Abstract
Description
本出願は、米国特許法119条の下に、2018年7月10日出願の米国仮出願第62/696,020号の利益を主張し、上記出願の開示の全体は、参照することにより本出願に包含される。
技術分野
本発明は、強直性脊椎炎(AS)の診断、予後および治療のためのバイオマーカーおよび標的に関する。本発明はまた、ASの動物モデルを作製する方法、それから作製された動物モデル、およびそのような動物モデルを使用してASの治療において薬学的に活性な薬剤をスクリーニングする方法に関する。
本発明において、我々はAS MSCを確立し、AS MSCにおける強化されたTNAP(ALPL遺伝子産物)が、AS MSCによって誘発される骨付加の病理学的メカニズムに必要であり、TNAPの遮断がAS MSCの異常な鉱化を阻害することを見出した。また、ASの治療に有効な薬剤のスクリーニングに有用なAS MSCを使用したAS動物モデルを確立した。さらに、ALPL遺伝子産物(血清TNAP/または骨ALPレベル)が、強直性脊椎炎(AS)、特にASのレントゲン写真の重症度と高度に相関し、したがって、ASを診断するため、およびまたAS患者の進行を監視するためのバイオマーカーとしても役立つことを被験者で実証した。
(a)第1の時点で、患者からの第1の生体サンプルを提供すること;
(b)第1の時点より後の第2の時点で、患者からの第2の生体サンプルを提供すること;
(c)第1および第2の生体サンプル中のASマーカーとしてのALPL遺伝子産物のレベルを測定すること;および

1.1 ヒト対象
すべてのドナーは、ヘルシンキ宣言に従ってサンプリングする前に書面によるインフォームドコンセントを提供した。研究プロトコルは、Taipei Tzu Chi Hospital、Buddhist Tzu Chi Medical Foundation、Taipei Veteran General Hospital、Academic Sinica(すべて台湾内)および英国のRoyal National Hospital for Rheumatic Diseasesの研究倫理委員会によって承認された。
脊椎短縮骨切り術を受けた3人のAS患者(A1、A2、A3)からの脊椎強直に関与する組織内のBMからのMSC(AS MSC)を単離した。同様の部位にてBMから外傷手術を受けた3人の非AS個人(C1、C2、C3)のMSC(N MSC)を対照として使用した。すべて台湾人である。これらの検体を細かく刻み、抗生物質-抗真菌溶液を含むアルファ最小必須培地(α-MEM;Gibco、Grand Island、NY、USA)でリンスした後、1mg/mLコラゲナーゼD(Roche、Basel、Switzerland)で消化した。37℃で一晩インキュベートした後、消化した組織を40μmナイロンフィルターでろ過して破片を除去した。細胞を遠心分離によって収集し、培養皿にプレーティングして付着させた。48時間以内およびその後3日ごとに培地を交換することによって、非接着細胞を除去した。コンフルエンスに達したら、細胞を回収して継代培養した。継代3にて、初代MSCのアリコートを液体窒素で凍結保存し、実験のために増殖させた。ASMSCとNMSCの同じ継代(p3〜p5)を比較した。
MSCのマーカーの細胞表面発現を分析するために、MSCを以下の抗ヒト抗体で標識した:CD31-PE、CD34-PE、CD44-PE、CD29-FITC、CD45-FITC、およびCD105-FITC(すべてAncell、Bayport、MN、USAから)。マウスアイソタイプ抗体(Ancell)を対照とした。全体として、FACScanto Flow Cytometer(BD Biosciences、Franklin Lakes、NJ、USA)を使用して、100,000個の標識細胞を取得して分析した。データ分析にはFCSExpress v3.0(De Novo Software、Glendale、CA、USA)を使用した
