この独創的な手法は、特にsiRNAによって、末梢ミエリンタンパク質22(PMP22)の過剰発現を標的とすることによる、CMT-1A病に対する新たな療法を開発することを目指す。

- 前記アンチセンスRNAが、前記アンチセンスRNAを含むナノ粒子の形態で提供される、請求項1から8のいずれか一項に記載の使用のためのアンチセンスRNA。
- 前記アンチセンスRNAが、静脈内に、腹腔内に、皮下に、又は神経内に、好ましくは座骨神経に投与される、請求項1から9のいずれか一項に記載の使用のためのアンチセンスRNA。
- 前記アンチセンスRNAが、シャルコー・マリー・トゥース1Aの治療において有用な少なくとも別の薬物と組み合わせて使用される、請求項1から10のいずれか一項に記載の使用のためのアンチセンスRNA。
- PMP22タンパク質をコードするmRNAを標的とするアンチセンスRNAであって、前記アンチセンスRNAが、
(i)
- 配列番号9の配列のヌクレオチド970〜988、
- 配列番号11の配列のヌクレオチド431〜449、
- 配列番号9の配列のヌクレオチド429〜447
からなる、その中に含まれる、又はそれと重複する一部分、
(ii)
- 配列番号11の配列のヌクレオチド1721〜1739、
- 配列番号9の配列のヌクレオチド1726〜1744、
- 配列番号11の配列のヌクレオチド1805〜1823、
- 配列番号9の配列のヌクレオチド1809〜1827
からなる又はそれと重複する一部分、
(iii)天然に存在するバリアントに存在する(i)又は(ii)の一部分と相同な一部分
に相補的な核酸である、アンチセンスRNA。 - 前記アンチセンスRNAが、siRNA、shRNA、miRNA、dsRNA、及びインビボで切断されてsiRNAを形成し得るRNA種からなる群から選択される、請求項12に記載のPMP22を標的とするアンチセンスRNA。
- 前記アンチセンスRNAが、1つ又は2つの一本鎖突出を含むsiRNAである、請求項12又は13に記載のPMP22を標的とするアンチセンスRNA。
- 前記アンチセンスRNAが、(i)配列番号16、配列番号17、及び配列番号18の配列からなる群から選択される配列、並びに(ii)任意で、1つ又は2つの一本鎖突出、を含む又はそれからなるsiRNAである、請求項12から14のいずれか一項に記載のPMP22を標的とするアンチセンスRNA。
- 請求項12から15のいずれか一項に記載のアンチセンスRNAを含むナノ粒子。
- 前記アンチセンスRNAが、スクアレン又はその誘導体に結合している、請求項16に記載のナノ粒子。
