Abstract
Description
本発明は、小児におけるパーキンソン病の、そしてレボドパ応答性症候群の治療の分野に関する。
本発明は、静脈内注入に似た、パーキンソン病のカルビドパ/レボドパ経口治療における新規のアプローチに関する。前記の改善された治療は、正確な用量分割を可能にして、初期患者における運動応答合併症の発症を遅延させそして回避さえし、中期患者におけるこれらの合併症の重篤度を軽減し、そして後期患者における既存の運動応答合併症を安全に治療するかまたは排除さえする、新規の多割線2層カルビドパ25mg/レボドパ100mg医薬単位形態を使用することによってなされる。
そしてレボドパ応答性症候群に罹患した小児の治療の必要性に応える。

シネメット(登録商標)米国ラベルによれば、「治療は1錠のCD/LD 25/100を1日に3回で最良に開始される。この投薬スケジュールは、1錠当たり25mgのCDを提供し、典型的な1日当たり3回の1日投薬スケジュールでは1日当たり75mgに相当する。投薬量は、必要に応じて、毎日または1日置きに1錠ずつ、高くても1日当たり8錠のCD/LD 25/100まで拡大し得る投薬量まで増加され得る。いくつかの場合において、CD/LD 25/250錠剤が使用され得、そしてCD/LD 25-250の投薬量は、毎日または1日置きに半錠または1錠ずつ、最大1日当たり8錠まで増加され得る。

パーキンソン病(PD)および関連する運動障害には、米国において150万人近くが罹患していると現在推定されている。世界的には、約700万人から1000万人がこの障害を有して生存していると考えられている。約60,000人のアメリカ人が毎年PDと診断されている(検出されないままの数千のケースを反映していない数である)。PDの発生率は年齢とともに増加するが、この障害を有する人々の推定4%は50歳の前に診断されている。稀に、小児がPDの若年形態を発症し得る。男性がPDを有する可能性は、女性より1.5倍高い。治療、社会保障の支払い、および労働できないことに起因する収入の損失などの、PDの直接および間接の費用を合わせると、米国単独で1年当たり250億ドル近くであると算出されている。時間が経過するにつれて、PDの負担は、人口の老化とともに増加し、そして持続不可能になる。

Claims (14)
- 運動応答合併症の予防または対症治療と同時に、パーキンソン病または関連するレボドパ(LD)応答性障害を治療するための方法であって、それを必要とする患者に、各々がカルビドパ-6.25mg/レボドパ-25mgおよびカルビドパ-8.3mg/レボドパ-33.3mgからなる群より選択される量のカルビドパ/レボドパを含む、1~6の単位用量を、1日当たり5回~9回投与する工程を含む、方法。
- 前記患者が、パーキンソン病の初期において新たに診断された患者である、請求項1に記載の方法。
- 前記患者が、カルビドパ/レボドパでの標準治療が以前に投与されたパーキンソン病患者である、請求項1に記載の方法。
- 前記患者が、運動応答合併症に罹患している、請求項3に記載の方法。
- 前記患者における前記運動応答合併症が緩和される、請求項4に記載の方法。
- 前記単位用量が、活性成分なしで医薬担体を含む不活性底部層に隣接した、即時放出のための医薬担体と混合された総量25mgのカルビドパおよび総量100mgのレボドパを含む完全に割線を付された頂部層によって形成され、前記頂部層の各々の割線が底部層に部分的に貫通する、2層錠剤からなる3割線/4区分医薬単位形態の1区分としての、カルビドパ-6.25mg/レボドパ-25mgである、請求項1に記載の方法。
- 前記単位用量が、活性成分なしで医薬担体を含む不活性底部層に隣接した、即時放出のための医薬担体と混合された総量25mgのカルビドパおよび総量100mgのレボドパを含む完全に割線を付された頂部層によって形成され、前記頂部層の各々の割線が底部層に部分的に貫通する、2層錠剤からなる2割線/3区分医薬単位形態の1区分としての、カルビドパ-8.(3)mg/レボドパ-33.(3)mgである、請求項1に記載の方法。
- 活性成分なしで医薬担体を含む不活性底部層に隣接した、即時放出のための医薬担体と混合された総量25mgのカルビドパおよび総量100mgのレボドパを含む完全に割線を付された頂部層によって形成された3割線/4区分2層錠剤であって、前記錠剤の前記頂部層の4つの区分の各々が6.25mg/25mgの量のカルビドパ/レボドパを含み、前記頂部層の前記割線が不活性底部層に部分的に貫通する、錠剤。
- 活性成分が口腔内崩壊処方物に処方される、請求項8に記載の錠剤。
- レボドパ(LD)に対して過敏性である患者においてパーキンソン病またはLD応答性障害を治療するための方法であって、それを必要とする患者に、カルビドパ-10mg/レボドパ-100mg、カルビドパ-25mg/レボドパ-100mgからなる群より選択される量のカルビドパ/レボドパを投与する工程を含む、方法。