MSCを6ウェルプレートあたり8×104細胞で播種し、レバミゾール(100 μM;Sigma-Aldrich)(非競合的TNAP阻害剤)、硫酸ベリリウム四水和物(競合的TNAP阻害剤)(100μM;Santa Cruz Biotechnology)、またはパミドロネート(非競合的TNAP阻害剤)(1μg/ml;Sigma-Aldrich)の存在または不在下にて、5%CO2インキュベーション、37℃で骨形成誘導培地で増殖させた。形態学的分化後、細胞をアリザリンレッドS染色に付した。
全RNAを、製造元の指示に従ってRNeasy Miniキット(Qiagen、Hilden、Germany)を使用して抽出した。大容量cDNA逆転写キット(AppliedBiosystems、Waltham、MA、USA)を使用して、2μgのトータルRNAからファーストストランドcDNAを合成した。RT-QPCRを、Prism 7300シーケンス検出システム(Applied Biosystems)にてSYBR GreenPCRマスターミックスで実行した。マーカー遺伝子の相対的定量を、内部対照遺伝子としてグリセルアルデヒド3-リン酸デヒドロゲナーゼ(GAPDH)を使用するΔCt法に従って行った。RT-QPCRに使用される特定のプライマーを以下に示す。
細胞溶解物を、M-PER(登録商標)タンパク質抽出試薬(Pierce、Rockford、IL、USA)とプロテアーゼ阻害剤カクテル(HaltTM; Pierce)を使用して調製した。タンパク質溶解物(20〜40μg)を10%ドデシル硫酸ナトリウム-ポリアクリルアミドゲルで分離し、ポリフッ化ビニリデン(PVDF)膜に移した。PVDF膜をTBST(20mM Tris-HCl [pH 7.6]、137mM NaCl、1%Tween 20)中の5%ブロッティンググレードミルクでブロックした。次に、PVDF膜を指定の一次抗体で4℃で一晩プローブし、洗浄しまし、次に、対応する二次抗体でさらに1時間プローブした。免疫反応性タンパク質を、製造元の指示に従って、ウエスタンブライトシリウス化学発光検出試薬(Advansta、Menlo Park、CA、USA)によって検出した。化学発光シグナルの画像を、LAS 3000システム(富士フイルム、東京、日本)を使用して捕捉した。使用した一次抗体は、ラット抗TNAP抗体(1μg/mL;R&D Systems、Minneapolis、MN、USA)、ウサギ抗GAPDH抗体(1:10000;Gentex、Zeeland、MI、USA)およびマウス抗ベータ-アクチン抗体(1:10000;ThermoFisher scientific)であった。使用した西洋ワサビペルオキシダーゼ結合二次抗体は、ヤギ抗ラットIgGおよび抗ウサギIgG(すべて1:5000希釈、Sigma-Aldrich製)であった。イ免疫ブロット実験を少なくとも2回行った。すべてのタンパク質バンド密度を、GAPDHまたはベータアクチンローディング対照の密度に正規化した。
蛍光ALP活性検出キット(Abcam、Cambridge、UK)を使用して、細胞内ALP活性を測定した。簡単に説明すると、細胞(5×104)を100μLのアッセイ緩衝液でホモジナイズし、4-メチルウンベリフェリルホスフェート二ナトリウム塩基質を添加した。ALPは、非蛍光性の4-メチルウンベリフェリルホスフェート基質のリン酸基を切断し、蛍光性の4-メチルウンベリフェロンを生成する。30分後に停止液を加え、蛍光マイクロタイタープレートリーダー(Spectramax Gemini;Molecular Devices)を用いて360/440nmの励起/発光波長での蛍光強度を測定した。サンプル中のALP活性を、次の式を使用して計算した:ALP活性=A/V/T(mU/mL)、ここで、Aはサンプルによって生成された4-メチルウンベリフェロンの量(nmol)、Vはアッセイウェル(mL)に添加されたサンプルの量、およびTは反応時間(分)である。
TNAP(TRCN0000052005、標的配列:GCGCAAGAGACACTGAAATAT、(配列番号:7))およびTRCN0000052007、標的配列:ACTGCCATCCTGTATGGCAAT、(配列番号:8))のためのショートヘアピンRNA(shRNA)発現プラスミドおよびバクテリアクローンは、RNAi Core Facility、Academia Sinica(Taipei、Taiwan)から入手した。ALPL cDNAフラグメントは、Genome Research Center、National Yang-Ming University(Clone:066116)の「哺乳類遺伝子コレクション」(クローン:066116)から得られたプラスミドから切り出され、Phusion High-Fidelity DNAポリメラーゼ(Thermo)を用いたPCRによって増幅された。ALPLフラグメントをpLAS2w.Ppuroベクター(RNAi Core Facility、Academia Sinica)にNheIおよびEcoRIサイト(NEB)でのライゲーションにより挿入した。レンチウイルスベクターの調製および形質導入のための手順は、以前に記載されたように実施された36、37。サブコンフルエントなMSCに、8μg/mLのポリブレン(Sigma-Aldrich)の存在下で多重度5のレンチウイルスベクターを形質導入した。24時間後、培養培地を、ピューロマイシン(1μg/mL)を含む新鮮な増殖培地と交換して、形質導入された細胞を48時間選択した。
MSCを骨形成誘導下で14日間培養した後、PBSで2回リンスし、4%パラホルムアルデヒドで室温で20分間固定した後、0.1%TritonX-100で室温で5分間透過処理した。次に、細胞を、PBST中の1%ウシ血清アルブミンによって室温で1時間ブロックした。ブロッキング緩衝液中のマウス抗オステオアドヘリン抗体(1:500希釈、R&D)を添加し、4℃で一晩インキュベートした。細胞をPBSで3回洗浄した後、二次抗体(Alexa fluor-488と結合したヤギ抗マウス抗体)(1:400;Invitrogen)とともに室温で1時間インキュベートした。細胞核を、PBS中の4',6-ジアミジノ-2-フェニルインドール(DAPI)(Sigma-Aldrich)で対比染色した。蛍光画像を共焦点顕微鏡(Carl Zeiss LSM510)で検査し、Zeiss EC Plan-Neofluar 40x/1.30 Oil DICM27で撮影した。
免疫組織化学染色用のBM(骨髄)スライスを、3人のAS患者(A1、A2、およびA3)、2人の健康な個人(C4、C5)および1人の非AS患者対照(C6)から得た。非AS患者対照は、リンパ腫の精密検査のために骨髄生検を受けて、リンパ腫がなかった患者であった。その中で、健康な対照(C4、C5)は白人であり、それらのBMスライドをUS Biomax(Rockville、MD、USA)から購入した。AS患者および非AS患者対照は、台湾人であった。BM組織のパラフィンブロックを4μmのスライスに切断し、標準的なプロトコルを使用して処理した。切片をTris/EDTA(pH9)溶液(Thermo Fisher Scientific)中のAntigen Retrieval Reagentで処理し、リン酸緩衝生理食塩水中の0.1%Triton X-100で15分間インキュベートした後、3%ヒト血清(Invitrogen、Carlsbad、CA、USA)でブロッキングした。切片をウサギ抗TNAP抗体(1:200希釈;Abcam)とともに4℃で一晩インキュベートした後、西洋ワサビペルオキシダーゼ(HRP)標識二次抗体(Dako)とともに室温で1時間インキュベートした。褐色の発色は、色原体3,3'-ジアミノベンジジン(Thermo Fisher Scientific)とのインキュベーションによって実施された。必要に応じて、核をヘマトキシリン(Thermo Fisher Scientific)で対比染色した。切片を水道水で5分間洗浄し、脱水し、カバースリップを取り付けた。
動物実験プロトコルは、Taipei Tzu Chi General Hospitalの動物管理利用委員会によって承認された。NOD-SCIDマウス(8〜10週)をBioLASCO(Taipei、Taiwan)から購入し、特定菌フリー環境で維持した。ヒトMSCを、培養物中のβ-グリセロホスフェートで5日間処理し、フィブリン(5μlフィブリンに106細胞)(Baxter、Vienna、Austria)に包埋し、NOD SCIDマウスの腰椎セグメントL4〜L5の右椎弓板に隣接する領域に移植した。腰椎の椎弓板を、鉗子を使用して皮質剥離した。手術後、マウスに0.9%リン酸塩食(Dyets、Bethlehem、PA、USA)を給餌した。腰椎のマイクロコンピューター断層撮影(SkyScan 1076;Kontich、Belgium)を移植の3週間後に行った。AS MSCを移植し、H2Oレバミゾール(10mg/kg)、硫酸ベリリウム四水和物(7.5mg/kg)、またはパミドロネート(0.3mg/kg)を12週間毎日経口投与した後、NOD-SCIDマウスのマイクロコンピューター断層撮影によって大腿骨の骨塩密度を測定した。新規骨付加の定量的量は、Bruker CT-Volumeバージョン2.0ソフトウェアによって取得された。
ヒト対象からの血液サンプルをEDTA処理チューブに収集した。LIASONアナライザーによるDiaSorin LIASON BAP OSTASEアッセイ(1ステップ遅延添加サンドイッチ化学発光免疫アッセイ)を使用して、骨特異的TNAP(BAP)のレベルを測定するために血清を収集した。製造元によれば、5.1〜20.2μg/Lの血清BAPレベルの基準範囲は正常であると見なされた。
サイトカイン産生を決定するために、MSCを骨形成誘導下で培養した。3、7、および10日目に上清を収集した。Human Milliplexキット(Merck Millipore、Billerica、MA、USA)を製造元の指示に従って使用して、サイトカインを検出した。データを、Luminex 200機器(Luminex、Austin、TX、USA)によって取得し、Milliplex analyst v5.1(Merck Millipore)によって分析した。
総RNAを、0、3、7日目に骨形成誘導下のAS MSCとN MSCから調製し、中央研究院(Academia Sinica)のGRC Microarray Core Facilityにて、ヒトマイクロアレイチップ(Affymetrix、Santa Clara、CA、USA)のために使用した。GeneChip(登録商標)3'IVT Expressキット(Thermo Scientific、Waltham、MA、USA)を使用した、逆転写、ビオチン結合ヌクレオチドの取り込み/断片化、および線形RNAの35〜200ntのビオチン標識フラグメントへの変換の逐次手法を着手した。次に、サンプルをGeneChip Human Genome U133 Plus2.0アレイにハイブリダイズさせた。アレイの洗浄と染色を、Fluidics Station 450(Thermo Scientific)を使用して行い、アレイデータを、GeneChip Scanner 3000(Thermo Scientific)を使用して取得した。最後に、アレイデータをGeneSpring GX v12(Agilent Technologies、Santa Clara、CA、USA)によって分析した。
データを、平均±平均の標準誤差(SEM)または平均±標準偏差(SD)として示す。データを、スチューデントのt検定、マンホイットニーのU検定、またはフィッシャーの直接確率検定によって分析した。変数間の相関を、スピアマンの順位相関検定で決定した。P<0.05を有意であると見なした。データを、SPSSまたはPrismv6.01によって分析した。
2.1 骨形成誘導下のAS MSCにおける加速された鉱化の病理学的表現型はRunx2に依存しない
ASにおける間質前駆細胞の活性化を研究するために、3人のAS患者(AS MSC)の脊髄強直に関与する組織内のBM-MSCからエクスビボ細胞培養モデルを確立した。これらの細胞を、外傷手術を受けた非ASコントロールのBM-MSC(N MSC)と比較した。これらの細胞は線維芽細胞様の形態を採用し、BM-MSCの特徴的な表面マーカーを発現した。それらは、特定の標準的な誘導法によってアッセイされるように、三系統の分化の可能性を有した16。
AS MSCの加速された石灰化の調節メカニズムをさらに調査するために、マイクロアレイ分析により、0、3、および7日目の骨形成誘導後のAS MSCとN MSCの間の遺伝子発現を分析した。骨形成誘導後、AS MSCでは、N MSCと比較して、2倍以上、それぞれ188個の遺伝子と136個の遺伝子がアップレギュレートおよびダウンレギュレートされた。これらのうち、骨形成に関与する遺伝子は、Ingenuity Pathway Analysisによって明らかにされた。RT-QPCRによる骨形成に関与する遺伝子のさらなる検証により、組織非特異的アルカリホスファターゼ(TNAP)発現およびアルカリホスファターゼ(ALP)活性の上昇は、骨形成誘導の前後の両方で、N MSCと比較して、AS MSCにおける異なる鉱化の加速速度に最も密接に関連していることが明らかになった。ALPは、脱リン酸化およびトランスリン酸化反応を触媒することができる非遍在性外部酵素の大きなスーパーファミリーである。それは、別々の遺伝子によってコードされるTNAP、胎盤、生殖細胞、および腸のALPを含む4つのアイソザイムからなる。その中で、TNAPはALPL遺伝子によってコードされ、選択的スプライシング転写変異体とともに肝臓/骨/腎臓組織に分布する20、21。ピロリン酸塩を加水分解し、無機リン酸塩を提供して鉱化作用を促進する20、21。AS MSCの異常な鉱化におけるTNAPの役割を決定するために、骨形成培養物を非競合(レバミゾール22またはパミドロネート23)および競合(ベリリウム硫酸塩24)TNAP阻害剤で処理した(図2D、図2E)。TNAPが、TNAPに対する2つの独立したshRNAによってサイレンシングされた場合、AS MSCにおける加速された鉱化作用の同様の減少が観察された(図2F-2J)。さらに、N MSCにおけるレンチウイルス形質導入を介したTNAP過剰発現は、強化された鉱化作用を示した(図2K-2M)。さらに、β-グリセロホスフェート(TNAPの基質)の存在下で増殖培地(GM)中でMSCを培養した21(図5)。予想通り、GM中で培養されたNMSCは石灰化しなかった。しかしながら、AS MSCへのβ-グリセロホスフェートの添加は、N MSCよりも高い鉱化率を示し、AS MSCでのTNAPの発現増加は、その基質の存在下で加速された鉱化を誘導するのに十分であることを示唆する。まとめると、これらの結果は、TNAPの発現の増強、がAS MSCにおける鉱化の加速に不可欠であることを示す。
次に、TNAP阻害剤の治療可能性をテストするために使用することができる病理学的骨付加を模倣するために、AS MSCベースのインビボ疾患モデルを確立した。特に、N MSCではなく、AS MSCを移植されたNOD-SCIDマウスは、腰椎セグメントL4-5の右椎弓板に隣接して、新規骨付加を発達させた(図3A)。さらに、対照のshRNA形質導入AS MSCと比較して、TNAPがshRNAによってサイレンシングされた場合、AS MSCの移植によって誘発された異所性骨付加はブロックされた(図3B)。一貫して、TNAP阻害剤の経口投与は、AS MSCを移植したNOD-SCIDマウスにおいて新規骨付加を大きく無効にしたが(図3C)、全体的な骨塩密度(BMD)は変化させなかった(図6)。群間の新規骨付加の定量的体積を示した(図3D)。これらの結果は、TNAP遮断が、NOD-SCIDマウスにおいてAS MSCによって誘発される新規骨付加を阻害する可能性があることを示唆する。
次に、我々は、AS患者のBMまたは末梢血でTNAPの発現増強が検出することができるかどうかを疑問に思った。BM標本の免疫組織化学(IHC)染色は、健康な個人(図4A)および非AS患者対照(図7)と比較して、AS患者のBMにおけるTNAPの発現の増加を明らかにした。IHC二重染色は、ほとんどのTNAP陽性細胞が骨髄細胞系列(ミエロペルオキシダーゼ+)、単球(CD68+)系統またはMSC(CD44+)であることを示した(図4B、4C、4D)。さらに、血清TNAPの代わりに、2つの別々のコホートからのサンプルを使用して肝臓と腎臓からのTNAPの干渉を除外するために骨特異的TNAP(BAP)の血清レベルを測定した:1つは104人のAS患者と50人の健康な対照を含む台湾のコホートであり、コントロール、他方は184人のAS患者を含む英国のコホートである。それらの人口統計学的特徴およびサブグループ/コホート間の比較的差異を示しれた(表1-3)。台湾と英国のコホートで血清BAPレベルが上昇した(>20.2μg/L)AS患者の割合は、それぞれ9.61%と25.54%であった。全体として、血清BAPレベルは、台湾のコホートでは健康な対照(12.954±5.538μg/Lおよび6.296±2.110μg/L;P<0.001)よりAS患者で有意に高かったが、血清BAPレベルは、英国のコホートでは17.607±8.055μg/Lであった。
ここでは、脊椎骨切り術後のAS患者(AS MSC)の強直脊椎の強直組織の骨髄から間葉系間質細胞を確立した。これは、骨切り術中の間質前駆細胞の活性化の研究、およびAS患者の強直を解決するための薬物を特定するための潜在的な細胞ベースのプラットフォームに使用することができる。本発明に由来するAS MSCは、骨形成誘導後の加速された鉱化の異常な表現型を示し、その組織の非特異的アルカリホスファターゼ(TNAP)活性が高度に発現された。本発明に由来するAS MSCは、NOD-SCIDマウスの脊椎に移植された場合、動物に異所性骨付加を誘発することができ、良好な動物モデルとして役立つ。

Claims (34)
- 強直性脊椎炎(AS)を検出するため、および/またはASのレントゲン写真の重症度の進行のリスクを予測するための方法であって、
(i)対象からの生体サンプルを提供すること;および
(ii)サンプル中のASマーカーとしてALPL遺伝子産物を検出すること;
を含む、方法。 - 遺伝子産物が、タンパク質またはRNA転写物を含む、請求項1に記載の方法。
- ALPL遺伝子産物あ、非特異的アルカリホスファターゼ(TNAP)である、請求項1に記載の方法。

- マーカーが、ALPL遺伝子産物に特異的に結合する薬剤で検出される、請求項1に記載の方法。
- 検出が、イムノアッセイ、質量分析アッセイ、核酸ハイブリダイゼーション検出アッセイ、および/または逆トランスフェラーゼ−ポリメラーゼ連鎖反応(RT-PCR)によって実施される、請求項1に記載の方法。
- 生体サンプルが、体液サンプルまたは組織サンプルである、請求項1に記載の方法。
- 検出の結果を参照レベルと比較すること、比較がALPL遺伝子産物のレベルの上昇を示す場合、対象をASを有する、および/またはASの放射線写真の重症度の進行のリスクがあると特定することを含む、請求項1に記載の方法。
- ASの発生またはASのレントゲン写真の重症度の進行のリスクを確認するために、対象にさらなるAS診断アッセイを適用することをさらに含む、請求項8に記載の方法。

(b)第1の時点より後の第2の時点で、患者からの第2の生体サンプルを提供すること;
(c)第1および第2の生体サンプル中のASマーカーとしてのALPL遺伝子産物のレベルを測定すること;および
(d)第1および第2の生体サンプル中のALPL遺伝子産物のレベルに基づいて患者のAS進行を決定すること(ここで、第1の生体サンプルと比較して第2の生体サンプル中のALPL遺伝子産物のレベルの上昇は、AS進行を示す);
を含む、方法。 - ASの進行が、ASのレントゲン写真の重症度のレベルが高いことを含む、請求項13に記載の方法。
